Hairline crack developed on the transmission cooling line at the connection of the thermal bypass block. After looking up it seems to be common. Yes, there have been user-reported issues with transmission cooler lines leaking or cracking on 2024 Chevrolet Silverado models equipped with the 3.0L Duramax diesel engine, including at or near the thermal bypass block connection. These reports primari
... ly come from owner forums and appear to affect a subset of vehicles, often discovered during routine maintenance or after noticing fluid spots under the truck. Symptoms can include hard shifting, fluid seepage on the undercarriage, or low transmission fluid levels, with some cases progressing to require towing if not addressed early. For instance, multiple owners of 2024 Silverado 1500 models (including ZR2 trims with the LZ0 3.0L Duramax) have described leaks originating from the cooling line block or thermal bypass area next to the oil filter, sometimes involving hairline cracks in the lines themselves. In one detailed account, a 2024 model with around 35,000 miles experienced fluid leaking from the cooling line block, leading to replacement of seals and the entire line assembly due to cracking discovered after initial repairs. Similar problems have been noted on closely related 2023 models, where leaks at the thermal bypass valve necessitated replacing the valve, O-rings, and all cooler lines, with some users reporting hairline cracks as the root cause. Parts like the lines have been on backorder in some cases, causing repair delays, and resolutions are typically covered under warranty (e.g., powertrain coverage up to 100,000 miles), though repeated visits to dealerships are common. These issues seem tied to the 10L80 10-speed transmission paired with the 3.0L Duramax, where the thermal bypass valve can contribute to overheating or fluid flow problems if faulty, potentially exacerbating line wearRead more

I am submitting this complaint regarding multiple open safety recalls on my Jeep, including two fire-risk recalls that currently have no available repair. Jeep/Stellantis has informed me that they cannot confirm whether my vehicle is safe to drive, and they instructed me to park the vehicle away from buildings, trees, and other vehicles due to the potential fire hazard. I am unable to comply with
... these instructions because I live in an apartment complex where all parking spaces are close to other vehicles and surrounded by trees. At work, I must park inside a parking garage with no alternate options. I have small children, and if the vehicle were to catch fire, they cannot exit the vehicle independently, which significantly increases the danger. I have contacted the dealership, who confirmed they cannot authorize a rental or loaner for this recall. They instructed me to escalate directly to Jeep Corporate. I have attempted to contact the Jeep recall department by phone and received a “closed” message during normal business hours. I have also sent written documentation to Jeep Corporate and my leasing company (Ally Financial) outlining the safety concerns, my inability to safely comply with the parking instructions, and my request for a rental or loaner. Ally has opened a case to document the issue. At this time, Jeep has not provided a fix or a safe transportation alternative, and I have no safe way to operate or park the vehicle. I am filing this complaint to document the safety risk and request assistance in obtaining a safe transportation solution until Jeep provides a remedy for the fire-related recalls.Read more

