We have owned our 2016 Pilot since 2023. We have never had a problem with it and have had the recommended maintenance performed on schedule. The incident of our engine failing, out of the blue, occurred when my husband drove from MN to Indiana and then he made it back to MN and the engine died literally at the exit to our hometown. Our Honda Pilot ran perfect when my husband drove from MN to IN wi
... th no problems at all. He made it all the way back to MN and when he took the exit to our town, a bunch of lights went off on the dashboard. There were several lights and I don't know exactly which ones unfortunately. He was able to get it into our driveway and it was unable to be started once he turned it off. We had to get it towed to the dealer (Tom Kadlec) . After diagnostic testing, wewere told we needed a new engine. I realize our vehicle has higher mileage, but this is so unlike a Honda vehicle to just quit and then to have such a drastic repair of a new engine. Of course, completely unexpected and disappointing, out of the blue, just am shocked and would like to know if it had something to do with the recall. I appreciate your assistance. Thank you very much. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

During a routine transmission fluid service on a 2024 Toyota Corolla Hybrid, I discovered that the transmission model (PA10) is completely unlabeled and cannot be identified anywhere under the hood, in the owner’s manual, or on any visible tag. The only label is buried behind the firewall and not reasonably accessible to consumers or technicians. As a result, I initially attempted to service the
... wrong fluid port because Toyota provides no visible or clear identification of the transmission model or fill/drain points. Additionally, the factory-installed transmission drain and fill bolts were tightened with extreme torque well beyond normal service specifications, requiring application of high heat and excessive force to loosen. This resulted in personal injury and risk of property damage. The combination of (1) inaccessible transmission identification, (2) inadequate service guidance, and (3) extreme bolt torque creates a foreseeable safety hazard to vehicle owners or technicians attempting basic maintenance. Toyota’s official service documentation (Toyota Technical Information System, “TIS”) that clarifies the correct fill ports is paywalled and not included with the vehicle or manual, making it impossible for consumers to safely perform essential maintenance without paying daily access fees. This lack of accessible labeling and over-torqued components presents an avoidable risk of physical injury, vehicle damage, and mis-servicing. I believe Toyota should be required to provide visible transmission identification and safe torque specifications for consumer maintenance.Read more

Name: [XXX] •Address: [XXX] •Phone: [XXX] •Email: [XXX] Date of Incident: October 7, 2025 Description of Problem: While driving our 2025 Chevrolet Traverse on October 7, 2025, our son was sitting in the third-row seat and somehow managed to buckle two seatbelts at once—one from the right side and one from the left—crossing them over his chest and neck. Once both belts were latched,
... they locked in place. When we tried to loosen one side, the opposite side only tightened further, causing him to be choked. We tried multiple ways to release the seatbelts, but the locking mechanism prevented any movement. Several adults tried to help, but no one could free him. We ultimately had to cut one of the seatbelts to safely release him. He was left with visible red marks on his neck, and the incident caused significant fear and distress. The seatbelts were crisscrossed, which explains why attempting to loosen one side made the other side tighten. This appears to be a dangerous design issue, as there was no safe or quick way to release the belts in this situation. We are deeply concerned that this design could pose a serious strangulation hazard to children or other passengers in the future. Attachments: •Photos showing the crisscrossed seatbelts •Photos showing the red marks on the neck after the incident We request that NHTSA review this complaint to determine if this is a vehicle safety defect and take any appropriate action to prevent similar incidents from occurring. Purchased 12/2023 2015 Ford Expedition April of 2024 we began to have issues with the Sync System and Climate Control. We took it back to the original dealer and a second Ford dealer and neither was able to diagnose the issue. As they could not get it to duplicate while there. However it has happened numerous times since. The symptoms seem to have no rhyme or reason as to when they will happen, b
... ut they do follow the same pattern each time. The Sync screen will turn itself off completely and randomly. Screen goes Black System shuts down as if the power button was pushed. When you do push the power button to turn the system back on the temperature display in the left hand corner will no longer be displayed. This is the indication the system is still not working properly. The Air Conditioner will then turn itself off. Then the blower will switch to defrost and high heat, all in its own. You cannot adjust the setting or turn the heat off. It continues to blow on high heat in defrost mode until it decides to switch back to AC. This could last 2 seconds could be 20 minutes It will also read the gas gauge wrong, registering 20 miles left of gas even if you just filled up. This has all happened in the summer months causing extreme heat in the vehicle even with all the windows down. We replaced the Blower motor resistor and its wiring harness. Checked the blower at the same time, it is running smoothly. We also replaced the relay #3 responsible for climate control. We attempted the replace the intake manifold temperature sensor, but have been sent the wrong sensor twice, and it does not fit correctly. Now we have a fan running so loudly it sounds like out engine is idling high when it is not. We also now have no heat at all in the vehicle. When I checked the recalls the dates for the 2023 (22S56) are within the recall years but our vin says it's not. Manufactured 9/2014 Recall is for 1/2014-6/2017 No heat in the winter and extreme uncontrollable heat in the summerRead more

