I have a 2023 GMC Yukon AT4. The OnStar/telematics system stopped working. The blue OnStar button does not function and the vehicle no longer communicates with the myGMC app, resulting in loss of connected and OnStar services. A GMC dealership diagnosed a failed OnStar/telematics module and advised that the module must be replaced at a cost of approximately $1,200. I am concerned because this fail
... ure affects access to OnStar communication and emergency-related services. I have found reports from other GM owners describing similar telematics/OnStar module failures, and GM has issued service information addressing K73 telematics-module communication faults. I have also opened a case with GMC Customer Assistance. I am reporting this because I believe the repeated failure of a system used for emergency communication warrants investigation.Read more

I was driving down [XXX] at approximately [XXX] this morning and heard a loud bang, then an air sound like window not fully closed, then heard crumbling glass falling into Sunroof shade. I reported the incident to Orr Toyota in Hot Springs, ar. and they informed me that Toyota claims that something impacted the window. There was no impact to the window as it would have caused the window to bulge inward if that occurred. The 12 volt battery exhibits a consistent parasitic drain. This is intermittent, but results in the vehicle failing to start. The failure to start is an issue because it is happening frequently and could endanger individuals who live in rural areas or areas with cold or hot climate. This issue has happened to us multiple times and Toyota has not recalled or provided guidance to its dealer service
... network. Many effected owners have a diagnostic data backed theory that links the drain on the 12 volt battery to Toyota digital communication module. That diagnostic can be found here. Toyota should be forced to investigate and fix this as it is a big safety concern for those who are in rural harsh climates! It is impacting a lot of new BZ & CHR owners, but has been a known issue with previous models and even covered by some media outlets. Because this is a parasitic drain there often isnt a low battery warning before the car won't start. Another reason this deserves attention.Read more

... those 10-15 seconds. Suddenly, the car spontaneously started to accelerate - not slowly rolling forward, but actively accelerating with significant forward propulsion. Fortunately, there was no one in front of me and I was paying close attention so I was able to immediately step on the brake pedal to stop the car. The car only traveled about 2-3 feet, but that's only because I reacted so quickly. COMPONENT: Electric power steering (EPS) rack and pinion assembly. WHAT HAPPENED: The steering on my 2019 Civic EX binds and catches. Returning the wheel toward center after a left turn, the steering does not return smoothly — it hangs up, then releases suddenly, with an audible clicking noise. The condition is worse after the vehicle has been driven and has warmed up. It occurs at parking lot
... speeds and at highway speeds during small lane-keeping corrections. It is also reproducible with the vehicle stationary and the engine running. SAFETY RISK: When the steering binds, more force is required to move the wheel. When it releases, the wheel moves further than intended, causing overcorrection that must then be corrected back. On the highway this makes it difficult to hold a consistent lane position. There has been no warning lamp and no diagnostic trouble code at any point. CONFIRMED BY DEALER: An authorized Honda dealer reproduced the condition. Their technician documented that with the engine running and the wheel turned to full left lock, the wheel binds and will not return until physically touched. A master technician at the same dealer confirmed the binding and the clicking noise and identified the power steering rack as the failing component. The dealer noted that this condition is discussed on Honda's internal technical line under the description of Civic steering feeling sticky and pulling in either direction. Their inspection found no other damage or worn suspension or steering components. QUOTED REPAIR: The dealer quoted approximately $5,068 to replace the steering rack assembly. The vehicle is out of warranty. ADDITIONAL NOTE: Honda has issued a recall for the same sticky steering condition on 2022-2025 model year vehicles, attributed to excessive friction from the worm wheel inside the steering gearbox. No equivalent action covers 2016-2021 vehicles, which use a comparable EPS design and generate similar owner complaints. The component is availableRead more

I am reporting a potential safety defect involving the rear windshield glass on my Acura MDX. The rear windshield unexpectedly shattered while the vehicle was stationary, with no known impact or contact with an outside object. I was present when the glass shattered and immediately took multiple photographs and videos before the vehicle was moved. Based on what I observed, the glass appeared to br
... eak outward rather than from an outside object striking the vehicle. I later drove the vehicle from my home to the Acura dealership after being advised to do so. By the time the vehicle was inspected, portions of the shattered glass were understandably missing. The dealership later identified an area of missing glass as a possible “point of impact,” although I witnessed the event and there was no impact. I am submitting this complaint because I am concerned that the rear glass may have failed spontaneously and could represent a safety issue. Unexpected glass shattering could potentially occur while a vehicle is being driven and could startle a driver, expose occupants to broken glass, or impair visibility. I am also aware that Acura has previously issued technical service information regarding rear windshield glass on certain RDX vehicles shattering without external impact. I understand that my vehicle is an MDX and may not be covered by the same issue, but I believe the circumstances warrant documentation and review for a possible pattern.Read more

