Incident date: Oct 17, 2025
On October 17, 2025 at approximately 3:15 PM in the parking lot of a shopping mall at 2040 Lomita Blvd, Lomita, CA, our 2025 BMW iX M60 suddenly accelerated forward unintentionally while the driver was attempting to park. The vehicle made a loud “whirring” sound as if the motor engaged on its own, and it lurched forward rapidly without any acceleration input from the driver. The car traveled o
... nly about 2 feet before crashing into a wall in front of the parking space. All front and side airbags deployed. The driver (my father) suffered a chest fracture and hand laceration, and the passenger (my minor son) sustained severe bruises to the chest and shoulder from seat belt impact. The Driver (my father) was hospitalized for four days. The vehicle was towed to a body shop and is currently under insurance inspection for total loss. This appears to be a case of Sudden Unintended Acceleration (SUA) possibly related to the vehicle’s electronic drive control system. We request that NHTSA and BMW investigate this incident to determine if there is a safety defect in the 2025 BMW iX EV platform.Read more

Subject: Rear Differential Mount Failure and Driveshaft Breakage — Sudden Loss of Vehicle Control at Highway Speed Complaint Summary: While driving at highway speed, I experienced a sudden and extremely loud bang from the rear of the vehicle. Immediately afterward, I lost complete control of the rear end, accompanied by violent clanking and grinding noises from the driveline area. The gearbox t
... hen stalled, leaving the vehicle disabled in the middle of the highway. Upon inspection, it was determined that a rear differential mounting bolt had sheared, causing the differential to shift violently out of position. This sudden movement resulted in the driveshaft breaking, which further destabilized the vehicle and led to a severe and immediate loss of control. This failure occurred without warning and created an extremely dangerous situation. The sudden loss of drivetrain integrity at highway speed placed both myself and nearby motorists at serious risk of a crash. A component as critical as a differential mount or driveshaft should not fail under normal operating conditions. This incident strongly indicates a serious design or manufacturing defect affecting the vehicle’s powertrain and structural safety. Requested Action: I urge the NHTSA to investigate rear differential mounting and driveshaft failures on this vehicle model. Such failures can lead to catastrophic loss of control, and immediate attention is necessary to prevent future incidents and ensure driver safety.Read more

The engine and fuel pump system failed while driving the vehicle. It is available upon request to inspect. I just towed it to the shop today on 10/22/25 to get a full inspection. My sister and I were put at risk while driving towards a hill in San Francisco. It was on a busy main road when suddenly the vehicle began to jerk and thud back and forth. While I pushed the gas pedal, I began to question
... why the vehicle was suddenly reacting this way. In efforts to pull over to the side, a huge smoke fog began to come out of the exhaust pipe. As I tried to reach the curb the car didn’t want to move any further as I pushed on the gas. I put it in park and called the tow truck (receipts below) while there were cars passing me by almost causing an accident. I have looked online to see if there was a problem or recall with this vehicle having the same issue I was having. Come to find out there was a recall issued for certain vehicles only, with the same issue as my vehicle. The vehicle was taken into a local auto shop, where they diagnosed it and inspected it. The reports show it has the same exact issues with the car as those that were recalled. No there were not any warning signs nor failure signals. Due to this reason, I was completely caught off guard.Read more

There have been many times when driving, specifically over bumpy ground, but it has also happened over smooth/dry ground, where it feels like the anti-lock brakes suddenly kick in when they are not supposed to causing an inability to brake. You have to take your foot off the brake and brake again. This has caused several close calls of rear-ending someone or running through a stop sign or light.
... You must be keenly aware that this sudden braking inability/shaking of car, etc may happen and there always needs to be a lot of room in front of you. I mentioned it to the dealer, they noted there was nothing they can do but that they have reported it to Hyundai and Hyundai is aware and they have had other complaints. It has not been reproduced or inspected by anyone else at this time. I have not had any warning lamps or messages.Read more

