The vehicle displayed a warning lamp indicating a Cooling System Performance failure. The specific defect relates to the Integrated Thermal-Management Module (ITM) and the thermostat opening prematurely, corresponding to Diagnostic Trouble Code (DTC) P218100. This issue requires a repair outlined in Technical Service Bulletin 25-EM-007H, involving a spring installation and ECU update. This failur
... e creates a serious safety concern because improper thermal management can cause the engine to overheat or force the vehicle into a reduced-power 'limp mode' without warning. A sudden loss of acceleration or engine stall while driving in traffic significantly increases the risk of a crash.Read more

Incident Date: [XXX] Vehicle: 2023 Kia Sportage Component(s): Power Train (Automatic Transmission Shifter), Electrical System (Instrument Cluster/Display), Back Over Prevention (Rearview Camera) Description of Incident: On [XXX], while performing a low-speed maneuver in Pittsburgh, PA, the vehicle experienced a catastrophic Shift-by-Wire (SBW) logic failure resulting in a collision. After shiftin
... g the rotary gear selector from Reverse to Drive, the transmission failed to engage the selected gear and remained in Reverse without any auditory or haptic warning to the driver. Simultaneously, the Rear View Monitor (RVM) experienced a "system hang," continuing to display the rear camera feed and blocking the instrument cluster’s gear-position indicator. Due to this dual failure—unintended gear retention and display latency—the vehicle traveled in an unintended direction (Reverse) while I attempted to accelerate forward, leading to a property damage accident. Post-Incident Documentation: The vehicle was presented to Performance Kia (Moosic, PA) on 12/19/25. Technician #957 confirmed the software logic was corrupted and performed a "Clear and Relearn of Adaptive Values" to reset the Transmission Control Module (TCM). This diagnostic confirms that the vehicle was operating with failed control logic. Safety Risk: The failure of the Shift-by-Wire system to accurately execute gear changes, combined with the instrument cluster's failure to provide real-time gear position as required by FMVSS No. 102, creates an unreasonable risk of collision and injury. I was never notified of Recall SC270 or SC275, which address similar software and hydraulic failures in this model year. Component/System: Brake system – specifically brake pedal assembly / braking performance. Description: The vehicle experienced recurring issues with braking performance, including inconsistent or reduced deceleration during braking. Availability for inspection: The vehicle is currently in my possession and available for inspection upon request. The braking issue reduced the vehicle’s abilit
... y to slow or stop as expected during normal driving conditions. This created a safety risk to myself and other drivers and contributed to a rear-end collision. Although no one was injured, the inability to decelerate as expected increased the risk of a crash without prior warning. The issue was reported to an authorized dealer on three separate occasions over the past year. The dealer was unable to fully resolve the braking concern, and the issue persisted after each visit. Additionally, a safety recall was issued on December 9, 2025 (NHTSA Recall Number 25V-846) identifying a brake pedal assembly defect that may collapse during braking. The vehicle has been inspected by an authorized dealer multiple times for the braking concern. The vehicle was also inspected by insurance representatives following the collision. Repairs have not yet been completed pending further documentation and recall-related review. No specific warning lamps or messages were observed. The primary symptom was inconsistent or insufficient braking response during normal operation. These symptoms began prior to the first dealer visit and continued intermittently despite multiple repair attempts.Read more

There is an open safety recall on my vehicle since September 2025 with no updates on when a remedy will be available from Stellantis. My car is sitting idly in the garage and I'm paying hundreds of dollars every month in loan and insurance costs without being able to drive. I have called my dealership several times and they don't have an update from the manufacturer yet either. The car engine and hybrid system shuts down randomly and without warning when I am driving, and the car has no power. The first time it happened was on Wednesday, Dec. 11th 2025. I had just picked up the car a few days prior, when they replaced the high voltage generator under warranty. I was on the highway and the car shut down, with a red turtle on the right side of the instrument display. I rol
... led to a stop on the shoulder and managed to get it running again. I drove it in E-SAVE mode for about 40 miles and it shut down again, and I rolled to a stop on the side of the road. I got it restarted and in E-SAVE mode I managed to get it to the dealer. It was returned to me on Wednesday, Dec. 17. The dealer, Ron Lewis Alfa Romeo, said it was not programmed properly after installing the part on the prior repair job. They claimed to have driven it 50 miles without issue after the reprogramming, and said it was fixed to good working order. Today (Dec. 26th, 2025) I was going up a hill and it shut down again, with a red turtle on the dash, and the car stopped. I got it started and drove it home in E-Save mode. I then had Alfa Romeo roadside assistance have the car towed back to Ron Lewis Alfa Romeo. The car is unsafe and I cannot drive it without fear of it shutting down while underway. Having a car engine and hybrid system completely shut down in live traffic, either on the highway or in a suburban or business district, suddenly and without warning is incredibly dangerous to the driver, passengers and those in other vehicles. They have been unable to find and fix the problem, and keep giving me back an unsafe car to drive.Read more

