I have had issues with oil consumption in this vehicle for about 2 years. I have taken it to the dealership Vandergriff Hyundai in arlington, tx advising the oil light keeps coming on before oil change is due this was before my warranty had expired on my vehicle. I was told that they could not find anything wrong and a oil change was completed, but no oil consumption test was done. I asked about a
... ny recall for knock sensor and i was advised that had already been completed. I have no proof of this being completed. After researching the issue, I discovered that this is a known engine defect covered under Hyundai Technical Service Bulletin 23-EM-008H, which relates to excessive oil consumption. This condition can lead to engine failure, posing a significant safety risk if the engine fails while driving at high speeds or in traffic. This issue is part of an active class-action lawsuit and has affected numerous vehicles with the same engine type. I had my oil changed on April 26th, 2025 and as of June 24, 2025 it was 3 quartz low. this is not normal and it is dangerous.Read more

On April 4, 2025, my wife was driving at about 40 miles per hour when her vehicle struck an obstruction in the road. The impact caused relatively minor collision damage to the vehicle (a bent wheel strut). However the side curtain airbags deployed, startling my wife and limiting her vision on either side of the vehicle. Fortunately She was able maintain control of the vehicle, steer to a safe loca
... tion and stop off the roadway. When inspected by the auto body repair shop where the vehicle is currently being repaired, the technicians were puzzled that the side airbags went off for such relatively minor accident, particularly since the impact to the vehicle had been to the front and not the sides. Under the circumstances, the premature deployment of the airbags could have actually made the accident worse if my wife had lost control of the vehicle. Under the circumstances, we felt compelled to report the incident to you for investigation in the event other RAV4 owners are experiencing the same problem.Read more

Reporting an important safety issue I experienced due to random engine stalling on my 2003 Honda Pilot, typically after driving for a few minutes. The engine would crank for several seconds or sometimes start immediately after stalling, but no diagnostic trouble codes were ever stored. The vehicle had stable fuel trims and idle, and multiple components including the EGR valve and valve clearance w
... ere serviced without resolving the issue. Ultimately, the root cause was traced to a failing PGM-FI (fuel pump) relay located under the driver’s dash. After replacing the relay close to two months ago, the stalling stopped completely. This issue could pose a safety risk if the engine stalls in traffic, especially in a hazardous area where there’s no breakdown lane (construction zone, etc.) , and I believe this factor should consider this a known failure point in aging vehicles of this model. Attached are two images of the new relay zip tied under the driver’s footwell (could not remove the bad relay since it was too difficult to remove from its bracket).Read more

I am reporting a serious safety concern involving my 2021 Chevrolet Blazer, which experienced brake failure while driving, without any warning lights or diagnostic alerts from the vehicle’s onboard system. This created a highly dangerous situation for myself and others on the road. Prior to this incident, I brought the vehicle to the dealership after consistently hearing squealing or screeching
... noises when braking. The issue was dismissed and attributed to a change in federal regulations limiting the use of copper in brake pads—without any further inspection. This resulted in a missed opportunity to diagnose what appears to be a deeper mechanical failure. After the brake failure, service technicians discovered that the vacuum pump had failed, which impacted braking performance and also caused engine damage. While I’ve since learned that GM has acknowledged vacuum pump failures in several vehicles manufactured between 2014–2018 (via formal recalls), the 2021 Chevy Blazer is not currently included in those campaigns However, the symptoms and outcome I experienced are strikingly similar to the documented defects in older GM models. The lack of early diagnostic detection, coupled with dealership dismissal of clear warning signs, signals a potential emerging safety defect in this newer model that may not yet be officially recognized. Given the safety-critical nature of the braking system and the failure of both mechanical and procedural safeguards, I strongly urge NHTSA to investigate whether this issue is more widespread in recent GM models, including the 2021 Blazer. Other consumers may be at risk without knowing it.Read more
