The integrated central control unit failed on my Hyundai Ioniq 5. A warning flashed on my dashboard stating that the car needed to be turned off immediately because of a power issue. I was in traffic on a very busy road and was trying to navigate to a safe place when my car stopped and turned off. After several attempts, I was able to start it enough to get off the street and to a safe place. I wa
... s fortunate that I was stopped in traffic or someone may have hit me. The tow truck driver said that he is towing at least one or two 2024 or 2025 Hyundai Ioniq 5 vehicles every day for this issue. The Hyundai service center said the ICCU failed and do not have an estimated date for replacing it because of the number of ICCU units that are failing is so high and they are backordered. I do not feel that this car is safe to drive. These cars should be recalled. If I had been driving on a freeway, I could have been hit and killed because I would not have had time to find a safe place to stop.Read more

I am experiencing a steering defect in my vehicle that closely mirrors the condition described in Recall No. 6524A, although my specific vehicle VIN is not currently included in the recall. While driving, particularly at highway speeds, the steering wheel intermittently becomes difficult to turn and appears to “stick.” During one incident, I was traveling approximately 55 mph on the freeway w
... hile navigating a right-hand curve when the steering wheel suddenly became resistant and did not move smoothly in the direction of the turn. The steering felt as though it momentarily bound or stuck while I was actively turning to the right, requiring increased force to maintain control of the vehicle. In addition to this event, I have repeatedly noticed resistance during right-hand turns, even at lower speeds. The resistance does not feel normal or smooth and gives the sensation that the steering mechanism is catching or binding. This issue occurs without warning and presents a significant safety concern, especially at highway speeds or while navigating curves. The steering resistance reduces the driver’s ability to control the vehicle predictably and could increase the risk of a loss-of-control event or collision. Because the symptoms I am experiencing closely match those described in Recall 6524A, I am concerned that my vehicle may be affected by the same or a related steering defect despite not being officially included in the recall at this time. I am submitting this report in hopes that this issue will looked into immediately.Read more

At some point in June 2025, between approximately 11 a.m. and 2 p.m., I was driving southbound on the 110 Freeway in Los Angeles, just south of the 134 interchange, at approximately 45 mph. Traffic was light, with no vehicles in front of me and no vehicles attempting to merge into my lane. Without warning, a bright red alert appeared on the dashboard of my 2019 BMW x3 and the vehicle suddenly app
... lied full braking. The position of my arms at that moment caused an injury to my right elbow, which has remained painful since the incident. In January 2026, under nearly identical circumstances, the emergency warning appeared again. This time, the brakes did not engage. In July, I contacted BMW Corporate and service center regarding these incidents. I found their response dismissive, stating that “sometimes that happens.” A service center later informed me that this is a known issue referred to as “phantom braking.” Having experienced this firsthand, I believe this represents a serious safety concern. In addition to my injury, I could easily have been rear-ended at freeway speed. Despite acknowledging the issue, BMW Corporate requested that I sign an indemnity waiver in exchange for covering only the inspection cost. This response feels inappropriate, as their initial focus appeared to be limiting liability rather than addressing a known safety defect. Given that this is reportedly a known issue, I believe an inquiry is warranted into how often this has occurred and whether others have been injured.Read more

NHTSA Complaint Narrative I am filing this complaint regarding a 2010 Mazda 3, VIN [XXX] . The vehicle experiences intermittent transmission control failures, including difficulty shifting out of first gear and warning indicators triggered by vibration. The transmission and transmission control module have already been replaced. The dealership has identified the wiring harness as the cause of th
... e remaining failure. Mazda North American Operations has confirmed that the required wiring harness (BBM9-67-020H) is discontinued and unavailable. However, Mazda has not provided a written determination of whether the known wiring harness recall (engine wiring harness chafing affecting control systems) applies to this VIN, nor confirmed recall completion history. Mazda has advised the customer to seek aftermarket or salvage parts, but has not provided an approved repair remedy or goodwill assistance despite the issue affecting drivability and safety. I am requesting that NHTSA review whether Mazda has met its obligations regarding: 1.recall applicability determination by VIN, and 2.providing an adequate remedy when OEM repair parts are no longer available for a safety-related defect. On the morning of January 12, 2026, I attempted to unplug my 2023 Volvo XC90 Recharge (plug-in hybrid) from my home charging station in order to drive to an appointment. I had turned on the climate control remotely from the Volvo app while inside the house. When I went to unplug the vehicle, the charging connector was locked in the vehicle’s charging port and could not be removed. I contacted Vo
... lvo customer service, who instructed me to try several troubleshooting steps, including locking and unlocking the vehicle multiple times with the key fob and resetting the infotainment system. These steps did not resolve the issue. Because the charging plug remained locked, I was unable to use my vehicle and had to borrow a neighbor’s car, causing me to arrive late to a scheduled appointment. While away from home, I contacted my Volvo dealer, whose service representative advised me to use force to remove the plug, stating it would likely break the internal clip in the charging port. When I returned home several hours later and unlocked the vehicle, the charging plug released normally without any physical intervention. This suggests a software or electronic malfunction rather than a mechanical obstruction. The incident temporarily immobilized the vehicle and could pose a safety or reliability concern if it occurs in other situations. Furthermore, we have been experiencing intermittent issues with the infotainment system, including the backup cameras not coming on, random radio stations playing and complete blackout of the screen and/or flickering. The last software update was completed on Aug 15, 2025.Read more

