I was driving down a regular road by my house, full sunshine, perfect road, nothing unusual. In fact, it was such a nice day, the sunroof was open and windows were down. My dog was in the back seat after just being picked up from grooming happily enjoying the day with his head out the window. I was on a road with the speed limit of 30. I was going 30 mph. Nothing unusual. Literally, out of nowhere
... , like my car was possessed, is the best way I put it, my car abruptly braked in an attempt to stop on a dime. My car screeched to a stop while simultaneously my windows rolled up, my sunroof closed and my seat belt tightened, locking me in place. My dog was thrown forward. It was terrifying! Luckily, no one was behind me or they would have certainly rear-ended me. Nothing was in the road. Absolutely nothing. It happened so quickly in a split second. I put my hazards on and finished the next 2 miles home. I called Audi for a service appointment. I took my car in, again going down back roads with my hazards on. Audi charged me $400 to run a diagnostic report and shows there is a fault code J428. This fault code I researched is for the front/right ACC radar sensor that powers Adaptative Cruise control and Pre-Sense braking system. The fault code erroneously detected an obstacle and did abrupt braking. Audi told me that they would charge me $4,000 to fix it. I contested that as a safety issue it should be fixed as there is also Audi TSB2063502 referencing problems. This is a major safety issue as if I was going 70 MPH, I could cause a massive accident where I could be killed, anyone in my car, as well as others behind me in a massive car pile up. I feel ethically, this must be a safety recall to replace this fault in all vehicles. I am also requesting payment to fix from Audi. As an attorney (#[XXX]), Audi is under notice that they have a major safety defect on their hands and should take steps to protect drivers safety. Thank you. I am reporting a severe safety hazard on my 2020 Audi Q8 directly caused by the software update for NHTSA Recall 25V900 (Audi 90TV). The recall shows as "completed" on my VIN, but it left the vehicle with an active driver assistance defect that did not exist before. No accident has occurred, but there is a severe risk of a crash. Following the recall, the Audi Side Assist (blind-spot monitoring)
... behavior changed dangerously. When I signal a lane change while a vehicle is in my blind spot, the mirror LEDs blink only 3 times and then return to a solid, dim state—even though the vehicle is still actively there. If you miss that brief 3-blink window, the dim LED falsely implies the lane is clear. This has caused 3 separate highway close calls (no crash occurred) where I almost merged into a vehicle because the warning period is inadequately short. This defect followed a catastrophic failure during the recall execution. I dropped the car at Audi Chantilly on Jan 13, 2026. When picked up on Jan 29, the lane-keep assist was broken (bouncing the vehicle left and right between lanes) and an SOS malfunction light was on. It took the dealer over 2 additional months to "fix" the corrupted camera/localization modules, finally returning the car in early April. While the major module errors are gone, the underlying side assist logic remains broken and hazardous. Because of a bitter, 3-month waiting experience where the vehicle was degraded, I do not trust Audi Chantilly to fix this. I need guidance on how to compel Audi to investigate this post-recall software defect and allow me to have it corrected at a different dealership.Read more

I am reporting a serious safety defect involving the door locking and electrical systems in my 2024 Audi SQ8. On two separate occasions the vehicle experienced a complete electrical malfunction. During the first incident, I and my child were trapped inside the vehicle. The doors would not open from the inside and the vehicle would not start. After the first incident, the vehicle was taken to an Au
... di dealership for inspection and repair. I was informed that the issue had been identified and repaired. Despite those repairs, the same safety issue occurred again. During the second incident, which was captured on video, the vehicle would not start despite repeated attempts. The vehicle malfunctioned again after the dealership had already stated the problem was fixed. Dealership personnel acknowledged that they could not guarantee the problem would not happen again. I was also provided a video demonstrating a manual emergency exit procedure because the electronic door locking system could become inoperative again. The dealership showed how occupants would have to use a mechanical override to exit if the electronic system failed again. This defect creates a significant safety risk because occupants can become trapped inside the vehicle unable to exit during an emergency. The inability to start the vehicle combined with failure of the electronic door locking system could delay escape in the event of a fire, collision, medical emergency, or other hazardous situation. The vehicle has been inspected by an authorized Audi dealership multiple times. The problem was reproduced by the vehicle itself on multiple occasions, and I have video evidence documenting one of the incidents. Because of these repeated failures and the safety risk posed to me and everyone on the road I no longer feel it is safe to drive it.Read more

