Within days of purchasing the vehicle, multiple warning lights and safety-related alerts appeared, including repeated check engine warnings, an alert stating “Driver Assistance Support Reduced,” and a wrench indicator referencing an auto lighting and wiping system malfunction. While driving, the vehicle felt unstable and would sway side to side, making it difficult to maintain confident contro
... l. The vehicle was brought in for service shortly after purchase due to these concerns and was removed from my possession because of the seriousness of the issues. During the service process, I was informed that the vehicle had previously been involved in an accident and that a critical structural safety component (the front crash bar) was damaged. This damage had not been disclosed prior to purchase. The crash bar is a key part of the vehicle’s body structure designed to absorb impact and protect occupants in a collision. Driving a vehicle with damage to this component posed a potential safety risk. The combination of structural damage, reduced driver assistance alerts, visibility system warnings, and unstable handling raised concerns about crash protection, vehicle control, and overall safety. The issues were identified by the selling dealer during inspection after the vehicle was returned for service and are available for further inspection if requested.Read more

Reporting a failure of the engine cooling system on my 2019 BMW X3 that developed over time despite repeated repair attempts and ultimately resulted in complete engine failure. Over the past year, the vehicle has experienced ongoing coolant loss and overheating warnings despite multiple coolant-related repairs. When the problem continued, I brought the vehicle to a BMW dealership, where the servic
... e department determined that the engine had suffered internal damage consistent with overheating and advised that a full engine replacement was required. These issues occurred during normal driving and created unsafe conditions, including the risk of sudden loss of power or breakdown in traffic. Based on my experience and the repeated nature of the failures despite proper maintenance, it appears that coolant and overheating issues may be persistent in this vehicle model rather than isolated to a single component, and I am concerned that other owners may be experiencing similar safety-related defects. A investigation into this is warranted.Read more

I contacted the corporate office with hopes they would assist with a good will service request. However it appears not only do they not stand behind their products but apparently bmw’s are only meant to last 4-5 years. When questioned if it’s normal for components to continuously melt under the hood he agreed it is not. However refused to offer an equitable solution considering the issues I’ve had over the years and their mechanics currently not being able to state 100 percent what’s wrong with the automobile. Summary: Remote start causes repeated, predictable failure of critical safety systems including traction control, hill descent control, and automatic emergency braking. Description of Problem: When the vehicle is started using the factory app for phones, the lower infotainment screen (the climate/drive assist panel) frequently fails to initialize properly. As a result, the icons and functions for
... Traction Control, Hill Descent Control do not illuminate or activate. The vehicle then displays warnings such as “Hill Descent Control Not Available” and “Automatic Braking Not Available.” This failure occurs approximately 99% of the time when using remote start. If I begin driving with the vehicle in this state, all of these systems remain deactivated. These features are essential for safe driving especially in winter conditions yet the malfunction leaves the vehicle without stability control, hill descent functionality, and automatic braking assistance. Safety Concern: This issue creates a dangerous situation where the vehicle behaves unpredictably and lacks core safety features required for traction, stability, and controlled braking. This is particularly unsafe in cold, icy, or low-visibility environments where remote start is most likely to be used. Reproducibility: The issue occurs consistently after remote start. If the vehicle is parked, fully shut down, locked, and left undisturbed for 10–15 minutes, the systems return to normal. The malfunction clears before the dealership can inspect it, leaving no fault codes stored. Dealer Response: The dealership has been unable to diagnose the issue because the fault resets itself before technicians can observe it. They report “no codes stored,” yet the problem persists daily and is easily reproducible by any owner using remote start. Why This Needs Investigation: This is not an isolated glitch. It disables multiple federally-regulated safety systems. A potential software or integration failure is affecting the control moduleRead more

