The headlight module failed in the middle of driving at night and caused me to pull over and get the vehicle towed. Took to the dealership and apparently it’s been an issue with these trucks and the headlight module fails and then burns out both headlights. There is no recall and is not covered by my factory warranty.

Yet, Tesla imposes no restrictions or clear warnings on its use in such areas, even where the vehicle’s software could easily identify elevated risks. Autopilot handles highways reasonably well, following lanes, making turns, and maintaining distance from other vehicles. This creates a misleading impression that it will perform just as effectively on city roads. However, city roads are far more complex, and the brief transition from Autopilot to human control is often insufficient to prevent accidents. Our experience painfully illustrates this. What concerns us most is that Tesla includes Autopilot as a standard feature but fails to adequately educate users about its limitations. There are no effective restrictions to prevent the system’s use in high-risk scenarios, nor are there built-in warnings when the system is engaged in unsuitable driving environments. This creates extremely dangerous situations for customers. Every life is valuable and deserves proper protection. In addition to the physical damage, the emotional toll after the incident has been significant. What feels especially unjust is Tesla’s response. The company has shown indifference, refusing to take responsibility or provide any compensation for an incident caused by clear system design flaws. We are submitting this report in the hope of preventing similar accidents and receiving the compensation we rightfully deserve. As background, our family purchased this vehicle used from a MI dealer in July 2023. It was a trade-in by the previous owner, and the dealer did not disclose any known issues. ... 000 miles, her safety was put at risk when she lost acceleration as gray smoke poured out of her tailpipe (had she been on a highway, both she and other drivers would have been at grave risk). She had the vehicle towed to a local service station and was told the engine needed to be replaced. After searching Google for problems with this vehicle's engine, a 1.5L Ecoboost, we discovered that not only is this a known issue (coolant intrusion) for which Ford did not issue a recall (instead, "Program #21N12 one-time repair for vehicles included in 19B37"), but there also is a class action lawsuit being litigated by Capstone Law APC. Given this, we towed the vehicle to the local Ford dealer, who also advised we need to replace the engine, we will receive no financial relief from Ford, and it will cost $12,000 for the replacement (we paid $16,000 for this vehicle less than two years ago). We have spoken to various individuals at the national Ford customer satisfaction organization, and they will not help us. So we now have an inoperable vehicle that my daughter can't afford to repair, and since we are not getting any financial relief from Ford, she is forced to buy a much older, high-mileage vehicle that may also put her safety at risk. The only indication of a problem was when the check engine light came on, but since the vehicle was driving fine, our daughter did not realize she should take it for service right way (this is her first vehicle, and she lives close to campus so drives only infrequently). The second place did not check anything and said they would when I come back for my 15,000 oil change. The brakes were inspected by the first service center but they did not drive it. I bought the Highlander Hybrid brand new and it started immediately. I traded in the exact same vehicle that was a 2019 Highlander Hybrid Platinum and it did not have this problem. The suv had a PCS that does show on dashboard but that is because the brake isn’t engaging and you are so close to hitting someone. Normally you apply pressure to the brakes to start slowing down and it does. But in some cases the brake has been applied and nothing happens. It finally does engage after the brake is almost completely down. I don’t know if that is the safety system braking or the brake actually working. The engine shut down in active traffic. The smell, ticking sounds, and dashboard messages all returned. The vehicle once again would not shift into neutral or respond to restarts. This failure happened with traffic traveling 30-45MPH. Despite police presence and hazard lights, multiple near-accidents occurred because the vehicle looked fully intact and other drivers didn’t understand it was inoperable. Mercedes-Benz claimed this was a starter motor issue, which is inaccurate. A starter motor does not cause a moving vehicle to shut off, nor does it lock out neutral. These symptoms point to a critical failure of the powertrain or electrical system. Most alarming is the inability to manually override the system and move the vehicle to safety. Bystanders and even police were unable to shift or push it. This is a serious design flaw that turns a technical malfunction into a dangerous roadside emergency. This vehicle was in pristine condition with no damage or misuse. I kept up with all maintenance and care. Technology should never prevent human intervention during failure. They want to charge 11K to replace with new one. Ford rebuilt this transmission at approximately 29k due to issues and 80k miles later having issues again. This transmission always shifted hard and would drop down in gear while traveling creating an unsafe situation. Ford knows about the issues with these transmissions in the 2018 year F150's and hasn't implemented any recalls, only TSB's that don't fix the issue. I am still getting quotes from other repair shops to replace the transmission.Read more

