Dear National Transportation Safety Board, I would like to bring a situation that happened to me while driving my 2019 Subaru Forester last Saturday March 7th, 2026 at 10:40am. I was coming back to my house from an errand. ... hest freezer that was against the wall in the garage which was against the back wall. This freezer was also taken through the wall and into my laundry room. The freezer was crushed. The front of the car and tires were up through the wall and in the home. There was a water spigot also in the garage that came into my home that broke and dispersed water. The furnace was crushed, water heater damaged, washer and dryer damaged, a water cooler damaged along with crushed metal bifold doors that were ripped off HVAC closet. And other things in garage totally crushed. Thankfully, no-one was killed. I was barely able to get out of car as when it suddenly accelerated, and clipped the bottom of garage door that wasn’t finished being up, then it went towards the left and corner of garage instead of of how I would normally pull in and park which would have been straight and middle of garage. The car went so fast and would not stop until it had hit the items in house that would make it not be able to go any further. This vehicle definitely had something faulty with it that caused this car to suddenly take off and crash. And the air bags did not go off. Surprised with such a forceful impact. I have had regular maintenance done on my car and have even replaced the transmission fluid twice and the shudder is continuing to happen. Driving north on I-25 I encountered the snow/sleet storm. All of a sudden I found myself hydroplaning...or sliding...in the poor visibility and there was no nearby exit. I white-knuckled to the motel, and only later realized that my low-quality, already-worn tires were the likely problem. Yesterday I was shopping for new tires and learned that the factory-installed Continental ProContact RX 255/45R19 is not electric-vehicle rated, is a "passenger touring" tire and is not recommended for the weight of an EV. Additionally, it has a 55K mile warranty...I am at ~32K miles, and not even due for my next tire rotation, which is when I thought I would have to buy new tires. My local tire dealer, private in a small town, is not able to access the Continental warranty until the tire is down to a tread of 2/32". Discount Tire will not let me drive away if the tread is below 4/32"...and at 5/32 (I think) last tire rotation they weren't sure I'd even make it to my next rotation...clearly I haven't, even though I'm below the 8K interval for rotation. The price of the now-discontinued Continental ProContact RX was $211. I must now spend over $300/tire for a suitable replacement. Tesla knowingly installed a low-quality tire on my $53K car, further compromising my safety and that of other drivers and persons in the vicinity of moving Teslas. Upon further investigation, we learned that there was a recall on the Brake Actuator Assembly in 2016. Which is the part that has failed and almost caused my 16 year old daughter to crash the car she worked hard for. I contacted the Toyota Brand Engagement Center. It was stated to me that the recall has expired, however, I feel that the fact that a recall was issued means that it was recognized by Toyota that the vehicle was sold with a faulty brake system. This, in and of itself, is a hazard and could be fatal. This should be addressed and the brake system should be replaced as it is no fault to any owner regardless of the recall expiry date. We look forward to your timely response on how we can remedy this very serious safety issue. Vehicle experienced a catastrophic mechanical failure characterized by a percussive "shotgun blast" noise from the rear chassis, followed by an immediate, total loss of braking power (pedal to the floor) and a violent loss of directional stability. This incident represents an aggravated safety failure of known GMT900 platform defects. The technical chain of causality began with the factory exhaust
... manifold fasteners suffering a metallurgical shear, a universal defect documented under GM Bulletins 19-NA-218, PIP5075, and PIP4701. This shear created a high-velocity, high-heat exhaust leak directed at the brake master cylinder and vacuum assembly. This "heat soak" event compromised the engine-mounted mechanical Brake Vacuum Pump (Recall N192268490 / NHTSA 19V645) and destroyed the internal seals of the Master Cylinder (Special Coverage 15143 / NHTSA 11V599), resulting in total depletion of vacuum assist and hydraulic pressure. Simultaneously, the "shotgun blast" and violent swerve were caused by structural failure consistent with Rear Brake Caliper Bolt Fracture (GM 13247 / NHTSA 13V342) and Rear Axle Shaft/Pin Fracture (GM 14892 / NHTSA 14V819). This mechanical collapse caused the caliper to strike the wheel, resulting in a catastrophic rim blowout and immediate loss of tire pressure. Despite the vehicle having aftermarket long-tube headers installed, the root cause of the heat-related brake failure was the failure of the OEM GM manifold fasteners which snapped off in the cylinder head, as documented in the aforementioned bulletins. ... manifold fasteners suffering a metallurgical shear, a universal defect documented under GM Bulletins 19-NA-218, PIP5075, and PIP4701. This shear created a high-velocity, high-heat exhaust leak directed at the brake master cylinder and vacuum assembly. This "heat soak" event compromised the engine-mounted mechanical Brake Vacuum Pump (Recall N192268490 / NHTSA 19V645) and destroyed the internal seals of the Master Cylinder (Special Coverage 15143 / NHTSA 11V599), resulting in total depletion of vacuum assist and hydraulic pressure. Simultaneously, the "shotgun blast" and violent swerve were caused by structural failure consistent with Rear Brake Caliper Bolt Fracture (GM 13247 / NHTSA 13V342) and Rear Axle Shaft/Pin Fracture (GM 14892 / NHTSA 14V819). This mechanical collapse caused the caliper to strike the wheel, resulting in a catastrophic rim blowout and immediate loss of tire pressure. Despite the vehicle having aftermarket long-tube headers installed, the root cause of the heat-related brake failure was the failure of the OEM GM manifold fasteners which snapped off in the cylinder head, as documented in the aforementioned bulletins. After multiple attempts to fold the third-row power seat in my 2022 Ford Explorer, the seat mechanism failed completely. The seat will no longer lock securely in either the upright or folded position. The seat frame appears to have failed and the seat constantly springs back and will not stay in place. The issue was inspected and diagnosed by a Ford dealership, which confirmed that the third-row seat frame had failed and would require replacement of the seat frame assembly. The dealership quoted approximately $3,622 to repair the issue. Because the seat will not lock securely in the upright or folded position, this may present a safety concern for passengers using the third-row seating area. The radio head unit randomly reboots. This occurs usually when the car is initially started, and you are unable to utilize the backup camera or the camera freezes. Rear cross traffic breaking also fails and can cause an accident. I have also had issues with the powertrain, where the vehicle refuses to start, with a red lobster error message saying Service Electrical System.

