- What component or system failed or malfunctioned, and is it available for inspection upon request? The rearview backup camera system seems to be misaligned or improperly calibrated or connecting to the wrong camera when put into reverse. When the vehicle is placed in reverse, the camera image points downward and primarily shows the rear bumper rather than the area behind the vehicle. This signif
... icantly limits the rear field of view that the camera is intended to provide. -How was your safety or the safety of others put at risk? Because the backup camera view points downward and does not clearly show the area behind the vehicle, it does not show me pedestrians, small children, vehicles or objects directly behind the vehicle when reversing. This creates a risk of backing into a person, vehicle, or object because the driver cannot rely on the camera display to safely check the rear area. - Has the problem been reproduced or confirmed by a dealer or independent service center? The dealership did confirm that this is an issue and advised me that they could not figure out what the problem is and sent me on my way without resolution. -Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The component has been inspected by the dealership. They confirmed it has an issue by comparing the view with another Escalade. -Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warning lamps, messages, or alerts appeared prior to or during the issue. The backup camera display functions normally but consistently shows an incorrect downward viewing angle that primarily displays the rear bumper instead of the area behind the vehicle.Read more

The vehicle is a 2025 Ford Maverick. The engine experienced a major internal failure that was later diagnosed by the dealership as a gouge in the #4 cylinder wall. This defect caused significant engine malfunction and required major repair/replacement. This issue which triggered a CEL created a serious safety concern because a failure of this type can result in sudden loss of engine power while
... driving, which increases the risk of a crash, especially when traveling at highway speeds or when attempting to merge into traffic. Loss of propulsion can place the driver and other motorists at risk. The problem was confirmed by an authorized Ford dealership, which inspected the vehicle and documented the gouged cylinder wall in cylinder #4. Repair documentation from the dealership is available.Read more

While operating the vehicle in reverse at a low speed (under 5 mph), the Advanced Driver Assistance System (ADAS) failed to provide any audible or visual alerts regarding a stationary vehicle positioned directly behind my path. The vehicle is equipped with a Rearview Video System and Rear Proximity/Parking Sensors, both of which were active. Despite a clear line of sight and the secondary vehic
... le being a standard passenger car, the sensors did not "ping" or trigger an emergency brake/alert, resulting in a rear-end collision. The sensors and camera lens were clean and free of debris at the time of the incident. This represents a failure of the safety system to perform its primary function of obstacle detection in a standard backing scenario, posing a significant risk for back-over incidents involving pedestrians or other vehicles.Read more

The transmission shift cable bushing at the transmission end of the shifter cable has completely degraded and detached, which is a component available for inspection upon request. This failure caused a significant safety risk as the gear shifter inside the cabin moved freely while the transmission remained in its last gear, creating an immediate rollaway risk and preventing the vehicle from being
... shifted into Park. The safety of others was put at risk because the vehicle could not be secured and could have moved in an unintended direction. This specific problem has been confirmed by a Ford dealership, yet the dealer refused to perform the necessary repair under Recall 22S43 (NHTSA Campaign 22V413), claiming the VIN was not covered despite the vehicle exhibiting the exact failure and being within the recalled model year and assembly plant range. Prior to this total failure, there were no warning lamps or messages, though the instrument cluster occasionally showed a gear position that did not match the physical shifter before the bushing finally disintegrated. The vehicle has not yet been inspected by the manufacturer or insurance representatives.Read more

On [XXX], I received a warning on my 2023 IONIQ 5 that said “stop vehicle and check braking system” and the car went into “turtle mode”. I took it into the dealer for repairs and they said the issue was related to the 12V battery and returned the car to me as repaired. Then, on [XXX], I was driving the car and I got the same error message but this time the car’s brakes didn’t work. Tha

I then had it towed to the dealer and they are currently in the process of repairing it. My son and I were in the car at the time of the [XXX] incident and were very fortunate that we did not hurt anyone or ourselves when the brakes didn’t work. As of today, the Hyundai dealer (Roseville, CA) says they are repairing a brake component that was defective but they missed this on the 2/28/26 inspection. on 2/27/26 I heard a knocking sound in my car, no warning lights came on, took it to local car place and was told there was no oil in the tank. I immediately made an appt to take it to River View Ford dealership for service. They looked at it on 3/2 and found the engine to be damaged from the turbos pulling oil out of the oil tank and sucking it into the engine. They informed me nothing was being
... offered to replace the engine for that year 2020 and my warranty just ended 11/25. This car is worth $70,000 new, just paid it off early, otherwise in great condition. I called the Customer service line and was told there is nothing they can do or offer and filed the complaint. My husband called back and spoke to 3 different rude people requesting someone call our dealer and talk with the service people- this didn't happen. Earlier this year this car had a differential leak that cost $3000; it also had the undercarriage of the car that holds the flex pipes up, fall apart and required the entire undercarriage to be replaced. I find it appalling that you google ST engine issues and they specifically say 2020 models have issues with and suffer from severe oil-related issues, including high oil consumption, oil starvation, and, in some cases, turbocharger failure or engine knocking. How is this not a recall? No warning signs til its too late.Read more