On 12/10/25, my 2019 CX5 NA had major oil leak after smelling a burning smell for a few days. Vehicle went to the Mazda dealership on 12/15 and it was confirmed that there was a crack in the cylinder head and received an estimate of $11,000 needed for repairs. $7000 of that was for a new cylinder head. This was after an independent mechanic inspected my vehicle and stated everything looked good besides the main issue and needing new rear breaks in the next few months. 12/15-contacted Mazda customer experience and and case file was opened. 12/16-contacted customer experience back due to not receiving a call and received the name and extension of the assigned case manager. 12/17- still had not received a call back and left voicemail for case manager. Requested a call back to myself and the dealership; received a call back a few hours later and I was never asked what happened but was informed that Mazda corporate could not help me due to the warranty being expired. Summary On September 30, 2025, the passenger in a 2017 BMW i3 REX was involved in a severe frontal collision with a Gas Company Utility Vehicle. The impact struck the driver’s side extending toward the passenger side, causing the vehicle to spin violently and nearly flip. The passenger reports seatbelt failure with excessive slack, leading to partial ejection from the seat, mid-thigh restraint,
... and collision with the steering wheel. The head lodged under the steering column, resulting in loss of consciousness and a severe traumatic brain injury (TBI) with ongoing neurological issues. Only the driver’s curtain and seat airbags deployed; the steering wheel and passenger frontal airbags did not. Seatbelt Failure Analysis Seatbelts in modern vehicles like the BMW i3 include retractors and pretensioners designed to lock and tighten upon impact detection via accelerometers. Excessive slack, as described, suggests a malfunction in the pretensioner system, which failed to eliminate belt looseness during the high-G force event. This allowed forward momentum to propel the occupant, exacerbating injuries. Common issues in BMW i3 models include seatbelt twisting and retractor malfunctions, as reported in owner forums and repair services. Pretensioner failures can stem from sensor errors, mechanical wear, or manufacturing defects. In this crash’s severity, proper function should have secured the occupant, preventing ejection and secondary impacts. Airbag Failure Analysis The BMW employs a supplemental restraint system (SRS) with frontal, side, curtain, and seat airbags, triggered by crash sensors evaluating impact direction and force. Non-deployment of the passenger frontal and steering wheel airbags in a near-head-on collision indicates potential system failure. Possible causes include faulty occupant classification system (OCS) mats that misdetect presence/weight, leading to deactivation; sensor malfunctions; or wiring issues. My 2019 Honda CR-V, which has the 1.5 liter turbo engine, with 78,000 started up with the dashboard lit up like a xmas tree and reported approx. a dozen faults of all safety features EPB, LKAS, Emissions, etc. It was diagnosed by Lester Glenn Honda in Wall, NJ with oil dilution caused by the leaky fuel injectors. The mechanic noted TDC P0172 - System Too Rich & P0301- cylinder 1 misfire and all f
... uel injectors failed A/F test. This vehicle had its fuel pump replaced earlier this year pursuant to the Honda recall for this (wondering if related). The same underlying problem was subject to a safety recall for the 2017-2018 model years, but the defect was not included for my 2019 model year even though it's the same 1.5L Turbo motor and there are numerous complaints from customers on the same exact issue. I was quoted over $2300 since it was past the warranty period (8k miles past the extended 7/70k emissions warranty that would have covered this 100%). Not only is this premature failure of critical engine component making the engine run very rough with a sudden and severe loss of powers, it produces a safety hazard of a direct rear-end crash risk due to the inability to maintain a safe operating speed. This is a significant expense for a defect Honda is already aware of. I believe this is a manufacturing defect that should be fully covered by Honda, along with any related systems affected by this issue. I respectfully request that NHTSA investigate this widespread defect affecting 2017–2019 CR-V models and issue a recall. I have not had it repaired yet as I asked the dealer to contact Honda Corporate and ask for a goodwill/good faith warranty.Read more