I purchased my 2022 Volvo XC60 in April 2025. I had no problems with the backup camera, until I installed a new version of software, 3.7, over the air, in September. Since the recall update, I've had issues with the backup camera not working, at the same time all of the cameras, pr

I took my 2024 Chevrolet Suburban to get repairs associated with GM safety recall N252494000 - L87 Engine Loss of Propulsion. My vehicle "passed" the test, so the "correction" employed by GM is that "vehicles that pass inspection will be provided a higher viscosity oil, which will also require a new oil fill cap, an oil filer replacement, and an owner's manual insert." All of this was done free
... of charge. However, the owner's manual insert states that "For future engine oil changes, use SAE OW 40" oil. This updated owner's manual insert, which is provided as a solution to GM's recall of a faulty connecting road and/or crankshaft, causes the future costs of oil changes to increase from $160.00 to $230.00. Assuming quarterly oil changes, this will increase the cost of operating this vehicle to increase by $2,800 over an estimated 10 year lifespan. This increase should be covered by GM. GM sent me a letter that states, "If your dealer fails or is unable to remedy this defect without charge, or within a reasonable amount of time, you may submit a written complaint to the Administrator, NHTSA." By passing on the cost of the increase in oil change for all future oil changes, GM failed to remedy this defect without charge. I submit this claim because GM filed to remedy this defect without charge. Please see highlighted portions of attached documents in support.Read more

I own a 2021 Toyota Camry XSE (VIN: [XXX] ) with a malfunction in the SRS airbag system related to the front passenger seat frame Occupant Classification System (OCS) sensor. The airbag warning light remains on, and diagnostics indicate a failure in the OCS sensor that could prevent the passenger airbag from deploying correctly, or could deploy it at the wrong time during a collision. I contacted
... Toyota for assistance, but they declined to cover the repair, stating the vehicle is out of warranty and not covered under any current recall or extended warranty. However, this is a critical safety issue that affects the airbag system’s ability to protect occupants properly. I’ve discovered that Toyota has already issued multiple airbag- and sensor-related recalls in recent years, including: – The Takata inflator recall, affecting many Toyota models for potentially explosive airbag inflators. – A December 2023 recall for approximately 1 million Toyota and Lexus vehicles (model years 2020-2022) due to a front passenger seat OCS sensor defect—the same component involved in my Camry’s issue. – A June 2024 recall of about 145,000 U.S. vehicles (certain 2024 SUVs/hybrids) for side-curtain airbags that may not deploy correctly. – A January 2025 recall affecting 2023 Toyota and Lexus vehicles for steering-column spiral cable defects that could prevent the driver’s airbag from deploying. Given these ongoing airbag and OCS sensor problems across multiple Toyota models and years, it appears my 2021 Camry may suffer from a related or similar defect. I am requesting that NHTSA investigate whether this issue should be included in or trigger an expansion of existing Toyota airbag recalls. Toyota’s refusal to assist despite the safety risk raises concern that other 2021 Camry owners could be affected without being aware of the danger INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