... me after checking all fluids - they were fine. I trailered the car to my mechanic and after diagnosing it, he stated the #1 cylinder had no compression. I brought it home and checked online to find out that this seemed to be a common occurrence with the Sportage. To double check, I took the car to a second mechanic to get the same results except he provided more detail. Compression on #1 Cylinder 0, #2 was 140, #3 145, #4 140. He stated the compression should be above 175. My safety issue was I could have been stranded on I49 and if this had happened at night, I may not have been able to get it home. I took the vehicle to Crain Kia, Fayetteville, AR. when the first Check Engine Light came on and they stated it was shown as low engine oil even though it had oil in the engine and that it showed low compression on Cylinder 4. I now have a vehicle with 106,048 miles that without major engine repairs is worthless to me.Read more

This car is a very well-maintained, low-use vehicle with regular oil changes and routine maintenance performed on schedule. Early in this drive, I noticed an unusual hissing sound as I approached a stop. When I tried to brake, I realized my brakes didn't seem to be responding. I pressed the brake pedal harder and realized that with 50-100 pounds of pressure (guess), I could slow the car down.
... As I came to a stop, the engine died. I restarted the car, and it died again immediately. After the following restart, I realized I could manually keep the RPM up in neutral while at stops, and press with significant force to get the car to stop. Had I not been comfortable two foot driving or understood the issue, this surely would have resulted in a crash of some sort. The car indicated a check engine light, loss of traction control, a flashing BRAKE light, and a flashing cruise control light. I took the car immediately to a local shop where they identified the problem as a vacuum leak at the brake booster. The plastic piece that the hose clamps onto had cracked off, leading to this problem. This make and model does not seem to have a check valve downstream of this component.Read more

Either Fuel injector not properly installed or issue with fuel pump. The problem has not been confirmed Hyundai has informed me it will take 5/6 weeks to determine if an inspection is warranted. -Thursday February 19 We got the vehicle to the closest Hyundai dealer Metro West in Framingham Massachusetts on Thursday February 19 - I explained to the service representative the history (recall and repair in 2017) and a request to investigate recurrence of the recall defect as well as to prepare an estimate for repair. I never received a phone back so I called them on Wednesday February 27 and was informed that the vehicle estimate and inspection was in progress. At that point I contacted Hyundai corporate offices and was given a case number. On March 3, I called again and completed the preliminary investigation report - i was informed they would get back to me in approximately 48 hours. - On 3/9/26 after a phone conversation I was emailed a request to inspect my vehicle authorization form and another form (11 questions that had been answer numerous time by phone already) both were completed and submitted the following day 3/10/26. Dealership now threatening storage fees - Because the vehicle experienced failure of a component that was previously repaired under a federal safety recall, I am formally requesting that Hyundai preserve all evidence relating to the hood latch assembly and related components for purposes of regulatory review and potential insurance subrogation.