INCIDENT SUMMARY: On January 9, 2026, at approximately 1:30 PM, while traveling at 50 MPH in North Carolina, the vehicle experienced a sudden, catastrophic engine seizure without any prior "Limp Mode" warning or Check Engine Light. This resulted in an immediate, total loss of motive power in active traffic. I was forced to navigate a dead multi-ton vehicle across lanes without power steering or va
... cuum-assisted braking, narrowly avoiding a high-speed collision. TIMELINE & NEGLIGENCE: This vehicle was purchased from an independent retail dealer on December 20, 2025—less than 21 days prior to this life-threatening failure. The vehicle was sold with an "Open" status for Safety Recall 23V-707. The failure occurred exactly as described in the recall’s warning: a catastrophic stall at speed. TECHNICAL EVIDENCE: A non-invasive borescope inspection of the cylinders has confirmed the presence of metallic "silver chunks" (sheared bolt heads) and "half-moon" impact marks on the piston crowns, indicating a total timing failure and internal mechanical collision (piston-to-valve contact). REQUIRED REMEDY: This incident is a direct manifestation of the safety risk documented in Recall 23V-707. Per BMW Service Bulletin SIB 11 09 23, the presence of sheared bolt heads and seized internals constitutes "Catastrophic Consequential Damage." I am filing this report to ensure federal oversight of BMW North America’s obligation to provide a complete engine replacement, as the safety defect was present at the time of sale and resulted in a near-fatal incident within 300 miles of ownership.Read more

I am reporting a safety-related engine failure involving my 2011 Hyundai Sonata 2.0T. In November 2025, at approximately 90,000 miles, the vehicle experienced a sudden engine failure while driving, including loss of power and abnormal engine noise. This created an unsafe driving condition and required the vehicle to be removed from service. Component/System Involved: The engine (Theta II 2.0T) s
... uffered a catastrophic internal failure. The engine and internal components remain available for inspection upon request. Safety Risk: The sudden loss of engine power reduced my ability to maintain speed and maneuver in traffic, posing a safety risk to myself and other motorists. Confirmation and Inspection: The failure was confirmed by an authorized Hyundai dealership, which inspected the vehicle and documented the internal valve train condition through photographs. Hyundai Motor America reviewed the claim and issued a written denial. No independent third-party technical inspection was performed. Prior Symptoms: Before the failure, the vehicle had a history of excessive oil consumption requiring frequent monitoring and oil addition despite regular maintenance. Abnormal engine noise was present for approximately one minute before failure. No warning lights adequately predicted the sudden failure. Additional Context: This vehicle is equipped with a Theta II engine, which has been the subject of prior recalls and settlements related to internal engine defects. Hyundai denied warranty coverage alleging “exceptional maintenance neglect”; however, inspection photos show a well maintained valve train with no sludge or oil flow obstruction, only light surface varnish consistent with normal engine aging. I am concerned this failure represents a continuation of a known Theta II engine defect (confirmed by Hyundai technician who performed inspection with OBD code P1326 followed by engine failure) and poses a serious safety risk due to sudden loss of engine power while driving.Read more

