We own a 2024 Cadillac Lyriq EV. We began experiencing safety concerns related to battery performance and overall vehicle reliability during the active bumper-to-bumper warranty period. These concerns were reported to an authorized GM dealer, Napleton Cadillac, prior to warranty expiration. Service records at Napleton Cadillac document the history of these complaints. The vehicle has been unable
... to process required over-the-air (OTA) software updates through GM's intended automated update process. During a dealer visit on February 17, 2026, at GM's direction, the Napleton Cadillac service technician confirmed that the vehicle had multiple open notices and that our reported concerns are consistent with known issues affecting the 2024 Cadillac Lyriq across multiple units — indicating this is not an isolated occurrence. The dealer advised that correcting the update failure would require approximately five hours of manual labor at an estimated cost of $1,250 — a cost GM has declined to cover, despite the fact that the OTA update process is the manufacturer's intended method for delivering safety-critical corrections and the failure to update is a vehicle deficiency, not a customer-caused condition. Due to these unresolved safety concerns, we have not been driving the vehicle. We do not have confidence that the vehicle is operating safely or as designed. We formally requested a repurchase from General Motors through the EV Concierge program (Case No. 87486271). That request was denied on March 14, 2026, citing two rationales: (1) the vehicle is out of warranty, and (2) system updates do not qualify for repurchase. We believe these rationales fail to account for the fact that the original safety complaints were filed during the warranty period and that the root issue is battery and system safety — not a routine software preference. We are filing this complaint to create a formal federal safety record and to request that NHTSA evaluate an investigation or recall.Read more

COMPONENT: Vehicle Speed Control — Automatic Emergency Braking / Anti-Lock Braking System (ABS) COMPLAINT: I am filing this complaint to report a POST-REMEDY FAILURE of the repair performed under NHTSA Safety Recall 24V589 (GM Program #N242453471) on my 2024 Cadillac Lyriq AWD. RECALL REMEDY COMPLETED: My vehicle received the Electronic Brake Control Module (EBCM) software update on August 13, 2
... 024, under GM Program #N242453471. The recall was marked "Complete" by the servicing dealership on that date. FAILURE INCIDENT: On March 15, 2026, at low speed (under 25 mph) on a dry surface, my Lyriq failed to activate its Automatic Emergency Braking system while approaching a stationary object. The vehicle did not decelerate or stop as designed. The ABS system appeared to release braking pressure rather than maintain it, allowing the vehicle to continue forward and strike the object. The front grille assembly sustained significant physical damage as a direct result. This failure is identical in nature to the defect described in Recall 24V589 — specifically, the ABS falsely activating and releasing braking pressure during low-speed operation on a dry surface — despite the recall remedy having been applied and certified complete ten months prior. SAFETY CONCERN: The software remedy applied under Recall 24V589 did not correct the defect on my vehicle. This raises serious concerns about the effectiveness of the EBCM software update as a remedy across all affected vehicles. If other vehicles received the same software update and the defect persists, there is a significant ongoing safety risk to the public. I am requesting that NHTSA: Investigate whether the Recall 24V589 remedy (EBCM software update) has been effective across all affected vehicles Determine whether a supplemental or revised recall remedy is necessary Require GM/Cadillac to cover physical damages resulting from this post-remedy failure I have also contacted Cadillac Customer Care directRead more

The engine in my 2015 Cadillac Escalade failed unexpectedly at approximately 53,000 miles. Immediately prior to the failure, the engine developed a noticeable knocking noise, indicating a potential internal issue. While attempting to park in a public parking garage, the vehicle suddenly lost power and the engine shut down without any warning lights or alerts. This created a hazardous situation in
... a confined area with other vehicles and pedestrians, as I was unable to properly control or maneuver the vehicle. The sudden loss of power and complete engine shutdown significantly increased the risk of a collision or injury. The engine would not restart after the failure, leaving the vehicle disabled in the garage. Due to the confined space, the vehicle had to be removed using specialized towing equipment. The vehicle has been dealer-maintained since purchase, and I am the original owner. There were no prior major engine issues before this incident. The vehicle was towed to a dealership, where it was confirmed that the engine had seized and failed internally. I was quoted approximately $20,000 for a rebuilt engine replacement. This type of catastrophic engine failure at relatively low mileage, without warning, presents a serious safety concern.Read more

On February 21, 2026, my 2022 Cadillac Escalade ESV experienced a sudden and complete engine seizure at highway speed at 57,825 miles just 8,613 miles after passing GM's PicoScope inspection under Safety Recall N252494000 (NHTSA Campaign 25V274). After fueling, I entered a highway on-ramp and accelerated to approximately 70 mph. Within 30 seconds, the engine lost power completely with no prior war
... ning lights, no diagnostic trouble codes, and no audible warning. The only dashboard notification was a prompt to press the Start button. The vehicle behaved as if shifted into neutral, decelerating rapidly from 70 mph to 40 mph while I was navigating an exit merge ramp. I reached the right shoulder safely. Under normal weekday traffic, this failure on an active merge ramp could have caused a serious collision. The engine would not restart. The vehicle would not shift out of Park. OnStar could not remotely start the vehicle. A tow truck was dispatched; the operator could not shift the vehicle to neutral and required a dolly. The dealership confirmed the engine had seized and the engine would not crank. A complete engine replacement was performed under the recall warranty (RO 606490). The recall remedy was performed August 20, 2025 at 49,212 miles. GM records confirm the PicoScope result: "Engine Passed." The engine seized 8,613 miles later with no warning. This is consistent with 30-plus NHTSA complaints reporting loss of propulsion after the prior remedy, and demonstrates the inspection-based remedy is insufficient to identify engines at imminent risk of seizure.Read more

Vehicle: 2026 Chevrolet Silverado 1500 Incident Description: While driving at highway speed, the transmission fluid line failed and transmission fluid rapidly leaked from the vehicle. There were no warning lights, alerts, or messages before the failure occurred. As the fluid was lost, the vehicle began shutting down while still moving in traffic and stopped in the middle of the highway with vehic
... les behind the driver. The truck attempted to place itself into Park before it had fully stopped, and once stopped in the roadway it could not be shifted out of Park. The driver had to move the disabled vehicle off the roadway while traffic was approaching from behind. Safety Concern: The vehicle experienced a sudden loss of transmission fluid and shut down in an active traffic lane without any warning, creating a serious safety hazard for the driver and surrounding vehicles. Photos show the truck stopped in the middle of a busy highway with traffic passing while the owner documented transmission fluid pouring from underneath the vehicle. Additional photos show the dealership repair report stating the transmission cooler line was left unclipped from the fan shroud assembly, allowing it to contact the A/C compressor clutch and wear a hole in the line, causing rapid transmission fluid loss. The service manager also stated that other trucks have arrived from the factory with the transmission cooler line not properly clipped or secured. Videos available as needed. Since this incident and repair, the vehicle has continued to exhibit abnormal drivetrain behavior including shuddering and harsh or unexpected downshifts during acceleration and when using cruise control. The vehicle has been returned to the dealership multiple times for inspection, but no confirmed cause or documented successful repair has been provided.Read more
