While driving approximately 10–15 mph approaching a green light, a truck in front of me suddenly slammed on its brakes. As soon as I saw the brake lights I immediately applied my brakes before any system alert activated. I stopped within approximately 3–5 feet of the vehicle in front of me. At the same time, a truck behind me was approaching quickly. To avoid being rear-ended I pressed the acc
... elerator to move out of the way. However my vehicle would not respond. Despite pressing the gas pedal the truck remained in a braking or non-responsive state and would not accelerate. I attempted to switch between the brake and gas pedal but the vehicle still would not move. This condition lasted approximately 30–60 seconds leaving me stopped in active traffic and unable to take evasive action. This created a serious safety hazard as I was at high risk of being rear-ended due to the vehicle’s failure to respond to acceleration input. The delay was critical and could have resulted in a collision. There were no warning lights or messages prior to or during the incident. The issue occurred suddenly without any indication. This appears to be related to the Pre-Collision System (PCS) and/or the vehicle’s throttle/acceleration system. The vehicle is available for inspection upon request. The issue has not yet been reproduced or confirmed by a dealer or service center and the vehicle has not yet been inspected by the manufacturer or other parties BUT I did call yesterday and haven’t received a response so I will be taking it to Gulf Coast Toyota today [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Infotainment Center: I am submitting this complaint regarding my 2021 Honda Accord Sport and the ongoing issues I’ve experienced with its infotainment system. This problem has been persistent, unpredictable, and increasingly concerning from both a usability and safety standpoint. Since purchasing the vehicle, the infotainment center has repeatedly malfunctioned without any warning. The screen
... will freeze, switch from setting to setting, or become unresponsive to touch input. At times, it will randomly reboot while I am driving. These issues occur regardless of whether I am using navigation, Bluetooth audio, or even when the system is idle. This also affects the ADAS, periodically, warnings will be displayed, and then not show up. What makes this especially troubling is how it affects my ability to safely operate the vehicle. For example, when the system freezes, I lose access to navigation directions mid-route, which forces me to either drive without guidance or attempt to troubleshoot the issue while on the road. Similarly, when using hands-free calling or music controls, the glitches can be distracting and require repeated attempts to regain control, taking my attention away from driving. I rely on my vehicle daily, and this ongoing issue has caused a great deal of frustration and concern. A modern vehicle’s infotainment system is not just a convenience—it is integrated into essential driving functions. When it fails, it directly impacts driver awareness and safety. I am requesting that this issue be investigated further, as it appears to be a systemic problem rather than an isolated defect. I hope that by reporting this, it may help lead to a resolution for myself and other drivers experiencing similar problems.Read more

The failed component is the engine assembly in my 2017 Hyundai Sonata SE with the 2.4L Theta II GDI engine. The failure involves excessive oil consumption and internal engine failure, specifically suspected piston ring deterioration allowing oil to pass into the combustion chambers. The vehicle is available for inspection upon request. Safety was put at risk because the engine was losing oil rapi
... dly. If the oil level drops too low, the engine can seize, stall, or lose power while driving. Sudden engine failure at road or highway speeds could increase the risk of a crash. The issue required constant oil monitoring and made the vehicle unsafe to rely on. The problem was confirmed by both an independent repair shop and a Hyundai dealership. At about 99,813 miles, an independent shop diagnosed oil burning from the tailpipe. I then paid for major repairs, including cylinder head and valve work, but the oil consumption continued. A later independent inspection found internal oil consumption consistent with failed piston rings and oil bypassing into the combustion chambers. The Hyundai dealership performed Hyundai’s oil consumption testing process, including combustion chamber cleaning and rechecking oil level. The vehicle failed and engine replacement was approved. The dealer quoted $11,400-$12,000 for replacement. The vehicle has been inspected by an independent repair facility, a Hyundai dealership, and reviewed by Hyundai Motor America Customer Care under case number 42489659. Hyundai denied assistance, stating the vehicle is outside warranty due to time and mileage. Symptoms first appeared around 99,813 miles and included excessive oil consumption, burning oil from the tailpipe, reduced power/dragging, abnormal rattle/noise during low-speed acceleration, and poor fuel economy. After repairs, the oil level still dropped below minimum quickly. I am filing this complaint because this condition may cause sudden engine damage, stalling, or loss of power while driving.Read more