- Engine water pump failure and coolant intrusion into the vacuum system. Parts were replaced on 5 Nov 2025 and possible still at the dealer's location. - The defect poses the following safety risks: It resulted in contamination of vacuum lines; potential for sudden engine stalling or loss of power while driving; potential for catastrophic damage to engine-management systems dependent on vacuum in
... tegrity; potential for damage to emissions and combustion-control systems; potential for reduced braking system performance if vacuum supply is compromised, expensive and unexpected component failures, and possible recurrence since the cause appears related to design rather than normal wear. Any failure can create cascading hazards affecting safety-critical functions. - The problem was confirmed by an authorized Audi dealer. Dealer name and contact info can be provided upon request. - I do not know if the dealer delivered the involved parts to the manufacturer or insurance representative for inspection. Police or others did not inspect it. - There were no warning lights, messages, or other symptoms of the problem prior to the failure. It was identified by the Audi technician on 5 November 2025 while the vehicle was in the dealer for other services. The failure occurred at 47,855 miles which is far below the average lifespan of a water pump or vacuum system.Read more

Yet, there have been recalls for 2020 Ford F-150s related to the emissions system, including a recall for the 2.7L EcoBoost engine's catalytic converters, which requires a Powertrain Control Module (PCM) reprogramming, and another related to a loose battery cable that could cause stalling, fire, and other malfunctio
... ns. Catalytic converter recall (24E12) Who is affected: 2018–2020 F-150 models with the 2.7L EcoBoost engine. : The recall addresses inaccuracies in the catalytic converters' effectiveness. The primary fix is reprogramming the PCM to more accurately detect issues, which might trigger a check engine light if the converter is faulty.Read more

My 2017 Alfa Romeo Giulia, VIN [XXX] , is currently disabled and inoperable at the authorized dealer, Alfa Romeo Fiat of Daytona (Service Advisor: Kevin Chapman). The vehicle is subject to the active and known Safety Recall 93C (Low-Pressure Fuel Pump). My vehicle is experiencing a total loss of motive power and engine stall, which is the exact safety hazard described in the recall. Furthermore, t
... he initial failure of the low-pressure fuel system appears to have caused secondary consequential damage to the expensive High-Pressure Fuel Pump (HPFP), and the vehicle is also exhibiting a severe and recurring electrical defect (new battery dying, random dash warnings/brake lights flashing). The manufacturer, FCA/Stellantis (Customer Care Case #XXX...), and the dealer are engaging in a runaround to deny basic safety recall provisions: 1. Refusal to Cover Consequential Damage: FCA Customer Care stated that if the issue is 'not the 93C recall then it will all be customer pay,' which conflicts with NHTSA guidelines requiring the manufacturer to remedy the primary defect and all damage proximately caused by it. 2. Refusal to Provide Loaner/Rental Car: FCA stated the loaner is up to the dealer, and the dealer stated it is up to the diagnostic. This runaround has created an unreasonable delay in the repair of a federally mandated safety recall, leaving the owner without transportation for a disabled, unsafe vehicle. I am requesting NHTSA intervention to compel FCA/Stellantis to: a) Authorize a complimentary rental car/loaner until the vehicle is fully repaired. b) Provide a formal commitment that all damage proven to be a direct, proximate consequence of the 93C fuel pump failure will be covered at no cost. My 2018 Alfa Romeo Stelvio experienced a complete electrical failure and will not start at all. The vehicle suddenly lost all functionality and remains fully inoperable. The dealership has had the vehicle for an extended period and has been unable to diagnose or repair the defect. This appears to be related to known electrical issues in 2018 Stelvio models, including BCM (Body Control Module) fai
... lures, wiring faults, and fuel system defects reported in recalls and TSBs. A no-start electrical failure is a serious safety concern because similar defects in this model have been linked to unexpected engine shut-offs. The vehicle has been out of service for over 30 days. This is a major safety and reliability issue. I am requesting that NHTSA review this defect, because the manufacturer has not provided a resolution.Read more

When arriving at the high school parking lot to pick our grandson up from band practice, we experienced Sudden Unintended Acceleration of our vehicle as we attempted to pull into a parking spot. This resulted in a collision with another vehicle that had backed into the parking space in front of us. That driver was inside her vehicle and claimed injuries. As you can imagine we were pulling into the
... space at a very slow rate of speed when all of a sudden the car lunged forward at a high rate of speed, jumped two parking lot cement barriers and slammed into the rear of her vehicle. The car is equipped with all the latest collision avoidance equipment, but did not stop before hitting the other vehicle ... and no automatic warning sounds went off that I am aware of. A police report was filed and a tow truck had to be called to tow the vehicle. The vehicle had less than 5,ooo miles on it and had only been owned by us for 4 months (it was new when we purchased it). The accident happened on August 27, 2025 and we did not get our vehicle back from the body shop until Monday, November 17. Not only was our safety at risk, but the other driver and several students in the parking lot at the time. This incident could have been so much worse, or could have ended in a fatality. Volvo did an inspection of the vehicle, but the results were inconclusive and our insurance company paid the claim. As soon as we got our vehicle back we took it to our local Volvo service manager to have him check it over. He has indicated the car is safe to drive ... but I am very nervous about driving the vehicle now. The cost to repair the vehicle was $14,145. I was not distracted at the time of the accident and I am certain that I did not hit the accelerator pedal. NOTE: I never received a copy of the accident report, but I believe our insurance company has a copy.Read more
