02/04/2026 69k miles. I brought my vehicle in because I got a check engine light on the dash and had loss of power. Was informed that my fuel pump had failed and sent metal shards through the whole fuel system. The damage was covered by the recall, which also included reprogramming of the tcm and pcm. Was then told that the reflash of those systems was a failure, and that the Ford Motor Company wa
... s working on a remedy. My dealership has since completed a full fuel system replacement, and there still isn't a fix for the programming issue. This is a revenue generating vehicle, and the only one, for my small business. I lost many clients due to inability to perform the work I was contracted to do. My dealership did offer me a loaner vehicle, but it's not one that can perform the work that my truck was able to do, and was told not to use the vehicle for any commercial work or towing, as thats not allowed. Ive reached out to customer relations to get updates, and have had no help from them, save for telling me they're working on it, and will hope to have a remedy soon.Read more

Eyesight camera randomly turns off and on while driving. Has been consistent over several months pierced to be an ongoing issue that was previously recorded from 20 14–20 21 super Forrester. Very similar issue now with my 2023.

The vehicle exhibits multiple critical safety defects that pose a severe risk to my life and the lives of my children. Most notably, the steering system performs sudden, unintended maneuvers and the steering wheel moves with dangerous abruptness without driver input. Additionally, severe electrical failures frequently prevent the doors from opening or the vehicle from powering down, creating a hig
... h risk of entrapment. The seat belts frequently jam, making it impossible for passengers to release them in an emergency. Power windows operate spontaneously, and the car often refuses to lock or unlock. This vehicle has been out of service for a total of 63 days across 5 separate repair attempts. As of today, the vehicle has been at the dealership for 8 consecutive days with no updates or progress reports from the manufacturer. Despite these repeated failures and the car being unusable for over two months, Audi refuses to provide a buyback, claiming the vehicle is 'operating as designed.' This is a blatant disregard for public safety, as the vehicle remains unpredictable and prone to total system failure while on the road.Read more

RE: NHTSA Recall Number 25V668 Defective Unsafe 2025 Toyota Sienna 2nd row seat rails. I purchased a brand new 2025 TOYOTA Sienna Hybrid on May 20th, 2025 as a family vehicle to transport my grandchildren in their car safety seats. In July, Toyota discovered that the 2nd row seat rails were defective and posed a serious safety risk. Dealers were sent a DO NOT SELL notice on October 7th for the un
... safe Siennas. I was not notified until December 12th that no one should occupy the 2nd row seats while the vehicle was in motion due to a serious safety risk, so my grandchildren were strapped into unsafe seats without our knowledge all that time. Since then, the vehicle has been unusable for its intended purpose due to this significant safety risk. I have been waiting for months for the vehicle to be fixed without any update of any kind from Toyota on when this will happen. I have called the Toyota Brand Engagement Center several times and they have had zero information to provide beyond that listed in the Interim Notice. This is completely Unacceptable. Toyota has continued to produce new 2025 Siennas and 2026 Siennas with corrected seat rails since July 25th 2025 without supplying any replacement seat rails to remedy the recall and end the hardship it has caused for the families affected. This needs to be addressed and Toyota needs to be compelled to put their pursuit of profit on hold and fix these vehicles NOW.Read more

My vehicle has now experienced a serious electrical malfunction a few times, including one again today. Each time, the car suddenly loses power and drops to 40 mph, the steering wheel shakes, and all warning lights come on. This is extremely dangerous and puts my safety—and the safety of others—at risk. The Car is Brand new and has about 1200 miles. I took the car in previously, and the dealer

At this point I do not feel safe driving it or having anyone else in the vehicle. This is severely impacting my daily life and mobility. I need immediate action and a resolution to this unsafe situation. I only get run around when I call Audi Fletcher Jones and my service adviser does not resolve my issue. I am reporting a recurring safety issue involving sudden loss of power while driving in my 2023 Ford Escape. The vehicle has experienced multiple incidents where, while accelerating or pulling into traffic, it suddenly loses power, hesitates, or fails to respond to the accelerator. In some cases, the RPM drops unexpectedly, and the vehicle will not move or respond for several seconds. After this
... delay, the vehicle may jerk or surge forward suddenly. This creates a dangerous situation, especially when entering busy roads or intersections, as it puts me at risk of being struck by oncoming traffic. This issue has occurred multiple times and has been brought to the dealership on several occasions. The vehicle has undergone multiple repair attempts, including major engine-related repairs such as cylinder head and cooling system work, as well as a fuel system component replacement. Despite these efforts, the problem persists. On multiple visits, the dealership has stated they were unable to duplicate the issue, even though the condition continues to occur during normal driving. The vehicle has been out of service for an extended number of days due to these ongoing issues. This defect is intermittent but poses a serious safety hazard due to the unpredictable loss of power and delayed acceleration. I am concerned about the risk of an accident or injury if this issue is not properly identified and corrected. I am requesting that this issue be investigated, as it appears to be a recurring defect that may affect other vehicles of the same make and modelRead more