The failure has been reproduced and confirmed by an independent mechanic, even with the aftermarket springs removed. Despite the clear evidence of a factory component failure, SOA refused to cover the repair under warranty, stating that the presence of aftermarket parts voids the claim. This is contrary to the Magnuson-Moss Warranty Act, which requires proof that a modification caused a failure. No warning lamps or messages appeared prior to the issue. For 1.5+ years, the transmission will unexpectedly and suddenly downshift into 1st gear without warning. Which causes a sudden deceleration. No warning lights before it happens. It occurs randomly and at different speeds. It has happened over 20 times. I've experienced it happening at speed ranging from 35mph to 65mph. The gear shifter was in the drive position each time this issue occurred.
... The suv is the 2017 ford expedition EL with the 3.5L ecoboost engine with 6 speed automatic transmission. No check engine light before or after the issue (so no OBD2 code to read) a big yellow warning appears during/after saying "see manual". Sometimes the issue will resolve in a few seconds. I believe that there is a safety issue with the design of the adaptive cruise control on the 2024 Honda Pilot. The vehicle applies the brakes at about 1 mph over the selected speed. ... ehicle several mph below the set speed causing cars behind me to swerve or come close to me. No human driver would apply the brakes in that situation. It also appears to irritate those same drivers. I’m glad that none of them has a weapon so far. It also applies the brakes going downhill on the interstate highway. This occurs at any speed below 55 mph. This also makes it look like the driver is continually tapping the brakes, when no one normally would. I suspect that there may be extra brake wear. My family asks what is going on with the car because of the aggressive braking. I previously owned a 2018 Honda Pilot with adaptive cruise control. It used transmission downshifting to slow the vehicle. I admit that there were some over speeds, but nothing serious. The 2024 Owner’s Manual states that downshifting is also used, but I’ve only seen the brakes used for over speed control. There are no user selectable adjustments to the system. I am getting used to this (feature, not a bug) by clicking off the cruise control whenever the conditions are close for a brake activation. This causes me to pay more attention to monitoring for those conditions, rather than the actual driving and traffic scan. I suggest that the system use downshifting followed by brakes after a 3 or 4 mph excursion from the set speed.Read more

Experienced complete engine failure. Car was at a Certified Hyundai dealership 2 weeks prior for new plugs and new coil pack. Multiple point inspection with no noted issues. Driving on a 3 hour trip when suddenly the engine started knocking without an engine light or oil light coming on. Pulled over and no oil on the dip stick. The same dealership did an oil change at 86,000 miles now the car

I am submitting this complaint regarding a serious engine defect in my 2014 Hyundai Santa Fe, which suffered rod bearing failure and now requires a full engine replacement at a cost of approximately $8,600. This failure occurred on April 13, 2025, despite regular oil changes and proper maintenance throughout my 1 year and 7 months of ownership. According to my certified mechanic, the vehicle was
... evaluated using Hyundai’s own rod bearing clearance test protocol, which it failed. The results were submitted to Hyundai by the repair shop, but the manufacturer denied the claim—despite their own diagnostic test confirming the issue. I later discovered that my vehicle may fall under NHTSA Campaign Number 17V226000 (Recall 162), which addresses machining errors in the Theta II GDI engine that can cause premature bearing wear. I am requesting that NHTSA investigate whether my vehicle should be formally included in this recall or covered under Hyundai’s extended warranty program for rod bearing failures. This issue represents a serious safety risk. The engine began knocking suddenly, without any warning lights or codes, and still had another month or 1,419 miles left till next service and oil level was normal. Vehicle had to be towed to a service facility. A failure while driving could have resulted in a complete engine seizure, placing my family and others at risk. I respectfully request that NHTSA review this issue and investigate Hyundai’s handling of such claims, especially where their own testing confirms a known defect.Read more
