A vehicle was taken to a collision repair facility following minor front-end damage. At intake, the repair facility documented visible exterior damage but did not perform or document an underbody inspection, which is necessary to assess high-voltage battery integrity in electric vehicles. After the vehicle entered teardown, the repair facility later claimed that the high-voltage battery was damag
... ed and recommended that the vehicle be declared a total loss. This determination was made without documented evidence of required OEM diagnostic procedures and without initial intake documentation supporting battery damage. The sequence and timing of documentation raises concern that the battery damage determination was made after teardown rather than through proper inspection protocols. A subsequent manufacturer-level evaluation did not support the claim of battery damage. This situation presents a safety concern regarding whether proper high-voltage battery inspection and handling procedures are being followed by collision repair facilities. Misidentifying or improperly handling high-voltage battery conditions in electric vehicles can pose safety risks, including potential thermal events, improper classification of vehicle safety, or unsafe return to service. This complaint is submitted to request review of whether proper safety and OEM procedures were followed in evaluating the high-voltage battery, and whether this reflects a broader safety issue in electric vehicle collision repair practices.Read more

Driving shaft malfunction.
Incident date: Jan 10, 2026
Component/System: Windshield washer fluid monitoring system (level sensor and warning). Observed Behavior: The system provides no advance or “low level” warning. The only alert occurs when the reservoir is completely empty, with no intermediate notification of low fluid. Failure Mode / Functional Impact: Washer function is lost suddenly without prior warning. The driver is not given an oppor
... tunity to refill fluid before the system becomes non-functional. Safety Risk: Loss of washer fluid prevents clearing road spray, salt, or debris from the windshield, causing immediate degradation of forward visibility. This is most critical during highway driving, winter conditions, and repeated washer use. The absence of advance warning creates a foreseeable risk of reduced visibility while operating the vehicle. ADAS Impact: The vehicle uses a forward-facing camera behind the windshield for driver assistance features. When the windshield becomes contaminated and cannot be cleaned due to depleted washer fluid, camera visibility may be impaired. This may reduce performance or temporarily disable features such as lane keeping, lane departure warning, and traffic sign recognition, resulting in simultaneous degradation of driver visibility and certain assistance systems. Reproduction / Confirmation: An authorized Audi dealer confirmed this behavior as normal and consistent with design. Audi customer care supported this position. Inspection History: The vehicle has been inspected by an authorized Audi dealer. No inspection by police or insurance representatives. Warnings Prior to Failure: No warnings, messages, or symptoms appear prior to complete depletion. The only alert occurs when the system is empty. Requested Evaluation: Evaluation is requested as to whether this design adequately supports driver visibility and safe operation, and whether any modification exists or is planned to provide earlier warning.Read more

Purchased a used 2022 Audi E-Tron GT with an open NHTSA safety recall for high-voltage battery fire risk (Recall 24V726 / Audi 931A). The recall remedy had not been performed at time of purchase. The selling dealer completed a pre-delivery inspection two weeks prior to sale with full knowledge of the open recall. Within 7 weeks of purchase, the vehicle entered limited performance mode (turtle mode
... ) while driving and had to be towed. A high-voltage coolant pump was diagnosed faulty and replaced. Days later, the vehicle powered down completely while driving. Fault codes indicated an implausible signal from the HV battery controller. The battery management controller (J840) was replaced. After replacement, additional faults were found. The HV battery switch box (SX6) was also diagnosed faulty and replaced. The vehicle was out of service approximately 18 days for this event alone. Within 11 months of purchase, the vehicle developed water intrusion in the trunk area traced to a faulty spoiler motor sealing surface. This caused corrosion to the center tail lamp connector, electrical shorts to all three tail lamps, and required repair of six wiring harness connections. A body electrical wiring repair was also documented in service history prior to purchase, suggesting a pre-existing sealing defect. Total: 9 dealer visits and 50+ days out of service in under 14 months. All repairs performed under manufacturer warranty. The open safety recall was not completed until 8 months after purchase.Read more