Dear Sir or Madam, I am writing to report a serious safety incident involving my Tesla Model Y (2026) licence plate : [XXX] that occurred on [XXX], at approximately [XXX]. While attempting to slowly park in a parking lot located at [XXX], East [XXX] and forcefully accelerated forward. I was performing a low-speed parking maneuver and did not intentionally press the accelerator. The vehicle
... suddenly moved forward with very strong power and high speed, making it extremely difficult to control the steering wheel. If there had not been bushes directly in front of the vehicle, the car would have continued accelerating forward and could have caused a serious accident involving property damage or personal injury. This was a very dangerous and alarming situation. Despite my attempts to stop and control the vehicle, it continued to move forward rapidly. As a result of this unexpected behavior, the vehicle struck bushes within the parking lot. The vehicle’s response was sudden, abnormal, and completely inconsistent with a normal parking situation. This incident raises serious safety concerns regarding the vehicle’s behavior during low-speed parking and a possible malfunction of the automatic or driver-assistance systems. I own a 2020 Chevy Silverado 1500, the truck currently has 51,400 miles on it. I started noticing a vibration while driving home from work that continued to get worse. After some quick online research I learned that the torque converter on the newer Chevy and GMC trucks is a weak point in their assembly. This is a very common issue and complaint for owners. The manufacturer has done nothing to res
... olve it or honor repairs on the defective product. My truck was one month out of the 5 year/60,000 mile warranty, they would not honor repairs because it was outside of the time window. The internal parts break up and destroy a transmission. Luckily i caught it early enough before it caused my transmission to fail while driving. I do not have the part with me. I took the truck to a shop for repairs and they did not return the defective part. No warning lights came on the dash.Read more

Safety issue involving loss of power and braking Prior to receiving a PTU recall notice in January 2025, my vehicle began experiencing sudden and unexpected loss of power while driving, particularly when braking. Since that time, the issue has worsened significantly and now occurs more frequently. On multiple occasions, the vehicle has failed to accelerate properly after braking, creating danger
... ous situations in traffic. In at least one incident, there was a loss of braking power that nearly caused me to run a red light. Several of these incidents were near-accidents and occurred without warning. I have brought the vehicle to my local Jeep dealership three separate times and fully explained the loss of power and braking concerns. Each time, I was told that all issues are “directly related to the PTU recall,” that there is currently “no solution,” and that “nothing can be done.” The vehicle was returned to me without repairs despite the safety risk. The dealership has acknowledged the issue but has not provided a fix, alternative transportation, or assurance that the vehicle is safe to operate. With a longer commute and the problem occurring more often, I no longer feel safe driving the vehicle and believe an accident outside of my control is inevitable if this continues. This defect presents a serious safety hazard involving loss of acceleration and braking while driving.Read more

One to two times per month upon starting my Cybertruck I receive multiple notifications for systems not working including "Driver Assistance - Automatic Emergency Braking is unavailable" and "Steering & Braking - Adaptive ride control degraded/Ride comfort may be reduced." These alerts cause the disablement of the regenerative braking, auto-hold (braking), emergency braking, adaptive ride control
... , traction control, and the self drive features. While the vehicle is still drivable, my ability to control the vehicle is greatly diminished, as is the safety of the vehicle without any of the accident avoidance and stability systems working. After driving in this condition, if I park the vehicle, exit it, and let it sit for at least 10 minutes, all systems are normal upon reentry. On 1/5 I experienced this issue and opened a service request for warranty work and Tesla demanded that I agree to a $200 charge before they would even look at it and said if they were unable to see the issue then it would not be considered as warranty. Since I cannot reproduce this issue on demand I cancelled the service request as I do not want to pay them $200 to say they couldn't reproduce the issue. Internet searches have revealed that this is a common issue among Cybertrucks and despite multiple software updates since owning my vehicle, it has not gotten any better.Read more

Transmission seems to be faulty. While accelerating the transmission will shift from 4th gear to 1st gear then up to 3rd gear. Sometimes it will return to normal operation at the point, sometimes it will continue to shift up and down at random. I have been told by the shop (more than 1) that the transmission is fine. This random and hard down shifting cause the vehicle to lose speed which is very
... unsafe. I first noticed the problem about 6 months after I purchased the truck (Oct. of 2024) as a hesitation when shifting from 3rd to 4th. This is an ongoing problem that continues to worsen as time goes on. Currently the vehicle is available with reasonable accommodations to be inspected.Read more

My wife [XXX] ) was traveling in her 2024 Nissan Rouge at highway speed on her way to work on 1/5/26, in Southeast Alabama, no defroster on, or other car in sight. While traveling her rear windshield randomly busted into a thousand pieces. She immediately notified myself [XXX] ) and the local Sheriffs Office, first believing someone had shot at her car. After deputies arrived and a thorough inve
... stigation, revealed that fortunately no one had shot at the car. That it appeared to be a random flaw/defect with the back glass windshield. They also noted that that a similar incident occurred last week mid morning, with a gentleman in a same make and model rouge, as he was traveling, busted without warning. I attempted to call dealership, but they send unbothered and informed me nothing could be done due to no active recall. Upon my own investigation today, I have noted and located many instances of the same occurring in similar models, and even some class actions suits potentially in the works. Please review this with the correct persons and hopefully find some safe resolve. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
