On 5/8/2024, at approximately 18,969 miles, the vehicle suffered a sudden and complete loss of engine power while driving on the highway in the rain with my family inside. The engine died, the vehicle lost all propulsion creating an immediate and severe safety hazard. Dealer diagnosis revealed a catastrophic crankshaft main bearing failure, requiring a full engine replacement. Since the engine rep
... air, the vehicle has exhibited persistent intermittent/unintended brake activation. The brakes engage without driver input causing "Service Brake" messages, excessive and premature brake pad/rotor wear (replaced three times in a matter of months due to heat and dust from repeated activation), grinding, squealing, and locked caliper incidents. The vehicle has been inspected by two Cadillac Dealerships without any remedy except replacing warranty parts. An independent mechanic inspected the vehicle and confirmed the issue is likely related to the Advanced Driver Assistance Systems. To drive safely, we must disable the collision avoidance and traction control features. This has been reported on multiple repair orders and matches complaints from other 2023-2025 Escalade and GM SUV owners. Total documented downtime since first failure exceeds 160+ calendar days. Problems remained unresolved despite warranty repairs, software updates, recalls, and the L87 engine replacement. The sudden engine failure endangered my family and the ongoing phantom braking creates risk of collision or loss of control, especially at highway speeds. I have been forced to disable critical safety systems to prevent unintended braking. This vehicle is directly affected by the L87 defect that prompted federal recall and investigations. Please include my complaint in the ongoing EA25007 and RQ26001 investigations and consider my case in any further actions regarding the adequacy of GM's remedies. Please contact me for dealer repair orders and independent mechanic findings.Read more

The rearview backup camera system malfunctioned. The camera was replaced under a safety recall on my 2021 Hyundai Santa Fe. Within four days of the recall repair, the camera continued to fail intermittently, displaying a blank/black screen and not providing a rearview image when the vehicle was placed in reverse. The vehicle is available for inspection upon request. I also have video documentation
... clearly showing the malfunction occurring. The failure of the rearview camera puts me and others at risk because the system is required to provide visibility behind the vehicle when reversing. When the screen goes blank, I cannot see pedestrians, children, vehicles, or objects behind me. This creates a significant risk of a backing collision, especially in parking lots and residential areas. The vehicle was brought back to the dealership four days after the recall repair because the issue continued. The dealer stated they could not duplicate the issue at the time of inspection and are requesting a diagnostic fee to continue evaluation. However, I have video evidence clearly showing the malfunction occurring after the recall repair. The vehicle has been inspected by the dealership that performed the recall repair. It has not been inspected by police or insurance representatives. The manufacturer has not personally inspected it beyond the dealership performing the recall repair. There were no warning lamps. The issue presents intermittently when shifting into reverse, at which time the display screen fails to show the rearview camera image. The issue first appeared prior to the recall repair and continued within four days after the recall component was replaced.Read more

At approximately 29,000 miles, my 2023 GMC Sierra 1500 experienced a transmission failure. The transmission began slipping, shifting improperly, and operating unpredictably during normal driving conditions. This occurred without warning and well below the expected lifespan of a transmission, indicating a serious defect rather than normal wear or maintenance-related issues. The vehicle became unre
... liable and unsafe to operate, particularly during acceleration and highway driving, where delayed or improper shifting created a potential safety hazard. I relied on this vehicle for daily transportation and work, and the worsening transmission condition made continued operation unsafe and impractical. I later became aware that transmission failures are a widespread and documented issue affecting this model year GMC Sierra 1500, suggesting a possible manufacturing or design defect. Due to the severity and premature nature of this transmission failure, I was forced to return the vehicle early. General Motors is now charging me a balance of over $10,000 related to the early lease termination, which was directly caused by this defective transmission and not by customer misuse or negligence. My requested resolution is for General Motors to waive the full balance of over $10,000, as this financial obligation resulted solely from a premature transmission failure caused by a defect outside of my control.Read more

1. The engine starter motor (specifically the pinion starter) has failed, and the auxiliary battery is also showing reduced capacity (only a portion of its 660 cold cranking amps). The vehicle is currently at a service center where the technician has diagnosed these issues. The components should be available for inspection upon request, as they have not yet been replaced. 2. The failure of t
... he starter motor and battery primarily affects the vehicle's ability to start. While this could potentially strand you in an unsafe location, the information provided does not indicate any specific safety incident or risk that occurred during operation. No direct safety hazard has been reported. 3. Yes, a technician has diagnosed the problem and confirmed a fault code in the drivetrain related to the failed starter motor. The technician also identified the auxiliary battery's reduced output. This confirmation comes from a service center, which could be a dealer or independent shop. 4. The only inspection mentioned is by the technician at the service center. There is no indication that the manufacturer, police, insurance representatives, or others have inspected the vehicle or components. 5. Yes. Prior to the failure, when starting the car in the early morning, a "drive train service" message appeared on the display, advising you to visit a service center as soon as possible. This warning was the first symptom of the problem before the starter ultimately failed and the battery drained. This aligns with the technician's diagnosis of a drivetrain fault code related to the starter motor.Read more

