Over ~3 years of ownership, the vehicle has experienced recurrent electrical and engine-management issues, resulting in 15+ service visits and ~100 cumulative days out of service at an authorized dealer. Patterns include: (1) electrical—keyless entry intermittently unresponsive, instrument cluster flickering/rebooting, multiple warning messages including a battery/electrical icon, HVAC/A/C becom
... ing inoperative without cooling; and (2) powertrain/engine—check-engine light (MIL) illuminated, hesitation/limited power on initial acceleration, and rough operation after start. Safety risk: intermittent failure to unlock/start can leave occupants stranded; loss of power/hesitation creates risk when merging or crossing intersections; multiple warning messages/flicker are distracting; and loss of A/C in hot conditions has required operation with windows down to avoid heat stress. Reproduction/confirmation: The concerns have been observed by the dealer on multiple visits with fault codes recorded (codes UNKNOWN to me). Repairs/updates have been attempted, but symptoms recur after service. The vehicle has been inspected repeatedly by the authorized dealer and remains available for inspection upon request. Warnings/symptoms prior to failure: intermittent MIL, battery/electrical warning, cluster glitches/flicker, and A/C blowing warm air; some events followed periods where the vehicle would not unlock or start on the first attempt. Root cause: UNKNOWN.Read more

While driving at night in a poorly lit area, I could barely see. I turned on the high beam but it blinded on coming traffic. No recall issued even though Nissan was well aware of this. But when I tried to get it fixed, the dealership

Incident date: Sep 7, 2023
The contact owns a 2019 Ford F-150. The contact stated that the vehicle lost motive power while depressing the accelerator pedal, and hesitated while accelerating after a complete stop. The contact stated that occasionally the check engine warning light illuminated, and the dipstick was checked, and the engine oil was low. The vehicle was taken to a dealer, and an oil and oil filter change was per
... formed as part of an oil consumption test. On the next visit for an oil change, the dealer discovered that the engine was consuming a quart of engine oil between oil changes. The contact inspected the vehicle and discovered that the engine was consuming three quarts of engine oil between oil changes. The vehicle was taken back to the dealer ,where a diagnostic was performed, and the contact was informed that the engine needed to be replaced. The manufacturer was notified of the engine failure and the manufacturer spoke with the dealer about the repair. The contact was later informed that the manufacturer would not assist with the cost of replacing the engine. The manufacturer referred the contact to the NHTSA Hotline to report the failure. Recently, while driving 15 MPH and towing a trailer, the electric power steering assist failed and the messages "Steering Assist Fault", Traction Control" and "ABS Fault" were displayed. The contact used excessive force to steer the vehicle back to the residence. The vehicle was taken back to the dealer and was diagnosed with a defective rack and pinion. The vehicle was not repaired. The failure mileage was approximately 55,000.Read more

I am submitting a safety concern with my 2016 Chevrolet Colorado truck. This vehicle is experiencing intermittent loss of power steering assist, which poses a significant safety risk. This issue mirrors the defect described in the GM Recall 15595A, which affected 2015 Chevrolet Canyon and GMC Canyon models. The root cause appears to be a poor electrical connection in the torque sensor harness co

