My 2024 BMW X5 experienced a complete megatronics failure at 54,500 miles. The megatronics unit controls critical vehicle functions including transmission and engine management. This failure caused the vehicle to become completely inoperable — posing a significant safety risk. This failure occurred just 4,500 miles beyond the factory warranty on a barely 2 year old vehicle. A 2024 mo
... del year vehicle should not experience such a critical safety-related failure at this mileage. BMW Corporate has been unresponsive for 7 days despite opening case number XXX and promising a 3-5 business day response. I am requesting NHTSA investigate whether this is a widespread defect that may affect other 2024 BMW X5 owners. Incident Description: "Vehicle suddenly entered limp mode while driving at highway speed on XXX in [XXX] . This caused an unexpected and dangerous loss of power while surrounded by highway traffic creating a significant safety hazard. Vehicle was unable to maintain highway speeds and had to be carefully maneuvered to the shoulder of the road. Vehicle was subsequently driven to BMW Dealership in [XXX] where it was diagnosed with a complete megatronics failure." Sudden loss of propulsion at highway speeds - 2023 Escalade. We easily could have been killed. I drive a 2023 Cadillac Escalade that I purchased from Motor Werks Cadillac in [XXX] . It was very expensive. The car has under 33k miles on it, off memory.I informed the dealer I would be taking the car on a family vacation road trip from [XXX] to [XXX] (for spring break), and requested they perform a
... ny necessary preventative service on it to ensure it was in perfect shape to make the trip. They performed the service (basic air filter change etc) and returned the car to me two days before I packed my family [XXX] and [XXX] daughters and wife) into the car and began the journey. After driving less than three hours, the engine completely and suddenly died, without warning, while we were traveling at 75 miles an hour on the highway in the left lane in the middle of rural [XXX] . At the same time all of the electronics in the vehicle died as well. It was an extremely dangerous situation because the road is heavily trafficked by semi trucks, which I was barely able to avoid as I attempted to coast across all the lanes of traffic to get onto the shoulder of the road on the right hand side. Because the car was without propulsion, I was not able to get fully onto the shoulder, but mostly. This left the car in a precarious position because the semi's that we're driving by could easily have hit the car had they not been careful and veered slightly to the left. We easily could have been killed. There's more to the story, like the dealer telling me Cadillac is aware that catastrophic engine failure at highway speeds is a known issue impacting Escalades around the country, and including significant mental trauma that my children experienced by being stranded on the side of the road for several hours in the dark in a car that had no engine, no electronics and no heat. My 2024 Ford Ranger Raptor has been suddenly losing power or stumbling upon initial acceleration. This has been occurring intermittently since it was nearly new. My truck now has approximately 10,500 miles. The problem has become increasingly frequent. It seems to occur most often when accelerating from a complete stop. The loss of power can last for several seconds, then afterwards, normal power
... is regained. It poses a safety risk since it has nearly caused collisions. Several times, the truck has lost power while making a left turn in front of oncoming traffic. It simply will not accelerate, and I find that I am stuck, unable to complete my turn and clear the oncoming traffic. Once when accelerating away from a stoplight, the truck lost power, and I was nearly struck by following vehicle. I have taken my truck to a local dealer who attempted to diagnose the problem and they didn’t offer a solution since they were unable to reproduce the problem during their road test. It has happened many times since then and I am scheduled to return the vehicle to the same dealership to attempt to diagnose and fix the problem again soon. The problem seems to occur, regardless of which drive mode the truck is in and also occurs with Electronic Stability Control both enabled and disabled. There are no warning lights or check engine codes shown.Read more

On or about 02/09/2026, the vehicle experienced a catastrophic failure directly related to a known defect identified in Technical Service Bulletin 19-NA-066. The engine "sucked" in the oil filter due to a poor design on GM's part. The stand pipe was not present apparently, and must have broken off at some point. This bulletin acknowledges an issue affecting this vehicle platform. This should have
... been recalled to address this engineering flaw that GM acknowledged in their TSB. Despite multiple attempts and ongoing communication with authorized representatives to have the vehicles engine replaced under an extended warranty, the vehicle was inoperable and unusable, causing significant financial hardship. I owned the vehicle for approximately 5 months and had the vehicle in the shop multiple times for other repair issues that were known per forums from other owners of this model/year of vehicle. I have exhausted all reasonable efforts to resolve this matter through dealership, GM customer service channels, and GM's executive team without satisfactory resolution. As a direct result of this defect and resulting failure, I have incurred the following damages: Loss of use of the vehicle ($24,578 purchase price on 09/20/2025) Rental and alternative transportation expenses ($90.58 rental vehicle from 02/12/2026 - 02/14/2026) Diminished vehicle value ($24,578 - $12,000 (Selling Price) = $12,578) Out-of-pocket diagnostic and related expenses ($2,036.50) Total loss and seeking reimbursement: $14,705.08Read more

