1. What component or system failed or malfunctioned, and is it available for inspection upon request? The engine and the HVAC systems. Specifically, a persistent "ghost misfire"—where the engine vibrates and shudders without triggering a dashboard warning light—and a total heater failure. The vehicle is currently held at the dealership; all service records (RO# 367641, 367891, 367980, 368179)
... are available. 2. How was your safety or the safety of others put at risk? The "ghost misfire" causes shaking and hesitation during acceleration, creating a risk of losing power or stalling in high-speed traffic. Because the vehicle's diagnostics do not trigger a warning light for this condition, the driver has no advance notice of a potential engine failure. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Dealership technicians confirmed the misfire on Feb 9 (RO# 367891) and found a defective spark plug gap (.024” vs. .036”). On Feb 25, the service consultant confirmed the "ghost misfire" remained active. The heater failure was confirmed via a 'Heater Support Pump Dry Run' fault code (RO# 367641). 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle has been inspected by dealership technicians. Formal complaints have also been filed with the California Bureau of Automotive Repair and the California Attorney General. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Symptoms began following a service on Jan 30. Feb 8: Exhaust malfunction light (MIL) appeared briefly with severe shaking. Feb 9–present: Exhaust malfunction warning lights appeared briefly and then disappeared, but the engine vibration and "ghost misfire" symptoms remain active and undetected by the vehicle's onboard diagnostics. Feb 17: Total loss of heat occurred during a highway drive without a dashboard warning lampRead more

All four of my door lock actuators are not working properly. All four usually don’t work, however once in a great while they will actually lock or unlock. I always have to check all 4 doors a couple times, because sometimes they will unlock but not

While driving, the vehicle displayed a 'Drivetrain malfunction' warning on the dashboard and began jerking. I safely pulled over and turned off the vehicle. Immediately after stopping, there was a distinct, strong electrical burning smell coming from the engine compartment. The vehicle was inspected by an authorized BMW dealership. The dealer diagnosed a catastrophic failure of the starter motor.
... The vehicle's computer logged fault codes 21611A (excessive counter-torque), 21A511 (Start system: Timeout), and 216119 (Line disconnection). These codes confirm the starter motor suffered a severe electrical overload and thermal event. This premature failure and fire hazard identically matches the safety risks outlined in BMW's recent massive starter motor recalls (such as NHTSA Recalls 25V-636 and 26V-056), but my specific vehicle is currently excluded from those recall campaigns. The starter requires complete replacement due to this overheating damageRead more

I purchased a used 2023 Genesis GV60 on [XXX] from a franchised Genesis dealer in New Jersey (Genesis of Wayne, Wayne, NJ). At the time of delivery, the vehicle had an open safety recall with an available remedy and it was not completed prior to sale/delivery. After purchase, Genesis’ official recall/campaign lookup for my VIN showed at least one open safety recall listed as “Incomplete.” T

The vehicle is experiencing a serious door locking malfunction. Even after the car is locked, the doors repeatedly attempt to lock again every few seconds. During this time, the horn sounds and the lights flash continuously, as if the vehicle is trying to re-lock itself but failing. This behavior continues repeatedly. This issue raises serious safety concerns for the following reasons: •The veh
... icle may not be securely locking. •The repeated locking attempts may indicate a failure in the door lock actuator or body control system. •The malfunction could potentially cause the doors to unlock unexpectedly. •The constant horn and light activity drains the battery and creates public disturbance. The problem occurs intermittently but frequently enough to cause concern. There have been no recent modifications made to the vehicle. I am requesting that the National Highway Traffic Safety Administration (NHTSA) investigate this issue to determine whether it is a widespread defect affecting other 2015 Nissan Rogue vehicles. Vehicle Details: •Make: Nissan •Model: Rogue •Year: 2015 •Mileage: 127,000 miles •VIN: [XXX] Thank you for your attention to this matter. The dealer was kind enough to provide a smaller loaner vehicle but had no idea when my car would be returned, as there were others with similar failures ahead of me that were waiting for the part to arrive. Apparently, this is a known failure issue of the ICCU that Hyundai has done a poor job of addressing. I continue to make my lease payments while waiting for the part to arrive. The Safety Hazard The primary risk is a sudden loss of motive power. When the slide fails, oil pressure becomes unregulate

