The second-row seatbelt in my 2017 Tesla Model X does not retract, preventing proper restraint of a passenger. Tesla acknowledged the issue but refused to repair it, stating the vehicle is out of warranty. This is a failure of a federally mandated safety system and a clear safety hazard under FMVSS 209. I’m transporting children in this vehicle and cannot rely on this seatbelt to function.

I am reporting a serious safety concern involving my 2021 Chevrolet Blazer, which experienced brake failure while driving, without any warning lights or diagnostic alerts from the vehicle’s onboard system. ... noises when braking. The issue was dismissed and attributed to a change in federal regulations limiting the use of copper in brake pads—without any further inspection. This resulted in a missed opportunity to diagnose what appears to be a deeper mechanical failure. After the brake failure, service technicians discovered that the vacuum pump had failed, which impacted braking performance and also caused engine damage. While I’ve since learned that GM has acknowledged vacuum pump failures in several vehicles manufactured between 2014–2018 (via formal recalls), the 2021 Chevy Blazer is not currently included in those campaigns However, the symptoms and outcome I experienced are strikingly similar to the documented defects in older GM models. The lack of early diagnostic detection, coupled with dealership dismissal of clear warning signs, signals a potential emerging safety defect in this newer model that may not yet be officially recognized. Given the safety-critical nature of the braking system and the failure of both mechanical and procedural safeguards, I strongly urge NHTSA to investigate whether this issue is more widespread in recent GM models, including the 2021 Blazer. It was very dangerous as their were flames and burning hot coolant and shards of metal shooting everywhere as my son was on the highway being passed by big trucks. Let me start by stating that this incident is just one of many occurrences that has happened to me with this vehicle. I was driving down a straight highway that was pretty heavily congested with traffic of other vehicles, when all of a sudden I felt a loss of power with the vehicle and some error codes came up on the dashboard indicating the engine needed to be serviced, start & stop was unavailab
... le, and to service electronic throttle control. I also noticed that the vehicle driving mode had switched by itself from Dynamic to Natural mode. After a few short seconds, I felt the RPMs drop completely to zero and I felt a loss of the power steering as well because the engine had shut down completely in the middle of highway traffic. As I was driving on the left lane of a two lane highway, I had to immediately pull over to the right side shoulder, nearly being hit by vehicles behind me as the Alfa Romeo Giulia stalled and immediately cut my speed. I was parked on the right shoulder of the highway with my hazard lights on and put the vehicle into Park. When I tried to restart the engine, it just cranked over for a long time before finally turning on. The same error codes popped up on the dashboard indicating the problem was still there. I tried to continue driving down the highway but the engine kept on shutting itself off and the issue persisted. Eventually I had to call a local friend who has a trailer to help me tow my vehicle back home. The vehicle, when scanned, released the code P008A which indicates a low pressure fuel system error. ... component that failed was the chassis battery disconnect switch, which was installed on the chassis by the RV builder's (final stage manufacturer) chassis preparation division. The failed component is a BEP Marine battery switch (part number unavailable at the time of this report). The resulting condition is that there is NO power to the dashboard, display console, headlights, taillights, running lights, nor any other chassis-related system, resulting in a completely unpowered, unlighted, unmovable coach. In this case, the switch failed under light load while the vehicle was parked in my driveway. However, if the same failure had occurred while the coach was being operated on the open road, such as a freeway, in the dark, the result would be an unlighted, barely-visible, 21,000-pound stationary object in the lane of traffic, possibly resulting in a serious collision with another oncoming vehicle.Read more

This is a major safety issue --- we got an error that steering assistance was "reduced" for about 12 hours. Then on [XXX] at around [XXX], we were pulling out of a parked area (after being parked for 2 hours) -- and proceeded to increase speed on [XXX] in Newark, the car steering wheel abruptly LOCKED ITSELF - you couldn't even move the steering wheel. It just locked and shut down completely.
... We proceeded to do a rest (hitting two scroll buttons) to reset the system over 5-6 times. Each time it takes about 10 mins to complete. It was pure HORROR - sitting on the road with cars just passing by and honking at me behind. I had the hazard lights on, but it was a dangerous situation. I was completely STUCK and immobile. After some time, the steering wheel "magically" started working again. We've dropped the Tesla off at Springfield and they are charging us over $6,000 in damages for this. Subaru of America sent an engineer to inspect the vehicle, but the evaluation was limited to measuring vibration levels. He did not place the vehicle on a lift or inspect any physical components, such as the suspension, wheels, or drivetrain. Despite this limited assessment, he concluded the vibration was “normal.” I respectfully disagree with this conclusion, as the vibration significantly affects the drivability, comfort, and confidence I feel while operating the vehicle. Consistent faulty battery warning back in the winter. Recently the brake pedal warning comes up very often, brake also feels like I need to apply more pressure when stopping. The lane assist also always gives me a warning light saying to drive with care.

When driving down a busy road, the car completely stalled with loss of power steering, unable to accelerate or brake, power completely lost in car with all safety lights illuminated on the dashboard. ... several attempts, restarted car with warning lights still present- moved to side of Road. Currently at dealer for inspection- stated that problem was "digital crash input code failure" and that there is not a fix for this at this time. How was your safety or the safety of others put at risk? Car stalled with complete loss of control in the middle of traffic, while accelerating at approximately 15-25 mph. This could have caused an accident, but luckily did not. However, car was stalled in traffic for about 30 mins and unable to move car at all- blocking traffic on busy street. If happened on highway, could have been much worse and also had the potential to cause an accident. Has the problem been reproduced or confirmed by a dealer or independent service center? The problem was confirmed by dealer, and said that there is "no fix". According to them, all recalls have been fixed in this vehicle prior to incident and should have fixed this problem by replacing the battery. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Vehicle has been inspected by both dealership service and Chrysler/ Stellantis engineers. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear?
