I am submitting this to supplement NHTSA Complaint No. 11704996 regarding a 2024 INFINITI QX60. Current mileage approx. 31,500. No injuries. Crash/Incident (property damage only): Within the first 2 months of ownership, while backing into a parking space at a grocery store parking lot, the Parking Collision Warning did not alert and Automatic Emergency Braking did not activate. I struck a shopping
... cart I did not see, causing minor damage/dent to the liftgate (no injuries). Additional ADAS safety concern (near-miss / repeated events): During the first year of ownership, on at least two occasions while using Intelligent Cruise Control / driver assist (steering assist) on a highway, when the vehicle ahead braked, my vehicle did not slow down or brake as expected and continued approaching at the prior speed. I had to brake immediately and steer to avoid a collision. No crash or injuries occurred. Similar instances have occurred additional times; however, I was monitoring the system and manually braked in time when it did not slow/stop from a safe following distance. I am concerned about intermittent non-response of these driver-assist braking functions. Ongoing safety-related issues since about the 6th month of the first year: 1) Windshield washer system leak/empty reservoir (reduced visibility). Concern began after prior dealer visit on 06/03–06/04/2025 for panoramic sunroof noise. 2) Rear driver-side window intermittently lowers by itself (electrical). 3) Side mirror intermittently fails to unfold/open (visibility). 4) Water intrusion into cabin from top of front passenger door area during rain/car washes (photos available), raising concern for electrical/safe operation. 5) Panoramic sunroof creaking noise (distraction). Request NHTSA document and evaluate for potential defect trend.Read more

It seems that this vehicle was not included in a brake booster recall which included this model from 2005 through 2015. The brakes are not working properly, and brake fluid has entered the master brake cylinder damaging the seal. It is extremely unsafe to operate this vehicle in this condition. Somehow, this vehicle was not included in the recall, but definitely should have been.

Approximately 10 minutes into an ordinary drive home from work, the engine abruptly began making extremely loud noise, as if metal or bricks were being thrown inside the engine. Within moments, the engine rapidly lost power and then completely shut down. At no time did any warning lights, engine lights, or malfunction indicators appear on the dashboard prior to or during the failure. This incid
... ent represents a serious safety hazard. This vehicle is my primary tow vehicle for transporting a horse trailer, and had this failure occurred while towing, I would have had no control, no engine power, and no ability to maintain speed, creating a high risk of a major collision or highway accident. The fact that the failure occurred with zero warning makes the danger even more significant. Toyota has confirmed that this engine failure falls under the active recall. Despite this, the mode of failure—sudden shutdown while driving without any alert—poses a severe and immediate safety risk to drivers, passengers, and the general public.Read more

Supplemental Update – Manufacturer Response to Safety Complaint (BMW) I am submitting this supplemental update in connection with my existing NHTSA safety complaint regarding my BMW vehicle. Following my report that the vehicle continued operating without the key present while transporting minor children, BMW of North America acknowledged the concern and indicated it was under review. However,
... BMW has now stated that it is closing my case file without: • Conducting any inspection; • Issuing written findings; • Providing a safety explanation or analysis; or • Confirming whether the reported behavior is a defect, system failure, or normal operation. BMW has conditioned further assistance on dealership inspection while simultaneously closing the manufacturer-level case, creating a gap in accountability and preventing meaningful review of the safety issue at the corporate level. I am not refusing inspection and remain willing to present the vehicle. My concern is that BMW’s attempt to close the case prior to evaluation undermines safety reporting and limits transparency regarding potential risks to drivers and passengers. Please add this update to my complaint record for consideration in any review or investigation.Read more

Subject: RECALL VIOLATION: Kia on the Boulevard (Philadelphia) - Refused Remedy, Charged $199, Returned Unsafe Vehicle Vehicle Information: [Year] Kia Optima Hybrid Dealership Name: Kia on the Boulevard Dealership Address: 11295 Roosevelt Blvd, Philadelphia, PA 19154 Complaint Description: 1. The Safety Incident: While operating the vehicle, I experienced a catastrophic engine failure. Thick sm
... oke poured from the engine compartment, followed by immediate stalling. This presented a severe fire risk and safety hazard. 2. Dealer Misconduct & Illegal Charges: I towed the vehicle to Kia on the Boulevard (11295 Roosevelt Blvd, Philadelphia) for the active engine recall remedy. The dealership: * Refused to perform the recall repair free of charge. * Forced me to pay a $199 "diagnostic fee" just to look at the recall-related defect, which should be covered by the manufacturer. * Threatened me with $45/day storage fees if I did not pay the $199 and remove the vehicle immediately. 3. Current Status: Under the threat of accumulating storage fees, I was coerced into paying the $199 and towing the vehicle back to my residence. The dealership knowingly released a vehicle with a catastrophic engine failure (smoke/fire risk) back to the owner without performing the federally mandated remedy. The unsafe vehicle is now sitting at my home unrepaired. 4. Conclusion: I request an immediate investigation into Kia on the Boulevard's refusal to honor the recall and their practice of charging customers for recall-related inspections.Read more