I urge the NHSTA to investigate and consider expanding the scope of Recall 15595A to include affected 2016 models. I am writing to express my serious concern and frustration regarding the unresolved recall on my 2020 Chevrolet Silverado 2500 (VIN: [XXX] ). Despite multiple visits to multiple dealerships, I was repeatedly told that the necessary parts to complete the recall repair were on back order and instructed to “wait for parts.” No interim solution or assistance was ever offered. As a result of this
... delay, my vehicle broke down without warning, leaving me stranded on the side of the road. This created a hazardous situation and significant disruption to my day. To make matters worse, I had to pay out of pocket to have the truck towed to a repair shop in an effort to get it operational—an expense I should never have incurred given the known recall and repeated attempts to have the issue corrected by Chevrolet. I have been a loyal Chevrolet owner and have always trusted the brand for its quality and dependability. However, this delay and lack of proactive support is causing significant inconvenience and eroding that trust. This situation is unacceptable for several reasons: 1.Safety Risk and Recall Obligation: Chevrolet acknowledged the defect by issuing a recall, yet I have been forced to operate a vehicle that does not meet your own safety standards for months. 2.Repeated Service Attempts with No Resolution: I followed the proper process and visited an authorized dealer multiple times, only to be turned away each time with no solution beyond “wait for parts.” 3.Breakdown and Financial Loss: The vehicle failure and subsequent towing expense are a direct consequence of Chevrolet’s inability to provide timely parts or alternative accommodations. Dear Sir/Madam, I own a 2016 Ford F-150. Over the past few months the vehicle will suddenly hesitate and downshift while driving and the mechanics symbol will appear on the dashboard. This happened to me a few times. At the end of June I had two different mechanics look at the vehicle and both kept the truck for a couple of days and drove it around and neither one could see a problem. I drove the
... vehicle from Rhode Island to Pennsylvania and this happened 4 times during the trip driving on the highway!!!! It then happened again (with a witness in the vehicle) while driving. I could have been killed driving 70 mph on the highway during these times that the vehicle hesitated and downshifted. I researched possible recalls that I might have missed and learned of the investigation into by NHTSA by other consumers and I contacted Ford. When I spoke to Ford and filed my complaint I let them know I knew about the investigation by NHTSA and told them I could have been killed. It has been two weeks and I have yet to hear from them. I would like my vehicle repaired free of charge. We paid thousands of dollars for that truck and since this is a safety issue it should be recalled. Please contact me if you have any other questions. The OnStar Telematics Module has malfunctioned and is causing SAFETY FEATURES to malfunction. It is causing the following to occur to the vehicle: 1. The faulty module is causing the car to shut OFF UNEXPECTEDLY MID DRIVE. Losing control of the vehicle instantly. No steering, no gas, no brake, no gear change. THIS can cause a CATASTROPHIC ACCIDENT at higher speeds on the highways. The battery is
... completely drained when technicians inspect it but the alternator is working properly. 2. Causes the vehicle’s safety features to malfunction and give alerts such as “Service Driver Assist”, “Lane Departure Unavailable”, “Forward Collision Assist Unavailable”, “Steering Control Unavailable Soon”. 3. Causes GPS, WiFi, and location system to fail. 4. OnStar safety button do not work 5. Causes the engine light to go on and off and causes the cars idling to run erratically. 6. The module causes NEW batteries to drain quickly, less than a year after purchase. I have had the car shut off on me twice MID DRIVE, I was lucky to be in areas of less concern and harm. I’ve had to purchase two batteries since the module malfunctioned. Less than a year apart. The dealership confirmed it was the Telematics Module January 2025. They have confirmed the part is on backorder and not covered under warranty. NO INDICATORS were given when the car shut off BOTH times that I may have noticed or heard.Read more

I have been experiencing an intermittent issue where the vehicle displays a “Shift to Park” message, even though the gear shift lever is clearly in the Park position. When this happens, the vehicle will not fully shut off, and I have to repeatedly move the shifter back and forth or restart the vehicle to clear the message. This issue began around January 2024, and it has become more frequent
... over time. I understand that this is a known defect in GM vehicles, and that GM issued Technical Service Bulletin #19-NA-206 to address it. According to the TSB and owner reports, this problem is caused by a faulty park switch or microswitch inside the shifter assembly, and the solution involves installing a jumper harness and/or replacing the shift control assembly. This is both a functional and safety concern, as the vehicle may not recognize it's in Park and could potentially result in a failure to turn off or secure properly. I’m requesting that General Motors or an authorized dealer perform the recommended repair per TSB #19-NA-206 at no cost, as this is a widely reported manufacturing defect. Please advise how this issue can be resolved as soon as possible. I am attempting at uploading a photo and a video. The system does not allow videos and it is rejecting the photo.Read more

I purchased my vehicle in 2020. In 2022 I experienced an issue where my car stopped in the middle of the road without warning. One office tried to put the vehicle in neutral but the wheels locked and the shifter
... was locked as well. The battery has been replaced 3 different times since I have owned the vehicle. A mechanic friend of mine (who is a trainer for GMC) hooked his computer up to it and found no issues. He did find a loose wire which he the reconnected. I was able to drive my car for 6 months before the same thing happened, same situation the vehicle just stopped in the middle of the road only this time I couldn't turn the vehicle off, we had to manually disconnect the battery and again the police came out due to traffic hold up. My car was towed twice afterwards as I could get the car running for a few minutes before another error. To date my car has been sitting in my driveway since March of this year, this time it won't hold a charge and will not even start. Im not sure if this is an engine or an electrical issue. I did call 2 Buick dealerships that told me that it would take about 5 hours to diagnose the issue where I would be charged per hour at the mechanic hourly rate. At this point I think my vehicle needs to be recalled, even if I could get it repaired I dont feel safe driving it. Thankfully it stopped on a minor freeway and no major incidents occurred.Read more

