Session ID: [XXX] EA Case No: [XXX] Time of incident: [XXX] While DC fast charging my 2022 Porsche Taycan at an Electrify America station, I experienced a critical malfunction during cable disconnection. Upon unplugging, there was a visible spark, fire, and smoke on the car's charging port. The DC charge port showed heat damage, with melted pins that rendered it unusable. A Porsche dealer confirm
... ed the damage likely resulted from the charging event, not a vehicle defect. A repair invoice of ~$5000 has been issued. This incident posed a serious fire and electric shock hazard. If someone had been closer to the port, injury or fire could have occurred. No warnings or errors were displayed by the vehicle before or during the event, which happened suddenly and without shutdown or alert. The dealership inspected the damage and noted signs of thermal/electrical malfunction. A copy of the dealer's report is attached with this form. The component has not yet been examined by Porsche North America, insurance, or third parties, but I am escalating the issue. The damaged charger remains nonfunctional as of June 11, and Electrify America has stated the charger's connector pins need replacement. Electrify America has also refused to reimburse any damage fees to me. A picture of the out of operation charger as of 6/7 is attached. The vehicle charge port is available for inspection if needed. I can't confirm whether the malfunction was caused by Electrify America’s charger, the Taycan’s port, or both, however evidence suggests the HV charging cable triggered the fault. This event presents a credible safety risk to people and vehicles. EA's chargers are widely used, and if this issue affects compatibility with vehicles like the Taycan, other users may be at risk of fire or shock. No injury occurred, but the potential was significant. I am reporting this to NHTSA to urge investigation into the broader safety implications. Incident date: Jun 9, 2025
The car in recent months has twice lost power and been rendered in operable on the side of the road until it could be limped home. I had the car serviced last year for a recall of a fuel filter gasket and at that time I was informed that I also had an update done for a (k knock?) sensor prior done on the car before purchase. I inquired about what that was, and they told me at the time it was for a
... n engine recall because that particular engine I have is known to have rod bearing failure and consume oil without any trace of where it goes and piston rattle. I told the gentleman after my fuel gasket was taken care of that I would like to look into that because I have the symptoms and would like to know. At that point in time, I was told that I may have to do an oil test to see if the car loses oil. I said well can we do that and he told me it's $300 out of pocket. I asked on average before anything ever done by Hyundai, how many times do people have to do this? He said 4 to 6 in his experience. I told him I did not have the money, but I will keep an eye on the car. Everything's been fine for six months a few months ago Randomly driving to work the car shut down on me. We got the car home and my fiancé did an oil change and new spark plugs and the car worked fine. June 9th 2025 on my way to work the issue happened again in the middle of traffic. I called my fiancé and we got the car home. He performed another oil change and spark plug replacement and took pictures this time because the spark plugs are gunked up. The car now runs again. He called Hyundai of America on my behalf to just ask them if they could set up the test to see if the car was failing and they told him that they were all independent Hyundai and he would have to pay the test fees. He told them in a formal email that he doesn't believe under the federal recall He is responsible for that and they answered back that he may be reimbursed. We are contacting you because we were advised to on the issueRead more

I have now experienced multiple instances where the vehicle suddenly brakes on its own while driving at normal speeds, without any obstruction or hazard ahead. This is not only unacceptable — it's extremely dangerous, putting both myself and other drivers at risk of serious injury. Despite bringing the vehicle in twice to the dealership, the issue persists. I continue to receive dashboard malfun
... ction alerts, and the automatic braking problem has not been resolved. Each time, I am left without a vehicle, impacting my ability to work. I did not purchase a $100k+ vehicle to live in fear of sudden, unpredictable, and life-threatening malfunctions. Today, while driving, the vehicle completely shut off in motion, creating a highly dangerous situation. This follows multiple previous incidents where the car automatically braked without cause and displayed repeated malfunction notifications on the dashboard. I have brought the car to the dealership twice, and yet the problem persists. This is not only unacceptable — it's an imminent risk to my safety and the safety of others on the road. The fact that a brand-new luxury vehicle with this price tag continues to experience such serious failures is beyond frustrating — it is life-threatening.Read more

