My vehicle is subject to NHTSA Safety Recall 14S06 related to loss of electric power steering assist. The Power Steering Control Module was reprogrammed in 2014, and Ford now considers the recall closed based on that reprogramming. In 2025, the vehicle again experienced loss of electric power steering assist and currently exhibits the EPAS diagnostic trouble code associated with the recall defect

On November 13, 2025, the dealer attempted reprogramming of the steering control module as a diagnostic and customer service step to determine whether reprogramming would resolve the condition. This attempt did not correct the defect, and the loss of steering assist persists. Supplement 3 to Recall 14S06, issued in 2019, includes a Dealer Q&A section (Attachment 4, Question 3) that specifically addresses what happens if a customer experiences loss of steering assist after the module has been reprogrammed. Ford’s guidance states that any subsequent loss of assist after reprogramming may be eligible for extended coverage under a related program. This guidance acknowledges that reprogramming alone may not prevent recurrence of the defect and that further corrective action may be required when the loss of assist returns. Despite this acknowledgment, Ford has denied further remedy solely because the recall is marked “closed” in its internal system and because the vehicle exceeds the time and mileage limits of customer satisfaction program 15N01. The recall defect remains present in the vehicle, and the original remedy has not been adequate to correct it. I am requesting NHTSA review whether Ford has met its obligation under 49 U.S.C. §30120 to provide an adequate remedy for this safety defect. On November 12, I was on the highway in the right lane, there was a car about 4 car lengths ahead of me in my lane and the car in the middle lane was quite a distance ahead. Just as I got in the middle lane, BOOM! the vehicle's CMBS malfunctioned and slammed on the brakes - no warning, visual or audible, just came to an abrupt stop. Thank goodness no one was behind me or I would have been dead.

... ely 11:45 AM when I went to leave in my truck and noticed the rear sliding window had shattered. The window shattered and distributed tempered glass fragments all over the inside of the cab. Only approximately 10% of the window remained intact. I then reviewed the security camera footage from my driveway and discovered that the window failed entirely on its own with nothing hitting/striking/touching the window, nor any humans or animals in the vicinity. I attempted to take the vehicle to a GMC service department this morning (Dec 13) but due to their demand to keep the vehicle for 2-3 days to investigate yet unable to provide a loaner vehicle, I was unable to leave the vehicle in their possession and returned home. I plan to continue to contact GMC dealership service departments in hope I can find one which can provide a loaner vehicle, as I will need transportation to and from work in the coming days. The hybrid system effectively shut down and the vehicle could not maintain normal road speed. I brought the vehicle to a Toyota dealership. The dealership performed only ECM and hybrid ECU software updates and cleared codes. No mechanical inspection, oil pressure analysis, particulate inspection, turbo system evaluation, hybrid motor synchronization testing, or engineering-level diagnostics were performed. Toyota Corporate subsequently stated that the software update was the “final” resolution and declined further diagnostics. I was advised to return to the dealership if the issue reoccurs. This failure mode matches the documented loss-of-propulsion behavior described in Toyota’s V35A engine machining-defect recall (sudden power loss, limp mode, cascading system failures). Hybrid models are currently excluded despite using the same engine platform and exhibiting identical symptoms. Sudden loss of propulsion at road speed presents a serious crash risk. This appears to be a safety-related defect that is not being adequately investigated or remedied for hybrid models.Read more