Defective side‑marker LED inside sealed headlight assembly Component: Exterior Lighting / Headlights Make/Model/Year: 2017 Audi Q7 prestige Issue: Defective side‑marker LED inside sealed headlight assembly Complaint: The side‑marker LED inside the headlight assembly on my 2017 Audi Q7 has failed due to an internal LED driver defect. The LED module is sealed and non‑serviceable, so the e
... ntire headlight assembly must be replaced at a cost of $2,000–$5,000. The vehicle displays a “Side Marker Light Defective” warning even though the LED is still illuminated, indicating reduced output that triggers the fault. This failure reduces nighttime visibility and creates a safety hazard. This is a widespread issue reported by many Audi Q7 owners. Audi designed the LED module as a non‑replaceable component, leaving full headlight replacement as the only repair option. This is a design flaw that forces an extremely expensive repair for a basic safety function. I am requesting that NHTSA investigate this lighting defect.Read more

I and my wife were emerging from an intetstate highway entrance ramp when our car suddenly and completely lost power. A "Transmission Malfunction" warning suddenly (and for the first time) appeared on the dashboard. We had just completed a merge with heavy traffic going 70-75 miles per hour. Immediately behind our car was a very large tractor-trailer that hit its brakes but came within inches of th
... e rear of our car before we could get on the highway shoulder--i.e., we came extremely close to suffering serious injury or death. The car was towed to the nearest Audi dealer, where it was determined that the Transmission Control Module had to be replaced, at a cost of $8,000. The warranty on the car is 4 years/40,000 miles, while our car is 5 years, 2 months old, and has 47,000 miles on the odometer. The dealer (Flow Audi of Charlottesville) confirmed the car's problem and determined that, because of the threat to safety, the cost of the repair, and the lack of any previous warning, it should be covered by warranty in spite of the car's age and mileage, but Audi has refused to do so. Audi has been aware of this problem with Q5e (plug-in hybrid) models such as ours, as they have published a Technical Service Bulletin about it (without provision for the affected vehicles to be examined or repaired). The vehicle is still at the dealer for repair (as of 06/08/2026).Read more

I was traveling at highway speeds in active, high-velocity traffic when my vehicle suffered a sudden, catastrophic mechanical failure that nearly resulted in a severe multi-car accident. Without any warning, the vehicle completely lost its ability to accelerate, flatly refusing to pick up speed or maintain momentum despite pressing the gas pedal. This unexpected, immediate drop in propulsion ma
... rooned me in high-speed lanes, leaving the vehicle dangerously vulnerable as oncoming traffic rapidly closed the distance behind me. Drivers behind me were forced to aggressively swerve and brake to avoid a high-speed rear-end collision. Moments after this life-threatening loss of power occurred, the Check Engine Light illuminated on the dashboard. Subsequent technical inspection revealed a severe engineering defect: the internal seals of the factory coolant control components failed, forcing pressurized engine coolant backward into the engine’s primary vacuum system. This pressurized fluid completely contaminated the vacuum lines, the vacuum reservoir tank, and the electronic solenoids—including the N75 boost control solenoid. By filling the vacuum infrastructure with liquid coolant, the engine was instantly starved of its ability to regulate turbocharger boost, triggering a critical "limp mode" and a P0299 underboost fault. The catastrophic failure of an internal cooling seal directly compromising an active drivability safety system represents an unreasonable and severe defect. It transforms a routine drive into an immediate, uncontrollable highway hazard. I am filing this report because this known coolant migration pattern presents a clear, present, and life-threatening danger to owners operating this engine platform.Read more

