I am reporting a serious safety defect involving my 2013 Hyundai Santa Fe Sport equipped with a Theta II engine While driving and exiting the freeway with my wife as a passenger, the engine suddenly failed after knocking symptoms and completely seized/locked while the vehicle was still in motion. The vehicle immediately lost motive power and came to a stop, creating an immediate and dangerous tra
... ffic safety hazard. Had this engine seizure occurred moments earlier while traveling at freeway speed, it could have caused a catastrophic accident involving serious injury or death. The Hyundai dealership diagnosed the vehicle with rod bearing failure, which is the known defect associated with Hyundai Theta II engine failures. My VIN falls under the Theta II extended engine warranty program, and the dealership confirmed eligibility and accepted the vehicle for inspection under that coverage. Despite this, Hyundai denied the repair claim, first alleging maintenance-related issues and later stating the vehicle was out of warranty. I maintain that the vehicle was properly maintained with oil changes performed approximately every 6,000/8,000 miles. Some calendar-time gaps occurred because the vehicle was stored and driven very little during my recovery from a personal injury surgery and recovery, but the vehicle was maintained on mileage intervals. This engine defect resulted in sudden engine seizure and total loss of motive power while driving, creating a serious safety risk to vehicle occupants and surrounding motorists. I am reporting this defect so it can be documented and investigated.Read more

Re: Vehicle Safety Defect Complaint – 2023 Chevrolet Blazer To Whom It May Concern, I am submitting a formal safety defect complaint regarding my 2023 Chevrolet Blazer. This vehicle has exhibited multiple system failures that present a serious risk to driver safety and public roadway safety. The vehicle was purchased new on July 20, 2023. Beginning on April 21, 2025, at approximately 27,610 mile
... s, the vehicle developed a progressive defect initially presenting as oil-related issues. Despite multiple repair attempts, the condition worsened and required a full engine replacement in January 2026. Following the engine replacement, the vehicle continued to experience critical safety failures, including engine stalling, reduced engine power warnings, and electronic stability control (ESC) alerts. On April 26, 2026, the vehicle experienced a driver assistance system malfunction in which it attempted to steer off the roadway. I was forced to manually disable the system to maintain control of the vehicle. On April 27, 2026, the vehicle exhibited a fuel system failure, including fuel leakage and fuel odor, along with multiple simultaneous warning indicators: • Reduced Engine Power • Service ESC • Steering Assist Reduced / Drive with Care • Power assist driver system disabled This combination of failures presents both a fire hazard and a loss-of-control risk. Additionally, I was informed by the dealership that a high-pressure fuel line, a one-time-use component, was not replaced during the engine installation. This raises concerns regarding improper repair and continued unsafe operation of the vehicle. The vehicle has undergone multiple repair attempts and a major powertrain replacement, yet continues to present serious safety concerns. Due to these ongoing issues, I do not feel safe operating this vehicle. I respectfully request that NHTSA review this matter for potential safety defects that may impact other vehicles of the same make and model.Read more

Knudtsen fixed that issue. Low battery voltage light did not come on however battery was older so had battery installed at park to ensure the issue was not due to the battery. The oil was fresh and showed no issues, changed 2 weeks before fresh oil no issues found during service. Computer was hooked up and it would try to start, however, it would not know it was already running and try to start again on it's own...the codes that came up were the PO101 code came up for Mass Air Flow issue...and then the codes would go in a wild sequence of fail.....mechanic checked for loose connections around air filter, took air filter out and it was clean no issues, checked pipe no visible issues. We walked away and walked around the park to give it some time to sit and then we tried it again and it tried and sputtered out and died again would not stay idling. Then all of a sudden we heard it try to start itself without us trying to start it and it clanked. We hauled it to Knudtsen for a diagnosis. While driving in traffic, my 2023 Toyota Mirai suffered a sudden and total loss of motive power. The vehicle became completely unresponsive while in 'Drive', presenting a severe safety hazard. The dashboard displayed multiple cascading warnings, including 'FCV System Malfunction', 'Pre-Collision System Malfunction', and 'PKSB Malfunction'. The vehicle had to be towed to Dublin Toyota (Repair Order
... #653103). The dealership retrieved critical diagnostic code P1DC500 (FC AIR PRESSURE TOO HIGH) and verified the vehicle would not go into 'Ready' mode. The dealer opened Toyota TAS Case #TA26079027. However, the dealership performed no mechanical repairs to address the root cause of the sudden power loss. They simply cleared the safety code, test drove it, and explicitly wrote on the final invoice: 'ADVICE CUSTOMER TO DRIVE VEHICLE UNTIL PROBLEM OCCUR'. Furthermore, they refused to document my stated concerns that the vehicle is unsafe to drive. Returning a vehicle to a customer with an unresolved, life-threatening defect that causes total loss of power at speed is incredibly dangerous.Read more