From the time of purchase we have had an issue with the fuse blowing when we retracted the kitchen slide. We have had our Brinkley 5th wheel in for service at approved Brinkley locations for this issue multiple times with the issue reproduced. Service has occurred for this issue from March 2024 until April 2026. In April 2024 when we picked up the trailer we were told by Mike Thompson dealershi
... p where we purchased the RV, that this is common and we should invest in fuses. Despite multiple trips for service the issue has yet to be resolved. We have ultimately been told to use resettable fuses, however we are extremely worried about a potential fire hazard. There is an overcurrent that occurs with the slide retraction. Although a resettable fuse may resolve the symptom it does not treat the cause. When a 30A fuse has an audible pop, you know that there are multiples of 30A hitting the fuse resulting in 100+amps of current coursing through the wiring, stressing the motor and the printed circuit board until that fuse opens. Resettable fuses are good for approximately 100 cycles before they are no longer viable. If these fuses short, it is unknown how long the PCB trace will be able to handle the current prior to it burning and becoming a fuse itself. This would ruin the main fuse panel board and possibly the motor while potentially causing a fire.Read more

While driving at approximately 45 mph, the steering system suddenly failed without any warning. The steering wheel became extremely stiff and nearly impossible to turn, resulting in a sudden loss of control of the vehicle. I was unable to safely steer, especially when attempting to turn right, which nearly caused a crash. There were no warning lights, alerts, or prior symptoms before this failure.
... The issue occurred abruptly and without any indication, creating a serious and unexpected safety hazard. Due to the severity of the problem, the vehicle had to be towed and is unsafe to drive. This appears to be a failure of the electronic power steering (EPS) system. A sudden loss of steering assist at driving speed presents a significant risk of injury or death to the driver, passengers, and others on the road. This is a critical safety issue that should be investigated immediately.Read more

The power steering system in our 2024 Mazda CX-90 intermittently "sticks" while steering, requiring noticeably increased effort to turn the wheel before it suddenly releases. The symptom matches the defect described in NHTSA Recall 24V-022 (sticky steering due to worm gear friction in the power steering assembly) and the ongoing NHTSA Recall Query into vehicles where the 24V-022 remedy has failed
... to resolve the issue. Component: Power steering gear / worm gear assembly. The vehicle is available for inspection upon request. Safety risk: The intermittent increase in steering effort followed by sudden release creates a real risk of overcorrection, particularly during lane changes, highway driving, and turns from a stop. The driver cannot predict when the wheel will resist or release, which compromises the ability to maintain a consistent lane position and respond to road conditions. Dealer/service confirmation: UNKNOWN at this time — we are scheduling a dealer inspection. We will update if the dealer reproduces or documents the condition. Manufacturer/police/insurance inspection: None to date. No crash has occurred. Warning lamps or messages: None. No dashboard warnings, no error codes, and no messages have appeared before or during the sticking events. The condition is purely tactile through the steering wheel and is intermittent. The vehicle's recall status shows no active open recalls, suggesting any prior remedy under 24V-022 was already performed, yet the sticky steering symptom is still present — consistent with the complaints driving the current NHTSA investigation.Read more

At approximately 43,044 miles, my 2021 Audi A7 55 TFSI e quattro (PHEV) was diagnosed with a failed N632 coolant changeover valve — a component of the PHEV thermal management heating circuit. The vehicle has a complete, documented service history with no maintenance gaps. The N632 is an electronically controlled valve that routes coolant between the combustion and hybrid/electric circuits and is
... essential for cabin heating in EV mode, battery pre-conditioning, and engine warm-up. Failure of this valve can result in loss of cabin heat, coolant leaks into the engine bay, and potential cascade of electrical faults if coolant migrates into the wiring harness. The repair estimate is $1,700. The factory warranty, Audi Care plan, and third-party extended service contract (United Car Care Protection Plus) have all denied coverage. This failure on a low-mileage PHEV with full service history suggests a premature component defect in Audi's PHEV thermal management system. I am requesting NHTSA investigate whether this constitutes a pattern of failure across 2021 Audi PHEV vehicles.Read more

