Component/System: Passenger Occupant Detection System (PODS) / Airbag system — Recall 74E3 / NHTSA 21V874. What Happened: The passenger airbag may switch off even when the seat is occupied due to a faulty PODS wiring connection. The warning message “Passenger Airbag Off” appears with a chime. This is an active safety recall that was issued before I purchased the vehicle, but the recall was
... not disclosed at the time of sale. Safety Risk: If the system shuts off the airbag incorrectly, the passenger may have no airbag protection in a crash, creating a serious risk of injury or death. Inspection Availability: Vehicle is available for inspection. I live on the Big Island of Hawaii, where there is no Audi-authorized repair facility. Reproduction/Confirmation: Symptoms match the known recall defect. Audi has confirmed the recall applies to my VIN, but no local dealer exists to inspect or repair it. Manufacturer/Dealer Inspection: Audi has refused to cover shipping to the nearest authorized dealership on Oahu, which is the only location where the recall can be performed. Therefore, the defect has not been inspected due to lack of accessible remedy. Warnings/Symptoms: Airbag warning light illuminates and a message appears indicating the passenger airbag is off, along with a chime. Additional Notes: I cannot safely use or sell the car because the required safety repair is inaccessible. Audi’s refusal to provide transportation makes it impossible to complete the mandated recall.Read more

Dear NHTSA, On two separate occasions—May 2025 and December 2025, approximately six months apart—I experienced fuel leaks from the upper fuel filter on my 2017 F-350 while driving. Both filters had previously been replaced with genuine Ford parts. I am deeply concerned that my safety was compromised during these incidents. In both cases, diesel fuel was leaking down the engine compartment wit
... hout any warning indicators or alerts on the dashboard. The only indication of a problem was the smell of fuel, which I initially assumed was external. It wasn’t until I stopped the vehicle that I discovered a steady stream of fuel leaking. The most recent occurrence happened while I was on vacation, approximately 650 miles from home. There were no prior symptoms or warnings, making it extremely difficult to detect while driving, as the smell dissipates in the wind. In my opinion, this represents a significant safety hazard. Diesel fuel streaming down a hot engine could easily result in a fire, posing a serious risk to both the driver and passengers.Read more

The contact owns a 2021 GMC Sierra 1500. The contact had received notification of NHTSA Campaign Number: 25V274000 (Engine and Engine Cooling). The vehicle was taken to the dealer where an inspection was performed on the engine. The contact was informed that the vehicle had passed the inspection, and the mechanic added a higher viscosity oil, installed an oil fill cap, replaced the oil filter, and
... updated the Owner's Manual. The contact stated that the remedy failed to address or repair the possible defect with the engine. The contact stated that the remedy resulted in a higher cost for oil changes, and fuel economy had decreased from 21 miles per gallon to 18 miles per gallon. The manufacturer was made aware of the issue and the contact was provided a case number. The contact stated that the vehicle had not been repaired. The failure mileage was unknown.Read more

The Diesel Exhaust Fluid system on my 2016 Range Rover Sport has a recurring defect that triggers the warning “Incorrect Diesel Exhaust Fluid Quality Detected” followed by a “No engine starts in XX miles” countdown. This defect has happened repeatedly over several years despite multiple repairs including injector replacement, injector housing replacement and eventually a complete DEF tank
... assembly replacement. The failure always returns. This defect disables the vehicle and creates a safety risk because the car will eventually refuse to start, which can leave the driver stranded without warning. The problem appeared again this month, and the vehicle displayed the countdown message even though only OEM DEF fluid was used. Land Rover’s own internal service bulletins describe this issue, but dealerships have been unable to permanently repair it. I was instructed by Land Rover corporate to bring the vehicle in for a goodwill review, but instead I was told I would need to pay for diagnostics, and corporate later stated they would not cover repairs at all. A respected independent garage reviewed my service records and told me the dealership should have performed a BlueDEF regeneration procedure. They also told me this is a known and continuous problem on these vehicles and that it will never go away without a redesigned system. This is a long-standing emissions-related defect that repeatedly renders the vehicle non-operational and forces owners into unsafe situations. There is no permanent fix available through the manufacturer or dealers. I am submitting this complaint because this defect is widespread on Land Rover diesel models and carries clear safety and reliability consequences.Read more

