On [XXX] while traveling on the interstate the car issued a warning engine light and turtle image to pull off the road as the car would shut off, immediately it had no power, maybe could only go 30 mph. Took the car in on 5/10 which was the earliest appt. Said they did a software update, gave the car back. Took the car again on [XXX] when the same warning appeared again on the interstate, same sit
... uation disabled vehicle at high speeds on the interstate and no power. Said they changed a module related to the error and gave me the car back. Yesterday as I was driving on [XXX] 70 mph the car gave the same warning and shut off again, immediately I had no power, 30 mph. The dealer acknowledged my car is not the only one in his shop with this issue. They had my vehicle for nearly a month the time before. This vehicle is not safe to drive on the highways as it can shut off at any time. It's a known problem by Chrysler and their idea of fixing it, is issuing a warning which gives you almost no time to get off the highway before your speed is greatly reduced and even then you're stuck on the side of the highway with cars speeding by in an area with no shoulder or construction zone you'd be stuck in the middle of the road. Someone will wind up dead on the highway, I am lucky that in the 3 times this has happened I have not been hit and have been able to safely get to the shoulder, but I don't see Chrysler being able to fix this issue, the dealership did reach out to the manufacturer each time. I know my car is not the only one that has this issue and I'm concerned for the safety of anyone who owns one of these. A simple google search of this vehicle will show the problem is wide spread. On July 1, 2025, I was involved in a severe rear-end collision while driving my 2024 Mazda3 manual hatchback. The impact was forceful enough to shatter the rear windshield, leave a deep dent in the tailgate, and bend the trailer hitch mount under the vehicle by several inches. Despite this, none of the airbags deployed, and there were no airbag warning lights before or after the crash. After the
... incident, I checked my vehicle’s VIN on NHTSA’s recall lookup tool and discovered that Recall 7525E had been issued on June 6, 2025, for a critical airbag sensing system defect (SAS module), which can disable airbag deployment after battery depletion or improper power conditions. According to Mazda and NHTSA documents, this defect can completely disable airbag function, increasing the risk of injury in a crash. As of July 11, 2025, I have not received any recall notice from Mazda by mail, email, phone, or dealer contact—despite being the registered owner. Mazda’s documentation shows that: Dealers were notified as early as June 3, 2025 Owner letters are not scheduled until July 28, 2025 This means that I was driving a defective vehicle without knowledge of a serious safety issue at the time of the crash. I believe that: Mazda failed in its legal obligation to notify me in a timely and adequate manner. The non-deployment of airbags in my crash is consistent with the failure described in Recall 7525E. Other affected Mazda owners may remain unaware and at significant risk. I respectfully request that NHTSA: Investigate whether Recall 7525E was the cause of airbag failure in this incident. Determine whether Mazda’s recall notification practices met regulatory requirements. Ensure immediate outreach to affected owners to prevent further injury or risk. I am also seeking legal counsel due to the serious safety implications and potential liability arising from this defect. I will be retaining all documentation, vehicle records, photos, and post-Read more

I am filing this complaint regarding an ongoing safety issue with my 2021 BMW 740i involving the lane departure warning system, lane keep assist, and adaptive cruise control, all of which have stopped functioning properly. These features are critically tied to the windshield-mounted KAFAS camera, which I’ve now been told needs to be replaced. Initially, the BMW dealership attributed the issue t
... o a software problem and charged me $500 for a software update. After the update, the driver assist systems remained non-functional. The dealership then diagnosed a failure with the front camera assembly and is now quoting me approximately $3,000 for replacement of the windshield and camera module. I have reviewed NHTSA Technical Service Bulletin SIB 66 08 23 (NHTSA ID: MC-10236236-9999), which outlines this exact issue, suggesting that BMW is aware of system limitations and hardware failures in this critical safety feature. However, this has not resulted in a recall or manufacturer-paid repair, despite the significant safety risk involved. I am requesting that NHTSA investigate this issue for the 2021 BMW 740i and similar BMW models equipped with the same KAFAS camera system. The sudden failure of key driver assistance systems—especially lane departure warning and adaptive cruise control—can lead to dangerous driving conditions, particularly on highways. I believe a recall or service campaign should be initiated requiring BMW to cover the cost of diagnosis and camera replacement due to the safety-critical nature of these systems. Please consider this complaint as a formal petition for further investigation and potential recall.Read more

PLLEASE PROVIDE ME INFORMATION ABOUT TRANSMISSION RECALL
Incident date: Jul 6, 2025
On [XXX], while driving from Orlando, FL to Atlanta, GA, I experienced one of the most terrifying moments of my life. I was traveling on the freeway at approximately 80 MPH, in lane 3 or 4, when my vehicle suddenly began to hesitate and lose power without any warning. In that moment, fear and panic took over. Cars were flying past me, and I could feel my truck rapidly decelerating. I immediately
... turned on my hazard lights and began honking to alert other drivers, praying they would give me space to move over. Thankfully, I was able to carefully navigate across lanes and reach the far-left shoulder. Once I got into the acceleration/deceleration lane, the vehicle suddenly lurched forward and began accelerating again—completely on its own. I knew I couldn't risk staying on the road, so I exited at the next ramp and pulled into a gas station, still shaken. After refueling, I attempted to restart the truck, but it would no longer shift into Drive or Neutral. The truck was stuck in Park and completely inoperable. A roadside mechanic responded to my call for help and made several attempts to restore mobility, but nothing worked. I spent the evening stranded and alone, unsure of what to do next. The following day, [XXX], I had the vehicle towed to Honda South near Atlanta, GA. After inspection, I was informed that the issue was a complete transmission failure. In addition to needing a new transmission, the vehicle also required replacement of critical sub-assembly components, including the transmission control module (TCM). This situation was not just a mechanical failure—it was a serious safety event that put my life and the lives of others at risk. I’m still shaken by what happened and deeply concerned about how such a failure could occur so suddenly and without warning. ... The following conversation was exchanged from myself & someone named Ben, from the Sewell dealership in North Austin: [From Ben: I am not as familiar from the surface, but everything was explained in the video from the technician. The failed component is the bearing of the turbocharger which caused a major oil leak. This oil leak was the smoke you were seeing from the exhaust. We know for sure that the turbocharger will need to be replaced. The engine has a high likelihood of needing replacement as it was oil starved due to the major oil leak from the turbocharger. There is metal in the oil from inside the engine indicating internal failure from not having enough oil. From Me: which makes no sense, having just had all these things checked, back in May, with no indication of any leaks of anything other than what was shown.] ANDDD, which was supposedly fixed.
