Jeep has a design defect in vehicles equipped with the 2.0L turbocharged engine where the coolant reservoir is positioned directly adjacent to the turbocharger and turbo heat source with insufficient heat shielding. Due to the constant high heat exposure, the plastic coolant reservoir and/or connecting plastic fittings degrade, warp, and become brittle over time, eventually leading to coolant leak
... s, erratic coolant temperature readings, and/or sudden coolant loss. This failure often occurs without warning, and many vehicles do not initially trigger a check engine light. In my case, the vehicle exhibited intermittent coolant temperature readings, abnormal fan operation, and heating inconsistencies before the leak was discovered. Multiple repair attempts are frequently made before the root cause is identified. Mechanics often replace components such as the thermostat housing, coolant temperature sensors, or related assemblies, not realizing the coolant reservoir heat damage is the underlying cause. This results in unnecessary repairs, significant consumer expense, and extended vehicle downtime. This defect poses a serious safety risk, as coolant loss can result in: •Engine overheating •Sudden loss of power •Potential engine failure while driving •Increased risk of breakdown in traffic or unsafe driving conditions Numerous owners report similar failures, indicating this is not an isolated incident but a widespread design flaw affecting Jeep vehicles equipped with the 2.0L turbo engine.Jeep has not issued a Technical Service Bulletin or recall addressing the placement of the coolant reservoir or providing guidance to technicians to inspect for heat-related degradation in this area.I am requesting that NHTSA investigate this design defect and require Jeep to issue a Technical Service Bulletin or recall addressing this. The vehicle was properly maintained and not modified and The issue occurred at relatively low mileage.Read more

We had to replace the engine about 4 years ago and we’re still getting gas smell into the cabin. About 3 years ago, we broke down on the road because the transmission quit. All our repairs have been done through Ford service department. We are now broke down again because of the transmission. Something has to be done with the Ford Escape!

The safety hazard began with a pattern of ignored warnings by the authorized dealer network. On Aug 15, 2025, I brought the vehicle to Luther Kia of Bloomington for a Check Engine Light. The dealer simply cleared the code without diagnosis, failing to catch early signs of engine trouble. On Sept 22, 2025, the engine failed again. I drove the vehicle in unsafe "limp mode" to Luther Nissan Kia [XX
... X] ). They found the engine was "4 quarts low on oil" (severe starvation just 37 days after a dealer oil change). Despite this catastrophic finding, they failed to diagnose the root cause and released the unsafe vehicle to me after selling only a spark plug replacement. On Sept 26, when I attempted to pick up the vehicle, the Check Engine Light was flashing immediately at handover. I refused to take the unsafe vehicle. The dealer then replaced an Oil Control Valve (OCV) and released the vehicle to me again later that day. After this second "repair," I drove the vehicle for only a few miles before the engine failed again with a Check Engine Light. The dealer has since admitted in writing (BBB complaint #XXX) that the engine was "deprived of lubrication" due to the initial low oil condition. By ignoring early warnings and releasing a vehicle with a dying engine twice, the dealer put me and other motorists at risk of a sudden loss of power at highway speeds. On November 27, 2025, my 2019 Honda Odyssey experienced a sudden and catastrophic engine failure while traveling at highway speed. The vehicle had been serviced at an authorized Honda dealership the previous day (oil change and tire rotation), and the failure occurred approximately 55 miles after that service. While driving on the interstate, the engine abruptly lost power, expelled a large quant
... ity of engine oil onto the roadway, and then caught fire in the engine compartment. The driver managed to pull over safely and extinguish the fire. This was a significant safety hazard to the driver, passengers, and surrounding motorists. The dealership later reported that the engine suffered internal mechanical failure involving broken connecting rods, which caused extensive engine damage and oil loss. I reviewed public information and discovered that certain Honda 3.5L V6 engines from similar model years have been recalled for internal engine defects that may lead to connecting-rod issues or catastrophic engine failure. However, my vehicle’s VIN is not currently included in those recalls, despite experiencing a similar failure pattern. This incident involved: Sudden loss of engine power at highway speed Immediate oil loss onto the roadway Engine compartment fire Risk of collision or injury Given the severity of the incident and the similarities to known recalls for related engine failures, I am submitting this report to request that NHTSA review this event as part of potential broader safety concerns involving this engine family. The underlying cause should be evaluated to determine whether a defect or pattern exists that could affect additional vehicles not currently included in recall campaigns. No injuries occurred, but the incident posed a serious risk to life and traffic safety.Read more