Safety concerns of Sam Leman Mazda located in blooomington IL installed a radiator from auto zone instead of using Mazda-approved parts. Mazda corporate was then involved due to the unnecessary vehicle modifications due to Sam Lemans using a non approved Mazda part (Auto Zone radiator). Sam Leman compromised safety & my vehicle has been compromised due to component alternations and non Mazda approved parts. The auto zone part was not designed for my specific car and could fail during normal driving conditions. By altering the fan housing to make the unauthorized Mazda part fit, Sam Lemans created a risk of mechanical failure or accident. I am have a video showing the dealership discussing “zip tie” repair to hold the weight of the radiator (but this site will not allow me to upload a .mov video for your review only a photo of the video is there an alternate email I can send the video for review of Sam Leman’s mechanic discussing using zip ties to)? I am requesting NHTSA review this as a safety risk due to the wrong part, alteration of other parts, and non mazda approved part used by an authorized Mazda dealership, even after Mazda corporate was notified.Read more

Before the problem was properly identified, I was repeatedly told the issue was “just the gas cap,” despite reporting fuel smell, rapid fuel loss after filling the tank, and the check engine light. During this time, I unknowingly drove a vehicle with a significant fire hazard. The cabin fumes caused physical symptoms and unsafe driving conditions. I was not notified of this recall at the time I purchased the vehicle in March 2022, nor was I notified of a second recall affecting the passenger occupant detection system (69BY), despite having purchased a one-year maintenance warranty. I only became aware of the recalls after experiencing the hazardous symptoms described above. The problem was confirmed and reproduced by the Audi dealership. Fuel odor, fuel loss, and warning lights first appeared within 24 hours after the initial service visit, and the safety defect was later linked to the recall-related component failure. This defect presented a serious risk of fire and exposure to gasoline fumes to both myself and other road users. After complying with Recall 24V436000, the Navigation screen is stuck in Night Time Mode. This causes the streets to become very hard to see during the day. While entered routes are highlighted, cross streets are almost invisible. I have missed turns because of it. I believe this diverts the driver's attention away from looking ahead to attempting to read the Nav Screen. FCA knows about this a

Description of the Problem: I own a 2025 Hyundai Santa Fe Hybrid Limited with approximately 3 months of use. While driving on highway, the vehicle suddenly stalled displaying “Hybrid System Failure-Stop by side”. I had to stop on shoulder and restart, drove it couple of days and it happened again. Engine would not start, leaving the vehicle stuck in EV mode only with no ability to accelerate p
... roperly, Vehicle wouldnt go beyond 35mph. The vehicle then stalled and became unsafe to operate on a public road. After restarting, the vehicle displayed a Check Engine Light. Tookk it to the dealer and the dealer reported DTC P1122000, but did not provide clear evidence of an internal system fault. The dealer claimed the event was caused by “20% ethanol in the fuel,” which is inconsistent with U.S. pump labeling laws. When I checked my credit card entries around the time for refuel, all of them are reputed gas stations with nothing lower than "87" gas which is in alignment with car manufacturer's recommendation. This sudden stall created a serious safety hazard, and I am concerned this may be a broader issue with the 2025 Hyundai Santa Fe Hybrid drivetrain, fuel system sensors, or hybrid control module. I am requesting NHTSA to look into whether similar complaints exist, as this failure mode can result in loss of propulsion at speed, putting occupants at risk.Read more

My 2025 Ram 1500 TRX has recurring safety-related failures of the infotainment system affecting both Uconnect 5 (12” screen) and earlier Uconnect 4 units. These issues are widespread in 2021–2024 Ram TRX and Ram 1500 models and began or worsened after forced over-the-air updates. The primary defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses
... all functions controlled through the display, including HVAC/defrost, backup camera, vehicle settings, seat controls, and performance gauges. This creates an immediate safety hazard: I cannot adjust climate/defrost, cannot see the required rear-view camera when shifting to reverse, and cannot access key vehicle information. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including: [Insert brief personal symptoms: e.g., black screen while driving, loss of HVAC, camera not available, system rebooting, etc.] Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