I am reporting a recurring safety issue with my 2023 BMW i4 (VIN: [XXX] ), currently at 28,426 miles. The vehicle has experienced three separate incidents of unexpected and complete loss of power while driving. The first incident occurred in March 2025. The vehicle suddenly lost all propulsion without warning and came to an immediate hard stop. I was later informed this was related to a wheel-pos
... itioning sensor. The second incident occurred in October 2025. I had been told that a recall software update would resolve the problem, but the vehicle experienced another loss of power while in motion, showing the issue was not corrected. The third and most concerning incident occurred in November 2025. Multiple error messages appeared instructing me to stop driving immediately, and the vehicle again lost power while moving. A complete loss of propulsion in traffic—especially while merging or pulling into a roadway—creates a severe safety hazard because I am unable to move the vehicle out of the way of oncoming traffic. This puts both myself and other drivers at risk. BMW has acknowledged awareness of issues related to these systems during previous service visits, but the defect continues to occur despite attempted repairs and software updates. I am requesting that NHTSA investigate this matter, as repeated power-loss events pose a significant safety risk that could lead to crashes, injuries, or fatalities. Vehicle Year: 2023 Make: Nissan Model: Pathfinder Platinum 4WD VIN: [XXX] Mileage: 24,500 Date First Noticed: October 8, 2025 Problem: The panoramic sunroof drain system is defective, causing water to leak into the cabin during rain, dripping from the driver-side overhead console and door area. This matches Nissan TSB NTB23-073 (Oct 9, 2023), which admits a design flaw in the front drain grommet
... s (Part #76938-5NA0A) leading to poor drainage and clogs—even from normal environmental debris like sand in Florida. The vehicle was built November 2022, well within the 3-year/36,000-mile warranty. Nissan dealer (Nissan of Naples) inspected (Oct 9, 2025) but performed an inadequate diagnosis: only a photo of sand in the tray, no water intrusion test, no drain line analysis, and no TSB verification. They charged $207.54 and denied warranty, claiming "environmental concern" and "outside influence" (sand clog, not defect)—contrary to the Owner's Manual (pp. 2-121/2-122), which requires zero drain maintenance. Nissan Consumer Affairs (Case XXX) upheld the denial, refusing written explanation. BBB complaint (NIS2519919) and FL Lemon Law Form 1A filed Nov 6—still unresolved. Safety Risks: - Mold/mildew growth in headliner (health hazard). - Electrical shorts in overhead console (lights, controls, airbags). - Driver distraction from water dripping near visor/windshield. This defect impairs safe operation and could affect others. Nissan must recall/fix under warranty. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

During unrelated work, a mechanic discovered the retaining ring for recall N08 (rear axle pinion nut) was not installed and the pinion nut was loose enough to turn by hand. He stated the recall had either been performed incorrectly or not at all, and that the vehicle was unsafe to drive. The recall shows “completed” in 2013, but the defect is still present. I contacted Stellantis Customer Car
... e, submitted photos, and they confirmed in writing that if the defect still exists, the recall must be performed free of charge regardless of age, mileage, or ownership. I sent the photos of the missing ring, screenshots showing the recall marked completed, and Stellantis’ confirmation to the dealership before having the vehicle towed to verify coverage. The truck was towed to Time Chrysler Dodge Jeep Ram of Portland. My initial contact was [XXX] at that location. However, all recall decisions and communication later came from [XXX] , who works at a different dealership. Over the next month, the dealership repeatedly stalled the recall, saying they were waiting on approval from the district manager and then from the recall department, but no action was ever taken. Despite having Stellantis’ written confirmation and clear photographic evidence of the unresolved defect, the dealership refused to perform the recall. They insisted on treating it as a “warranty” issue and cited reasons such as mileage, age, and not being the original owner—none of which apply to safety recalls. They then stated that teardown and diagnosis “would run $1000,” even though the defect has already been documented by both the mechanic and Stellantis. All communication, including photos, the mechanic’s findings, Stellantis’ confirmation of coverage, and the dealership’s refusal, is available upon request. The truck has now been sitting at the dealership for over a month with an unresolved safety defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I own a 2021 Chevrolet Silverado 2500HD Duramax diesel with approximately 100,300 miles. The truck began displaying a “Service Emissions System” warning and a countdown message that in a certain number of miles the vehicle will be limited in speed (for example to 65 mph). GM’s own Duramax diesel owner’s supplement explains that when a “Service Emission System” message appears it is acc
... ompanied by a distance-until-speed-limit countdown and that the truck will progressively limit speed first to about 65 mph and then to about 55 mph if the condition is not corrected, telling the owner to see the dealer. This creates a safety risk because the derate can occur while driving on highways or rural roads, where an unexpected limit on maximum speed can interfere with merging, passing, and keeping up with traffic. This is our only vehicle, and we must use it for important medical appointments, including cardiology and surgery-related visits. The condition started only about 300 miles beyond GM’s 5-year / 100,000-mile powertrain warranty. I opened a case with Chevrolet / GM Corporate (Service Request No. XXX) and took the truck to Sons Chevrolet in Columbus, Georgia on November 17, 2025 for this issue and a rear wheel lockup recall. The dealer kept the truck from about 9:00 a.m. to 3:00 p.m. and indicated they might need to keep it for at least two more days, but I was not given a clear written explanation of the diagnostic trouble codes or a firm repair plan. Because GM’s own manuals describe this “Service Emission System” message together with speed-limit countdowns, and because the vehicle can actively limit speed on the road if the fault is not resolved, I am concerned this represents a broader emissions/derate defect on late-model Silverado HD Duramax trucks. I request that NHTSA review similar complaints and consider whether further investigation or a safety recall is warranted. The forward-facing camera on my vehicle repeatedly fogs up inside the sealed camera housing, creating a light to severe haze across the camera lens. This internal fogging obstructs the camera’s view and affects the vehicle’s safety systems that rely on it, including forward collision warning, automatic emergency braking, lane keeping, and adaptive cruise control. The components are available f
... or inspection upon request. This obstruction puts my safety at risk because these systems either disable themselves or operate unpredictably when the camera view is blocked. The issue occurred again this year under normal outdoor temperatures, and the fogging cannot be cleared by the driver or by using defrost or cabin heat. The problem has been confirmed previously by a Tesla service center, where technicians inspected the camera area and told me my vehicle had one of the worst cases they had seen and told me this is a common issue. They attempted a repair, but the issue has returned, making it a recurring defect. No police or insurance representatives have inspected it. During the first occurrence, the vehicle displayed messages indicating that features like Autopilot, cruise control, and lane assist were disabled due to camera obstruction. The current recurrence produced visible internal haze over the camera lens and a warning message appeared which I do have that Tesla acknowledges this is an issue and that rather than fixing it, you can bring your car in for expedited cleaning. I have read others stating Tesla changed their windshield design and it doesn't heat up this area properly any longer. This issue has happened in multiple winter seasons and directly affects the proper operation of safety-critical driver assistance systems.Read more