A cooling system failure was diagnosed, which caused critical safety systems to stop functioning: the forward collision warning sensor, radar, and driver-assistance features all became disabled. A technician also discovered that a component requiring nine bolts was fastened with only two, indicating improper repair or assembly and creating a serious safety hazard. I brought the vehicle to AutoStar CDJR in Hendersonville, NC, where additional issues were documented: rattling/thumping noise under the vehicle at highway speeds, wind noise entering through the front windshield, and the Check Engine light still on. The advisor noted in writing that the vehicle had been in the Florida dealership for nine months and now shows new faults. As of December 7, 2025, the vehicle is again in the repair shop. The dealer reported: • Battery “low but fine after charging” • Alternator “normal” • Starter “normal” • No warning codes On Dec 2, 2025 the vehicle had been released without repairs. Only a few minutes after leaving the dealer, the car stalled in live traffic during an Auto Stop/Start event. Symptoms: • Immediate loss of all electrical power • All warning lights flashing erratically • Hazard lights did NOT illuminate externally • Gear shifter locked again • Vehicle would not restart on battery • Subaru roadside assistance jump-started the vehicle With Auto Stop/Start disabled, it was barely drivable and returned to the dealer. This appears to be a serious electrical or charging-system failure, possibly involving: • intermittent alternator failure • voltage collapse during Auto Stop/Start • BCM or power distribution fault The vehicle can stall unpredictably in traffic without warning, creating a significant risk of collision. This is not normal battery discharge — the battery was fully charged at the dealer minutes before the second failure. I am filing this complaint because this issue is dangerous and the dealer cannot reproduce or diagnose it. A formal investigation or recall may be necessary.Read more

The AWD light came on while the vehicle was in normal operation on October 25, 2025. Two days later, a diagnostic service was done at Bay Ridge Honda (Bay Ridge Honda did not previously worked on my vehicle), and it was determined that the rear differential on my vehicle failed. A differential is quite durable and does not fail easily. The mechanic who performed the diagnostic, stated that it look
... ed like someone tried to repair it. In addition, an independent mechanic confirmed that the damage occurred as a result of improper service at a Honda dealership. Because of the failure, I also noticed the rear of the vehicle seemed to be dragging. The failure occurred after the vehicle had been serviced at authorized Honda dealerships, Plaza Honda and Middletown Honda. Those are the only two places where routine and repair services have been performed on my vehicle. This created a safety hazard because a failed differential can lead to loss of vehicle control, putting the safety and lives of others and myself at risk. I'm concerned that improper workmanship caused the failure. Since I don't know which dealership is responsible, I reached out to American Honda Motor Co, Inc. for assistance, but they have not provided a satisfactory resolution and insisted that I resolve the issue with both dealerships. I'm requesting that NHTSA investigate and document if improper servicing contributed to the differential failure.Read more

The Brake Hydraulic Control Unit (HCU) covered under Safety Recall C4B has been unavailable for several years, leaving my 2018 Ram 2500 with ongoing ABS, traction control, and electronic stability control malfunctions. This defect has already caused an actual accident. In January 2024, during snow conditions, the ABS and traction control failed while I was braking in my driveway. The truck slid u
... ncontrollably and struck a tree, causing over $10,000 in damage that had to be repaired through my insurance. This incident occurred specifically because the brake and stability systems did not function properly. The issue continues to create serious safety risks, especially in winter. The warning lights for ABS and traction control are on constantly, accompanied by continuous dinging that is extremely distracting and stressful to drive with. The truck loses stability on snow or ice, braking distance is increased, and the vehicle behaves unpredictably. I am now driving again in snow and ice this winter, and I am genuinely afraid this defect will cause another accident. Despite this being an acknowledged safety recall, every dealership I contact says the replacement HCU part is still “not available” for my vehicle. This has been confirmed repeatedly by multiple dealerships and by the manufacturer’s recall hotline. I have attempted to get this recall repaired for several years with no success. Because the recall remedy is unavailable, the vehicle remains unsafe to operate, yet I have no alternative transportation and must drive it daily. I also cannot sell or trade the truck, which has caused significant financial harm. I am requesting NHTSA assistance because the recall remedy has not been provided within a reasonable timeframe, the vehicle remains unsafe, and the defect has already contributed to a crash.Read more