Vehicle: 2022 Ford Escape VIN: [XXX] Dealer: Westfield Ford, Countryside IL Problem: Rear light repeatedly fails even after multiple repairs; vehicle becomes unsafe and illegal to drive. The rear tail light on my 2022 Ford Escape has failed repeatedly since early Spring 2025. The dealer (Westfield Ford) has attempted repairs 5 times, including replacing bulbs and diagnosing the wiring/computer s
... ystem. Each repair fails again shortly after. On June 12, 2025, the dealer refused to provide a loaner for a safety repair, forcing me to rent a car at my own expense. On October 9–10, 2025, I left the car again for diagnostics and had to pay for Uber rides because the vehicle could not be legally driven with the light out. The dealership now says the computer module (BCM) is defective, but they still refuse to open a warranty claim, so no repairs have been completed. The vehicle remains unsafe to drive due to loss of exterior rear lighting. This defect makes the vehicle non-compliant with FMVSS lighting standards and creates a serious safety hazard for nighttime or low-visibility driving. I am filing this complaint because the defect is ongoing, Ford and the dealer have failed to resolve it, and the car continues to have a recurring lighting failure that affects road safety. I am reporting a recurring safety defect involving hydraulic lifter failure in Ram trucks equipped with the Cummins diesel engine (model years 2019–2024). This issue is widely acknowledged by Stellantis and multiple dealers. It typically begins with a knocking sound and has been documented in many cases to progress to complete engine failure. My vehicle has fewer than 20,000 miles. The lifters
... were already replaced once, which required the cab to be removed. Within less than 400 miles of that repair, the knocking sound has returned. I brought the truck to a second dealer, who confirmed the problem and informed me that Stellantis has an active STAR case acknowledging the defect. They stated there is currently no known permanent fix. This defect presents a serious safety concern. Sudden engine failure while driving, especially under load or at highway speeds, could lead to loss of power, loss of steering/braking assist, and potential accidents. In my situation, I frequently travel with family members who have medical conditions, and being stranded in hazardous or remote conditions poses significant risk. At this point, my vehicle is not reliable or safe to operate. I am requesting NHTSA investigate the hydraulic lifter defect in 2019–2024 Ram Cummins trucks due to the safety implications and the lack of an available permanent repair.Read more

My 2019 Audi E-tron has experienced two major issues during my ownership. At around 87k miles, I was driving into a parking lot and suddenly a “electrical malfunction” warning light appeared on my dashboard followed my many warning lights including 4wd, brake system, and battery. The vehicle was not moving properly and I had to slowly move it into a parking lot and had it towed to the dealersh
... ip. The issue was that the front motor is toasted caused by coolant leaking into the motor. The second issue occurred around 109k miles. My vehicle couldn’t start in a parking lot and the same “Electrical Malfunction” warning light went on. The vehicle was towed to Audi dealership and the diagnosis was a failing SX6 battery control module, which costed me over $8000 to repair. Those two issues both make the vehicle unmovable and could be catastrophic if they occur at high speed. There are numerous complaints from Reddit and E-tron forum regarding the same two issues. Both issues involved moisture getting into critical electrical components and could put the safety of passengers and other vehicles on the road in danger. The Audi USA has extended the warranty for the motor issue but hasn’t done anything regarding the SX6 modules. The vehicle’s electrical motors and HV battery should be closely inspected since those issues are prevalent and could be related to serious design flaws from factory. Audi USA hasn’t addressed those issues properly and it didn’t have a proper fix for its self discharging battery as well, which puts this vehicle unsafe to operate on the road.Read more