While making a left turn at approximately 10 mph in light snow conditions, braking became severely impaired with pedal pressure present but vehicle failing to decelerate, and steering became significantly resistant and insufficient to redirect the vehicle away from the obstacle. Vehicle continued toward a tree despite brake pedal input and steering input away from the tree. Slippery road warning l
... ight activated before and during the incident. Multiple additional warning lights activated during and after impact. Airbags did not deploy despite collision. Post-incident, steering required significant opposite steering input to drive straight for approximately 1.7 miles to reach a safe parking location, consistent with front-end damage affecting steering alignment. Prior to this incident, I experienced multiple separate occasions where braking felt abnormal and insufficient in conditions requiring higher traction including rain and snow. I verbally reported this concern once to an authorized Nissan dealer in December 2025 and was told this was normal AEB system behavior. No documentation was created for this verbal complaint. Vehicle had a prior manufacturer recall performed at an authorized Nissan dealer on June 1, 2024 reconfiguring the IPDM/USM — the module controlling electronic steering column lock and CAN bus safety system communications. At that same visit, the Nissan technician documented abnormally dirty brake fluid at only 14,410 miles on this 2023 vehicle. A second manufacturer recall reprogramming the ECM was performed at an authorized Nissan dealer on December 4, 2025, approximately two months before this incident. The vehicle is available for inspection. No independent or dealer post-incident inspection has occurred. An insurance claim has been opened. Incident occurred within the 3-year/36,000-mile manufacturer warranty period at under 31,000 miles.Read more

I purchased this used car in the end of July 2025. Within the last month the service transmission light came on. Within a 24-hour period, the message changed to, "service transmission unable to shift Soon". Also alarm bells go off when I first start the car and sometimes during driving. The car feels like it drives correctly and smoothly with no shifting issues. Sometimes when that second message
... and check engine light icon being comes on, there seems to be a third message that is trying to pop up behind it but it does not. It seems like there is a glitch in the system. This was very concerning and scary for me. My research shows me that this has been a chronic problem for my make and model and year of car. I've also heard it can be even time to check and or change the transmission fluid. That needs to be a different message all together without causing alarm that the transmission itself is not functioning properly. I have not driven this for 10 days because many repair shops in dealerships are telling me not to risk it and of course want me to bring this in for a diagnosis. It is costly and hard to discern and trust who to go to for the correct information. The dealership should be responsible for a recall and replacing this sensor or modular so people do not have a cause for alarm if it is something as simple as needing to change the transmission fluid. There is also no dipstick on this vehicle so no local fluid fill station can check this. Many people have complained and are at a loss. I see there is a bulletin on this subject but no recall. Please take action and turn this situation into a recall once and for all to help. The little folks like me stay more safe and better informed about the health and longevity of our vehicles. My 2025 Toyota 4Runners brakes have a very high pitched squeal. This is most noticeable when backing up but sometimes when applying the brakes going forward, especially in wet conditions. I feel the brakes may fail at some point causing an accident. Twice I’ve had other vehicles slam their brakes on or attempt evasive action thinking I was going to hit them because the noise was so loud they hea
... rd it inside their vehicle. This did happen in a parking lot and not on the road. It is my understanding Toyota had a service bulletin to fix the issue under T-SB-0046-25 which was issued in July 2025, however to date, Toyota has not repaired my vehicle. It is my understanding that some vehicles were repaired under the bulletin but the fix did not correct the problem. I was told by my local Toyota dealer this week that Toyota told them to stop all repairs because the TSB was not correcting the issue and they were waiting for Toyota engineers to find a new solution. Request your assistance in getting Toyota to repair all the vehicles affected by this problem before a serious accident occurs because of brake failure or another driver taking evasive action when they hear this noise.Read more