The issue first appeared during my initial test drive before purchase in 2019. The salesman stated it was a low-battery issue from the car sitting on the lot. Since then, the same problem has recurred multiple times. The vehicle displays multiple warning lights — “Electric Powertrain Fault,” Check Engine, EPS, and VSA — with reduced power, jerky acceleration, and temporary loss of throttl
... e response. This poses a safety risk when merging or crossing intersections. Vehicle: 2018 Honda Accord Sport 1.5T, approximately 52,000 miles. Diagnostic codes retrieved include U0401-68 (ECM/Electric Powertrain Communication Failure), U0416-68 (VSA/EPS System Malfunction), and P0302 (Cylinder #2 Misfire). These faults have occurred despite replacement of spark plugs, ignition coils, and fuel injectors (all OEM Honda parts). Dealerships have not been able to permanently correct the issue. The problem appears related to the ECM or communication network between the powertrain and stability systems. The sudden loss of power and unpredictable throttle response during normal driving creates a serious safety hazard.Read more

In July 2025 recall #25V467 was issued on my vehicle. Many 2025 Hyundai Tucson owners have reported significant issues with the driver attention warning feature, particularly when using cruise control and wearing sunglasses, leading to safety concerns and frustration. Common Complaints Constant Alerts: Many drivers have experienced the driver attention warning activating unnecessarily, especially while using cruise control. Users report that the syst
... em beeps and prompts them to keep their eyes on the road, even when they are attentive and looking straight ahead. This has led to frustration, with some considering disabling the feature entirely. 2 Sunglasses Interference: A recurring issue is that the driver attention warning system struggles to detect the driver's eyes when sunglasses are worn. This has caused the system to issue warnings and disable cruise control, which can be particularly dangerous in heavy traffic situations. Drivers have noted that even moderately tinted sunglasses can trigger these alerts, making it difficult to use the vehicle comfortably. 2 Safety Concerns: Some users have reported alarming incidents where the driver attention warning system caused abrupt braking, leading to near-collisions. For instance, one driver described a situation where the system suddenly applied the brakes while driving between vehicles, creating a hazardous situation. This has raised serious safety concerns among owners, particularly regarding the system's reliability and responsiveness. 1 Lack of Support from Hyundai: Many drivers have expressed dissatisfaction with Hyundai's response to these issues. Complaints about the driver attention warning feature have been met with minimal support, and some users feel that their concerns are not being adequately addressed by the manufacturer. 2 3 Sources Summary of Issues Overall, the driver attention warning feature in the 2025 Hyundai Tucson has been a source of frustration and concern for many owners. The combination of false alerts, interference from sunglasses.Read more

There is an issue with the brake booster and brake booster pump assemblies that causes this vehicle to brake poorly. These parts are known to malfunction in Lexus CT 200h models and recognized by Lexus/Toyota. This issue needs to be extended into a recall for the vehicle due to the danger it poses while braking. Lexus/Toyota have acknowledged the issue via their letter: Customer Support Program 21
... LE01. The original publication of this program was issued on May 20, 2021. Within the letter they recognize that these parts will commonly break down. They issued a brief coverage on vehicles affected. A primary coverage on vehicles prior to November 30, 2022, and a secondary coverage on vehicles within 10 years of operation and 150,000 miles. With the secondary coverage applying to whichever came first. As a secondary owner of this vehicle, I had no idea this letter had been sent out. I had purchased this car late 2022. Had there been a recall notice on these vehicles, I would have been able to speak to a service representative about getting this issue remedied. Seeing as my vehicle is at 143,273 miles and 14 years in use, I am no longer covered through the Customer Support Program 21LE01. During my years of operating this vehicle I have experienced issues when braking. While driving and braking over bumpy roads the vehicle's braking would feel less effective. A week before my vehicle's Brake, Brake System, ABS, and Traction Control lights came on, I nearly experienced a collision with another vehicle while turning at a street light. The brakes should have been more than enough to have stopped the vehicle in the moment, and the ABS should have kicked in when it sensed me attempting to come to a harsh stop. Once I had diagnosed the dashboard lights I received OBD2 code C1391. Indicating an issue with the Power Brake Booster. Seeing as the issue is recognized by Lexus/Toyota, and this issue posing danger while operating the vehicle, there should be a recall imposed.Read more

