* Recall Campaign Number : [018G, 019G] I. Statement of Complaint This complaint concerns a failure in a critical safety-related component that was previously addressed under a mandatory manufacturer recall, and the subsequent refusal by the authorized dealership to provide a second no-charge repair for the same repeated failure. II. Chronology of Events * Original Recall Repair: Approximately t
... hirteen (13) months ago, 8/30/2024 the vehicle was taken to Anaheim Hyundai to address a safety recall concerning the oil feed pipe. The prescribed remedy, a replacement of the oil feed pipe, was completed at no cost under the recall campaign. * Second Failure (Current Event): On or about 10/6/2025, while the vehicle’s engine was running, I observed smoke rising from the engine compartment. The vehicle was immediately taken to a local independent mechanic who diagnosed the issue as a burst oil feed pipe—the identical component subject to the prior recall. * Attempted Repair at Anaheim Hyundai (Denial): The vehicle was then towed back to Anaheim Hyundai. The service department acknowledged that the component failure was the same issue addressed in the previous recall. However, the dealership has now refused to perform the repair under the recall process, citing a specific 12-month policy set by Hyundai Motor America regarding the warranty on recall remedies, which has now expired. III. Request for Intervention The failure of the previously installed recall remedy (the oil feed pipe) exposes the vehicle to the original safety risk, including potential engine failure and fire hazards, and the manufacturer’s refusal to re-remedy the defect effectively negates the purpose of the mandatory recall. I request that NHTSA investigate this matter and compel Hyundai Motor America to: * Honor the original safety recall and provide a permanent, effective remedy at no charge to the consumer. * Clarify the consumer's right regarding the warranty on recall remedies, fails after installRead more

Our 2022 Ford F-150 lost power while trying to proceed into an intersection. I was turning right, exiting a parking lot onto a main road trying to go north. There was a good amount of time with no traffic so I pushed the gas to proceed with my right turn. When I pushed the gas, the truck responded bringing me out into the intersection however before I could get turned the gas pedal stopped resp
... onding so I was stuck straight out on the main road with traffic that was heading north coming straight toward my driver side of my truck. I almost got T-boned by multiple vehicles. Traffic slowed and had to stop to move around my truck while I was attempting to get the truck to respond. The truck responded after about 2 minutes of being stalled. We brought the truck into our local ford dealership again (had brought it in before for this same issue described below), they kept it for about a week to try to reproduce the issue again however were not successful again. The dealer said that the truck did not throw any codes when the stall happened and there were no warning lights or messages in the vehicle at the moment that it happened to me. They are now telling us that we need to pick up our truck which we truly do not feel safe in. This is a family vehicle and our daily driver that we drive around our 3 year old child in. It not only put my life in danger that day by stalling, but it put all of the others cars headed toward me - almost T-boning me - in danger. I had a previous instance of this same issue where my truck was non-responsive, but was not on a main road so it wasn't as scary. There were also multiple other times where it experienced the same issue, but did not fully lose power (just kind of lagged when the pedal was pushed). We brought my truck into a local ford dealership for these instances of the issue but they could not reproduce the issue, therefore did not fix it. There was a recall issued for this a couple months back, however currently no fix.Read more

My check engine light came on. Ford dealer diagnostics reveal coolant has leaked into the cylinder and ruined the engine. Now the engine needs to be replaced altogether. Summary of Issue: I am the owner of a Toyota Mirai that suffered a sudden loss of power due to contaminated hydrogen fuel. The vehicle was taken to Toyota of [XXX] for diagnosis and repair, where Toyota confirmed the presence of c
... ontaminated fuel. Toyota’s Stance: According to Toyota, the contamination issue was addressed by flushing the contaminated hydrogen from the system and refueling the vehicle. Toyota has stated that this process has restored power and that the company does not accept responsibility for any damage caused by the contamination. They have not disclosed any data on the type or level of contamination found, nor have they provided a written service report detailing inspection results or long-term corrective actions. Owner Concern: Independent research and fuel-cell engineering references indicate that contaminants such as sulfur compounds, carbon monoxide, or heavy metals can permanently degrade or “poison” the fuel-cell stack. Flushing alone cannot reverse such chemical or structural damage. As a result, there is a risk of future performance loss or total stack failure. Without transparency from Toyota regarding contaminant analysis or verification of system integrity, I cannot consider this vehicle safe or reliable to drive. Request for Resolution: I am requesting that Toyota: Provide complete written documentation of the contaminant analysis and testing performed on my vehicle. Disclose the long-term plan for ensuring the fuel-cell stack and related components have not been compromised. Offer either a full buyback/repurchase under applicable consumer protection laws or a complete fuel-cell stack replacement verified by engineering inspection. 1- ABS/Braking Issue The ABS system appears to be malfunctioning. On multiple occasions, typically once or twice a week, when I apply the brakes, the tires lock up and cause the vehicle to skid. This happens most often when descending hills or backing up on a sloped driveway. The wheels lock and slide until I release the brake pedal, posing a serious safety concern. 2- Severe Water Leak Interior
... Roof Area There is a significant water leak from the interior roof, specifically from the top center area near the rearview mirror, every time it rains. I’ve taken the vehicle to the dealer multiple times. They confirmed that all drain tubes are clear, all seals are functioning properly, and no water appears to be entering from the sliding moonroof. However, the issue persists. Water pours down like a waterfall through the vent (as shown in the attached photo), soaking the center console and creating an unsafe driving condition. I have also recorded videos showing the water intrusion. Living in Guam, where it rains frequently, this has become a major problem. 3- Malfunctioning Interior Ceiling Light The ceiling light on the driver's side (center dome light) turns on randomly and often remains on until manually switched off. The dealer has inspected this issue but reported no faults found. Despite this, the problem continues to occur intermittently. 4- Rear Camera Display Issue When shifting into reverse, the monitor occasionally turns completely blue, and the backup camera does not function. The blue screen remains until the vehicle is turned off and restarted. This happens sporadically and disrupts safe maneuvering.Read more

The first critical component was reported in my complaint tracking number 11459461 submitted on 04/03/2022. The component in that filing was the engine pan that supported the engine. He was not going to let me drive it home. Then he changed his mind and told me it was "at my discretion". I did drive it home (4.5 miles) with no problem. He told me he was going to try and find a new or used part to fix the issue. He made it clear that he was not hopeful of finding one. This is twice now that my life and the lives of any drivers on the road at the same time were placed in danger from excessive rusting problems. My 2018 Kia Rio S (VIN [XXX] ) suffered sudden engine failure at approximately 84 000 miles. I pulled over immediately when the oil light came on, but the engine began rattling and was later diagnosed by the dealer as a bearing/seizure failure requiring full engine replacement (estimate ≈ $15 865). This vehicle uses the 1.6 L Gamma II GDI engine, which is the same engine family already included
... in Kia’s “Engine Litigation II” settlement and KSDS (Knock Sensor Detection System) program for Forte, Soul, and other models. However, Kia claims the 2018 Rio is not covered by the extended warranty even though the failure mode is identical (rod-bearing wear and oil starvation). I never received any recall or settlement notice. The lack of notification and exclusion of the Rio model leave owners unprotected against a known safety-related defect that can cause stalling or fire.