I have now had three instances within the last three months where my car has stalled out at traffic lights when the Idle Stop was engaged. The safety of myself and my children in the car with me has been put at risk since each time it happed we were stopped at a red light at a major intersection on busy major roads. AMENDMENT TO SAFETY COMPLAINT: FAILURE OF RECALL REMEDY / USE OF LEGACY PART Vehicle: 2020 Honda HR-V NHTSA Recall Campaign Number: 23V858000 (Low-Pressure Fuel Pump) [The 2020 Honda HR-V is under an active, massive federal safety recall for low-pressure fuel pump failure.] Chronological Safety History: •January 2025: Mandatory fuel pump safety recall performed at an authorized Honda franchise.
... •September 2025: Catastrophic engine stall occurred at highway speeds. •June 12, 2026: Second catastrophic engine stall occurred at highway speeds. Critical Update: Following these severe stalling events, I presented the vehicle to franchised dealer Zeigler Honda. The dealership service department engaged in fraudulent diagnostic practices to evade liability. They falsely claimed the stalling was due to "bad gas," refused to perform a secondary repair, and released a hazardous vehicle back onto public roads. Due to the immediate threat to public safety, I brought the vehicle to a certified independent mechanic. Upon replacement of the system, a component breakdown revealed that the recall part installed in January 2025 was actually an identical, low-density defective legacy component subject to the original recall. The independent mechanic's invoice factually confirms a total mechanical failure of this fuel pump component [The 2020 Honda HR-V is under an active, massive federal safety recall for low-pressure fuel pump failure.]. The system breakdown confirms the plastic impeller deformed and jammed, completely disproving Zeigler Honda's fabricated "bad gas" diagnosis [Honda’s official recall documentation states that the low-pressure fuel pump fails because the plastic impeller absorbs fuel, deforms, and jams against the pump housing, causing the vehicle to stall.]. Regulatory Request: I request that the Office of Defects Investigation (ODI) audit this dealership immediately. Automated emergency braking triggering unsafely! Two separate occasions: 1) As I was driving about 40 MPH on a 4 lane street in Wyomissing, PA, the car slammed on the brakes. In the median to the left of the car ahead about 20-30 ft was a "your car is going XX MPH" sign. It was NOT in the road. If a car had been following me closely, this could have caused a serious accident. The road had a s
... light curve. The exact same thing happened the next day. I ended up avoided the road to prevent the braking issue. 2). I live on a busy city street. I have to reverse out of a tight garage and very slowly enter traffic. The chances of a car being 15 feet behind me are close to 90%. As I back slowly out, the car slams on the brakes and throws my head back, despite me going so slowly that the car behind me would be long gone by the time I actually got back that far. This happens to me DAILY. THIS IS A SAFETY issue for two reasons - 1) my head keeps getting slammed back, and 2) the car's sudden stop means in need to reset it into reverse - while cars are trying to go around me, and the backing up becomes erratic and a safety issue. I want automated reverse braking TURNED OFF. It it painful and triggers long before an actual accident would happen!Read more

Formal safety complaint against dealer for recall malpractice and non-compliance with federal safety repair procedures under Campaign 21V635000. During recall service, the service department completely bypassed the mandatory TOPIx Workshop Manual Section 310-01 electronic priming sequence, leaving high-pressure fuel rails completely dry upon reassembly. Bypassing this safety protocol introduced se
... vere air pockets into the high-pressure fuel system. Time-stamped OBD diagnostic screenshots taken on the dealer lot right before departure confirm the vehicle's computer had already registered severe fuel pressure drops and engine misfires due to dry-running High-Pressure Fuel Pumps and injectors. This mechanical deviation caused the mechanical High-Pressure Fuel Pumps to fail internally and the Bank 2 fuel injectors to seize permanently wide open, resulting in continuous fuel dumping, fouled spark plugs, a destroyed oxygen sensor, and a total loss of motive power on a public roadway. Furthermore, the dealership generated federal recall compliance documentation certifying a quality-control test drive was performed when the official repair order shows identical "In" and "Out" mileage, proving the vehicle never left the service bay. The dealer refuses to remedy the related damage under federal guidelines and refuses to communicate a resolution. Due to intentional document falsification, consumer demands the manufacturer fund a $0.00 balance remedy executed at an alternate certified facility including: (1) Replacement of BOTH High-Pressure Fuel Pumps (HPFPs), (2) Replacement of all Bank 2 injectors, plugs, and oxygen sensor, (3) Crankcase oil flush due to fuel dilution, (4) Engine compression and emissions back-pressure tests, and (5) A written 12-month nationwide parts/labor warranty.Read more

