To whom it may concern, I was on a back road late in the evening not far from home [XXX] and a small deer ran into the road. I needed to brake to avoid hitting the deer. After breaking, a Brake System Malfunction warning message and i-ACTIVESENSE System Malfunction warning illuminated on my dashboard. I brought the car to a Mazda dealership the next morning. The technician determined that the br
... ake fluid level sensor became stuck, requiring replacement of the brake fluid reservoir and the brake fluid level sensor. What failed: The repairs include replacement of the Brake Fluid Reservoir and the Brake Fluid Level Sensor. This is the link to the dealer report [XXX] . No one other than the dealer has inspected these parts to my knowledge. There was no accident of any kind. I personally felt unsafe after these parts failing and reached out to Mazda Customer Care to request investigation of the issue. The case manager just called me back today to say they could not help me. Mazda has denied warranty coverage, claiming the parts are not covered. However, I really do feel this is a critical safety system component that failed after normal emergency use with no collision or impact. Why I feel this is a safety concern: - The brake warning system became unreliable after a single emergency stop. - My car only has 42k miles and is in excellent condition and is fully dealer serviced. - A driver could lose the ability to receive proper brake system warnings if this failure occurs again. - A critical brake sensor should not fail under normal driving conditions, and its failure could lead to delayed driver response in an actual brake fluid loss scenario. It makes me worry for the future. For your consideration: I believe this represents a safety-related defect that should be investigated, as it affects the reliability of the brake system warning indicators in the 2019 Mazda 3. I appreciate your time and consideration. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

When I first brought the car for its first service appointment (6 months after purchase), I informed the service dept at the dealership that the car seemed to accelerate itself when I was driving. I took the car in again for service a few months ahead of time and they said that my car mat was wedged under the break pad and that was what was causing the car to slide when I hit the break. The car slid but also accelerated by about 5 miles, however I was able to quickly break and stop it without incident. This last service was on Friday, August 1st, and on Sunday August 3, the car slid and accelerated again - without the floor mat being wedged under the break. On Monday morning (August 4th) I called Hyundai so many times that I lost count. I was never able to speak with a service employee but I was given an appointment for a week and half later. According to ChatGPT, it is a common issue with the 2022 Kona and it should be recalled.Read more

While driving my 2021 Cadillac Escalade to Utah, the engine completely locked up and failed without warning. The vehicle had to be towed to a GM-authorized dealership. Upon inspection, the dealership confirmed the failure was directly related to an active safety recall on the vehicle. Prior to the failure, there were no check engine lights or no other warning lamps, messages, or symptoms. The fai

The dealership stated the needed part is currently on national backorder and that the vehicle cannot be repaired or safely driven until the part arrives. ... into the passenger-side floorboard) Yes, the vehicle is currently with the dealership (Stamford Hyundai, CT) and is available for inspection. 2. How was your safety or the safety of others put at risk? • The engine has shut off without warning while the vehicle was in motion, including in traffic, creating the potential for rear-end collisions. • The emergency brake has slammed on while reversing, without any obstacles present, risking whiplash and collisions with pedestrians or nearby objects. • The brake pedal becomes unresponsive, with a loose/rattling sensation, making it difficult to slow or stop the vehicle safely. • The HVAC leak creates slippery floor conditions, posing a secondary risk for occupants during ingress/egress. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? The dealership (Stamford Hyundai) acknowledged some issues but stated they were unable to replicate all of them. However, the brake pedal defect is consistently reproducible and occurs daily. Multiple videos and logs have been submitted to Hyundai’s consumer affairs team. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Summary of Problem: The infotainment screen on my 2018 Subaru Outback has delaminated and is malfunctioning, causing multiple safety issues. This is a known manufacturer defect previously addressed in a class-action lawsuit (Starlink Infotainment System Litigation, settled 2021). However, my vehicle’s screen failed shortly after the extended warranty expired. Detailed Description of Safety Haza
... rds: The malfunctioning screen creates the following safety risks: 1. Backup Camera Obstruction – The screen dims erratically, sometimes going completely dark, rendering the rearview camera unusable. This poses a serious safety hazard and appears to violate FMVSS 111 requirements for rear visibility systems. 2. Unintended Screen Inputs – The screen exhibits “ghost touches” that result in random phone calls, volume changes, and navigation menu disruptions, all without user input. This creates an unsafe level of driver distraction. 3. Driver Distraction and Interference – The system's erratic behavior pulls my attention away from the road, as I must manage or override its malfunctions, increasing the risk of an accident. Manufacturer Response: I contacted Subaru of America, who acknowledged the issue but only offered to cover half of the $1,200 repair cost. They declined full assistance, citing the expiration of the class-action extended warranty (5 years/100,000 miles), despite this being a latent defect with safety implications. Known Precedent: Many other Outback owners have received full coverage from Subaru for this exact issue, even outside of the warranty, suggesting inconsistent application of goodwill support. This is clearly a systemic defect. Requested Action: I am reporting this because the defect affects safety-critical features. I am requesting NHTSA to investigate whether this issue warrants a broader recall or compliance action. The unpredictable failure of the rearview camera and screen functionality poses a clear risk to driver and public safety.Read more

