See attached document for complaint. To Whom It May Concern, I am writing regarding the recall on my 2021 Chevy Suburban. While I understand there is currently no active solution for the issue, I want to stress the seriousness of the situation. As a mother of [REDACTED] living in Wisconsin, I am deeply concerned that if my truck were to shut down due to engine failure and we could not get help
... quickly, my family would be exposed to the risks from the fire that inevitably could arrive. Given the harsh winters there, this poses an unacceptable safety risk. Additionally, I paid for an extended warranty, believing I had plenty of coverage left. I recently brought my vehicle in only to be told that I was 500 miles past the 60,000-mile limit, I was under the impression the warranty had not expired, and if I had known otherwise, I would have brought it in much sooner. This issue has been ongoing for at least 6 months, and I have been aware of the mileage restriction, I would have scheduled service well before this point. On top of that, my Suburban continues to emit the smell of burnt oil. It is embarrassing to have my vehicle smell like it's burning every time I drive it, and it raises serious concerns about my health. Breathing in burnt oil fumes regularly cannot be good for my lungs or overall well-being. This issue has also created a financial burden for me, as I am currently trying to sell the vehicle. In its present condition—with the burning oil smell and unresolved recall—it is essentially unsellable. No buyer wants to take on these risks and uncertainties, leaving me stuck with a vehicle I can neither comfortably drive nor reasonably sell. This matter is not just about inconvenience. It is about safety, that matters, and other concerns, trust and accountability. Chevrolet has marketed its vehicles as reliable, family-safe transportation, and I want to feel assured that GM takes immediate action to properly address this oil consumption and/or issue. Anything less is unacceptable when it comes to the safetRead more

In fall 2024 we noticed our Nissan Leaf starting to no longer be able to hold a charge. There were no warning lamps, messages, or other symptoms prior to this problem. It would go from fully charged to zero with no warning (including when driving on the highway) and would stop accelerating above 35mph. This was stressful and unsafe. It took 5+ months to "repair" the battery at the dealership,
... only to have the same issue occur again a few months later. This time the dealership "fixed" the issue in a little over a month, only for us to drive it home and immediately have the issue occur again (on a busy highway - thankfully no one in my family was injured or killed). The car is currently at the dealership for a third time. Combine this issue with an active recall on the charging system for over a year with no known fix, this vehicle no longer feels safe for my family. The car battery is still well within it's warranty period. I do not feel safe driving this car any longer and have experienced dangerous situations three times in this vehicle in the past year (for myself, my family, and potentially others on the road). I do not trust any "fixes" by the dealership at this point - as we've been assured it has been fixed twice now only to find out it has not. Nissan acknowledges there is an issue, and I have copies of all the service records.Read more

There is a common and very dangerous electrical wire routing problem on my vehicle, along with 3 dozen others and my dads (separate but same truck) i confirmed with (FB group for owners of 17-19 L5P GMC diesel trucks) where the alternator positive wire from the alternator to the battery is run right along and across a rather sharp part of molding from the air intake tube. This routing location and
... rough intake plastic molding rubs through the alternator wire creating a possible electrical short/ corrosion problems that can lead to battery and electrical system compromise, as well as fire risk! A short-circuit condition can cause damage to engine components and, in rare cases, start a fire in the engine compartment. I have 92,000 miles on this 2019, my father has the problem starting on his at only 40,000 miles and may others have issues as well to very degrees of severity between 25 and 150k miles. I discovered this issue while performing routine air filter maintenance and happened to see the rub damage. no modifications have been done, this is the way the wire is routed from the factory. IMO and estimations This issue far supercedes the danger and severity of the recall that currently exists for the engine block heater cord for these models of trucks. 2 of the photos show the damage and exposed wire as soon as i found it. The last shows the location and the part that caused the rub damage along with a temporary mitigation method: electrical tape with a rubber hose installed as a buffer until a permanent fix can be installed.Read more

