I am writing to report a serious safety defect in my 2024 Acura ZDX concerning the Forward Collision Alert and Automatic Emergency Braking. The system exhibits hyper-sensitivity, leading to unwarranted and dangerous automatic braking events. On 3/17/25, at approximately 4pm PT, I was driving my Acura ZDX on US 101 at 65 mph. A vehicle in front of me changed lanes into my lane. There was ample spac
... e between my vehicle and the vehicle that changed lanes. However, the Automatic Emergency Braking engaged the brakes forcefully, bringing my vehicle to a complete stop in the middle of the freeway. The vehicle essentially stalled for approximately 5-10 seconds, and I was unable to accelerate or move forward during this time. This created a highly dangerous situation, as I was vulnerable to being rear-ended by oncoming traffic. This incident occurred even though I had disabled the Forward Collision Mitigation System through the vehicle's settings. The system's behavior indicates a severe malfunction or design flaw, as it overrides the driver's intent and poses a significant safety risk. The fact that the system engaged the brakes when it was supposed to be disabled is very concerning. This hyper-sensitivity and the inability to fully disable the Emergency Braking System are critical safety issues that could lead to serious accidents, injuries, or fatalities. I request that the NHTSA investigate this matter immediately.Read more

“The 2023 Pleasure-Way Ontour 2.2 motorhome is built on a 2022 Ford Transit 350 HD chassis, VIN [XXX] . Four times while driving on secondary roads at a low rate of speed, or while stopped at an intersection, warning chimes and 6 different types of error messages are generated on the instrument cluster. These messages persist over a 10 to 30 minute period, and then the 12-inch LCD display and
... instrument cluster shut down. The engine continues to run, but there is a loss of Blind Spot Alert, Pre-Collision Assist, Cross-Traffic Alert, TPMS, HVAC function, turn signals, speedometer, cruise control and all other display functions. Upon turning off the ignition, there is no way to start it again. The only remedy is to disconnect the chassis battery for approximately 15 minutes and then reconnect it. At that point, the engine can be restarted and all errors are cleared from the screen. After a scan of codes from the OBD port, approximately 62 messages are found which are all loss of communication errors.” INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The engine is consuming excessive amount of oil. Specifically, I had an oil change in December 2024 and now in March 2025 I had to add to add 2 quarts and it's just been 3 months and under 1000 miles since the last oil change. How was your safety or the safety of others put at risk? The low oil c
... ause the car to stall sometimes when in a curve and also displays warning on dash to put car in park and restart engine. This is very dangers as we sometimes get stuck in the middle of the road until the car is restarted. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? I have taken it to the Jeep dealership about 2 years ago for this issue, but they claimed it's normal to have to add oil in between oil changes. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? A warning lamp comes on while driving that says to put the car in park and restart the engine. This issue has been occurring on a daily basis. From my research online, it appears this is a known issue with these engines and should have a recall.Read more

In 2024, I had the transmission replaced due it leaking transmission fluid. The transmission is sealed and there is no way to add fluid. This created an internal problem for the transmission. In January 2025, the engine made a loud pop noise and began performing horribly. My wife and I were on the interstate in a major city and surrounded by traffic. I drove it to the side of the interstate, imme
... diately, and turned it off. I had to call someone to tow me back home(2 hours away) and my wife missed a very important doctor's appointment. Upon inspection, I found failed valve lifters, camshaft, pushrod and valve lifter tray. I managed to get everything replaced and the truck ran fine. March 2025, as I was driving on the highway, the truck stopped running. A message came on the dash screen saying, "Shift to Neutral and restart". I did and nothing happened. I had it towed to a shop. They told me it was a fuse block that was blown and the starter blew so they replaced them. It had struggled to start a couple of times and since I knew it had a new battery, a starter and fuse block made sense. I picked the truck up and in less than 24 hours, it happened again. It stopped running as I was driving and would not restart. The shop has checked GM tech forums and found where someone said to check if the motor would turn over manually. They have tried and failed. They, then, took the oil filter off and found metal shavings in the oil. Their diagnosis is that the engine bearing has seized and is causing the problems. My main issue is safety. My wife and I could have been seriously injured or killed and could have injured or killed someone else when the incident happened on the interstate. The same could've happened with just me in the vehicle or with our children. There were no warning lamps, messages or any symptoms whatsoever before January 2025 for the engine. The truck doesn't even have 100,000 miles on it and I have kept up all of the maintenance.Read more

I was put in a rental by enterprise from a rear-end. I was involved while my car was in the shop fixing the trunk. I was escorted to a near gas station by one of the cops that came out of the traffic that was on hold. They moved the car to the side and escorted us. From there, my ex's mom checked carfax for the VIN number, which came out to having 4 previous accidents of the same front area. I reported it to the enterprise agent that called me asking about the car being towed, and I explicitly told them I'm not ok and you guys failed to put me in a safe vehicle. This is the second time I’ve had overheating issues, and it now seems more severe. I’ve noticed both coolant and oil leaking, and the car is unsafe to drive. Also fault codes related to emission. Here are the symptoms I’m experiencing: •Smoking: The car I started smoking, likely from oil or coolant leaking onto hot engine parts. Caused me to pull over at midnight on side of highway and get

I suspect the issue may be related to plastic components, as I’ve read they can become brittle over time and lead to leaks Also, the these BMWs involves the water pump's electrical pump connector allowing water to drip from the positive crankcase ventilation system through the seal, potentially causing a short circuit.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? When the brakes were applied, the vehicle accelerated instead of slowing down/stopping. The vehicle is available for inspection and we are begging for someone to investigate this as we do not want to get behind the wheel of this truck if the issue is determined and rectified. How was your safety or
... the safety of others put at risk? Because of the above issue, the car accelerated and crashed into a building putting the driver and passenger at risk. The airbags deployed. The vehicle went over a walkway where, luckily, no one was walking or they would have been seriously injured or potentially killed. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Ford keeps telling us to work with our insurance company. The insurance company did an estimate for the repairs, but they are not able to inspect the vehicle and determine the cause of the malfunction. Our insurance adjuster said that Ford needs to be the one to access the black box and perform testing to determine the malfunction. Ford has been unresponsive to our claim and the service manager where the vehicle is sitting will not touch the truck due to it being a Ford liability. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No, the vehicle just suddenly accelerated when the brake was applied. The airbags deployed when the truck hit the building, the horn went off and wouldn't stop, and the brakes are stuck (unable to apply pressure).Read more