I believe the issue involved Super Cruise, forward collision warning, and automatic emergency braking systems. The vehicle is currently at the dealership and available for inspection. It has been there since 6/22/25 because the brakes and rotors are on backorder. While driving on the highway in the middle lane on a Sunday afternoon, I activated adaptive cruise after the vehicle had been parked for
... over 2 hours. After driving seven miles, the car suddenly jolted to a stop in the middle lane, as if we had been hit. We hadn’t—but cars were forced to swerve around us at highway speeds. If traffic had been heavier, this could have caused a serious crash. I exited safely to a mall parking lot, where the car began smoking from the brake area and gave multiple warnings, including “Reduce braking to avoid overheating.” The car remained parked, but the messages continued. A tow truck driver later said the brakes appeared to have caught fire because of how charred they were. I have video of the smoke and warning messages. This is a major safety concern. No. The issue has not been reproduced due to the parts being unavailable. The car has remained at the dealer since 6/22/25. Not to my knowledge. Only the dealer has had physical access to the vehicle since the incident. Just before the event, I saw a brief message on the screen, possibly “Auto Collision Not Engaged.” I had used Super Cruise previously without issue. What component or system failed or malfunctioned, and is it available for inspection upon request? My steering rack is damaged and needs replacement (confirmed by Honda Dealership). I just purchased (2 months ago) a 2012 Ford Expedition EL 5.4l v8 from a private party. I wish I would have done more research as I am ALREADY EXPERIENCING TRANSMISSION ISSUES THAT I BELIEVE ARE DIRECTLY RELATED TO THE VEHICLES LEAD FRAME RECALL. IT THREW A P0742 CODE ON ME Torque Converter Clutch Circuit stuck on and decided that it would only drive in limp mode unless I cleared the code, pulled
... over, cycles the key, and restarted the vehicle. Even as dangerous as this UNEXPECTED SHIFTING INTO 1ST GEAR AT HIGHWAY SPEEDS IS, IT IS EQUALLY AS DANGEROUS TO BE GETTING OUT OF YOUR VEHICLE IN MOVING TRAFFIC DUE TO FORD'S TERRIBLE DESIGN ON THIS VEHICLES POWER TRAIN/TRANSMISSION SYSTEM. I HAVE SCHEDULED AN APPOINTMENT WITH THE LOCAL Ford DEALER AND AL CERTAIN I'LL GET THE RUNAROUND/BLAME GAME FOR THIS ONE. 150K MILES AND AS I said, I HAVE OWNED IT FOR 2 MONTHS AND HAVE ALREADY HAD TRANSMISSION ISSUES, BRAKE ISSUES, BELT TENSIONER PULLEY LITERALLY FELL OFF.. AND FROM WHAT I READ, WHAT'S NEXT??? THE CAM PHASERS?? THAT'S IF THE TRANSMISSION ISSUE IS EVEN FIXED AND IT BECOMES SAFELY DRIVE ABLE AGAIN. COME ON FORD, GET YOUR ACT TOGETHER, YOU'RE BETTER THAN THIS!Read more

The embedded navigation system in my 2018 GMC Sierra is no longer supported. Paid map updates were available until 2023, when General Motors discontinued them without notice. This system was marketed as a core feature and provided offline GPS guidance independent of cell reception. I discovered the change while attempting to purchase an update and was told there are no remaining copies and that no
... further updates will ever be made. I was never notified. When I contacted support, I was transferred through six departments. One representative admitted that customers were supposed to be informed but were not. She stated they are trained to say “there’s nothing we can do” and acknowledged receiving hundreds of similar complaints. She also confirmed the final map update existed but was allowed to sell out. This puts safety at risk, particularly for drivers in rural areas, areas with poor cell service, or while towing. The system is not CarPlay-based and does not rely on mobile data. It was the only in-vehicle routing system many drivers depended on during travel, including emergencies. Its quiet discontinuation means drivers no longer have access to accurate, integrated navigation—reducing their ability to safely plan or adjust routes while on the road. The failure to maintain this system or provide reasonable access to the final update undermines long-term vehicle safety and owner trust. General Motors has not offered an inspection, recovery, or remedy.Read more