The ICCU (Integrated Charge Control Unit) failed. Fortunately I was in a parking lot at the time, as the vehicle limited its top speed to around 25 mph. Had the failure occurred on a highway it could have caused a collision. While this is a 2025 model year Ioniq 5, the ICCU hardware (part 36400-1XAA0) remains unchanged across all Hyundai/Kia/Gensis vehicles sharing the eGMP platform. NHTSA prev
... iously issued recall 24V-868 intended to address such ICCU failures. While the modified software is included from the factory in the 2025 model year Ioniq 5, it appears that the 24V-868 recall is not a complete fix. Indeed, ICCU failure continues to be one of the most commonly reported owner issues, both for vehicles which have already had the “fix” from recall 24V-868 and for newer vehicles outside of the recall’s scope. This can be confirmed from the numerous posts and videos online (YouTube, Reddit, Ioniq forums, etc.). Given that ICCUs continue to fail at a significant rate, resulting in reduced vehicle speed and vehicle shutdown within minutes (as soon as the 12 V battery is exhausted), I strongly urge NHTSA to do the following: 1. Consider the current ICCU software update to be an ineffective remedy for recall 24V-868. 2. Broaden the scope of recall 24V-868 or create a new recall for all Hyundai/Kia/Genesis eGMP platform vehicles to address this defect. 3. Require that Hyundai/Kia/Genesis create and deploy a truly effective remedy for this defect.Read more

... -old crash test dummy, the child restraint’s top tether system experienced repeated structural failures. The seat was installed using the lap-and-shoulder belt and the top tether, per manufacturer instructions. Across three out of three tests with the standard Hybrid III 6YO dummy, the top tether released before the dummy’s head reached its maximum forward excursion. The specific failures included: -Two tests where the top tether hook broke -One test where the tether anchor on the test sled broke In one out of one additional test using the weighted Hybrid III 6YO dummy, the top tether strap itself sheared (tore) off just past the double-webbing section, again before maximum head excursion. In all four tests, the tether system failed early in the crash event, eliminating the intended restraint effect and allowing for the potential of significantly increased head and neck excursion. The top tether is a critical safety component for forward-facing seats, designed to limit forward head movement and reduce head and neck injury risk. Early release or structural failure of the tether system can result in: -Excessive head excursion -Increased head and neck loading -Increased likelihood of head contact with the vehicle interior -Reduced effectiveness of the restraint system as a whole Because the failures occurred across multiple test samples, and involved different components of the tether system, this may indicate a systemic issue rather than a single defective part. The nature of these failures may also indicate potential non-compliance with FMVSS 213 performance requirements. INFORMATION REDACTED PURSUANT TO THE FREEDOM OFRead more

The tailgate control module has been water damaged and needed to be replaced twice. My local Honda dealership was unable to correct the issue and stated that it is due to a water leak in the frame of the vehicle/tailgate. The dealership has identified the tailgate control module as the cause of the electrical issues causing the battery to die, and advanced safety features to malfunction. Warning
... lights came on for break system problem, lane departure mitigation system problem, and many other systems all caused by the tailgate control module being water damaged from a leak in the frame.Read more

While operating the vehicle, the steering system suddenly developed a fault with no prior warning. A "Changed Steering Effort" warning appeared on the dashboard, immediately followed by significant resistance in the steering wheel and violent mechanical shuddering/oscillation. This resulted in a sudden, dangerous difficulty in maneuvering the vehicle. Despite the 2024 BMW iX and related models (5
... /7 Series) having known internal steering gear manufacturing defects (referencing SIB 32 01 24 and Recall 24V-714), the dealership is attempting to deny warranty coverage. They are attributing the failure to a tear in the rack boot, despite no evidence of impact to the surrounding undercarriage. The sudden electronic onset of the failure suggests an internal safety defect consistent with widespread reports on this model year, rather than environmental damage. There is significant online anecdotal evidence of this same issue occurring with 2023 and 2024 iX models, despite the above SIB only covering certain 2021 and 2022 models.Read more

It seems that this vehicle was not included in a brake booster recall which included this model from 2005 through 2015. The brakes are not working properly, and brake fluid has entered the master brake cylinder damaging the seal. It is extremely unsafe to operate this vehicle in this condition. Somehow, this vehicle was not included in the recall, but definitely should have been.