While driving the vehicle on an uphill stretch of highway with cruise control set at approximately 65 mph, the vehicle suddenly slowed down without warning. A distinct clunk was heard at the same moment. The cruise control system attempted to restore speed, and the engine became noticeably louder, but the vehicle struggled to return to the set speed. The driver tapped the brake to disengage cruise
... control and manually pressed the accelerator to resume normal speed. The trip was completed without additional events. After parking, the check engine light illuminated and the cruise control system became disabled on all subsequent drives. No warning indicators were present at the time of the initial failure, and there were no vehicles ahead that would have triggered the collision-avoidance system. Two days later, the vehicle became an intermittent no-start and eventually failed to start entirely. The vehicle was towed to a dealership for inspection. Technicians confirmed an electrical malfunction and reported that the multiplex ECU was likely experiencing an internal short. A parasitic draw test showed an abnormally high current draw of approximately 113 mA, which disappeared when the dome-light fuse and later the multiplex ECU were disconnected. The failure is intermittent and appears to involve an internal electrical short within the multiplex ECU. The sudden loss of cruise control function, abrupt deceleration, and subsequent intermittent electrical behavior created a safety risk, as the electrical malfunction occurred without warning while the vehicle was in motion. The issue was confirmed by a dealership service department.Read more

On the date in question. The following anomalies occurred: 1.After placing the car in reverse (action 2), instead of starting to back out, the car lurched forward. The brake was immediately applied and the car stopped about a foot before hitting the garage wall. 2.The brake was applied again to assure self-driving was disengaged and proceed with action 3. 3.Car was placed in reverse and accelerator was lightly pressed to slowly back out of garage. Rather than slowly back out (as was done hundreds of times) the car immediately accelerated and rapidly gained speed. 4.The brake was applied to stop the car but could not overcome the acceleration (pictures of skid marks were taken). 5.In order not to directly hit the wall located across from garage, Car steering wheel was turned to minimize any direct contact ( skid marks will show turn). 6.Car came to a stop when it hit the wall causing significant damage to the car and causing damage to the wall (pictures can be forwarded).Read more

Two weeks after purchase, experienced multiple, random driver assistance failures. After attempting to repair issue once unsuccessfully, The dealership was unable to provide a second service appointment for two and a half months. Was explained all 2025 Audis were having issues, so couldn’t get car in any sooner. Before that second scheduled service appointment and while driving in traffic, the

On several occasions I have a dead pedal situation on acceleration from a brief stop. When you press the accelerator, the truck does not accelerate immediately. Instead there is a second or two pause before truck responds to accelerator input. When crossing traffic, this hesitation has caused several situations in which i have almost been t-boned.

Having issues with the windshield wipers clearing rain when driving and multiple camera malfunctions that I have an appointment to have both checked on 12/2. Wiper stops more than 4 inches from the edge of the window which I could probably just deal with, but when driving it pulls water anywhere from 3-8 inches further across the windshield making it very hard to see when it rains harder or you dr
... ive faster. I have owned more than 15 vehicles and never experienced anything making it difficult to see during inclement weather. The other issue is the cameras when backing up or going forward will malfunction for a day or two, then be good for a few weeks and malfunctions again sporadically. Have video of windshield wipers clearing rain issues, but it won’t let me submit, so if you want I can email. Pictures don’t show issue as well.Read more

My truck started acting sluggish like the parking brake was applied while driving. At a traffic light I put the vehicle in park applied and released the parking brake. I put the vehicle back in gear an let off the brake. The vehicle did not move so slightly pressed the accelerator. The vehicle still acted as if the brakes were applied. I could feel the slight movement and torque being applied but
... only slightly lurching. The traffic light turned green so I proceeded up to 45mph. A vehicle in front of me slowed to make a turn. Applied the brakes and nothing happened. I pressed the brake so hard that I lifted myself off the seat. I swerved to miss the vehicle avoiding a collision. Then warnings started popping up; "brake system failure max speed 62mph", "brake system failure max speed 42mph", Brake system failure", service traction control", "service trailer baking system". among others I was too busy to see all of them. I coasted to a stop in a nearby parking lot with visual and audible warnings still going. I shut the vehicle off then restarted it. Some warnings remained like traction control and the engine light. The vehicle appeared to be operating correctly so I drove it to a local dealer. they said there was a code for low voltage in the brake control module but couldn't get me an appointment for over two weeks. I took the vehicle to a regular mechanic who said what I described was impossible. That mechanic kept my truck for the balance of the day but could find nothing wrong. I no longer trust this vehicles brake system. In many scenarios the failure, were it to happen again could be catastrophic.Read more