Our 2019 traverse transmission around 130,000 miles started shuddering when slowly accelerating or driving. Then I found a video about Shudder Fix and I added one of those packets to the transmission and it stopped the shudder but it now seems to be trying to creep back. This is a known issue with most people not being able to afford the $9000+ repair costs. The Shift to park issue is another issue that shouldn't be happening and the dealership will charge an arm and a leg for the repairs on this as well. Pressing the gear shift button about 5 to 10 times after shifting to park is a short term solution because that issue is tied to a spring action underneath the shifter to tell the car I'm in park. I found a bypass piece that is required to resolve the issue but I shouldn't have to do that to get this vehicle to work properly. Chevy needs to do a recall on this or they run the risk of having a bunch of them on the market where we all got rid of them and won't buy anymore Chevrolet vehicles. This is the second newer model vehicle I had from them. The first one was an Impala and the dealership didn't know why the screen would go black and cause the car to start hesitating horribly. I am reporting a serious safety issue with my 2021 Chevrolet Trailblazer related to transmission failure. The vehicle has approximately 83,000 miles. While driving, the vehicle experiences a sudden loss of power when attempting to accelerate. In some instances, the vehicle has stalled or shut off while I was actively driving, creating a dangerous situation, especially when merging into traffic or
... driving through intersections. The issue is intermittent but has become more frequent over time. The vehicle does not respond properly when the accelerator is pressed, and there is a noticeable delay or complete lack of power. At times, the vehicle will not move as expected, which poses a significant safety risk. Diagnostic codes associated with this issue include P2714 and P0700, which indicate a transmission-related malfunction. I have been advised that the vehicle requires a full transmission replacement. This failure occurred at relatively low mileage for a major powertrain component, and I am concerned this may indicate a broader defect. The stalling and loss of power create a hazardous driving condition that could result in an accident. I am submitting this complaint to report a potential safety defect and to request further investigation into this issue.Read more

I experienced multiple unintended gear changes while stopped in traffic in a 2026 BMW X2 xDrive28i (~11,800 miles, rental vehicle, prior usage unknown). The vehicle uses a small electronic toggle gear selector located immediately adjacent to the cupholders. Incident 1: While stopped with my foot on the brake, the engine shut off due to auto start/stop. When I released the brake, the engine restar
... ted but the vehicle did not move forward and instead revved slightly. I immediately pulled the selector back and the vehicle resumed normal forward movement. I did not intentionally shift gears and believe the vehicle had unintentionally shifted into Neutral. Incident 2: While stopped with my foot on the brake, I reached for a drink in the cupholder. During this normal action, the gear selector was likely contacted unintentionally. The vehicle shifted into Reverse without my intent, confirmed by the backup camera activating. The vehicle did not move because I remained on the brake and recognized the issue immediately. Afterward, I tested this interaction in a controlled environment in my driveway. While stationary with my foot on the brake, repeatedly reaching for a cup caused the vehicle to shift into Neutral or Reverse multiple times without intentional input or clear tactile feedback. The selector’s location next to the cupholder and its sensitivity allow incidental contact during normal use. As a left-handed driver reaching across the console (with a wristwatch and jacket), contact may occur without awareness. This creates risk of unintended gear selection, unexpected movement, or failure to move when expected.Read more