A bike rack that I purchased from 1UP USA in February of 2025, failed while attached to my car and carrying 2 ebikes. The incident occurred on Monday, October 13th, 2025. While driving home the bike rack disengaged and fell backwards, causing the bike rack to hit the road. I was driving on a 2 lane road with traffic approaching me and behind me. There was no place to pull off the road. Myself an
... d my passenger exited the car at great risk to ourselves to upright the bike rack. The traffic going both ways was at risk, as the traffic had to stop suddenly to avoid hitting us or my car.The ebikes were still attached to the bike rack. I drove home very slowly, using backroads in the event that this happened again. I followed all directions per the website on how to mount the bikes and what safety procedures needed to be followed. Prior to this date, there were no problems with this bike rack. I contacted 1UP USA on October15th, providing them with the narrative, and photos of the damage to the bike rack. One of the arms of the rack is bent and damaged, the EZ pull handle that hit the road is damaged and not usable, and the part of the rack that caused this failure is damaged as well; a bar that should lock into sprockets to prevent this type of failure. 1UP USA initially refused to replace any part of the bike rack. Subsequently, they did say they would replace ONLY the EZ pull handle. They are adamant that they will not replace the entire rack. Their website states they have a lifetime warranty on their products. This failure was caused by the EZ pull handle not engaging properly. I took all necessary safety precautions with this rack, and my passenger witnessed me doing this. In addition, the day before the ride, October 12th, I did a comprehensive safety check of the bike rack, which I do before every time I use it. I noted a missing washer and bolt, and purchased these at Ace Hardware and replaced the missing parts. I submitted a receipt from Ace.Read more

My backup rear camera does not work more than half the time. The screen will go black or just not change at all and it will also say camera unavailable contact dealership. I have not been able to take it anywhere to get it looked at due to money issues. No warnings or anything. Just started one day in July 2025 and has kept doing it ever since.

MY STARTER MOTOR OVERHEATED AND MY CAR WOULD NOT START AND MY CAR INDICATED THE FLUIDS AND COMPONENTS WERE OVERHEATED - MY DEALERSHIP CONFIRMED THE PROBLEM WITH A VIDEO THEY SENT ME. IF I WAS STILL DRIVING THE CAR, I DON'T KNOW WHAT WOULD HAVE HAPPENED TO ME MY DAUGHTER AND HUSBAND IN THE VEHICLE. i CALLED MY SERVICE REP AND THE DEALERSHIP IS LOOKING INTO IT AS MY VIN DOES NOT HAVE THE RECALL. NO
... THING WAS WRONG AND NO LIGHTS WERE ON THE VEHICLE PRIOR TO THE CAR NOT STARTING. I HAD BEEN DRIVING IT FOR ABOUT 25 MINUTES. In MAY of this year, not even 6 months ago, they had to replace a "heat management module", which cost me $2500 and they said it is not related. They found 8 fault codes at a frequency of 255. The dealership is telling me this is not related, but I have a hard time believing that.Read more

My vehicle is a 2022 Polestar 2. For model years 2025 and below within the State of California, Polestar does NOT include the drive shafts (from electric motors to the wheels) as part of their 100K drive train warranty. And for model years 2026 on-wards within the State of California, Polestar will include the drive shafts (from electric motors to the wheels) as part of their 100K drive train wa
... rranty. Mainly due to a new propulsion law in California. For those with model years 2025 and below, a RECALL for known defective drive shafts should be granted as Polestar has revised parts for the rear left and rear right drive shafts. I believe the new shafts are now on the 2026 model. My vehicle is covered up to 50K miles, and I am now a few hundred over this, I have asked for goodwill on their warranty coverage but I am not receiving any response from Volvo Marin (this is Polestar Marin's service dealer). I also attempted to contact Polestar Support and they told me to reach out to the dealer, but they have been very unresponsive with several emails to the Service Director. Symptoms - during deceleration, a ticking sound can be faintly heard by the rear wheels. It sounds like the wheel of fortune ticking sound. Issues - when a driveshaft breaks, the vehicle immediately loses power and will likely stop moving, accompanied by loud clunking or grinding noises. At higher speeds, broken pieces can fly off and cause significant damage to other components or the road, and a broken driveshaft can also lead to loss of control, affecting steering and braking. Car was taken in for service for a water pump recall. When I receive the car back there is a radiator photo on the dashboard as if there is low coolant. After returning the car to BMW they are stating that the water pump part that was fixed has nothing to do with why there is now a coolant leak from the back of the radiator. The fuel leak is coming from the fuel pump.