Immobilization: Vehicles may stall in active traffic or high-speed environments without sufficient warning. Environmental Risk: Failures peak in extreme cold, leaving occupants stranded in freezing conditions while awaiting recovery. Detection Gap: The electronic oil level sensor often fails to complete a measurement ("Measurement Aborted") before a low-pressure warning appears, depriving drivers of a clear precursor to mechanical failure. Critical Technical Challenges Obfuscated Parts History: BMW has superseded the original plastic-vane pump with a revised metal-vane version. However, the Electronic Parts Catalog (ETK) now lists the metal version as the "original" part for affected VINs, making it impossible for owners to verify which version is physically installed without engine teardown. High Repair Barrier: Correcting this defect requires extensive disassembly, often costing $6,000–$8,000. If the failure occurs at speed, total engine replacement is frequently required.Read more

On 2/23/26 my GMC sierra 1500 AT4 3.0L Inline-6 Turbo-Diesel, it auto stopped (which I will never keep on again) on an incline and would not start back up or shift gears and rolled a few feet. I had to turn it off and restart it, then the engine light came on and a notification about conditions not favorable for shifting? My app said service was needed for engine/transmission issue. I took it to
... the dealership and it now has to have the transmission control valve body assembly replaced. I did not receive a recall notice for my truck but, it seems to be the same issue that the recall was for on GM vehicles of the same time period. Of course the dealership said they have no loaner vehicles and I'm supposed to be without my truck for approx. a week and a half at this point. I am disappointed with GMC and the quality that I am paying $70k for. I had a 07' Silverado 2500 I purchased new and drove it for almost 15yrs. w/ no issues. Hopefully this is a good fix and it'll last but, from what I've seen, GM is not the quality it used to be. This is on top of being in the shop for a coolant control valve issue that wasn't recalled either but, a known issue on the pickups so beware. I did call GMC and they will reimburse up to $44 a day on a rental (has to be a GM vehicle btw) during this fiasco of course afterwards and with a bunch of paperwork but, I have to have a vehicle, in TX nothing is close by. On January 12, 2026, the vehicle shut off twice during the same trip while driving in traffic. All dashboard warning lights illuminated, the tachometer spiked, RPM readings did not correspond, the gear selector became unresponsive, and the vehicle lost propulsion while in motion. On February 15, 2026, the vehicle shut off again while driving under similar conditions. The vehicle was taken to a H

According to the repair order narrative, after the battery replacement, the stall condition recurred during dealership testing. Multiple extended road tests and datalogging were later performed. The dealership has stated they are unable to re-replicate the condition and that no active diagnostic codes are present. The vehicle has experienced multiple loss-of-power events while in motion. An intermittent stall while driving presents a significant safety hazard, particularly in traffic conditions. The root cause has not been definitively identified.Read more

Incident date: Aug 22, 2024
I'm reporting a safety issue with BMW vehicles equipped with the B58TU engine, primarily 2019-2021 models. These vehicles have an oil pump with an internal plastic component that regulates oil pressure, and this part is fracturing in the field, causing sudden loss of oil pressure. The failure typically occurs during cold starts in low temperatures. When it fails, oil pressure becomes erratic or is
... lost entirely, often with little to no warning before engine damage or vehicle disablement. Warning signs, when present, include inability to measure oil level electronically, abnormal oil pressure, or sudden drivetrain malfunction warnings. These can rapidly progress to engine shutdown or severe damage from oil starvation. This creates a dangerous situation because the vehicle can become disabled without warning while driving or after startup. This is especially hazardous when cars become immobilized in traffic or during winter conditions when failures occur most frequently, leaving occupants exposed to freezing temperatures while awaiting assistance. Pump replacement requires extensive disassembly and commonly costs $6,000-$8,000, assuming no engine damage. A critical concern: owners cannot determine if their vehicle has the original plastic pump or the later metal revision. BMW's VIN-based parts lookup now shows updated components even when original plastic parts remain installed, and BMW has removed the original plastic part number from dealer networks entirely. Failures are reported across multiple production years, and the scope of affected vehicles remains unclear. Because this condition causes sudden vehicle disablement without adequate warning, especially in cold weather, I believe this warrants investigation as a potential safety-related defect.Read more