I am the original owner of a 2018 GMC Sierra 1500 with the 6.2L L86 engine and 8-speed 8L90 transmission. I began experiencing severe transmission shuddering and harsh shifting, especially at lower speeds or under light acceleration. When the issue occurs, I press the gas and the engine revs up to 6,000 RPM without reaching 20 MPH, making it extremely difficult — and dangerous — to merge into
... traffic or cross intersections. This issue has worsened over time and is now a critical safety concern, especially with children in the vehicle. I have researched this issue and found that many owners report the same transmission defect, and there is an open class action lawsuit and a GM Technical Service Bulletin (TSB #18-NA-355) describing the exact symptoms. The vehicle has become unsafe to drive. GMC should issue a recall or mandatory repair program for this known defect before someone is injured or killed.Read more

I am writing to formally lodge a complaint and request immediate resolution regarding my 2022 GMC Sierra 1500 equipped with the L87 6.2L V8 engine, which has been out of service for more than 30 consecutive days due to a recall-related defect. The vehicle is inoperable, and I have been informed by the authorized dealership that no loaner vehicle will be provided, with no clear estimate for when pa
... rts or repairs will be available. I have contacted both General Motors Customer Assistance and the local dealership to request a loaner vehicle. In both cases, I was informed that no loaner was available. I was advised that I could rent a vehicle at my own expense, and that GM would only consider reimbursement up to $44 per day, but only after the recall is completed and only if my case is deemed eligible upon review. This reimbursement policy—requiring me to first pay out-of-pocket without guarantee of repayment—has placed significant undue hardship, financial burden, and emotional stress on me and my family. This situation is especially frustrating considering the vehicle: Remains under GM’s lifetime powertrain warranty; Has been completely unusable for 59 days without replacement; Has left me with no reliable means of transportation, and Involves a known defect acknowledged by GM, yet with no definitive timeline for resolution. As a result, I am suffering direct and ongoing consequences, including: Out-of-pocket transportation costs; Disruption to daily life and mounting stress that no consumer should face after purchasing a premium, modern vehicle from a manufacturer of GM’s reputation. Accordingly, I am requesting the following: Provision of a comparable loaner vehicle immediately, or full reimbursement for a rental vehicle retroactive to the date the truck became inoperable; Compensation for loss of transportation and impact on life. If GM cannot resolve the defect in a timely manner, a full repurchase or replacement of the vehicle under federal warranty law.Read more

Last week I was driving 70MPH on the highway when suddenly I got the red wrench icon with the message "Service Hybrid Electrical System" on the dashboard. After about 7 seconds I then got a "Put foot on brake and press start" message on the screen - while still going 70MPH on the highway. At that point the car died. I had no power - the accelerator pedal did nothing. Luckily, I was able to pul
... l over to the shoulder and stop. Then I did what it said and pressed the brake + start button. The car came back to life, but I didn't have the full hybrid system - I only had gas - the car was not using any electrical power as the battery level remained constant and did not drop as I drove. I continued for another 20 minutes where I stopped for lunch. When I returned to the car it started up normally and continued to run normally after that. This is the 3rd of 4th time the red wrench icon with the Service Hybrid system warning coming up, but on the previous times the message went away after about 5 seconds and the car drove normally. This is the only time that it caused the car to completely die on my while on the highway. Apparently, this has been happening to a *lot* of people after Jeep issued a software update to address the 95b recall last year. Many owners are now afraid to drive their Jeep 4xe for fear of it suddenly dying on the highway at high speeds.Read more

The Blizzard Pearl paint on my Toyota 4Runner is peeling in large patches, exposing the undercoat and compromising the vehicle’s integrity. Toyota issued a Customer Support Program (CSP) acknowledging this as a known defect affecting Blizzard Pearl paint, but I never received the notification. As a result, I missed the deadline to have the issue corrected, and Toyota has refused to assist. This
... widespread issue impacts resale value and could potentially lead to rust or corrosion. There should not be a time limit on addressing a manufacturing defect of this nature. Toyota should be held accountable for providing a lasting remedy, especially when customers like me were unaware of the CSP timeline.Read more