This was the hottest day of the summer, and I was left in terror, fully aware that one wrong move would have ended our lives instantly. The fact that my child could have lost their only parent because of a mechanical defect is beyond unacceptable. The stress, fear, and trauma of this incident cannot be overstated—it was a near-fatal event that never should have happened. My mechanic has since confirmed the transmission is destroyed, with black metal fragments throughout—clear evidence of internal failure. Only two months earlier, in June 2024, the car passed inspection with no codes or signs of a problem. This was not wear and tear; it was a hidden, catastrophic defect. In December 2024, GM recalled more than 461,000 vehicles, including certain 2018 Enclave models, for transmission defects leading to rollaway and internal failure. Source: The Lemon Firm, 12/18/24 [XXX] ). My case mirrors the defect exactly, yet I never received recall notice. I lease a 2024 Ford Mustang Mach-E. On [XXX] the vehicle suddenly displayed a High-Voltage Battery Fault, Powertrain Malfunction / Reduced Power warning. The dealer replaced the High-Voltage Junction Box (HVJB), however I am unsure whether the actual root cause was ever identified. This failure shows a major defect in the high-voltage electrical system. Separately, my vehicle is also subject to R
... ecall 25S65 / NHTSA 25V404, where a discharged 12-volt battery can prevent the electronic door latches from opening, risking passenger entrapment. Ford has no remedy available and has only projected a future software update. My car is still at the dealership. These two issues are connected: the HV system (through the HVJB) powers the DC-DC converter that keeps the 12-volt battery charged. A failure on the HV side can starve the 12-volt system, which directly creates the entrapment risk described in the recall. I live in Florida, where extreme heat makes this risk especially dangerous for children, pets, and elderly passengers. What is my emergency plan...carry a hammer to break the windows on every trip? This is an unreasonable and unsafe expectation for consumers. Ford has not provided any fix or assurance of safety. I have specifically asked Ford whether my car is safe to drive — they have not provided any assurances, merely repeating that Ford has not issued a stop-driving order on the vehicle. This unresolved defect substantially impairs the use, value, and safety of my vehicle, and Ford has failed to provide an adequate repair. The automatic transmission in my 2024 Nissan Titan PRO-4X intermittently malfunctions when slowing to a stop. As the vehicle decelerates to about 1–2 mph, there is a distinct “bump” or “lurch” as if the truck has been lightly rear-ended. The sensation occurs most often at traffic lights or rolling stops. •Component/system failed: Power Train – Automatic Transmission (9-speed). Vehic
... le is available for inspection upon request. •Safety risk: The unexpected jolt can cause distraction and abrupt braking. It raises concerns about long-term durability and could increase the risk of a rear-end collision in traffic. •Reproduced/confirmed: Yes. I reported this under warranty to a Nissan dealer. The dealer replaced a transmission bracket, but the issue persists. The downshift/lurch can still be reproduced. •Inspected by others: The dealer service department has inspected it. Manufacturer has not provided a lasting fix. •Warning lamps/messages: No warning lights or diagnostic messages appear before, during, or after the issue. •First appearance: Issue began when I purchased the vehicle (at approx 4700 miles) and continues intermittently (currently 19000 Miles). •Similar complaints: Other 2024 Nissan Titan owners have reported the same problem publicly, describing the downshift bump/lurch at low speed. Earlier Titan model years (2020–2023) had recalls and TSBs for TCM/ECM software updates (e.g., Campaigns R22B1, R22A9; TSBs NTB21-028A, NTB22-085) addressing low-speed hesitation and harsh downshifts. My request: I am requesting NHTSA investigate whether the 2024 Titan shares the same transmission calibration defect as prior years and determine if Nissan must provide updated software or other corrective measures to fix this unsafe low-speed downshift behavior. the adaptive cruise control, collision warning, lane keep assist and other safety features have stopped working as indicated by errors displayed on dashboard. Suspect front camera is malfunctioning but has not been confirmed by independent service center. At first, this seemed to be an intermittent issue, but now the error messages appear every time the car is used.

The bearings went out on cylinder number 5. The engine is torn down at the present time at the dealership. There was no warning. It is our understanding that other 3.5 l engines have had this happen. Our vehicle just wasn’t included in this recall. I think the recall needs expanded.

The contact was driving a company-owned 2017 Toyota Prius C. The contact stated that while driving at 65 MPH, another driver merging onto the highway attempted to cross multiple lanes of traffic, misjudged, and collided with the front passenger-side fender of the contact’s vehicle. As a result, the contact’s vehicle struck the left shoulder barrier, became airborne, and overturned three times
... before coming to rest in the middle of the roadway. Despite the severity of the crash, the air bags did not deploy. No warning lights were illuminated. The vehicle was towed to a storage facility and deemed a total loss by the employer’s insurance provider. A police report was filed, and injuries were reported. The contact was transported by EMTs to a local hospital and treated for multiple nondescript injuries, including a damaged right rotator cuff and body pain. The contact also underwent chest and torso X-rays due to concerns of possible internal bleeding. The manufacturer was not notified of the failure. The failure mileage was unknown.Read more