I am reporting recurring safety-related defects on my 2018 Audi A4 involving Automatic Emergency Braking, Forward Collision Warning, Audi pre sense/front-collision functionality, and related electrical/driver-assistance reliability. The AEB/forward-collision system has shown two unsafe intermittent behaviors. At times it has failed to activate as expected when crash-mitigation assistance appeared
... reasonably available. At other times it has unexpectedly warned, jolted, or applied braking when there was no apparent vehicle, object, pedestrian, or hazard close to the car. Unexpected braking or false intervention can startle the driver, create rear-end collision risk, and reduce confidence in the system. Failure to activate can also increase forward-collision risk. The system was not intentionally disabled by me, and I am not aware of any driver-caused obstruction, modification, or normal explanation. The condition is intermittent and may not appear during a brief service visit or basic fault-code scan. This concern was reported through Audi/dealer service channels, including a documented repair request identifying the AEB system as malfunctioning and failing to activate as designed. A later dealer communication stated that there were no stored AEB faults and later clarified that no work was done on the AEB system, no inspection or verification of AEB operation was performed, and confirmation of operation would require an additional estimate. I am reporting unresolved safety status because intermittent active-safety failures or false interventions may not store permanent DTCs, and lack of stored faults is not complete safety verification. I have not received final written safety closure confirming inspection, calibration review, road-test verification, factory scan/freeze-frame review, module communication review, and technical verification. No crash, fire, injury, or fatality is being reported. The vehicle is available for inspection if requested.Read more

Dear Sir or Madam: I am submitting a safety complaint regarding my 2022 Audi Q3 involving water intrusion from the sunroof. At approximately 41,000 miles, approximately a month ago, water began leaking through the front edge of the sunroof despite the sunroof never being opened prior to the issue. The leak was not caused by clogged drains or lack of maintenance. Water entered through the overhe
... ad console, resulting in failure of the interior lighting system causing lights to flicker on/off and water dripping directly onto me, the driver, while operating the vehicle. These issues created a significant distraction and potential safety hazard, especially during night driving. The dealership diagnosed the issue and confirmed that the repair required modification of the sunroof assembly and track to correct a water intrusion problem. This was not a routine maintenance issue. The total cost of repair was $1,259.18, which I paid out-of-pocket. Additionally, the service advisor at the dealership stated that this is a frequent problem seen on Audi Q3 vehicles and expressed surprise that it has not already been subject to a recall. Given that this issue involves water intrusion affecting electrical components and actively interfering with safe vehicle operation, I believe this may represent a broader design or manufacturing defect impacting vehicle safety. I respectfully request that NHTSA review this issue for potential investigation and possible recall action. Thank you for your attention to this matter.Read more

I was driving my car would not accelerate on the highway and it almost caused me to get into an accident with my niece in the car. He has found the coolant water pump has failed internally causing coolant to leak inter

In order to fix, the water pump and all compromised vacuum system components need to be replaced. It is a rather extensive job unfortunately.” My car only has a little over 45 thousand miles. Audi should be responsible to fix this problem because they sold this car knowing the water was faulty in these models. I recall should have been put out. I was quoted $6,000 to fix this problem.Read more

Our '22 Audi Q5 experienced a spontaneous sunroof implosion while driving on the highway. Temperature would not have been a considering factor here, nor did anything hit the sunroof but it exploded and showered the occupants with glass - both pebble-sized pieces and small shards. The curtain was open at the time and glass went everywhere. Thankfully, the driver had the good sense to pull over i
... mmediately without an accident. This incident interrupted their trip, they had to find someone to tape a plastic cover over the gaping hole, then have the sunroof replaced, all at considerable expense and hassle. This was both a very dangerous AND frightening experience. Audi headquarters was contacted and denied any responsibility, saying that we need to handle this through our insurance company which I find outrageous as it has been recorded that quite a few Audis have experienced this same situation. It should be covered under their warranty!!Read more