For several months I have been having problems off and on with my vehicle not shifting into drive, neutral, or reverse. It runs fine but becomes undrivable. Until this last time, I was always able to keep trying, let it sit overnight, etc... and it would eventually shift into gear. Currently it does not, I had to have it towed to a shop to be repaired. This is a problem, the vehicle has left m
... e stranded on several occasions. My daughter was recently home from collage and I could not let her use my car for fear she would get stranded somewhere. It was towed to a local shop, who was able to confirm that the transmission has an internal short. This is a very common occurrence with this vehicle. The transmission can not be repaired and it needs a new one. Each time the transmission would fail a dash light would come on with a P in a circle with an explanation mark next to it and an engine light. This happened several times, in the past I was able to clear the light and drive the car, but this time it would not clear. The transmission is shoot and needs to be replaced. I purchased the car new in February of 2017, and have always properly maintained the vehicle. It has approximately 79,800 miles on it. I have researched this issue and it is very common, this is a known problem with the 2017 Chrysler Pacifica. I have attached a Warranty Bulletin that seems like it should cover my vehicle, but I called the Chrysler Customer Hotline and they are telling me my vehicle is not covered. I have taken care of this vehicle, I drive it very gentle, there is no way this transmission should have gone bad. I am going to get this repaired at the shop, they have ordered me a new transmission. We have already filed a report regarding multiple unresolved warranty defects that Ford Motor Company has refused to address for over a year. Among these is a recurring fuel pump failure—an issue that has now occurred multiple times. The most recent incident happened just a couple of weeks ago in the Twin Cities after being told by our dealer that there was not an issue or fix for the fuel pump
... issue, where the truck stalled in active traffic, creating a situation that could have resulted in serious injury or worse, death! Despite repeated attempts to resolve this matter, Ford’s latest response is unacceptable and has directly led to this additional complaint. We have been told that no action will be taken unless we personally drive this unsafe vehicle to a dealership for inspection. When we made it clear that doing so would put us at risk, we were dismissively told to have it towed if we were unwilling to drive it. Let us be absolutely clear: we will not endanger our lives to accommodate Ford’s refusal to honor its warranty obligations. We are being asked to operate a vehicle with known, unresolved defects that have already proven dangerous. This is not only irresponsible—it is indefensible. If this matter is not addressed immediately and appropriately, we will have no choice but to escalate further.Read more

The vehicle was towed to Audi Beverly Hills for inspection. The dealership diagnosed the fault as: Control Module Main Processor Performance, Symptom Code 20079, per Technical Service Bulletin TSB 2074484/3. Per that TSB, the dealer was instructed by Audi of America to create a PSS (Pending Service Solution) record, erase the fault memory, and release the vehicle to me with no repair performed. The stated guidance from Audi of America to the dealer was to inform me that this is "an extremely infrequent fault" that is "being reviewed." No fix exists. No timeline for a fix has been provided. The vehicle was released to me with the underlying defect unresolved. This is a known, manufacturer-acknowledged defect with no current remedy. 2020 Volkswagen Passat SE 2.0L VIN: [XXX] Complaint Summary: I am reporting a persistent and unresolved fault involving the secondary air injection system (DTC P0491 – insufficient airflow) that has resulted in repeated, conflicting diagnoses across multiple repair facilities. This raises concerns about a potential systemic defect affecting this vehicle model. Detailed Description: The issue
... began prior to March 2025 while the vehicle was still under warranty. The vehicle displayed a Check Engine Light associated with diagnostic trouble code P0491 (secondary air injection system insufficient airflow). On April 3, 2025, a repair facility diagnosed the issue as a failed secondary air injection pump and replaced the component. However, the Check Engine Light returned within 24 hours with the exact same fault code, indicating that the repair did not resolve the issue. A second independent diagnostic evaluation was performed later in April 2025. This inspection confirmed that: The secondary air injection pump was functioning correctly Airflow through the system was present Electrical controls and PCM commands were operating properly The fault was attributed instead to a mechanically stuck secondary air injection combination valve Despite this revised diagnosis, the issue has remained unresolved. As of 2026, a dealership evaluation has now attributed the same fault condition to a potential failure within the cylinder head assembly, indicating a significantly more serious underlying condition. Safety Concern: This issue involves the emissions and engine air management system and has resulted in: Persistent Check Engine Light Repeated failure of emissions-related components Conflicting and escalating repair diagnoses across multiple qualified service providers I am requesting that the National Highway Traffic Safety Administration review this issue for potential investigation into a systemic defect. am writing to report a very concerning safety issue with my 2019 Audi Q5 and to request guidance on whether this may be covered under warranty or if my vehicle is experiencing a malfunction. I was n