On 04/28/2026, when diagnosing a reoccurring check engine light on the vehicle at a Chevy Certified Service, the mechanic "Borescoped cylinder 4 and found significant scuffs on the cylinder wall which along with the misfires indicate a cracked piston. The big picture appears to be the piston cracked which caused excessive oil consumption and misfires. The excess oil and unburned fuel overloaded th
... e converter causing it to fail. The engine and converter will need to be replaced." In 2017, a recall was issued by GM to mitigate Stochastic Pre-Ignition (SPI) Causing Cracked Piston (17019). Between the initial purchase of the vehicle (11/2016) and performed service required by this recall (03/10/2017), there were approximately 5000 miles driven. No engine diagnosis was performed or recommended. After the 17019 was addressed, the engine misfires continued along with reoccurring CEL. C.S.P. N182195660 Predictive Monitoring for Stochastic Pre-ignition (SPI) released in 2019 states the 1.5 L LFV engines in certain 2016 and 2017 model year Chevrolet Malibu vehicles may have a preignition condition that requires immediate repair. This condition may cause the engine to perform poorly, misfire, run rough, and/or excessively consume oil. It may also illuminate the vehicle’s service engine light. Over time, if not repaired, the condition can damage the vehicle’s pistons and engine. The VIN for this Malibu was not attached to this notice despite it having SPI recall history. On 06/2023 (after the N182195660 program ended), at 80k miles a diagnosis revealed a potential cracked cylinder with no mention of the pre-ignition condition that could cause the crack, and the only way to fix is to replace the entire engine (as per dealer recomendation). Since vehicle was out of warranty, the repair est. was nearly $10k. I firmly believe my 2016 Malibu has suffered the consequences of poor mismanagement on this recall, and am demanding mitigation from GM to fix this issue.Read more

I am experiencing multiple unresolved defects with my 2026 Chevrolet Equinox affecting drivability, braking behavior, and vehicle structure/fit and finish. The primary issue is a persistent vibration, shimmy, or wobble during moderate acceleration from a stop. This condition is felt through the steering wheel, seat, and floor, and occurs consistently during low‑speed launch and normal accelerati

In addition, the vehicle produces a snapping or grunting noise during hard braking, as well as rough and loud movement noises from underneath the vehicle during normal operation, which raises concerns about braking and drivetrain integrity. There are also body and interior fitment defects. The front passenger door does not sit flush with the rear door panel and appears slightly open when closed. The dealership service manager stated that correcting this condition would require removing and pulling the rear door panel outward so both doors align. The glove box door also does not sit flush when closed, and the service manager advised that replacement might make the condition worse. These concerns were reported to an authorized Chevrolet dealer. The dealer confirmed that General Motors has acknowledged the acceleration vibration condition in a Preliminary Information service bulletin (PIT6466) affecting 2025–2026 Equinox vehicles and advised that GM engineering is investigating the issue and that no repair is currently available. The combination of drivability issues, abnormal noises during braking, underbody movement sensations, and body misalignment affects the use, value, and safety perception of the vehicle. The vehicle has very low mileage, and several of these issues have been present since early ownership. I am submitting this complaint to document these conditions and request that NHTSA monitor potential powertrain, braking, suspension, and vehicle structure concerns if a defect trend exists. All of these defects have been acknowledged by the Dealership Service DeptRead more

The rear of the car would begin to slip during turns and when brakes were applied. When I had the issue checked out, my mechanic said that the rear subframe has severe corrosion, rust and holes. The subframe perforation is causing instability in the rear wheels, which is a safety issue. I took the car to Mercedes for an inspection and they confirmed the holes in the sub frame, with the inspector st

This appears to be a manufacturing defect given recalls for the same issue in other models of the 2015 c300. The car has cut off three time while at stoplights. I can get it restarted but the dealership cannot duplicate it even though there are codes in the system. Also the automatic braking will randomly stop when it shouldn’t and it is a hard braking.