This in addition to complaint form number:11699917 I have found the two forms issued by the GM global corp. One is a recall the other a bulletin about the HPFP fuel regulator to be replaced because it was known to fail. The first actual recall (13370 ) states that 25,000 vehicles were involved. The symptoms they stated are all the same that the vehicle I own are having including fuel fumes in the
... cab and under the hood . In 13370 right at the top it states urgent so this problem is definitely a safety concern for fire or explosion. In 13370A they eliminated a lot of these details including the words urgent and the amount of vehicles that were involved in this recall. And at the bottom of 13370A it also states that the following two paragraphs does not pertain to Canada , THIS is important because my vehicle was born in Canada 🇨🇦. So everything stated above that involves Canada manufacturing also. So this means my vehicle should be included in this recall also like I stated in my complaint to this safety commission 11699917 . My vehicle has the 5.3 , it’s a 1500 Silverado and it’s a 2014. Recalls don’t expire for safety as far as I heard. There is no reasonable explanation as to why this vehicle was excluded except it was manufactured in Canada and fell through the cracks of their recall . Without a recall they will not look at any bulletin for this safety issue on my vehicle. This is a very expensive repair to be done and GM knows this also.Read more

While driving my 2016 Kia Soul on the highway, the vehicle suddenly began shaking and immediately lost power. The check engine light came on and the car would not accelerate, even when pressing the gas pedal. This happened with no prior warning. I had to coast to the shoulder in moving traffic, which created a safety concern. The engine then stalled completely and would not restart. The vehicle w
... as towed to Findlay Kia in Las Vegas. Their technicians diagnosed a major internal engine failure. The repair report shows zero compression in cylinder 2, a cracked intake valve, oil contamination, and extensive internal engine damage requiring a full engine replacement. Diagnostic trouble codes included P0300, P0302, P0303, and P0326. There were no earlier symptoms, no overheating, and the vehicle has been properly maintained. The failure was sudden and left the car completely inoperable. I opened Case #[XXX] with Kia Corporate. They reviewed all documents, service records, and the dealer’s findings. Although they did provide assistance in reviewing the case, the final outcome was not in my favor, and they declined to cover the repair. This is concerning given the history of engine-related issues and recalls involving Kia Souls from similar model years, particularly involving sudden engine failure and loss of power. My vehicle remains inoperable at the Kia dealership. The sudden loss of power while driving presents a clear safety risk. Based on my experience and the dealership’s findings, I believe this engine failure is safety-related and should be evaluated further by NHTSA.