This submission is an additional report related to NHTSA complaint #11714381. At approximately 55,000 miles, my 2021 Chevrolet Blazer experienced a sudden throttle body failure while driving. The vehicle entered reduced engine power/limp mode without warning and acceleration was significantly limited. This created a safety concern while driving in traffic, particularly when merging and crossing i
... ntersections. Diagnostic code P2101 (Throttle Actuator Position Performance) was present. A Chevrolet dealer confirmed an internal throttle body failure and replaced the throttle body assembly. Although the repair was completed, this failure occurred at relatively low mileage for a critical engine control component. A sudden loss of acceleration can pose a safety risk depending on traffic conditions. Since filing my original complaint, I have identified multiple similar reports online and in owner forums describing premature throttle body failures in GM vehicles with similar symptoms (reduced power/limp mode and limited acceleration). This suggests the possibility of a broader pattern involving this component. I am submitting this additional report so that NHTSA has a complete record of the incident and potential pattern of premature throttle body failures. If this issue is widespread, it may warrant further review or investigation as it affects vehicle acceleration and safe operation.Read more

One of the original OEM tires (Pirelli Scorpion MS KS NCS XL - 245/45R21) had the acoustic dampening foam on the inside of the tire fall apart. Dealership indicated that there was 'no impact damage or modifications found with the tire during inspection; meaning this was likely a manufacturing defect due to adhesion issue. Despite the problem and the fact that this tire has less than 12K miles on
... it at the time of the incident, the local Hyundai dealership, Hyundai corporate and Pirelli all refused to cover the cost to replace the tire. Hyundai dealer said to file a complaint with Hyundai corporate, Hyundai Corporate said that this was a Pirelli tire warranty issue (no paperwork provided about Pirelli tire warranty at time of purchase) and Pirelli indicated that only the local dealer or an authorized dealer of their tires can submit warranty claims, and that it is contingent upon a Pirelli inspection and prorated based on the remaining tread of the tire. This leaves the consumer in a difficult position having to bear the burden of the tire replacement cost upfront, and little chances of recourse as these 3 parties skirt responsibility of the OEM tire. When the acoustic foam fails, the car shakes violently at high speeds, for us this stats at ~50mph. There is no warning light that appears for this, dealer originally thought this was related to ice build up which did not rectify the problem causing us to have to go to the dealer twice for diagnostics. Online forums seem to point that this acoustic foam adhesion issue is especially common in colder climates. Symptoms of problem first appeared in early January, with full diagnosis confirmed early Feb.Read more

This complaint is about other cars affecting me while I’m driving at night. Around 50% of oncoming traffic is using dangerously bright headlights. It is so bright that it’s physically painful to my eyes and leaves a blind spot in my field of vision that lasts for several minutes after each encounter. The head lights are so bright that when highbeams are switched off manually, there is barely a
... difference. They are so bright when they are NOT on high beams that the turn signals are very difficult or literally impossible to see. Some amount of vehicles are using automatic highbeams that do not adapt properly. They are sometimes on full high beam brightness during daylight hours (and are just as painful to look at in daylight). They do not register my vehicle’s headlights or approaching traffic so instead of automatically dimming, I am being blinded by full high beams. Sometimes flashing my highbeams will trigger them to dim, but their vehicle has to be pretty close to mine for it to work. Their automatic highbeams do not dim when they are behind my car, causing my rear view and side mirrors to be completely unusable. Flipping the rear view mirror does not help. While sitting at a stoplight at an intersection, vehicles opposite to mine often maintain their highbeams and are too far from mine for flashing my highbeams at them to do anything. Some of the bright headlights (not on high beams) I believe are LED retrofit and this is probably why turn signals are barely or not at all visible with some vehicles. Some of these suspected LED retrofits are as bright as the ones on a fire truck or ambulance. I saw them side by side tonight, prompting me to finally write this complaint. This problem needs to be regulated for the safety of everyone, especially night drivers and cars that sit lower to the ground than SUV/trucks. My windshield and my eye level is at the same height as an SUV’s headlights. Maybe a lumens test as part of the yearly inspection would help.Read more

