While driving northbound on US-280 freeway, the vehicle experienced sudden deceleration with maximum speed limited to approximately 40 mph. The dashboard displayed a "Check Power Supply" warning. The vehicle required a tow to the dealership. Dealer diagnosis indicates likely ICCU (Integrated Charging Control Unit) failure. This is a known issue affecting multiple Hyundai Ioniq 5 vehicles, with num
... erous consumer reports of the same failure mode. The sudden loss of propulsion at highway speed created a significant safety hazard though we were able to make it off the freeway with hazards on. However shortly after exiting the vehicle further slowed and eventually died on the side of the road as did the hazard signals.Read more

The Incident: While traveling at highway speeds on a public freeway, the engine suffered a catastrophic failure and stalled without warning. This caused an immediate loss of propulsion, power steering, and power braking. I was forced to navigate the vehicle across multiple lanes of high-speed traffic with no engine power, nearly resulting in a fatal collision. Background and Safety Risk: This is
... the second terminal engine failure this vehicle has suffered in under 60,000 miles. The first engine was replaced by a Ford dealer at approximately 30,000 miles due to a similar failure. The replacement engine has now failed at approximately 53,000 miles. This vehicle is also subject to Safety Recall 25S76 (Fuel Injector/Fire Hazard). Current Status: The defect has been confirmed by a Ford dealership which previously performed a full engine replacement. The manufacturer has not provided a permanent remedy for the current failure or the active fire recall. The vehicle remains a substantial and life-threatening safety risk to the driver and the public. No warning lights or symptoms appeared prior to the sudden engine shut-off at freeway speeds.Read more

Wednesday (02/18/2026), while driving to work, the vehicle stalled in the middle of the road at a red light. It would not move at all. I had to call roadside assistance, and the vehicle was ultimately towed. Being stranded in the middle of an intersection with traffic approaching at speed was not only humiliating — it was life-threatening. Vehicles were coming from behind, and I narrowly avoided
... what could have been a serious accident. Suddenly, my dashboard started showing all sorts or error and warning message. It showed ESC (Electronic Stability Control) error, Hill start assist error, Maneuver braking function currently not available, error start/stop, Rear traffic alert is currently not available, Transmission error etc. To make matters worse, the tow truck took approximately 1.5 hours to arrive, leaving me exposed at a busy junction the entire time. This incident caused significant mental distress and professional consequences, as I was unable to reach my office and had to formally explain my absence with supporting documentation. This level of unreliability from a new vehicle is simply unacceptable. It has not even been a year since I purchased this vehicle, and I have experienced repeated issues from the very beginning. This situation is extremely frustrating and completely unacceptable for a brand-new vehicle. Beyond these serious safety incidents, I have faced ongoing issues with the infotainment system since purchase. I was repeatedly told it was a software issue and that it had been fixed, yet the problem continues to persist.Read more

While using Tesla's Full Self-Driving (Supervised) software on the Pacific Coast Highway (PCH) and other highways, the vehicle’s automated steering system repeatedly malfunctions by failing to maintain lane centering. Specifically, the car drifts or actively steers across double yellow lines into the lane of oncoming traffic. This issue is a persistent failure of the Advanced Driver Assistance S
... ystem (ADAS) and Lane Keeping Assistance. The vehicle has attempted this maneuver multiple times at [Approximate Speed, e.g., 40-60 mph], requiring immediate and forceful manual steering intervention to avoid head-on collisions with oncoming vehicles. The system provides no warning chimes, error messages, or "Take Over Immediately" alerts prior to crossing the lines; it appears to perceive the oncoming lane as a valid path or fails to recognize the double yellow boundary entirely. This has occurred during [mention conditions: e.g., bright daylight with high glare / night time]. The vehicle is equipped with Hardware 3.0 (HW3). I believe the 1.2-megapixel camera suite or the processing power of the HW3 computer is insufficient to safely navigate the geometry of the PCH or any roads within mountains/hills. I have attempted to "Clear Calibration" and performed system resets, but the life-threatening behavior persists. The vehicle and its internal data logs are available for inspection. I have also sent "Bug Reports" via voice command to the manufacturer immediately following these near-miss events.Read more

Tire size: 255/40 R21 The original equipment Pirelli Scorpion Zero All Season VOL PNCS tires (255/40 R21 102V, DOT JXB RW582P) on my 2024 Volvo XC60 CPO developed significant tread chunking and active delamination, discovered at approximately 19,225 miles — less than 40% of the expected tire lifespan. Component: Both front tires are available for inspection upon request. Safety risk: Tread chunk

Prior symptoms: No warning lamps illuminated. Initial delamination was noticed earlier by a family member but was not fully examined at that time. Upon closer inspection at 19,225 miles, the extent of the tread chunking and delamination on both front tires was confirmed. The condition appears to have been progressive. Additional note: These tires are original equipment and have never been replaced. My 2022 Ioniq5 left me stranded late at night in a freezing rainstorm in March of 2024 when I was 75 miles from home at it failed to charge both D/C fast charging and A/C level 2 charging, despite my trying several different chargers. The car gave me the dreaded "Check E/V System" warning when it failed to charge. It turned out to be the notorious ICCU but also the AC inverter, which had to be r
... eplaced. It took 12 days. The dealership's initial diagnosis was something draining the 12-volt battery, so I asked if they also replaced that. They said no because it had tested fine, but 9 days after I picked up the car, it did not start because it turned out the 12-volt battery was dead, and my dealership spent 2 days getting Hyundai to approve replacement of the 12-vold under warranty. Last month, 17 January 2026, my Ioniq5 again left me stranded. This time 120 miles from home in a snowstorm. It D/C charged for 10 minutes before the charging stopped and it gave me the dreaded “Check E/V System” warning. I tried two nearby Tesla Superchargers with my Hyundai issued adapter, and they both failed. I had just enough charge to make it to a dealership 110 miles from my home, where the vehicle stayed for nearly a month while they diagnosed and repaired it by replacing the AC inverter (note that had been replaced the last time) and also the AC compressor. The ICCU failures are well-known and well-documented, but the AC inverters that failed on me twice do not seem to be as common and should probably be investigated. In both cases I was stranded on Saturdays when no dealership service departments were open until the following Monday, so I had to pay for a motel overnight, which Hyundai refused to reimburse because I was not at least 150 miles from home.Read more

I am reporting a manufacturing defect involving the dashboard on my 2022 Audi A6. The dashboard has begun bubbling and warping, particularly near the area close to the windshield. This defect appears to be common among Prestige‑trim Audi A6 vehicles, as other owners have reported similar dashboard warping. The warping causes the dashboard material to rise upward, which reflects additional glare
... and partially obstructs visibility through the lower portion of the windshield, especially in certain lighting conditions. This reduces my ability to clearly see hazards low on the road ahead, creating a potential safety risk. Additional Details: - The vehicle has not been modified. -The issue has progressively worsened over time. - The defect appears to affect normal forward visibility and could distract the driver. Audi has not issued a recall for this known problem, despite multiple similar complaints from other owners (Effected Models per online research: ETron, R8, A6, A7, A8, Q7, Q8). I am asking NHTSA to investigate this dashboard defect as a potential manufacturing defect and safety hazard due to obstructed visibility and distracted driving caused by glare/reflection from the warped dashboard surface.Read more
