I am writing to formally document serious engine defects I have experienced with my 2021 Chevrolet Tahoe equipped with the 5.3L L84 V8 engine. 1.Crankshaft Failure: My Tahoe suffered a catastrophic crankshaft failure that required major repair/replacement. I am aware that GM has recalled certain vehicles equipped with the 6.2L L87 engine for crankshaft and rod-bearing defects. Based on my experien
... ce, the same or a very similar defect has also occurred in my 5.3L engine, which is currently not included in that recall. 2.Valve Lifter Failure: In addition, my vehicle has suffered from valve lifter collapse—a well-documented problem in 2021–2022 GM trucks and SUVs with both the 5.3L and 6.2L engines. This caused severe misfiring, drivability issues, and expensive repair needs. Why this is a concern: •Both failures occurred well before what would be expected of a modern engine. •The crankshaft failure raises a safety risk: sudden loss of propulsion while driving could lead to accidents. •The lifter issue is widely reported by other owners and has been the subject of GM service bulletins. •Taken together, these problems suggest systemic defects in the 2021 Tahoe’s 5.3L engine that have not been adequately addressed by GM. Requested Action: •I ask GM to extend recall and warranty coverage to include the 5.3L L84 engine in addition to the 6.2L L87. •I ask NHTSA to consider investigating whether the scope of the current 6.2L recall should be broadened to include 5.3L engines, given failures like mine. •At minimum, I request assistance in covering repair/replacement costs for both the crankshaft and lifter defects, which are clearly not due to normal wear and tear. Vehicle details: •Year/Make/Model: 2021 Chevrolet Tahoe •Engine: 5.3L V8 L84Read more

My 2020 model Acura MDX is experiencing unexpected engine shutdowns while braking during motion. I drive 20+ miles each way to work every day. Much of this drive is on highways, and currently the bridge connecting Ohio and Kentucky is down to three lanes with no berm. On a windy road with low visibility and a steep hill near my home, the car completely lost power and acceleration without any warning. I wasn't able to drive it completely off the road before it came to a stop and due to the los
... s of power was unable to move into neutral to push it and the hazards would not work. Thank God that this didn't take place on the bridge or with my family in the car. I made the decision to exit the car while it was still on the road and find a safe area in the grass where I could call for help. When I exited the vehicle, it locked me out and the only way to get in was through the driver's side door, which would put me back on the road in traffic and at risk. The electrical failure prevented me from opening the other doors. The car's complete system failure put my safety/life at risk and could jeopardize the lives of others if it wasn't investigated. To make matters worse when the tow truck came via OnStar they weren't equipped to move a car stuck in park or drive due to the electronic gear shift so they drug it with the tires locked onto the bed. After several days of testing the electrical system, the dealership found a loose wire and informed me that it had been fixed. I picked up the car and after a mile or so, the check engine light came on. I contacted the dealership to come pick it up and that's where it stands at the moment. My intention was to pass this on to my boys when they start driving, but this experience will alter that plan.Read more

There is a button on the top of the steering wheel that puts the car in low gear. We have been going down the interstate twice and my husband accidentally hit the button slowing down the car immediately. This is very dangerous and could cause a rear end collision. We have reported it to Chevrolet several times, to the service department twice and the corporation also. Low gear has normally been
... on the gear shift with all the other gears, but for some reason, they put it on the top left side of the steering wheel. That is a very strange place to have that since the steering wheel also has buttons for cruise control, Bluetooth, stereo, etc. We think this button needs to be disengaged and put else where, but Chevrolet offered us $150 for the inconvenience. We are not concerned with convenience, but with safety. Also, the vertical buttons on the dash below the navigation screen are not visible during the day. At night, when it is dark, the light illuminate the buttons so they can be seen, but they are not visible during the day. However, you do need to be able to see the buttons. My husband used a label maker to put the names on the buttons just to prevent a wreck, but Chevrolet needs to address the issue. We have mentioned this to the service department and corporate office, but they do no seem to be concerned. We consider both items to be an issue that needs to be corrected, but no one seems to care. Approximately two months ago while driving about 65 mph on the interstate the power steering failed, and reverted to manual steering. The vehicle became extremely difficult to turn. I was near an exit, and as I slowed to leave the interstate, at 30-35 mph the steering began to work normally. I did not have the issue investigated. Two days ago while going to work, and descending a mountain which h

As I reduced speed to about 25mph the steering resumed normal operation. I was making a 90 degree downhill turn when this event occurred. I almost departed the road on a steep drop off. Had my wife been driving I do not think she would have possessed enough strength to keep from going off the road. I had the vehicle checked, by a mechanic and there were no codes or anomalies detected. I am a certified Airframe and Powerplant mechanic, as well as an Aviation Safety Inspector. This issue with my Jeep is an accident waiting to happen. There are a significant number of complaints for this same issue. NHTSA needs to address this issue.Read more

I am writing to formally request warranty service for my Toyota CH-R 2018, VIN [XXX] , currently with 88,341 miles. As you are aware, Toyota issued a warranty extension specifically covering transmission