Description of the Problem: •Component/system that failed: The engine starter, which is subject to an open recall issued September 23, 2025. The starter was not replaced during a service visit. The vehicle no longer exists due to the fire, but the VIN and service records are available for inspection. •Safety risk: The starter malfunction caused the vehicle to catch fire while parked, creating
... a serious safety risk to myself, my child, and others nearby. •Problem reproduction/confirmation: The fire and defect were confirmed by BMW’s inspection as originating from the starter irregularity tied to the recall. •Inspections: The vehicle was inspected by BMW North America’s engineering team after the fire, and insurance representatives confirmed the total loss. •Warning signs prior to failure: There were no warning lamps, messages, or symptoms indicating the starter defect before the fire. Additional context: The vehicle was serviced at an authorized BMW dealership on December 3, 2025. I was not informed of the recall, and the starter was not replaced. The vehicle caught fire on December 19, 2025, resulting in a total loss. While my insurance resolved the ACV, this incident was caused by BMW’s recall-related defect and procedural failure. I have escalated this matter to BMW North America and am requesting a resolution that restores me to a comparable position (replacement vehicle or financial support). Official confirmation: BMW North America confirmed in a confidential letter that the fire originated from the starter defect tied to the open recall. I was driving my vehicle home after a nearby out of town errand. I was on US-95 between Potlatch ID and Moscow ID which is a very hilly, curvy 2-lane 60mph highway with little to no cell service. As I ascended my first big hill, my engine light came on flashing and the ride became noticeably bumpy. I could not call anyone for many miles yet, so I put on my safety hazards and continued driving, hop
... ing to make it to town. With each hill my car became more unstable and seemed to be struggling to accelerate. Once I had cell service just outside of Moscow I called my husband to tell him what was happening, where I was at and proceeded to head to an auto repair shop. Once I drove into Moscow, my car was incredibly shaky and noisy and the engine seemed to struggle to stay engaged at stop lights. I brought it to Hive auto repair and they told me that my engine was misfiring, but that they would run a level 1 diagnostic that day. When they called with the results, they confirmed the following information: Scanned for DTCs and found P0301 - Cylinder 1 Misfire Detected. On this 3.6L engine, Cylinder 1 is located beneath the intake manifold, requiring intake removal for further access and testing.-- I continued to level 2 inspection and removal of the manifold which showed significant problems within the engine at Cylinder Head 1. There was oil intrusion and low compression, consistent with warping of the head. They said that this was a known issue with this engine on Stellantis vehicles because of an EGR valve on cylinder 1 2018/2022. Even though I had my Jeep regularly serviced at McGee Jeep in Barre VT, they never tested for this because it wasn't recommended or in the scheduled maintenance. But when I spoke with them they confirmed this is a known issue. I understand it is a severe, dangerous and expensive repair, but usually occurs after the warranty expires. I now have an unsafe, undriveable vehicle with a $14k repair pending.Read more

Passenger side mid row seat belt was completely pulled but won’t retract anymore. I just brought my Sienna to Toyota to get it fixed for a $299.00 plus whatever extra cost will be. I want to make sure this is not a safety factory issue vs unfortunate event.