The vehicle was inspected by an authorized GMC dealership on 01/14/2026, confirming the leak and potential safety hazard with continued driving of the vehicle. The issue was submitted to the GM Customer Care team, Case #[XXX]. The only warning of the defective oil cooler line leak was oil on the ground underneath the vehicle after driving. Vehicle: 2026 Cadillac Escalade IQL (battery-electric SUV). At approximately 4,500 miles the vehicle began displaying battery/high-voltage system warnings and charging system errors. An earlier incident involved a critical battery warning instructing service. When presented to the dealership, the vehicle was initially described as potentially requiring extensive diagnostic disassembly. The deale
... rship later reversed that position and stated the issue could be addressed by repeatedly "cycling the ignition" approximately 30–40 times. No permanent repair or engineering explanation was provided. More recently, during an attempted charging session, the vehicle terminated charging due to a battery system error and displayed a red warning indicator requesting service. This prevented the vehicle from accepting charge and therefore prevented normal operation. In a battery-electric vehicle, the ability to charge is the functional equivalent of refueling. When the vehicle refuses charging due to system errors, the vehicle becomes effectively unusable and raises potential propulsion and safety concerns. When the vehicle was presented again for service, the dealership stated they had been instructed by the manufacturer not to perform service because the vehicle may be retrieved by the manufacturer. As a result, the vehicle continues to exhibit battery/charging system faults without a confirmed repair path. The issue involves the electrical system, propulsion system, and charging capability. The vehicle remains under very low mileage and the condition has not been corrected.Read more

The airbag system failed to deploy during a side-impact collision. The vehicle is a 2013 Chevrolet Camaro LS. The vehicle was declared totaled after the accident and may be available for inspection through the insurance company if requested. Description of the Incident and Safety Risk: While driving down the street and coming around a curve, another vehicle experienced a tire blowout and lost con
... trol. The driver attempted to correct the vehicle but crossed into my lane. I attempted to turn my vehicle toward the right to avoid a head-on collision, but the other vehicle T-boned my car on the driver’s side. The impact was severe. The other vehicle struck my car twice, which caused my vehicle to spin and come to rest on a curb. During the crash, none of the airbags deployed, including the driver airbag, despite the severity of the side impact. Because of the damage to the driver’s side door, it was crushed inward, and I had to pull my leg inward and exit through the passenger side door. During the collision, my head struck the steering wheel, and my back became very sore from the force of the impact. The failure of the airbags to deploy placed my safety at serious risk, as they could have helped prevent or reduce head and upper body injuries during the crash. Inspection / Investigation: The incident has been reported to the National Highway Traffic Safety Administration (NHTSA). The vehicle has been evaluated by the insurance company following the accident and was declared totaled. Warning Lights or Prior Symptoms: There were no warning lights, airbag alerts, or system messages prior to the crash. My 2025 Cadillac Escalade ESV went into the dealership for routine maintenance and several software updates were applied to vehicle along with an oil and filter change on 17 February. It felt like something mechanical wasn't working in the suspension. I dropped it back off at the dealership Monday 23 February. I picked up the vehicle later that day and the same error popped up again. We then gave it back to the dealership on the 24th of February to resolve and they kept if for over a week and were unable to determine root cause of the issue as of 2 March. The error code was reset by the dealership and was told to notify them again if it happens a third time. I believe there is a software issue that is causing problems with the adaptive air ride suspension.Read more

Component: Electronic Stability Control / Navigation / GPS Complaint: Since the February 2026 over-the-air software update, the built-in Google Maps GPS has been severely malfunctioning. The system consistently fails to update my vehicle’s location, showing me as stationary at my home address even after driving for 30+ minutes and 20+ miles away. This is not an intermittent glitch — it renders
... the native navigation system completely unusable after nearly every drive. Prior to the February 2026 update, the GPS functioned perfectly. The issue began immediately following that update and has persisted despite infotainment system resets and checking for further software updates. I am aware that GM has internally acknowledged this issue via Preliminary Information bulletin PIC6644 (issued February 2026), which attributes the problem to a “software anomaly” and confirms engineering is investigating. However, no fix or timeline has been provided to customers. A GPS system that does not reflect the vehicle’s actual location is a safety concern — particularly when relying on navigation for routing, hazard awareness, or emergency response. I am filing this complaint to ensure this issue is part of the public record and to urge NHTSA to monitor the scope of this defect across 2024 Lyriq vehiclesRead more

