My 2021 Toyota Highlander Hybrid has a braking issue identified in Toyota Service Bulletin T-SB-0059-23, Hybrid Regenerative Brake Transition Feeling Improvement. When going downhill and applying the brake, the vehicle lurches forward temporarily as the braking system transitions from regenerative to hydraulic braking. The first time my vehicle did this, it was very alarming and I had to take add
... itional measures to slow my vehicle as I was driving downhill. My vehicle frequently demonstrates this problem, however, I did not report it to my local Toyota dealer because I read in an online chat group that it was a characteristic of this vehicle. Although the TSB recommends a Reprogram of the Brake Actuator Assembly Control Module, the repair is only covered under the Toyota Basic Warranty which is in effect for 36 months or 36k miles, whichever occurs first. My vehicle is past the warranty limit, and because I did not report it to my dealer previously, this repair is not covered under the TSB or the Toyota Basic Warranty. Toyota Highlander Hybrid model years 2020-2022 are affected. The Technical Service Bulletin was issued August 29, 2023, so if a 2020 model Highlander Hybrid has this problem, 2020 models would have already passed the warranty limit by the time the TSB was issued and thus would not be covered. This is a safety risk as drivers can lose control of the vehicle when braking, particularly when going downhill. The Warranty should be extended, or this problem should be elevated to a RECALL so that older affected vehicles, or those with higher mileage can be covered under warranty. Drivers should not have to pay for a recognized problem that affects the safety of this vehicle and has been identified in a Technical Service Bulletin. This problem puts drivers at risk of losing control of the vehicle and the problem can be mediated with a software reprogram. T-SB-0059.23 should be elevated to a RECALL, or extended regardless of age or mileage.Read more

I am writing to formally request an investigation into a safety defect affecting the dashboard on my 2004 Lexus GX470. Over the past several months, I have observed significant cracking along the top of the instrument panel. I believe this defect poses a serious safety hazard: if the dashboard material fractures or delaminates, it could interfere with proper expansion and deployment of the front p
... assenger airbag in the event of a collision. Despite the severity of this condition, Lexus has only issued a “limited-service campaign” rather than a full safety recall. Under that campaign, Lexus offers repair assistance only to owners located within certain regions and only if they pay a service fee. I contacted Lexus Headquarters, and requested a complete, no-cost replacement of the defective dashboard assembly, and was declined without satisfactory justification. I consider this response inadequate because: 1.Safety Risk to Occupants oThe dashboard crack traverses the airbag deployment zone. oIf the material fails, the passenger airbag may not be deployed correctly or on time. 2.Insufficient Remedy oA limited-service campaign does not guarantee replacement or cover all affected vehicles. oOwners outside specified regions or who have previously paid for dashboard repairs receive no assistance. 3.Regulatory Precedent oFederal Motor Vehicle Safety Standard (FMVSS) No. 208 mandates reliable airbag deployment. oA comprehensive recall would ensure all affected vehicles receive robust, no-cost repairs. I respectfully request that NHTSA open an investigation under 49 U.S.C. § 30162(a) to determine whether this dashboard cracking constitutes a safety-related defect warranting a nationwide recall. I am prepared to provide photographs, service records, and any additional information needed.Read more

Since purchasing the vehicle on September 27, 2024, I have experienced a series of troubling issues that raise serious concerns about its design, manufacturing, and overall safety, including: Airbag system malfunction: My airbags were nonfunctional for over a month. Tire-related issues: Required full replacement of the initial set of tires. Persistent alignment problems Bluetooth audio pro