My vehicle clearly falls within these parameters. Given the nature of the defect and Toyota’s acknowledgment of its impact, I respectfully request that your dealership initiate the appropriate warranty repair or replacement process without delay. I am prepared to bring the vehicle in for inspection and provide any additional documentation required. Please confirm receipt of this letter and advise on the next steps to resolve this matter promptly. I trust Toyota will stand behind its commitment to quality and customer satisfaction. Four days ago, engine started making a whining noise when accelerating, especially in lower gears. No warning signs. 2022 Highlander Platinum is 3 years old, 80K miles, out of warranty; no extended warranty purchased due to historical, stellar Toyota reliability. Transmission could last another 10K-20K miles they said, before it becomes undrivable or just drops, causing a potential safety issue. RE: 2025 HYUNDAI ELANTRA LIMITED AUTOMATIC BRAKE CONTROL On a recent road trip, the automatic brake system activated twice in a manner that could have put our lives at risk. The list below applies to both incidents: On a four lane interstate highway at approximately 70 mph. Wide, level median. Full daylight. Dry pavement. Unlimited visibility. Flat landscape. No objects in the median or a
... nywhere abutting the lanes. No wind. No car for at least a mile ahead of us, none next to us. Brakes were suddenly applied by the “assist.” Brakes so violent we were pushed forward in our seatbelts. Second incident so violent and persistent I started to pull off to the side of the lane for fear of being struck from behind. In both incidents, the “assist” eventually quit. Had this happened with a semi behind us unable to slow down fast enough we could have been severely injured or killed. Apparently, the “assist” cannot be turned off for good. Since the above incidents, we did try to shut off the’”assist" but it was back in operation the next time we started the car. This situation needs to be addressed. The “assist” seemed to want to assist us into an early grave.Read more

While driving down the highway at 70 miles per hour ater driving 2.5 hours our 2021 Pacifica hybrid minivan flashed a warning on the vehicles display saying approximately "Warning Engine Shutting Down". The engine immediately shut down in the middle of the highway and we were forced to coast off the highway to the shoulder. The engine warning light came on. We turned the car off for a few minutes
... and proceeded to start the engine. The engine warning light was still on, but the other message was gone. We attempted to drive towards our destination and approximately 30 minutes later the same thing happened again. We tried one more time since we were several hours from home and within 30 minutes the vehicle had the same issue. At that point we decided to try to get home. We drove the vehicle 15-20 mins at a time and then exited the highway over and over giving the vehicle time turned off. After returning home with no additional events the engine warning light went off the next warning. This was on a Sunday so we were unable to get it to a dealer. The next day we dropped off the vehicle at Nappletom Chrysler dealership. The called us the next day and told us they could not cause the event to happen again. They could see a set of error codes showing the problem occurred, but told us there was nothing they could do and it was up to us to cause the problem and then return the vehicle. Now I have a minivan with 70k miles on it that my wife primarily uses my with to get my daughter from place to place and I have no idea when this vehicle will shut the engine down at any time potentially causing a dangerous accident. I believe Chrysler has the responsibility to determine what the issue is. Researching on the internet show this is a common problem for my vehicle and typically requires a new transmission or and engine wiring short.Read more

The vehicle was sold with a “minor accident” reported, but the condition suggests the damage may have been more significant. After purchase, I discovered the rear seatbelt did not work and required interior panels to be removed and replaced. A non-working seatbelt points to possible prior impact or structural damage affecting the restraint system. The vehicle also has an active safety recall f
... or the side curtain airbags. The combination of a recall and a failed seatbelt raises concern that the vehicle may have been in a more severe collision than reported or repaired incorrectly. A failed seatbelt is a major safety hazard and should not be present at sale. The defect was confirmed by the dealer’s service department, and the vehicle has been in their possession since November 10, 2025. No warning lights appeared; the issue was found only when trying to use the seatbelt.Read more

My 2022 Kia EV6 developed a serious electrical/MCU malfunction on 10/19/2025 at approximately 64,300 miles. While driving, the vehicle began to stutter during acceleration, all driver-assistance and safety systems disabled (LKAS, collision avoidance, lane keeping), and the vehicle could not accelerate normally (topping out around ~60 mph when fully pressing the pedal). The Kia Connect app displaye
... d fault codes MCU P0A4100 and LKAS C164286. The malfunction recurred immediately after a dealer visit on 10/20/2025. The vehicle has since been inspected by Huffines Kia in Corinth, Texas, but no confirmed diagnosis has been provided, nor have they notified me of dropping/reconnecting the HV battery. On 11/18/2025, a new fault code appeared: MCU P0C1700. This failure affects the high-voltage electronics/MCU and causes sudden loss of acceleration capability and complete disabling of active safety systems while the vehicle is in motion, placing me and other drivers at risk. The issue is reproducible, continues to occur, and the vehicle remains at the dealership. I am requesting NHTSA review of this electrical/MCU malfunction as a safety-related defect.Read more

My 2017 Hyundai Santa Fe experienced a sudden and complete engine failure without any warning signs. There were no warning lights, no check engine light, and no unusual noise before the failure. The engine stalled while trying to start, ran very rough for a second, and then completely shut off. After that, the engine would not start again at all. A certified independent mechanic inspected the veh
... icle and confirmed the engine suffered a mechanical failure consistent with rod bearing wear or crankshaft damage, which is the same issue described in Hyundai’s known engine safety recalls. I later took the vehicle to a Hyundai dealership. They also confirmed that the engine had failed, but said it could not be repaired under Hyundai’s programs because my vehicle was not listed in the specific recall. I was never notified of any safety campaigns or recalls. I am not the first owner, so the required recall inspections and updates were never completed on this vehicle before I bought it. The sudden engine shutdown created a serious safety risk, especially because there was no warning before it failed. This failure matches the symptoms described in Hyundai’s Theta II engine defect investigations. I am requesting that NHTSA investigate whether this vehicle should be included in the affected group, and whether this engine failure is part of the broader safety defect already identified in many Hyundai engines from the same model years.Read more