Incident date: Feb 25, 2026
I am reporting a safety concern involving BMW vehicles equipped with the B58 Technical Update (B58TU) engine, commonly found in model years 2019–2021. These vehicles utilize a variable-displacement engine oil pump containing an internal thermoplastic adjusting component responsible for regulating oil pressure. Field failures are increasingly being reported in which this internal component fract
... ures, resulting in sudden loss or instability of engine oil pressure. The failure frequently occurs during cold-start operation in low ambient temperatures. When the component fails, oil pressure regulation becomes erratic or is lost entirely. In many cases, drivers receive little or no actionable warning prior to engine damage or vehicle disablement. Reported precursor symptoms include inability to electronically measure engine oil level, abnormal oil pressure behavior, or sudden drivetrain malfunction warnings. Failures may rapidly progress to engine shutdown or severe mechanical damage caused by oil starvation. Loss of lubrication can cause the vehicle to become disabled without warning while driving or shortly after startup. This creates a safety hazard, particularly when vehicles become immobilized in active traffic or during winter conditions where occupants may be exposed to freezing temperatures while awaiting assistance. Replacement of the oil pump requires extensive disassembly of the vehicle and repairs commonly exceed $6,000–$8,000 when engine damage occurs. Owners are currently unable to determine whether their vehicles contain the original thermoplastic oil pump design or a later revised version utilizing metal internal components, as VIN-based parts information now identifies updated components even when no replacement has occurred. Failures have been reported across multiple production years, and affected vehicle population boundaries remain unclear.Read more

Suspension failed at 50,000 miles. At 56,000 miles one of the valve components inside the engine (a lifter) has collapsed. This part controls the opening of the engine valve, and because it is no longer moving, that cylinder cannot build compression and is causing the misfire. This is an internal mechanical failure inside the engine and cannot be caused by body work, fuel quality, or anything ext

My son, [XXX] is currently driving this car. The car only has 29,200 miles so far. Last week, while at the stop light on the hill, he had near fatal accident! Forester suddenly completed shut down. The only functioning part was the brake. My son had to put the car in park and restart the engine. I reported the incident to customer advocate at Subaru and taking the car in for inspection. We never s
... kip any scheduled maintenance. My next maintenance not due until April. I also purchased Subaru safety monitoring software. The car never shows any malfunction. It’s a very serious issue. We strongly suspect it’s related to the start-stop system on this car. We strongly support to override the system at consumer level giving their unreliability. Thank you for your attention to this matter. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On 5/8/2024, at approximately 18,969 miles, the vehicle suffered a sudden and complete loss of engine power while driving on the highway in the rain with my family inside. The engine died, the vehicle lost all propulsion creating an immediate and severe safety hazard. Dealer diagnosis revealed a catastrophic crankshaft main bearing failure, requiring a full engine replacement. Since the engine rep
... air, the vehicle has exhibited persistent intermittent/unintended brake activation. The brakes engage without driver input causing "Service Brake" messages, excessive and premature brake pad/rotor wear (replaced three times in a matter of months due to heat and dust from repeated activation), grinding, squealing, and locked caliper incidents. The vehicle has been inspected by two Cadillac Dealerships without any remedy except replacing warranty parts. An independent mechanic inspected the vehicle and confirmed the issue is likely related to the Advanced Driver Assistance Systems. To drive safely, we must disable the collision avoidance and traction control features. This has been reported on multiple repair orders and matches complaints from other 2023-2025 Escalade and GM SUV owners. Total documented downtime since first failure exceeds 160+ calendar days. Problems remained unresolved despite warranty repairs, software updates, recalls, and the L87 engine replacement. The sudden engine failure endangered my family and the ongoing phantom braking creates risk of collision or loss of control, especially at highway speeds. I have been forced to disable critical safety systems to prevent unintended braking. This vehicle is directly affected by the L87 defect that prompted federal recall and investigations. Please include my complaint in the ongoing EA25007 and RQ26001 investigations and consider my case in any further actions regarding the adequacy of GM's remedies. Please contact me for dealer repair orders and independent mechanic findings.Read more