Vehicle: 2015 Audi Q3 | Engine: 2.0L TFSI 4-Cylinder Turbo Component: Engine Complaint Description: While driving on the interstate at highway speed, the red oil pressure warning light illuminated suddenly and without prior warning. The standard low oil level warning light did NOT illuminate prior to the failure. Oil level was verified as correct and within specification at the time of failure.
... The vehicle suffered a complete and catastrophic engine failure due to oil starvation and rod knock, despite a verified proper oil level and oil pressure. The engine requires full replacement. This failure occurred at only 3,432 miles since the most recent oil change. The vehicle has a documented history dating back to 2021 of recurring engine oil consumption concerns, fuel contamination of engine oil, and high-pressure fuel pump failure — all serviced at authorized dealerships. A multipoint inspection performed approximately four months prior to this failure returned a fully satisfactory result with no engine concerns flagged. This is believed to be related to the known piston ring defect in Audi/Volkswagen 2.0L TFSI engines documented in federal class action litigation (Rieger v. Volkswagen Group of America, Case No. 1:21-cv-10546-NLH-EAP), which explicitly named the 2012–2017 Audi Q3 as an affected vehicle. The engine failure occurred without warning while at highway speed, posing a significant and immediate safety risk to the driver and other motorists.Read more

On May 6, 2026, while operating the vehicle on a neighborhood street, the vehicle experienced a sudden, violent, and dangerous failure. After accelerating away from a stop sign, the vehicle was traveling at approximately 20-25 mph when, simultaneous with a warning message, it came to an immediate and abrupt stop. A number of error messages appeared in rapid succession on the vehicle’s MMI. Th
... ose messages referenced, among other things, drive battery errors, a major electrical system failure, and drivetrain issues. After the abrupt stop, the driver, attempted to restart the vehicle. Following one failed attempt, the vehicle restarted; however, after traveling approximately 30 feet, the vehicle again came to an abrupt, violent, and sudden stop. After several additional failed attempts to restart the vehicle, the driver allowed the vehicle to sit idle for approximately five minutes. The driver was then able to restart the vehicle and drive it, very cautiously and slowly, approximately two miles to a safe location. Once in a safe location, driver contacted the dealer who confirmed that the vehicle was unsafe to operate. The vehicle was towed to the dealership. On May 7, 2026, the dealer informed us that they had found “several different faults stored in the vehicle but those faults all basically contradict each other.” The dealer further informed us that they would need to contact Audi in Germany for guidance on how to proceed with diagnosis and repair. On May 22, 2026, the dealer informed us that "(w)e do need to drop the battery. There is a module in one of the battery cells that we are going to replace and also the module that controls the battery as well".Read more

2022 Audi Q5 (VIN: [XXX] ). Engine lubrication system failure — persistent abnormal oil consumption across two separate engines over 14 months. Beginning March 2025, the low engine oil warning light began activating repeatedly, sometimes after as few as 300 miles. AutoNation Audi Bellevue confirmed 1 quart low and initiated oil consumption tests. Their technician noted in writing that oil found
... externally was "not enough to cause a 1 quart low oil light" — meaning the root cause was never identified. A PCV valve replacement was attempted July 2025; the warning returned after only 300 miles — same as before the fix. Audi of America approved a full engine replacement in September 2025, completed December 2025. Within weeks of pickup in January 2026, the low oil warning activated again on the brand new engine, and again after 4,000 miles in April 2026. The defect has persisted across two engines despite multiple repair attempts. Audi now claims consumption is "within spec" at 0.16 qt/1,000 miles but has not explained why the warning light keeps activating. Safety risk: unpredictable oil loss risks sudden engine seizure at highway speeds with insufficient warning to stop safely. Confirmed and inspected by AutoNation Audi Bellevue and Audi of America. Vehicle available for inspection. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