I am reporting multiple safety-related issues involving my 2016 Chrysler 200 S. On 02/06/2024, after an oil change at a dealership, the underbody skid plate bolts were not reinstalled. In July 2022, following a battery replacement by the same dealership, the vehicle repeatedly lost power and stalled, leaving me stranded multiple times. I paid for diagnostic testing, but there is no documentation confirming proper diagnosis or repair. I was later advised incorrect hardware had been installed. On 02/27/2026, I was told the check engine light was on and diagnostic codes were cleared without repair. There is no documentation of the codes or repairs performed. I was also informed on 11/19/2022 that parts were ordered for a safety recall. From that date until 02/27/2026, I was repeatedly told parts were unavailable. I scheduled repair and dropped off the vehicle on 03/13/2026. I received notice that repair had begun, but later discovered the vehicle had not been serviced at all. No explanation was provided. Additionally, service records are inaccurate. A vehicle history report shows service on 03/17/2026 at a dealership I have never visited. The vehicle was not serviced anywhere on that date. On November 28, 2025, my 2025 Volvo EX90 displayed a headlight warning message on the dashboard and the headlights failed to function properly while parked and after starting the engine. The defect temporarily resolved but recurred on December 5, 2025. A headlight defect that is not immediately apparent to the driver poses a serious safety risk before or during nighttime driving. The symptoms are
... consistent with NHTSA Safety Recall No. 25V-071 (Volvo internal reference R10298), which covers 2025 EX90 vehicles and involves unexpected headlight malfunction. I request that NHTSA investigate whether my vehicle's defect is related to Recall No. 25V-071 and whether the recall remedy has been properly implemented. If unrelated to Recall No. 25V-071, I request that NHTSA investigate this as a separate unreported safety defect. Other 2025 EX90 owners may be experiencing the same issue without reporting it. Volvo has contacted me directly on multiple occasions to schedule a central computer replacement on my vehicle. I understand this replacement is being offered to all 2025 EX90 owners. I request that NHTSA investigate whether this widespread hardware replacement is related to the safety defect I experienced and whether it should have been conducted as a formal safety recall. When brought in for repair on December 13, 2025, the service department advised parts were on indefinite backorder with no estimated delivery date. The vehicle remained at the service center for 47 days while Volvo was unable to supply the necessary parts. I request that NHTSA evaluate whether this represents an adequate response to a safety-critical defect.Read more

During this same time trying to figure it out, my teenager opened the driver side rear door to get into the rear seat to see if something was getting it stuck. At the same time I opened the driver door and the driver seat automatically started moving backwards like always, but with my teenager being the driver seat and her foot got stuck under the driver seat because that sensor as well in the front seat didn’t stop when she was behind it. I think it is beyond just the serving and third rows and the front toe doesn’t have a proper sensor either. This was Friday at 4:50. I immediately called my dealer to see if they could help us get something fixed so we could leave for spring break as planned the next day and they said managers were already gone for the day and told my Hyundai corp needed to handle and gave me that phone number. Hyundai Corp. opened a ticket for me, but said that this was definitely something the dealer would have to handle in regards to getting me a rental and any matter of recourse. The vehicle experienced multiple electrical and safety-related malfunctions while driving. On several occasions the instrument cluster and vehicle screens suddenly went black while the vehicle was in motion, causing loss of critical driving information such as speed and warnings. In another incident the vehicle displayed a “High Voltage Malfunction” warning while driving and the vehicle shut
... down shortly after. This resulted in a sudden loss of propulsion and created a dangerous situation on the road. The vehicle has also displayed a “Coolant Too Hot – Stop Vehicle” warning which required replacement of the coolant pump at the dealership. In a separate event the vehicle lost battery charge unexpectedly after indicating a full charge and had to be towed to a charging station. The vehicle is also subject to Safety Recall 26V073 involving the high-voltage battery. According to the recall notice, the battery may fail due to an internal short circuit which could lead to a vehicle fire while parked or driving. The manufacturer currently does not have a repair available and advises owners to limit the battery charge to 80% and park the vehicle outside when not in use due to the potential fire risk. These repeated failures combined with the unresolved safety recall raise serious concerns about the safety and reliability of the vehicle. The issues began around September 2024 and have continued through multiple repair visits into March 2026.Read more

The vehicle experienced significant vibration at highway speeds and during braking within the first 6 months of ownership. The issue was confirmed by a dealership, and the rotors were resurfaced as a corrective action. ... pads remain in good condition. Multiple service representatives have stated they cannot explain this level of uneven rear brake wear. The brake defect causes strong vibration that impacts steering control and vehicle stability, especially at highway speeds and while slowing down. This creates a safety concern due to reduced braking performance and loss of consistent vehicle control. The issue has been confirmed by dealerships, but the root cause has not been identified or properly resolved. No warning lights or alerts were provided prior to the failure. I am experiencing repeated and unpredictable software failures in my vehicle that create serious safety concerns. While driving, the vehicle will intermittently trigger system faults that: •Disable advanced safety features (including driver assistance systems) without warning •Emit a frequent, high-pitched warning tone that is distracting and disruptive •Occur mid-drive, forcing me to manag
... e the vehicle without expected safety support At the same time, the vehicle exhibits broader system instability, including: •Inability to charge or unexpected charging interruptions •Failure of keyless entry and locking/unlocking functions, sometimes preventing me from entering the vehicle •General electronic system unreliability across multiple vehicle functions These issues appear to stem from software instability and occur without a clear trigger. The loss of safety systems while driving, combined with persistent audible warnings, creates a dangerous driving environment by increasing distraction and reducing available safety protections. This is not an isolated incident—it has happened multiple times. Safety Risk: The unexpected disabling of driver assistance and safety systems while the vehicle is in motion presents a significant safety hazard. The accompanying high-pitched warning noise further distracts the driver during critical moments. Together, these issues increase the risk of an accident. Frequency: Intermittent but recurringRead more