Below is a detailed timeline of the problems I experienced: Intermittent starting issue Over the past few months, the car has occasionally failed to start on the first attempt. It always started on the second try, so I planned to mention it at the next service visit. Complete MMI/display blackout while driving on the highway In the middle of my trip, the entire center display and instrument clu
... ster went completely black while I was driving at highway speed. All information disappeared (speed, fuel level, navigation, radio — everything). The car continued running, but I had no visibility into critical driving data. After pulling over and restarting the vehicle, only half the screen returned. Hours later, the second half came back on its own. Catastrophic front-right tire blowout in a remote area of Texas The following day, while driving through a very remote stretch of highway, the front-right tire suddenly exploded with a loud bang. I safely pulled over and discovered the tire was completely destroyed. No spare tire provided The trunk contained only a tire inflator kit — no spare tire or temporary spare. A new tire was required. Inadequate roadside assistance experience I called Audi Roadside Assistance twice. Both representatives appeared to be reading from a script, were located in another country, and could not understand the urgency or specifics of my situation. After repeated miscommunication, I received no meaningful help and had to resolve the situation on my own at significant personal expense. When the local tire shop inspected the vehicle, the mechanic immediately pointed out severe abnormal inner tire wear on both front tires and stated that the alignment is clearly defective. He said the left front tire was also about to fail and that the car was unsafe to drive in its current condition. Both front tires had to be replaced before I could continue.Read more

Airbags never inflated in a front end crash at 45mph Most recently I started experiencing I intermittent trouble staring the car. Then one day it didn't start at all and smoke was coming from under the hood. Towed to mechanic , starter burnt up 155,000. $ 900 to replace then a couple days later I lost power on the freeway and the car started jerking and vibrating. Stopped Vehicle cut off and would not start. After 1hr I started and limped home. Vibration under 2000 RPMs and when stopped. Knocking noise under hood. Brand new spark plugs. Mechanic stated Cylinder 3;missfire was identified as a hole in a valve due to weak material now I have to replace engine. Oil consumption and excessive friction and heat is to blame. In the fordpass app, we get a notification that the vehicle has turned off certain function due to inactivity in order to reserve power. However, we will have just driven the vehicle. This message should only populate when the vehicle is inactive for 14 days. It can also populate when the battery drops below 9V or due to extremely hot or cold conditions. It is 70 degrees F outside, so this reduces
... the reason to only one problem, and that is the battery less than 9V. We just spent over $700 at the dealership on a new battery and service charges because they said it was a battery problem. We spent over $100 the first attempt to find the problem with a sync update. Now, with a new battery and updated sync, out vehicle is still draining the battery. Low and behold, when the message populate in the app, the vehicle will not start, leaving us stranded on multiple occasions in town. The dealership at Long Lewis Ford in Muscle Shoals cannot find the problem.Read more

I am filing a complaint regarding Ford Recall 24502 / NHTSA Recall 24V031 for my 2017 Ford Police Interceptor Utility (Explorer-based). My vehicle’s A-pillar interior trim was identified as defective under this safety recall. For nearly two years, the repair has not been completed because the dealer claims the required parts are still on backorder. In that time, the A-pillar trim on my vehicle
... has become loose and is now detaching while driving, which creates a dangerous condition. The loose trim vibrates, moves, and could detach completely at highway speeds, posing a safety hazard to me and other drivers. Ford and the dealership have confirmed the recall but have not provided a repair, a temporary safety fix, or a loaner vehicle. I have repeatedly contacted the dealer, and they continue to tell me that Ford has not supplied the part. Because this is a federally mandated safety recall and the defect is actively creating a crash risk, I am requesting that NHTSA investigate Ford’s failure to provide parts or a remedy within a reasonable timeframe. This is an unresolved safety hazard, and Ford has not provided a solution.Read more