I am reporting a potential safety defect involving the driver seat lumbar support mechanism found in multiple vehicle models across the industry. The lumbar support feature applies excessive, uneven, or unstable pressure to the driver’s lower back during normal driving conditions. This occurs even when the feature is set to its lowest or neutral position. The defect presents in the following way
... s: •The lumbar support protrudes too far forward, forcing the spine into an unnatural posture. •The mechanism shifts or changes pressure during , creating sudden discomfort or pain. •Extended use results in lower back strain, muscle spasms, or nerve irritation. Safety Risk This issue poses a significant safety hazard because it directly affects the driver’s ability to maintain proper posture, comfort, and control of the vehicle. This pain is not noticeable until you step out of the vehicle and it difficult to trace that to the lumber support button as most people do not observed when it is on. The defect can: •Reduce reaction time due to discomfort or restricted movement •Increase the risk of long term musculoskeletal injury •Create fatigue or numbness that compromises safe operation Because this feature is present in many vehicles and consistently causes injury, the problem appears to be systemic and design related, not limited to a single manufacturer. Frequency of the Issue •Occurs: (e.g., every drive / frequent / intermittent) •First noticed on: (2020) •Conditions: (e.g., highway driving, long trips, stop and go traffic) Additional Notes •No aftermarket modifications have been made to the seat. •It may not be limited to Toyota vehicle only. I guess any vehicle driver seat that has lumber support is a suspect. •The issue persists regardless of seat height, tilt, or recline adjustments. •The problem appears inherent to the lumbar mechanism’s design rather than wear and tear. •The defect affects driver comfort, posture, and safe vehicle operation.Read more

My 2023 Yukon AT4 was taken to certified GMC dealer in November 2025 for the engine recall. The dealership gave my vehicle a clean bill of health at that time and the recall repair was cleared. On Friday, 2/13/26, myself and 5 other family members were en route to my godfather's veteran funeral service in Newtown, PA. We were going 75mph in the middle lane of the PA Turnpike when the motor seized
... up and vehicle lost all steering control and power. Since it was the side of the turnpike, we were limited in resources and stranded, missing the funeral service due to the length of time it took to get the vehicle loaded on a tow truck. Since the vehicle was unable to release the parking brake, the tow driver(s) were struggling to tow it. My husband, ended up having to go under the vehicle (on the side of the turnpike) to disengage the parking brake to help the tow drivers. This could have been a catastrophic situation. My vehicle was taken to a different GMC dealer than the original one that did the recall check. There are no rental vehicles available, so I am too out of a vehicle necessary for my work and family, with no time frame on the repair of my engine that only has 31,000 miles on it. Read more

The oil pressure switch on my 2019 KIA Sorento comes on when coming to a stop. This has occurred even after an oil change and checking the oil the dip stick showed full. I understand that Kia had a recall on these sensors for Kia Sorentos up to year models 2018 but evidently the problem with this sensor has not been resolved and presents a major possibility of fire occurring due to oil leakage. I
... believe Kia for safety sake should be required to replace this faulty sensor at no cost to the ownerRead more

The Integrated Charging Control Unit (ICCU) and High Voltage Fuse failed twice within 100 miles, causing a total loss of motive power while driving. The first failure occurred on Jan 24, 2026 (13,954 miles), and the second on Feb 2, 2026 (approx. 14,000 miles), immediately after the first repair attempt. In both instances, the 12V charging system failed, severing connection to the high-voltage bat
... tery and forcing the vehicle into a "limp mode" with reduced speed and steering loss in active traffic, creating a severe collision risk. The first authorized manufacturer technician stated that the issue was with the 12v battery and not the ICCU, however the second dealership confirmed that the issue was the ICCU all along, resulting in 17 days out of service. Symptoms included a loud "pop" followed by "Check Electric Vehicle System," "Stop Vehicle and Check Power Supply," and 12V battery warnings. Despite Recall SC327/SC302 procedures, the safety defect persists.Read more

While driving on a very busy highway going downhill when suddenly the vehicle started feeling abnormal like it was going to turn off automatically, heard an abnormal grinding noise coming from the engine the vehicle dropped speed drastically and the check engine light turned on , luckily we were on a part of the road where I merged quickly to the side of the road followed by a sudden engine shut
... down, the safety of myself and 3 other passengers was very compromised due to nearly being hit by other vehicles coming at a high speed on a main highway when my vehicle suddenly slowed down coming to a shut down due to the engine failure, I attempted to turn the car on again but would not turn on, was not getting any response at all, had to get the car towed, the vehicle did not have other warning lights before the failure, the check engine light came on after the engine failed, the vehicle was taken to an authorized Cadillac dealer where the engine was diagnosed as Engine seized, locked up and will not turn over. I received a Recall regarding Momentary Wheel Lock-Up which leads me to believe that might be related to what happened to my vehicle but did not have 8K dollars for a teardown to further inspect the root cause of my engine failure and definitely did not have over 11K dollars for a new engine/labor cost. The Cadillac dealer was quick to deny the possibility to be related to the recall without even doing a teardown of engine also the dealer denied the option to work on the recall repair due to the vehicle being inoperable.Read more