What component or system failed? When the BCEM failed on my car, it led directly to the transmission failing as well as the PIM. I was driving normally then had a completely inoperable car with no warning. I was told to pay over $17k for repairs by the dealership because there is no recall on the transmission, but I see proof that this is a common issue among Volt users. There needs to be a recal
... l and repair to keep the road safe. How was your safety or the safety of others put at risk? I was driving on a busy road to school when my car fully locked up, jerked like crazy, and became inoperable. If I had been somewhere like a freeway instead of in front of a school, I likely would have become badly injured when the wheels stopped even turning. The tow truck even had to use wood under my wheels to get it onto the tow truck. Has the problem been reproduced or confirmed by a dealer or independent service center? I took it to the dealership and worked only with them. They replaced the BCEM without charge but have left me with an inoperable vehicle because I am unwilling to pay twice the cost of the car to repair it. Has the vehicle or component been inspected by the manufacturer? Dealership. But also, as mentioned, this is clearly common and needs to be recalled. A 2018 car with 117k miles of gentle driving on a well maintained vehicle shouldn't be failing, and there shouldn't be so many Volts being sold for parts because their transmission failed. This is clearly a dangerous issue related to production and should have a recall. Were there any symptoms of the problem prior to the failure, and when did they first appear? I bought the car with just under 16k miles in 2021 and within 3 months I had symptoms, but the dealership couldn't figure them out, as they happened intermittently and only in heat, and the dealership always sent me away without repairs. The failure itself happened at 62°, but was foreshadowed by lesser versions of its demise.Read more

Current Unsafe Conditions: Post-Replacement Active Safety Failures. The vehicle presently exhibits two distinct categories of active, ongoing, and dangerous safety failures, both of which were introduced as a direct result of the engine replacement performed by Monument Chevrolet: (a) Severe Drivability Failure. Between approximately 1,500 and 5,000 RPM, the engine repeatedly and without warning l
... unges forward, drops all power, and lunges again — cycling in this manner between three (3) and six (6) times before stabilizing. This behavior occurs across all gears and driving conditions, which creates a severe and unpredictable hazard particularly when merging into traffic or accelerating onto highways. This failure constitutes an immediate and ongoing risk to the safety of myself and other motorists. (b) Complete ADAS System Failure. The frontal crash response warning light is permanently illuminated on the instrument cluster, rendering the following federally relevant safety systems entirely non-functional: (i) Automatic Emergency Braking / Forward Collision Alert; (ii) Cruise Control; and (iii) Lane Change Assist / Lane Control System. All of these systems were fully operational and properly functioning before the vehicle was submitted for the engine swap. Their failure is an assembly defect introduced by Monument Chevrolet during the course of the repair and constitutes a warranty and safety defect for which General Motors, as the manufacturer and warrantor, bears full responsibility. Medical Hardship and Urgency. This vehicle is my sole personal transportation. It is essential for travel to active medical appointments and upcoming medical procedures. The ongoing failure of General Motors and its authorized service network to restore this vehicle to a safe and fully operable condition has created a serious hardship that directly affects my ability to access needed medical care. Yes, it was driven by a service department supervisor. He did experience the issues.Read more

I am reporting two issues with my vehicle that I believe require further attention and repair assistance. I am requesting that both issues be properly repaired without any charges to me. There are no warning lights, error messages, or other dashboard warnings related to either issue. However, both conditions are abnormal and affecting the vehicle’s operation and ownership experience. 1. Steerin
... g Noise (Inside Cabin). The steering assembly inside the cabin. Whenever I make any type of turn, either left or right, I hear a noticeable clicking/clocking noise coming from inside the steering column area. I want to clarify that this noise is not coming from the wheels, tires, suspension, or steering components connected outside the cabin. The sound is specifically coming from the steering wheel/steering column area inside the vehicle. I have had this concern checked, and the only suggestion provided was to replace the suspected component. The dealer did not provide written confirmation of the diagnosis but verbally advised that they could replace the component and see if it resolves the issue. However, since there is no guarantee that the replacement will correct the problem, I do not feel comfortable spending my own money on a repair that has not been confirmed. I have also spoken with multiple third-party mechanics, and some have verbally indicated that this type of steering noise is a known issue that can occur after a few years and may be related to the clock spring (spiral cable/ribbon) or another internal steering column component. Because this appears to be a known manufacturer-related concern and not a result of misuse or neglect, I request that the issue be properly repaired at no cost. 2. Rear Brake Squeaking Noise. The rear brake pads and rotors are new, but the brakes continue to make a noticeable squeaking/squealing noise. The noise usually occurs after driving for some time, especially in warm weather when the brakes reach normal operating temperature.Read more

... ver removed the key from the ignition, which immobilized the vehicle. While in the middle of traffic, the driver contacted a family member for assistance and the vehicle was towed to the driver's residence. On the following day, January 6, 2026, the vehicle was towed to a service center for inspection. It is unknown if the warning messages observed January 6, 2026 were due to the vehicle being towed. At the service center, a tire was replaced and the brake system was flushed, at the owner's expense. Afterward, the driver continued to operate the vehicle. As of August 5, 2026, this incident had not recurred.Read more