It took quite a while for me to calm down and get my breathing back to normal after the incident. Then this morning, I received a dashboard warning that said: “Drive system: malfunction! Please contact service.” I am very concerned that this may be a malfunction of the automatic emergency braking or driver assistance system, especially after the unexpected hard braking incident. This feels like a serious safety issue. I am very concerned and very unhappy, as this situation felt dangerous and could have resulted in an accident.Read more

On March 22, 2026, my Check Engine light came on while driving my 2022 Audi A3. Prior to this, I had been noticing a slight jerk when accelerating from a stop. Because Audi dealerships charge approximately $250 just to read diagnostic codes, I went to Discount Auto Parts, where they retrieved code P27F500 — indicating the gear shift lock solenoid is stuck in the open position. I then contacted m
... y Audi dealership and asked directly whether this required a software update or if the gear selector was physically failing. They refused to give me a straight answer. I separately spoke with an Audi service technician who confirmed that the 2022 Audi A3 is well known to have this exact issue. My own research confirmed that this defect is widespread across both Audi and Volkswagen vehicles sharing the same platform. My vehicle is only 3 years old with no history of abuse or neglect. Repair estimates for this known manufacturer defect range from $1,500 to $2,000, which is unacceptable for a defect that appears to be a manufacturing issue, not owner caused damage. I am requesting that NHTSA investigate this as a potential safety defect and consider requiring Audi to issue a recall or provide repairs at no cost to affected owners.Read more

I am filing a safety complaint regarding a progressive engine defect. Beginning as early as 2019 (approximately 84,000 miles), I repeatedly reported excessive oil consumption and related engine concerns to authorized Audi dealerships. These complaints continued over several years. At no point was I informed that these symptoms could be associated with a known defect pattern or progressive engine
... failure. The condition has now escalated to a severe engine malfunction. The vehicle began exhibiting engine misfires, flashing check engine light, EPC warning, and unstable operation. The dealership confirmed misfire fault codes and oil contamination affecting the ignition system. Engine replacement was initially recommended. Excessive oil consumption leading to internal contamination and misfires presents a significant safety hazard. Loss of engine power or sudden failure while driving can create dangerous conditions, particularly at highway speeds or in traffic. Despite the long history of reported symptoms, Audi of America has declined to provide assistance, citing only warranty status and failing to address the defect progression. The vehicle has now been inoperable at an Audi dealership for approximately 60 days (nearly two months), with no resolution. This situation raises concerns regarding a safety-related defect, failure to disclose a known issue, and a potential broader pattern affecting similar vehicles equipped with this engine. I am requesting that this issue be reviewed for potential defect investigation. The dealer provided diagnostic findings with Failure Codes via a temporary link that is no longer accessible. I am requesting a copy of the full report and will upload this (once received), along with other documentation.Read more

I own a 2022 Audi e-tron Sportback that experienced multiple serious issues involving the high-voltage battery system. During service at Audi South Atlanta, the dealership documented diagnostic trouble code P29FF00 – Hybrid/EV Battery Thermal Runaway Detected. This code is related to battery overheating and potential thermal runaway, which is a critical safety condition in electric vehicles. D
... espite identifying this safety-related fault, the dealership did not perform any repair or replace any components. Instead, they cleared the fault codes and returned the vehicle to me stating it is operating as designed. Prior to and during this issue, the vehicle experienced: •Severe range reduction (approximately 60 miles at 60% charge) •Charging interruptions and warning messages •Regenerative braking malfunction •Abnormal energy consumption •Mechanical drag and braking irregularities These issues are consistent with a high-voltage battery or power system defect. The dealership’s decision to reset the fault code without addressing the underlying cause raises serious safety concerns. A thermal runaway-related fault should not be cleared and ignored, as it may indicate a risk of battery failure or fire. I am concerned that this vehicle may be unsafe to operate and that the defect was not properly diagnosed or repaired.Read more

Our electrical system is going craze. The electronic door handles will lock us in and out of the car, the instrument cluster will black out for periods, safety sensors go off at random, other screens in car will black out leaving controls in operable, and wireless car charger will heat the phone so hot that we have been burnt. We have take to the dealer several times. Some visits they claim they c
... an not duplicate. They have replaced two of the door handles but they still at random do the same thing. There have been numerous system updates done to the car but it does not seem to help. We are told that the solution for these problems have not been developed yet by the manufacturer. There are no warning signs as to when these items will fail, it just happens at random. We do not feel safe driving the car as all the safety components are tied to the electrical system such as steering, braking, lighting, etc.Read more