All components are available for inspection upon request. The vehicle was taken to an authorized Chevrolet dealership for diagnosis. The dealer
... acknowledged the emissions codes but stated that unrelated components would need to be repaired at owner expense before the emissions system would be addressed, despite these codes falling within the federally mandated 8-year/80,000-mile emissions warranty. The fault codes were confirmed by the dealership. No safety incident has occurred to date, however degraded NOx and EGR function can result in increased harmful emissions output and potential engine performance issues. Warning lamps appeared without prior incident or symptom. The manufacturer has not offered to cover the emissions components under the federal emissions warranty without the owner first paying for additional repairs that may or may not be causally related to the emissions system failure. When the dealer was directly asked, "Are you saying the emissions system itself is functioning correctly, and something else caused these codes to appear?" — the dealer did not provide an answer. This question was asked in order to clarify whether the emissions components themselves are degraded or failed, which would confirm warranty coverage eligibility. While entering a roundabout at a slow speed (~5MPH), the electronic power steering on my wife's car failed, almost resulting in an accident with another vehicle, as it suddenly became very difficult to steer. The vehicle's power steering warning indicator turned on, and then the following warning messages appeared on the dashboard: Check Forward/Side Safety system, Check Lane Safety system, Check
... Lane Following Assist system, and Check Inattentive Driving Warning system. After turning into a nearby parking lot and parking, the vehicle was turned off and then back on, and all warning messages and the power steering warning indicator turned off. My wife was approximately 1 mile from home and decided to try to drive home. After getting back on the road, the same issue occurred again with all indicators and warnings repopulating on the dashboard this time with the vehicle traveling at approximately 35 MPH as she crossed a major intersection. My wife was able to get the car home safely without power steering, and after turning the car off and back on again, all warning messages and indicators turned off again. The next day, 4/30/2026, we contacted our local Hyundai dealer and Hyundai roadside assistance. The vehicle was towed to the dealer, where it is currently being serviced for this issue. We have not yet received an update on what caused the issue, but we felt it was important to report it to NHTSA because, if it had happened at a higher speed, it could have resulted in a serious accident and possibly serious injury or death of my pregnant wife and child, who was with her in the car at the time. I can provide any and all updates and documentation I receive from the dealer upon request, and the vehicle can possibly be made available for inspection upon request.Read more

I bought my car in 2024 and while it was through a private party i have had transmission issues since the day i bought it. It surges very frequently and has since the day I bought it. While driving my 2021 Honda CR-V at highway speeds, the steering intermittently binds and becomes difficult to move, creating a dangerous “sticky steering” condition. The steering will momentarily resist normal input, and when I apply additional force to correct it, the wheel suddenly releases, causing the vehicle to jerk or swerve within the lane. This makes it difficult to maintain smooth, c
... ontrolled steering and creates a serious safety concern, especially at higher speeds where precise steering is critical. The condition is unpredictable, unnerving, and significantly affects my confidence in safely operating the vehicle. This defect creates an especially concerning hazard because I frequently transport my two young grandchildren in this vehicle. When the steering binds and then abruptly releases, it compromises my ability to maintain stable lane control and increases the risk of losing control or being involved in a collision. This is not merely an inconvenience—it is a clear safety issue that makes the vehicle feel unsafe to drive. The symptoms my vehicle is experiencing are identical to those described in Recall 24V-744 involving sticky steering in Honda vehicles. Although my VIN is not currently included in that recall, this same defect has been widely reported by other 2021 Honda CR-V owners. I understand the defective steering gearboxes were identified in vehicles produced beginning in February 2021, and my vehicle was manufactured in August 2021, which falls within that timeframe. Based on the identical symptoms and known defect history, I believe my vehicle is affected by the same steering gearbox issue and should be included in the investigation and recall population.Read more

Water entered tailgate through drainage hole in tailgate cap. The water then dripped on the tailgate switch causing it to short out. Safety Recall N232426400 Tailgate May Open Unexpectedly issued May 2024. I called the customer service number [XXX] to try and get reimbursed for the repairs and they told me there is no recall on my truck. While driving my Genesis G70 on the highway in rainy conditions, the vehicle repeatedly loses traction and hydroplanes at normal highway speeds. The car begins sliding across lanes with very little control. During a prior incident, driver-assist features (lane keeping assist) were active. Rain appeared to interfere with the sensors, and the system malfunctioned by steering incorrectly. The steeri
... ng wheel became difficult to control, and the vehicle attempted to steer into another lane while traction was already compromised. The vehicle hydroplaned across multiple lanes of the expressway, creating a high risk of collision. The system’s steering input did not match driver intent and made the situation more dangerous. The vehicle ultimately came to a stop near a wall but avoided impact. After this incident, I turned off the lane keeping assist feature. However, the problem continues. On a separate occasion, while entering the highway in rainy conditions, the vehicle immediately began sliding again, indicating an ongoing traction or stability issue even without driver-assist features active. This issue has occurred more than once and creates a serious safety risk to myself and others, as the vehicle becomes difficult to control and does not maintain proper traction in wet conditions.Read more