My 2016 Malibu has a severe EBCM-related safety defect causing all four brakes to drag and overheat (reaching 240°F), leading to unstable handling. This condition mirrors the 2016 Malibu EBCM recall (GM Recall 39440). I sought help from GM. A GM-certified dealer-Monken in Centralia, IL performed a “diagnostic scan” which documented the core hydraulic failure: a constant, uncommanded 6.839 psi
... residual brake line pressure when the pedal was released. (A healthy system should be near 0 psi) This 6.839 psi is actively forcing the brakes to drag. The dealer ignored their own scientific data and submitted a fraudulent diagnosis of ("rust buildup") to GM, which GM used to deny the claim based on mileage. “Rust buildup” doesn’t create hydraulic pressure. GM is aware of this specific, documented, quantifiable safety defect (the 6.839 psi pressure) and is refusing to investigate it further. I stressed to GM that the vehicle wasn’t safe to drive and that I have a small child who has to ride in the car with me. GM dismissed my concerns by stating the dealership won’t release my car unless its safe to drive. I then had to drive the car 22 miles back home with all 4 brakes dragging. I requested the full diagnostic scan report (that the mechanic confirmed he could provide me with when I dropped my car off there) numerous times and never did I get one. I informed them that I would be filing a complaint hadn’t I received it by a certain time. The new kid in service then did send me screenshots of a generic OBDII scanner where I saw the 6.839 psi. I knew what the issue was prior to taking my car to Monken. I had an independent scan done prior. I only took the car there to comply with GM’s protocol to be eligible for assistance. I am requesting NHTSA investigate GM for failure to address a documented hydraulic control safety defect that causes brake dragging and overheating, which is non-compliant with safety standards. Independent diagnostic scanning retrieved Diagnostic Trouble Codes (DTC) U0018 (Medium Speed CAN Communication Bus Short) and P0894 (Transmission Component Slipping). These specific codes and symptoms (intermittent electrical short causing stall/shift-to-neutral) are identical to the safety defects addressed in NHTSA Recall 15V-470 (FCA Recall R24 - Power Distribution Center Electrical Connector) and NHTSA 16V-529 (FCA Recall S55 - Transaxle Wiring Harness). Although records indicate Recall R24 was marked as "completed" on this vehicle in July 2015, the specific safety defect has recurred, presenting the exact same failure mode. The Authorized Dealer was contacted regarding the recurrence of this safety defect but refused to perform a no-charge inspection, citing an expired repair warranty. The Manufacturer (FCA US LLC) was also contacted to report the recurring safety failure but refused to open a case or authorize a diagnostic inspection, despite the life-threatening nature of the defect. I have a 2020 Porsche Macan S. My passenger side head light went out and upon taking it to the dealership, I was informed that it’s a a headlight assembly failure. My car is out of warranty, and the dealership quoted me $5400 approx for replacement of the assembly. ... /module). That is a manufacturer defect. I researched online, and saw many Reddit users as well as Porsche Forums talk about this very issue on 2019 and 2020 Macans. I’ve seen and failure is not isolated: I have found multiple owner reports of 2020 Macan headlight LED failures, which suggests a potential systemic defect. I also see one complaint already filed at NHTSA with a very same problem/concern and I am filing another, hoping this reaches attention and can be resolved. There was several customers nationwide, who also had this happen, and those headlight assemblies should last the lifetime of the car is what the dealership had told me. $5400 is a very expensive and steep cost for a headlight assembly, and it’s certainly an issue that causes great safety concern for 2020 Macan S users, and a major defect. I have reached out to Porsche North America several times over the month, and they have not offered to fix this at goodwill. I have informed them that I would file a NHTSA complaint as well as a BBB complaint, which also yielded no results. Given the cost and the fact that headlights are a critical safety item, I am formally requesting full recall to have Porsche take accountability and replace and or repair at no cost to its customers for the failed headlight assembly.Read more

The contact called an independent mechanic and was informed that the dealer needed to repair the vehicle. The vehicle was taken to the dealer. Once the vehicle was at the dealer, an employee used a handheld scanner, and the check engine warning light was no longer illuminated. The dealer informed the contact that the vehicle was repaired, and an emissions test was conducted without the contact's permission. The contact took the vehicle back to the residence. The contact stated that the cooling fan failed to function as intended. No warning lights were illuminated. The vehicle was taken to Auto Zone, and the contact was informed that DTC: P0128 and P0811were retrieved; however, the warning lights were no longer illuminated. The contact took the vehicle to an emissions testing center and was informed that the Transmission Fluid Temperature (TFT) Sensor "A" had failed and had not been repaired. Safety Issue Description On or about December 11, 2025, while the vehicle remained in service for unresolved safety system failures, the dealership performed a master reset of the vehicle’s modem/telemetry system. Immediately afterward: •My FordPass access was removed •All historical vehicle alerts and safety warnings were erased •No active alerts are displayed, despite documented prior ADAS failures •The vehicle now shows no alert history, no mileage data, and no retrievable safety logs •The vehicle appears as a new/uninitialized vehicle in the FordPass system This reset occurred without resolving the underlying safety defect and while engineering had previously indicated no fix was available.
