Incident date: Mar 31, 2025
On July 1st 2025, while slowly reversing out of my driveway at approximately 3–4 mph, the vehicle’s collision detection system errantly detected an imminent collision. Despite there being no obstacles, vehicles, pedestrians, or objects in the vehicle’s path, the system abruptly engaged the automatic emergency braking (AEB). This sudden and forceful braking caused me to experience whiplash du
... e to the unexpected jolt. There was no prior warning or indication of a malfunction, nor any alert from the vehicle indicating why the braking occurred. After exiting the vehicle and inspecting the surroundings, I confirmed there were no obstructions behind or near the vehicle. This appears to be a false positive detection by the vehicle’s collision avoidance system. This incident raises serious concerns about the reliability and safety of the AEB system, particularly in low-speed reversing situations. An erroneous activation of this kind can result in injury to the occupants, potential rear-end collisions (in other traffic scenarios), and may also contribute to driver mistrust of critical safety features.Read more

Driving home on a two lane road that is joined by a side road at a 45degree angle. A woman ran the stop sign and suddenly appeared on my road right in front about 40-50 feet away. The BMW did not show any visual or audible warning and the forward collision mitigation system functioned in a substandard way not mitigating the crash into the woman’s drivers door. Damages amounted to 25.8k to my v

If that was the case how come I had massive front end damage impacting about 30 mph. The BMW manual shows a frontal cross traffic intrusion and leads one to believe in the system. It is substandard and needs revision by BMW engineering.Read more

Bought my truck with 29,000 miles on it. Was making a noise in rear tire. Took it in and it was the break calilper. Dealership wanted $1500 to replace it a week after I bought it. The rear turn signal on the 2024 Jeep Grand Cherokee is very small. Following vehicles seem to have difficulty in seeing a signaled turn. This condition is exacerbated when driving into the sun. On several occasions, when attempting a left-hand turn and stopped for oncoming traffic, following vehicles have had to brake hard to avoid a rear end collision. This puts the occupants of both vehicles a

The vehicle is a Jeep Grand Cherokee 4xe plug-in hybrid SUV. While driving in heavy traffic on a 45 mph road, the vehicle suddenly lost propulsion, with the dash lighting up with a large "Service Hybrid System" warning. After several seconds of no response, with traffic trying to work its way around my stalled SUV, the vehicle began to move again, albeit very slowly, just enough to limp it to a si
... de street. After another 30 seconds or so, the problem cleared itself and the vehicle returned to a normal state of propulsion. I took the car to the dealer where I left it for more than a week. Apparently this is a known issue, and a code appeared (B273C-00, Digital Crash Input), but they were not able to replicate the issue while they had the vehicle and so they just reset the code. The automaker is also aware of this problem, but doesn't deem it severe enough to issue a warning to customers or provide alternative transportation while a fix is developed. The dealer's advice is to let them know when it happens again. At this point, I don't feel the vehicle is safe to drive, and it will remain parked until a fix is available.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The component that failed is the engine cooling system, specifically due to a known issue with coolant intrusion into the short block. This failure led to significant engine damage in my son’s 2018 Ford Escape. The vehicle is currently available for inspection upon request. How was your safety or
... the safety of others put at risk? The sudden engine failure posed a serious safety risk. If the vehicle had stalled while in motion—particularly on a highway or in heavy traffic—it could have led to a collision or left the driver stranded in a hazardous location. In this case, my son's car failed unexpectedly, which could have resulted in harm had the conditions been different. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, the issue has been confirmed by a certified service technician. It matches a known problem in 2018 Ford Escapes involving coolant leaking into the engine block, which has been documented in Technical Service Bulletins (TSBs) issued by Ford. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? At this time, the vehicle has not been inspected by the manufacturer, police, or insurance representatives. However, the diagnosis and confirmation of coolant intrusion was made by a mechanic. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? No. Only engine coolant container was empty without warning.Read more

Despite reporting this safety issue immediately, manufacturer Genesis Motor America failed to disclose that this was a known defect covered under Technical Service Bulletin 24-BD-008G. Customer service representatives did not mention existing TSB and refused emergency service appointments, forcing 2-month wait for service scheduling despite life-threatening electrical malfunctions. Customer had to independently discover TSB documentation through research. Only after customer escalated with TSB evidence did Genesis authorize repair. Dealer confirmed drainage system failure and completed repairs per TSB guidelines that manufacturer should have immediately referenced. Genesis's failure to disclose known safety defect and refusal of emergency service created extended exposure to dangerous electrical malfunctions that could result in accidents.Read more

The brakes on my Airstream travel trailer have malfunctioned. The brakes started to “grab”, and the only way I could stop the grabbing was to reduce the gain on the brake controller from 5 – 5 ½ to 3 – 3 ½. At one time in a parking lot the trailer brakes brought my entire rig to a halt after I had gently applied the brakes and then released them. I am concerned that this issue is a sa
... fety hazard. I have brought the issue up with Airstream, and they have twice tried to fix the problem. They replaced some of the brake components, and finally declared that the problem cannot be with the trailer, it must be in the truck. In attempting to eliminate the truck as the problem, I had a truck dealer replace the truck’s brake controller with the same factory controller. The result was no change in the condition. I finally brought the rig into my local mechanic, who determined that the problem was that the right rear trailer brake was not releasing fully, thus causing the problem. The mechanic replaced the right rear Right Hand Brake Kit (Complete Electric Brake Assembly), P/N K23-459-00 and the problem appears to be resolved (at least for now). Please keep in mind that only two of the four wheels have been inspected by my mechanic. The mechanic found that a part in the brake assembly had more play than the same part in the adjacent wheel, which allowed to part to move more than it was supposed to, thus causing the brake to hang up. I saved the defective part for further evaluation. In diagnosing the problem, the mechanic evaluated the rig with the trailer hooked up to my truck. He moved slowly, then applied the brakes and stopped the rig. He then disconnected the 7-way plug, thus eliminating any power or signal coming from the truck. When he went forward we heard a squeal from the trailer. He jacked up the trailer and found that the right rear brake was still applied.