I purchased my 2025 Chevrolet Silverado 2500HD new from Wilsonville Chevrolet in Oregon. Since owning the vehicle, I have experienced a noticeable shutter/vibration at highway speeds, consistently between 70–75 mph, whether towing or not. The shutter feels like it is coming from the transmission or driveline, and it occurs on multiple road types and conditions. I reported this issue to Wilsonvi
... lle Chevrolet, but they told me they “could not duplicate” the problem and offered no repair. I then took the truck to Power Chevrolet in Corvallis, who stated the tires were out of balance and replaced them. This did not resolve the issue, and the shutter at highway speed remains exactly the same. I also contacted GM directly, but was redirected back to the dealership, resulting in no progress. At this point the issue has been reported multiple times, yet no root cause or repair has been provided. I am essentially being sent in circles without a solution. There appears to be a growing history of transmission-related issues with newer Chevrolet HD trucks equipped with the Allison 10-speed transmission, and this symptom seems consistent with those reports. A shutter at sustained highway speeds raises a significant safety concern, as a transmission failure at 70+ mph could lead to loss of control or power. In addition to the shutter, I have repeatedly noticed a strong “burning brake pad” smell coming from the rear of the truck. This happens randomly and has also been reported to the dealership, but no cause has been identified. I am unsure whether this is related to the transmission issue, but combined, these symptoms suggest a potential mechanical defect. Given the safety implications and the lack of resolution from authorized dealerships, I am requesting NHTSA’s assistance in investigating this issue.Read more

My 2023 Kia EV6 experienced a sudden ICCU and 12-volt system failure while traveling approximately 50 mph. Without warning, the vehicle instantly lost propulsion and all electrical power. This included loss of steering assist, braking assist, hazard lights, shift capability (unable to shift into Park), and even the ability to lock the doors. The failure effectively “bricked” the vehicle in mot
... ion. I was only able to get the vehicle off the roadway because the road happened to slope downward into a nearby lot. Steering required significant physical effort, the brake pedal went completely hard (“dead”), and the vehicle offered no functional parking brake or Park engagement once stopped. Because I was on a hill, we had to allow the car to roll to the bottom until it met a curb to prevent movement. After power loss, the doors could not be locked at all. This type of unrecoverable electrical failure creates an extreme safety hazard for the driver, passengers, and surrounding traffic. There is no pre-warning and no diagnostic indicator. The ICCU fuse appears to fail spontaneously and renders the vehicle uncontrollable in several critical respects. This is not an isolated issue. ICCU failures have been occurring across the Kia EV6 and Hyundai Ioniq 5/6 platforms for more than three years, including on brand-new 2025 models. The persistence of this defect across multiple model years indicates that the underlying design or component defect has not been resolved by the manufacturer. Given the suddenness of the failure, the total loss of vehicle control systems, and the long history of similar incidents reported by other owners, I am now hesitant to drive the vehicle due to the unpredictable and dangerous nature of the defect.Read more

