The Keyless Vehicle Module security system on my 2023 Range Rover Sport deactivated both of my smart keys simultaneously without any warning or alert. The vehicle locked itself while both keys were inside and completely disabled them both at the same time, leaving my vehicle totally inoperable. I had no way to start or operate my vehicle. There was no warning light, no alert on the dashboard befor
... e this happened, and no information provided at purchase that this could ever occur. The only message displayed on the dashboard after the fact was Refer to Handbook Smart Key Deactivated. I am a single father with dependents and this is my only vehicle. I was left completely stranded at my home with no transportation for multiple days. This is a known and documented design defect affecting all Jaguar Land Rover vehicles manufactured from 2022 onwards. The vehicle has been inspected by Land Rover Alexandria dealer located at [XXX] . The dealer confirmed the issue and quoted me $3,000 to replace the Keyless Vehicle Module hardware. After I filed a BBB complaint and contacted Land Rover corporate the price was reduced to $640. My vehicle is a 2023 model and is still under full factory warranty. Land Rover corporate has an open case on this matter under case number XXX No other vehicle manufacturer allows their own security system to completely strand the owner with no warning and no safeguard. This is a serious safety defect because it can strand drivers in dangerous locations at any time without notice INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Recall/Loss of Power I am filing a safety complaint regarding a 2023 Chevy Equinox. The vehicle began losing engine power and will not go over 40 mph. My vehicle is a GMC Sierra 1500 Duramax diesel. The vehicle experienced a failure of the DEF (Diesel Exhaust Fluid) system, which was diagnosed by the dealership as a failure of the DEF tank heater. The heater is integrated into the tank assembly, requiring full replacement of the DEF tank. As a result of this failure, the vehicle became unusable and has been out of service since March 3, 2026. Th
... e dealership confirmed that the required part is on national backorder with no confirmed availability date. This condition creates a significant reliability and potential safety concern. When the DEF system fails, the vehicle can enter a reduced power or speed-limited mode, which may occur during normal driving and could create unsafe conditions in traffic. Additionally, there appears to be a broader pattern of similar failures affecting other vehicles of the same type, combined with extended delays in obtaining replacement parts. The lack of parts availability results in extended periods where the vehicle cannot be safely or reliably operated.Read more

Upon inspection at my local Toyota dealership, the UA80F 8-speed transmission on my 2021 Toyota Highlander has a known whining noise attributed to a failed transmission pump. The whining noise occurs when the car accelerates from 0 to around 50 miles per hour. When it gets to highway speeds, there is no noise. Anyone in the car can be put at risk if the transmission stops functioning on the roadwa
... ys, and we're left stranded or we're suddenly blocking traffic. This issue is a known issue according to T-SB-0008-21, and there are many people online who seemed to have encountered this issue even beyond the warranty limits of 5 years/60,000 miles. Even my local AAMCO tells me that this is a dreaded and known issue of past customers. This issue first appeared around the 60,000 miles of distance traveled. There is no warning for this problem. You just have to be fortunate enough to discover the noise before the warranty deadlines. I discovered the noise at around 58,000 miles around June 2025, but I didn't know it was an issue back then. Had I known about this, I would've still qualified for the warranty. This noise has been reproduced and confirmed by my local Toyota dealership, and they're quoting me $11,506.53 to fix this issue. Other 3rd party transmission shops are quoting me ~$6,000. I'm frankly frustrated that Toyota did not issue a recall for this known issue that I did not cause. I wanted to drive this car for life, but I'm unsure what to do now. Either pay an exorbitant amount of money or sell it.Read more

The transmission began making a whining noise at about 70,000 miles. It gradually became more noticeable. We had recently completed a thorough service, including transmission fluid exchange at 68,538 miles at a Toyota dealer. In March 2026 we took it to a Toyota dealer at 75,196 miles to report the noise and the dealer said the noise was coming from the transaxle and advised replacing the transaxl

While driving on the highway in light traffic at approximately 70 mph and changing lanes, the side airbags in my vehicle suddenly deployed without warning. There was no collision, no contact with another vehicle, no road debris, and no apparent external cause for deployment. I did not strike anything before, during, or after the deployment. There were no vehicles within several car lengths of my v
... ehicle at the time. Highway Patrol was dispatched to the scene. Officer TJ Carter with Guilford County responded. Upon inspection of the vehicle at the scene, we found no evidence of impact or contact with another vehicle, object, or debris. The vehicle was covered in yellow pollen at the time, and even light contact would have left visible marks or disturbance in the pollen. After examining the vehicle, no signs of contact were identified. The vehicle does have unrelated prior body damage from over a year ago on one side, but that damage predates this incident and was not caused by this event. The responding officer can confirm there was no collision or new impact associated with the airbag deployment. There was no accident associated with this airbag deployment. The airbags deployed while I was traveling at highway speed, which caused a sudden and dangerous driving event and required me to quickly maneuver to the side of the roadway. This created a serious safety risk to me and to surrounding traffic. Prior to this incident, there were no warning lights, safety alerts, or service notifications indicating an issue with the airbag system or vehicle safety systems. However, a notification did appear in the My Chevrolet app at or around the time of the incident. The vehicle has not yet been fully inspected following the incident. A safety claim has been submitted to General Motors. I also have video footage of the Highway Patrol Officer cutting down the airbags on-site but cannot add that below so I have added a screenshot.Read more

NHTSA Complaint Description — This complaint concerns two unrepaired safety recalls (23V591 and 25V375) on a 2021 Harley-Davidson Low Rider S (FXLRS) Softail motorcycle that were never remedied prior to a fatal crash on April 12, 2023. Recall 23V591 (issued August 2023) involves a fastener that may fracture, allowing the rear shock absorber adjuster to contact the rear tire, creating a groove a
... nd causing sudden loss of tire pressure — increasing crash risk. Recall 25V375 (issued June 2025) involves the rear shock pre-load adjuster mounting tab, which may fracture under normal use, with the same potential tire contact and pressure loss hazard. Neither recall was repaired prior to the fatal crash. The motorcycle was impounded following the crash and subsequently transferred before any inspection for recall-related defects could be conducted. It is unknown whether either defect was present at the time of the crash, as no inspection for recall compliance was performed post-crash. Safety was placed at severe risk — the operator died. The vehicle was not inspected by the manufacturer or dealer for recall defects after the crash. No dealer service was ever performed to remedy either recall. The vehicle is no longer available for inspection.Read more

Incident: While driving at highway speeds (65–75 mph), the vehicle issues imminent-shutdown warnings, the reduced-power (‘turtle’) light and check engine light illuminate, and the vehicle loses motive power. The driver must pull over to the freeway shoulder immediately and restart the vehicle. The condition is intermittent, occurs after cold starts, and sometimes with cruise control engaged.
... Safety Risk: Sudden loss of motive power at 65–75 mph without warning creates extreme crash risk. This is the exact risk stated in the recall notice. The software-only remedy appears insufficient. FCA’s own Warranty Extension Campaign XE1 — covering transmission replacement at unlimited years/unlimited mileage — suggests FCA is aware the software fix may not be adequate. There has been nothing sent out to owners to warn them of this issue. In the event of a fire or accident this could delay or prevent a passenger from being able to exit the vehicle. The problem has not yet been fully diagnosed by a dealer due to diagnostic costs, but the symptoms are consistent with the original issue that led to the recall repair. This condition could pose a safety risk because the driver may receive inaccurate overheating warnings or may not be alerted if a real overheating condition occurs. This is the same problem ford has experienced with other explorer models and there is an activr recall for those models. My car is a 2014 model and is experiencing the same problems.