I am reporting a safety-related defect involving my 2025 Audi vehicle. Shortly after leasing the vehicle in 09/2025, I experienced electrical and system malfunctions while driving, including warning lights, intermittent adaptive cruise control malfunctions, erratic acceleration, sputtering, and sudden braking without warning. These issues occurred during normal driving and raised safety concerns.
... On 11/03/2025, the vehicle failed to start and required multiple jump starts, displaying multiple system errors including a check engine light and malfunctioning displays. During a brief attempt to drive, the steering wheel began vibrating, additional warnings appeared, and the vehicle behaved as if on rough terrain despite normal pavement. The vehicle was towed to the dealership on 11/04/2025. While there, Audi’s Technical Assistance Center (TAC) and engineers were involved, and I was informed that internal technical guidance was issued related to this issue, indicating it may not be isolated. After more than two weeks, I was told the vehicle was repaired. On 11/19/2025, immediately after pickup, it malfunctioned again at highway speed. During lane changes, the vehicle resisted steering input and attempted to keep me in the lane despite signaling, creating a loss-of-control situation that required abrupt correction. A rear crash assist malfunction warning also appeared. I returned the vehicle the same day. Although it may start and run, I do not believe it is safe to operate due to the prior loss-of-control event and continued system instability. I have been asked to drive it again to reproduce the issue, which I do not feel is safe. This may involve a defect in driver assistance and electronic control systems.Read more

The display on the dashboard that includes the speedometer and other information goes completely blank. So does the heads-up display on the windshield. This could occur at the same time or individually. It may last only a few seconds or for several minutes. This has happened since I bought the car about 1.5 years ago. I've had it to the dealership and they have noted the problem but have not

Since the day of delivery, this 2025 Audi S5 has experienced daily electronic and safety system failures. At least once per day, on startup, the vehicle throws 10 or more warning codes disabling safety-critical systems including blind spot monitoring, lane departure warning, forward collision avoidance, rear traffic alert, and adaptive lighting. These failures occur without warning and persist for
... varying durations before clearing on their own. Additionally, on 5 occasions the adaptive cruise control refused to activate, displaying a false "door open" warning with all doors and trunk confirmed closed. On 4 occasions the power trunk failed to latch using the electronic controls. The 360-degree parking camera system intermittently stops rendering surroundings, displaying only the vehicle graphic with no camera feed. The vehicle was brought to the selling dealer for service. After one hour, the dealer stated no issue was found. A battery diagnostic was requested based on a documented case of an identical 2025 S5 with the same symptoms traced to a faulty 12V battery. The dealer declined to cover the diagnostic under warranty. The service manager later acknowledged these are known platform issues and confirmed a software update is expected from the manufacturer in July 2026. The vehicle already has the latest available software update (TSB 2079690, KD2, version 03.10.00/C) installed and the problems persist. The dealer has confirmed there is no current fix available. These failures put the driver and other road users at risk, as safety systems are non-functional during the error state with no indication of when they will restore.Read more

While driving in the far left fast lane at approximately 70mph the vehicle suddenly began to show multiple malfunctions to include the following but not the complete list. Malfunctions noted High Voltage Malfunction, Chassis Malfunction, Suspension Malfunction, Driver Assist Malfunction. The vehicle then proceeded to slow down and I lost acceleration. Another alert showed regarding auto-braking.
... So with my wife and child in the car I had to cross four lanes of traffic to get to the shoulder. At the time I was not even sure if the signal lights were working as the vehicle began to shut down. This posed significant safety to myself, my wife and child and to all the other vehicles on the freeway as traffic was heavy which included several semi trucks. To have the vehicle slow on it own with no ability to accelerate to match speeds of vehicles coming behind and along side and no indication that any of my vehicle signal lights were still operable I repeat posed a significant safety risk to everyone in proximity. There was no warning or early indications that a failure was going to occur as it was immediate. This vehicle has had many alerts in the past that have cleared within minutes such as assistive driving and central locking malfunctions. When on the shoulder at the full stop the steering wheel was vibrating and continued to vibrate through to the next morning. When on the shoulder the Emergency Call button failed. The vehicle was towed 70 miles to a Audi dealership. The following morning the service representative observed the car in the same state where they were not able to start the vehicle on its own. The had to jump start to get the vehicle to the garage in order to do inspection. I was contacted on Monday and was told that the diagnostic showed no errors or malfunctions which in and of itself is alarming that no record of this was captured by the vehicle. Engineers in Germany were contacted and Service was advised to perform an update to the vehicle.Read more