Central computer (motherboard), cameras (front/rear/side), autopilot/auto steer system. Error code: APS_w132. Description of Problem: On or around [XXX], Tesla pushed a mandatory over-the-air software update to my vehicle. Immediately after, multiple safety-critical systems failed: all cameras (including backup) became intermittent or non-functional, auto steer/autopilot stopped working, and the c
... ar emitted constant distracting beeping alerts due to a critical motherboard failure. This made driving hazardous—impaired visibility for parking/reversing, loss of collision avoidance features, and constant audio distractions that could lead to accidents. I had to drive in this unsafe condition for weeks while waiting for service. Tesla attempted a remote fix but it failed. I scheduled service on November 19 for December 3 (19-day delay). Diagnostics confirmed the update induced the hardware failure, a known issue reported by other 2021 Model 3 owners on forums like Reddit (e.g., software bricking motherboards post-update). Despite this, Tesla refused warranty/goodwill coverage, quoting $2,576.83 for repairs. Service advisors Josh and Andy were unprofessional: Josh was rude/condescending; Andy was aggressive, sarcastic, and retaliatory. After I mentioned filing complaints with the AZ Attorney General and News Channel 3, Andy threatened $100/day storage fees with an impossible deadline (called at 4 PM Friday, close at 5 PM, demanded pickup by morning, then noon). He closed my ticket without authorization, delaying fixes and forcing me to contact another location—prolonging my exposure to the unsafe vehicle. This defect poses serious safety risks: sudden loss of cameras/autopilot could cause crashes. Tesla's awareness (from prior reports) and poor handling (intimidation delaying repairs) exacerbates the danger. No crash/injury yet, but potential is high. Please investigate this update-induced failure as a widespread defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I'm submitting this complaint regarding an ongoing electrical defect on my 2022 Chevrolet Tahoe. The vehicle has experienced repeated sudden battery drain & no-start events, occurring approximately once per week. These failures happen without warning, sometimes after the vehicle is parked for as little as 6 hrs. All dealer updates & software campaigns were completed. Despite this, the vehicle con
... tinued to suffer no-start events. After further troubleshooting, we removed fuse F35RA (which powers only the hands-free liftgate module). Once this fuse was removed, the issue immediately stopped — confirming that the parasitic draw originates from the liftgate module, consistent with GM Technical Service Bulletin 24-NA-039. This indicates a known electrical defect. This problem has caused multiple safety concerns: The vehicle has left my family stranded late at night. My husband works in Chicago & relies on this vehicle. A sudden no-start or complete electrical failure in that environment is dangerous. We were unable to transport our children to scheduled activities due to the vehicle being completely dead. Repeated deep discharges are damaging the battery we purchased only months ago. A family vehicle shouldn't unpredictably become inoperative. This is not normal wear — it's a safety-critical electrical defect that could strand owners in unsafe conditions, especially in winter climates. GM hasn't issued a recall for this issue despite issuing a TSB documenting the defective behavior. The frequency & severity of electrical failure suggest this should be investigated as a potential recall. I'm requesting NHTSA review this matter due to the safety implications & the repeated nature of the failures. Incidents: No-start after 6 hours parked at a friend’s home Multiple weekly no-start events before fuse removal Occurred before & after software updates were performed Location: Incidents occurred both at home & away from home. On [XXX], I [XXX] ) experienced the following issues with my new 2024, Chevrolet Silverado - 1500: • Front Windshield Wiper Washer Fluid not spraying/dispensing. • No messages or symbols displayed on the Driver Information Center dashboard indicating Windshield Wiper Washer Fluid Reservoir low/empty. • Inspected the reservoir that holds the fluid and did not observe low/empty. • Took it
... to the GM dealer 4 times and they were unable to repair. My vehicle has a recurring defect with the windshield washer system where the washer fluid does not dispense when activated. This issue occurred on a new vehicle and has persisted despite service attempts. When the washer fluid fails to spray, I am unable to clear dirt, dust, road debris, or moisture from the windshield while driving. This significantly reduces forward visibility, especially at freeway speeds or in conditions where debris from other vehicles hits the windshield. The inability to clear the windshield immediately creates a safety hazard because it can obstruct the driver’s line of sight and increase the risk of a collision. I am reporting suspected odometer fraud and misreported mileage/VIN/title data by Greenway Kia North (Longwood, FL) involving my 2024 Kia EV6 GT-Line (VIN: [XXX] ) and my 2017 Audi Q5 trade-in. These actions appear to violate the Federal Odometer Act (49 U.S.C. § 32705 and 327 and Florida title/odometer laws. After attempts to resolve the matter, I issued a rescission and statutory-demand notice
... on Nov. 8; the dealer provided no adequate response. Title/Registration Misrepresentation: My Audi Q5 (dark blue) was submitted to FLHSMV as “Monsoon Gray.” The EV6 (Aurora Black Pearl) was submitted as “Blue.” I verbally corrected both errors at signing; the dealer ignored them, and the false data was transmitted to the State and later reported through Carfax. This indicates intentional or reckless submission, not clerical error. Odometer Fraud: The dealer certified 131 miles on the Odometer Disclosure Statement, though I informed them the mileage was higher. Post-purchase evidence shows the certification was false. Documented travel from Dec 29–Jan 4 totals ~546 miles. Charging records (49.2 kWh on Dec 30; 63 kWh on Jan 2) indicate regular use inconsistent with a 131-mile vehicle. On Jan 4, the dashboard displayed 965 miles; Carfax recorded 1,206 miles. If delivery mileage were truly 131, the vehicle would show ~677 miles minimum—yet readings were 965–1,206 miles, proving the EV6 had approximately 413–660 miles at delivery. Willfulness Indicators: I verbally corrected mileage and color errors; the dealer ignored the corrections, submitted false disclosures, and provided only a thumb drive instead of paper contracts, preventing immediate verification. Carfax confirms incorrect DMV data originated from dealer submissions. Requested NHTSA Action: Investigate for violations of the Federal Odometer Act, audit the dealer’s odometer/title practices for a possible pattern, pursue civil/criminal enforcement if warranted, and refer to DOJ as appropriate. The problem was confirmed by my Chevrolet dealer, who diagnosed the radiator as needing replacement. They initially quoted $1,800, later reducing the cost to $915.90 after acknowledging that approximately 20 customers per month at their dealership alone are experiencing the same issue. This indicates the defect is widespread, at least regionally, and may be systemic across the model. It occurred well within the 5‑year/60,000‑mile warranty period, yet the dealer claimed the radiator is not covered. I am requesting NHTSA investigate this issue, as it appears to affect many owners and poses a significant safety risk.Read more