I owned the car for two weeks when I had my dog in the front and it automatic braked going down the freeway. It threw him into the screen and broke it, luckily he was ok. (He was belted in.) This has happened three more times and been inspected but they are not able to reproduce the problem. This only happens when the dog is in the car with me. This last time, the dog hit the dash and screen ag
... ain and broke it, and I had to pay to have it replaced again. (Thankfully dog just fine.) They keep saying no fault found because they can’t get it to do it on their test drives. This is a serious safety issue and could cause an accident by someone rear ending me. The roads aren’t wet, and nothing is around when it has happened each time. They turned down the presensor this time, so i am hoping this helps.Read more

The Audi e-tron GT contains a design defect where the 12v battery cannot be charged in the event it becomes depleted. The battery is stored in the front trunk, but this can only be opened when the car has electrical power - there is no manual release. This leaves the vehicle immobilized to the point it must be towed to a dealer using special equipment because the four wheel drive train is locked i
... n position when de-powered. This is a safety issue because it will leave a driver unexpectedly stranded in a remote location or otherwise unable to use their vehicle in an emergency situation. There is no way to mitigate this risk (e.g. via routine replacement of 12v battery) because any electrical fault separate from the battery itself is sufficient. In my case, a malfunctioning light switch caused a parasitic drain on the 12v leaving the car totally immobilized. I was unable to start the car and it took two days for Audi to send a tow truck with the correct equipment to tow the vehicle. It then took them several weeks to diagnose and resolve the problem. Meanwhile, there are thousands of similar components on the car that could potentially cause the same issue to reoccur. The Porsche Taycan, which shares the same platform as the e-tron GT, allows the front trunk to be opened by connecting jumper cables to a fuse box in the driver's side foot well after accessing the passenger cabin with a manual key. The 12v battery in the front trunk can then be charged so the car can be driven in an emergency or to a service center. A recall should be issued requiring Audi to provide the driver some way of accessing the 12v system in an emergency. Failed Component: 12v battery / light switch. Unsure if dealer kept failed part. Safety Risk: Driver left stranded - no way to mitigate via proper maintenance. Confirmation of Issue: Dealer confirmed issue. Inspection of parts: Unsure beyond servicing dealer. Warning: No symptoms or warning of problem. Car simply died overnight.Read more

The ADAS is not working properly. Adaptive cruise control, emergency assist, lane departure, active front assist, distance warning, traffic sign recognition, adaptive light, etc. are all not working. The check engine light is illuminated, the key fob would not open car (not a battery issue) and the seat belt tensioner is not working properly. Those ADAS features are there for a reason and that is
... to enhance the safety of me, my passengers and other drivers. The key fob allows quick access to my vehicle, which enhances my safety when alone or with others. The seat belt tensioner provides restraint during an accident so neither I nor my passenger will go head first into the windshield, air bags and/or steering wheel/dashboard. The problems have been confirmed with the dealership. Yes, it has been inspected by the dealership. Yes, they first appeared January 2025. My car is currently at the dealership supposedly waiting for a backordered seat belt tensioner. It has been at the dealership for 22 days and I haven't any idea of when I will be getting the vehicle back. I purchased it in October 2025. It has been driven less than 5,000 miles.Read more

... in to Dealer for first oil change at 9,006 miles on odometer. Dealer inspected tires and reported both front tires were Red/32” and 2 rear tires were yellow at 5/32”. At 9,000 miles, this is obviously grossly premature tread/tire wear. For front tires to be unsafe to drive at fewer than 9,000 miles is unexpected and others need to be made aware of the risk of premature tread/tire wear to avoid dangerous blowouts that could result in accidents and injury. Feel lucky to have brought vehicle in 1,000 miles early for oil change and for Dealer to have detected. Dealer replaced all 4 tires at no cost and tires are being shipped to Bridgestone. I have Dealer service technician video showing tire/tread wear but was unable to download here — perhaps due to length. Happy to forward if you want it.Read more

I had 93u9 criteria 04 recall done in February. As of March 6, I had a no power supply message. Without being able to charge it went to the dealer. They said it was the charge port assembly but not from the recall since that was software. Once that was fixed they charged the car and it immediately had the fault electrical malfunction. Now they say i need to replace the rear drive motor. Then thy c
... alled again and said the 12v battery needs to be replaced as well. This all happened while still at the dealer. I didn’t have those faults prior, yet they want to say none of its related. This appears all related. After I got the car back from the recall, I had two times the charging didn’t want to start, then I had ghost wiping with the rear wiper and the infotainment system rebooted several times while driving. Safety had gotten much worse and now there are cascading failures. Audi does not want to admit the recall was the cause, but the car had no issues prior and now a month later had $17000 in repairs. Audi has had the error code reports requested by me, but they have not provided them including the corporate office. I feel this recall caused a catastrophic failure of my battery and electrical system.Read more