Defective paint: Blisters and bubbles in the rear quarter panel paint. Fuel module failure: Most concerning, the vehicle abruptly stopped on the road last week and required towing. I was later told the cause was a fuel module failure. Each of these issues is concerning on its own. Taken together, they represent a troubling pattern of unreliability and potential risk to my safety and that of others on the road. They’ve also resulted in a significant loss of my time. This vehicle has spent over a month at the dealership for service—astounding for a truck that’s only eight months old. The Radio emits static noise VERY Loud sometimes when the car is driven. It is so loud sometimes that my grandchild will scream and cry. Sometimes it is just a small static noise that doesn't get louder, but you never know. It seems to happen more when someone is in the passenger seat, so I thought it might have something to do with the weight sensor, but no.. it also does it when you are by yourself. And it is happening almost every single time we get in the car. I have been looking this up and a lot of people say they just unplug the fuse for the radio, but I shouldn't have to do that. If there are this many people having the same issue, they should do a recall on it. The noise is so loud and sudden sometimes that it shocks you and could potentially make someone have a wreck. There have been no warning messages on the screen and this first happened a few months ago. I have not had it inspected by anyone yet. Instrument Panel Cluster Failure: My instrument cluster failure has been an unrepairable issue for my 2019 Jeep Wrangler Sahara which I purchased from Marysville Jeep in Marysville, Washington back in 2021 or 2022. My vehicle is at the same dealership NOW as we speak and has been there since 23 April 2025. I have not had the use of my Jeep for the duration of this time. The time before that, they had my Jeep for about 2 weeks and did not repair the instrument cluster. My associate at the dealership name is Colin. He's constantly informing me that they ordered the cluster but they have not received it because my vehicle was made in Canada. I also saw online that a recall has been issued for certain 2018-2024 Jeep Wrangler vehicles, including the Sahara, due to potential instrument panel cluster failure. The recall number is 30B. I looked for this recall using my VIN on the NHTSA website, but it showed no recalls for my particular vehicle. However, I have this exact same problem with my Sahara. Colin informed me on 5 June 2025 that he has exhausted his last avenue to figure out why my Jeep still has a failed instrument panel that has not been repaired/replaced since 23 April 2025 and to basically come pick up my jeep or call someone else to help remedy this issue. Colin's phone number is (360)429-0384. He also gave me numerous dates that the new cluster was to arrive at the dealership, but to no avail. Also, in between the two times at the dealership, I took it to a repair shop who tried to order the cluster but could not get it, because the dealership is the only entity that can order the cluster. This correspondence comes to you in regard to Safety Recall Notice 24S79 / NHTSA Recall 24V954. I received a notice in February of this year from Lincoln regarding our vehicle (VIN [XXX] )- a 2021 Lincoln Corsair Plug-In Hybrid. The notice instructs us to cease charging the high voltage battery until further notice. The recall notification states that further charging the battery could result in a
... vehicle fire. The notice finally states that a resolution and parts will be available during the second quarter, which is now. Unfortunately, to date I have heard nothing from Lincoln. As we are both retired , my wife and I purchased a Plug-In Hybrid to help save money on gasoline. For the past three (going on four) months, we have had to pay for gasoline for every mile we have driven. Additionally each day that passes is another day less we have remaining on the original warranty. It would be greatly appreciated if you could respond to this request letting us know when this issue will be resolved. We also ask that Lincoln be obligated to extend a cash penalty to cover the cost of the last four months of gasoline purchases, and finally, to extend the manufacturer's warranty on the battery components to a lifetime warranty - in order for us to feel comfortable in the resolution offered to fix the issue. Incident date: May 20, 2025
The car has on 5+ occasions had the collision avoidance system shut down (photos of dash showing this are available and have been shared with the dealer). There were no weather incident at the time. Lane following........ same type of thing also shared with dealer. Most disturbing is on multiple occasions, with the speed control engaged driving on the interstate at 65 to 70 mph, the speed limit
... indicator on the dash changed to 40 (no speed limit change) and the car went into hard breaking mode until I disengaged the system. (also reported to dealer) Each time the car went into the shop, they "checked the systems" and found no faults and made no repair except on one occasion, they "flashed" the software. The car went into the dealer with 67,000 miles on June 3, 2025 with a promise that they would get to the bottom of this issue. The car was returned after a full "testing" and for good measure, one software update and the replacement of a forward facing camera lens. For each item submitted, they reported no history of any faults and found nothing wrong with the car. The car was returned [XXX] at [XXX]. On [XXX] going 60 mph in traffic, the speed limit indicator jumped to 80 mph and the car immediately and rapidly accelerated until I shut the system down. I took a photo of the dash showing the 80 MPH indication. There are NO locations in Virginia that have an 80 mph speed limit. Also, on several occasions, an alert comes up the the car has been in an accident..... when parked in my garage and a bogus date.