1 The vehicle’s dashboard upper trim panel (leather covering and stitching) has failed. The leather is lifting and separating from the dashboard surface, and the stitching is coming undone. The vehicle is available for inspection upon request. 2. The lifting and peeling leather creates a safety concern. The leather separation is present at all times and continues to lift from the dashboard. The
... condition becomes significantly worse during hot and humid weather, as the material expands further. This can obstruct the driver’s forward or peripheral vision, including blind spots, reducing the ability to clearly see passengers, other vehicles, or road hazards. Additionally, if the material continues to detach, loose sections could interfere with steering wheel components or dashboard controls, creating a distraction or potentially affecting safe vehicle operation. 3. Yes, the issue has been inspected and confirmed by a dealer. The dealer acknowledged the dashboard leather separation and stitching failure. Similar issues have also been reported in other vehicles of the same make and model. 4. The issue has been inspected and confirmed by an authorized dealer. It has not been inspected by police or insurance representatives. The manufacturer has not directly inspected the vehicle at this time. 5. There were no warning lamps or dashboard messages prior to this issue. The first symptom was visible lifting of the leather and loosening of the stitching on the dashboard. The condition has progressively worsened over time and becomes more pronounced during hot and humid weather, although the separation is present regardless of weather conditions.Read more

1. The engine starter motor (specifically the pinion starter) has failed, and the auxiliary battery is also showing reduced capacity (only a portion of its 660 cold cranking amps). The vehicle is currently at a service center where the technician has diagnosed these issues. The components should be available for inspection upon request, as they have not yet been replaced. 2. The failure of t
... he starter motor and battery primarily affects the vehicle's ability to start. While this could potentially strand you in an unsafe location, the information provided does not indicate any specific safety incident or risk that occurred during operation. No direct safety hazard has been reported. 3. Yes, a technician has diagnosed the problem and confirmed a fault code in the drivetrain related to the failed starter motor. The technician also identified the auxiliary battery's reduced output. This confirmation comes from a service center, which could be a dealer or independent shop. 4. The only inspection mentioned is by the technician at the service center. There is no indication that the manufacturer, police, insurance representatives, or others have inspected the vehicle or components. 5. Yes. Prior to the failure, when starting the car in the early morning, a "drive train service" message appeared on the display, advising you to visit a service center as soon as possible. This warning was the first symptom of the problem before the starter ultimately failed and the battery drained. This aligns with the technician's diagnosis of a drivetrain fault code related to the starter motor.Read more

Issue Description: My 2018 Honda Odyssey (VIN: [XXX] ) has a total failure of the Multipurpose Camera, permanently disabling the Collision Mitigation Braking System (CMBS), Forward Collision Warning (FCW), and Lane Keeping Assist (LKAS). In addition to these, the regular Cruise Control is also disabled. These are primary safety systems, not elective conveniences. Persistent Failure: Errors occur
... immediately upon every startup. Mid-Drive Danger: Systems have deactivated multiple times while in motion, causing sudden, unpredictable loss of driver-assist features at highway speeds. Total Loss of Control: Both Adaptive and Standard Cruise Control are completely disabled, forcing manual-only operation. Infrastructure Failure: The failure prevents the vehicle from recognizing lane markings (RDM/LKAS), leaving the car unable to assist in steering corrections. Safety Risk: The vehicle can no longer perform emergency braking or provide collision alerts, significantly increasing the risk of high-speed crashes. This is a premature failure of a non-wear safety electronic at 91k miles. It is a severe safety defect that leaves the vehicle unequipped with the crash-prevention technology it was designed to have. Manufacturer Response: American Honda (Case #16167889) and NCDS (Case #XXX) denied assistance, citing the expired 3yr/36k warranty. I contend this is an extension of the known electrical architecture defects in this model year (e.g., Recall 23V-431), and a component intended for the life of the vehicle should not fail such that it disables multiple safety systems simultaneously. While accelerating from a stop light, my 2023 F-150 Powerboost failed to properly accelerate and my gas pedal stopped working around 20-25 mph. I reported this to the Ford dealership immediately and drove straight to service bay. The failure repeated 3-4 more time before arriving. There was no engine light or error code so the dealership couldn't "fix anything". 16 hours later, the truck engine st
... ruggled to stay running, my gas pedal wouldn't engage again, I couldn't accelerate, and I had it towed to the dealership. I'm being told the Fuel Pump Driver Module failed and that it's not covered under the powertrain warranty - my concern is that this failed module creates a significant safety risk which leaves the vehicle inoperable while driving but Ford is treating it as a minor failure and not covering under warranty.Read more