Year: 2012 Make/Model: Jeep Grand Cherokee 5.7 L V8, 4WD Mileage: ~140,000 miles Date of Incident: [Insert date] Location: McKinney, Texas (near U.S. 75 exit) Description of the Safety Issue: While driving normally, the Service 4WD System warning illuminated. Seconds later, the lightning-bolt symbol (Electronic Throttle Control) appeared, and the engine stalled abruptly without warning. All power
... steering and power-brake assist were lost while the transmission was still in Drive. The vehicle rolled backward on a slight grade as if in neutral. Attempts to restart the vehicle failed. Even CarMax technicians, at whose lot the vehicle came to rest, were unable to start it. A strong hot/burning electrical odor was present immediately after shutdown. This indicates a possible overheating or shorting event in the electrical or fuel-relay system (TIPM or throttle-body circuit). Had this failure occurred on U.S. 75 at highway speed (80–85 mph), I would have lost steering assist and braking ability in heavy traffic — a situation that could have led to a severe collision or injury. I respectfully request that NHTSA review this case as a potential recurrence or expansion of the existing recall group. The combination of a sudden stall, complete loss of control systems, no-restart condition, and electrical-burn odor constitutes a serious and ongoing safety defect that endangers vehicle occupants and others on the roadway. Supporting Information: – CarMax technicians confirmed the vehicle would not restart. – Recent maintenance included new fuel pump, injectors, hoses, and cooling-system components (~$10 K). – Warning lights: “Service 4WD System” and “Electronic Throttle Control (lightning-bolt)” appeared immediately before stall.Read more

The Antilock Brake Module (part # 68228991AD) and Antilock Brake Hydraulic Pump (part # 68196033AA) have failed for no apparent reason. The module controls the anti lock braking system, traction control, and cruise control, which are all safety concerns. The module cannot work without the associated pump.
... This problem has been confirmed by two dealerships and an independent service center. There were no warnings prior to the failure, which occurred on 10/18/25. While driving and idling, my 2023 Chevrolet Silverado 1500 High Country equipped with the 6.2L v8 engine began experiencing a persistent audible ticking/clicking noise from the engine bay, rough idle during cold starts, and noticeable/abnormal engine vibration. Shortly after, the check engine light came on. When scanned, diagnostic codes P0304 (Cylinder 4 Misfire Detected) and P050D (Cold start Ro
... ugh Idle) were retrieved. The vehicle was inspected by an authorized Chevrolet dealership, and the service advisor and technician confirmed hearing a consistent clicking noise and documented that they contacted GM's Technical Assistance Center (TAC case #80413912). The service order report states: "Due to unknown noise cannot perform recall. Called TAC 80413912 and was told need to figure out top end noise and misfire before proceeding with recall. If repairs are related to top end, will not be covered under recall." The technician indicated this could be related to a camshaft and lifter issue, which matches the symptoms outlined in GM's active propulsion system recall for 6.2L engines. Suggests GM aware & has not yet confirmed full recall coverage/repairs. This issue presents a significant safety risk because the engine could experience sudden power loss or failure while driving, potentially leading to loss of propulsion or control. These symptoms are identical to those described by GM in their recall documentation concerning valve lifter and camshaft failures. The issue has been reproduced and confirmed by the dealership, and all documentation, diagnostics reports, and service notes are available for inspection upon request. Warning/check-engine lights first appeared in October 2025, and the problem persists. Based on GM's recall acknowledgment of similar failures, I believe this defect should be fully covered under the existing recall, and I am requesting NHTSA's review to ensure manufacturer addresses this safety-related defect promptly and without cost to owners.Read more

1. What component or system failed or malfunctioned, and is it available for inspection upon request? The issue involves the Dynamic Radar Cruise Control system (vehicle-to-vehicle distance control mode) in my 2018 Toyota Highlander. This system automatically cancels when the vehicle’s speed drops to approximately 25 mph (40 km/h) or below. The system disengages without driver input during norma
... l driving conditions such as heavy traffic or congestion. The component remains functional but poses a safety concern due to its operational design. The vehicle and system are available for inspection upon request. 2. How was your safety or the safety of others put at risk? This malfunction creates a significant safety risk when driving in stop-and-go or congested traffic. The sudden loss of adaptive speed control forces the driver to quickly resume manual braking and acceleration, which can cause delayed reactions and potential rear-end collisions. The unexpected disengagement may also confuse drivers, especially those relying on the system to maintain safe following distances. This behavior increases cognitive load and the chance of driver error, particularly for elderly or less experienced drivers. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The issue has been observed and replicated multiple times during normal driving conditions, such as during city traffic or highway slowdowns. It is also acknowledged in the vehicle owner’s manual, which specifies that the system automatically cancels below approximately 25 mph. However, this design flaw has not been formally addressed or corrected by Toyota dealerships or service centers as a safety concern. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle has not been inspected specifically for this concern by Toyota or other third parties. However, the condition is consistent across the same model and year,Read more
