Initially thought a seal on the windshield was faulty, so contacted the business who replaced the windshield. Business owner came to owner’s house Mon, Nov 4, 2024. He did a thorough inspection of all areas where water was identified by owner (dash, under the dash, on the driver side floor) and confirmed it was not the windshield. He then inspected the roof rack and determined the roof rack was
... defective and leaking water from the ceiling of the vehicle, down into the dash, and ultimately onto the floor. He recommended contacting Toyota because this was a known issue. By 12pm on Mon, Nov 4, 2024 owner contacted Toyota alerting them to the issue. An appointment was made to bring the vehicle in for inspection on Mon, Nov 11, 2024. Owners do not have a garage or carport, so a canopy was pitched over the car in an effort to prevent any further water damage and moisture absorbing canisters were placed in the car. The vehicle was not driven in the rain during the period after first noticing the leak and bringing it in to the dealership. Evidence of Electrical Damage: On Friday, November 1, 2024 the driver side seat became extremely hot, to the point of needing to pull over and turn the car off. The seat heater was not turned on at any point during this time. On Saturday, Nov 9, 2024 and Nov 11, 2024 while driving on the highway, the car (normally driven in Eco Mode) unexpectedly switched into Mud&Sand mode and all safety features switched off. Owner was unable to manually override this occurrence. The “Normal” button did not work. After a few minutes, the vehicle switched into the default setting and owner was able to put it back into Eco Mode. On Nov 11, 2024 owner brought vehicle into Toyota for diagnostics. Owner requested that the electrical components also be inspected and was told it was not covered under the Customer Support Program.Read more

I previously submitted a report to your agency on May 31, 2024 regarding my 2018 Honda Civic Sedan (VIN# provided) relating to what has been widely called a "sticky" steering action. After having one of their dealerships confirming there is a problem with my steering, I wrote Honda America on June 1, 2024 and advised them of that problem and requested my vehicle be added to a recall program that
... has included other Honda Civic's in the same manufacturing date range as my vehicle. Honda refused. To further confirm and document that my vehicle suffers from this problem, I took my vehicle to another local Honda dealer, who also confirmed my vehicle suffers from this problem. By letter of October 30, 2024 I again wrote Honda America and appealed for them to repair this dangerous steering problem. I must continue to drive this vehicle with this serious problem, and I am writing this to document my communications with Honda, and hope that your agency will intervene with Honda to add my vehicle to a recall program. I purchased a brand new 2023 Mazda CX – 30 from Cooley Mazda on September 27, 2023. I was excited to upgrade from my 2011 Mazda 3, however I have had issues early on. On June 7, 2024, I brought my 2023 Mazda CX – 30 in for diagnostic testing for front end noise and a shifting issue. I went out for a drive with the Technician and was told the noise I was hearing was from the weight of the car
... on the suspension, I disagreed. The Technician also experienced the shifting issue I spoke of, and I was told that the car needed to learn my driving technique, again I disagreed. At this point the vehicle only had 4014 miles on it. On July 25, 2024, I brought the vehicle in for its 1st oil change and tire rotation, as well as for the same noise and shifting issues. This time they found an issue with the front driver side front wheel bearing. They did not have the part in stock, so I had to wait for a call to schedule to come back once the part came in. Again, they claimed there was no shifting issue with the vehicle. At this point the vehicle only had 5079 miles on it. On August 7, 2024, I brought the vehicle in for replacement of the front driver side front wheel bearing. On my drive home I heard the same noise I brought it in for repair for. At this point the vehicle only had 5468 miles on it. On September 4, 2024, I brought the vehicle in for a NYS Inspection and for diagnostic testing on the noise and shifting issues, again they took the car out, did not experience the issue, but put it on the lift and did some testing which determined the car needs a new rear differential. At this point the vehicle only had 6219 miles on it. The rear differential was replaced and the vehicle still has shifting issues. It hesitates, sometimes doesn't advance when the gas pedal is pressed. The vehicle also hesitates at 38-40 and 58-60 mph while just holding your foot steady on the gas pedal.Read more

Since purchasing this vehicle new, I've had several occasions (not on just the most recent date noted in this case) where the car will issue a false Pre-collision Alert and apply the brakes for about 1 second while using the Adaptive Cruise Control. However, this happens while me and the drivers around me are driving "normally" and there are no vehicles in front of me nor attempting to merge into
... my lane. There does not seem to be a particular set of conditions that cause this false alert and they happen randomly, but it is startling when they happen since there is no observable reason for it. Fortunately, there has not been anyone tail-gating me when this happens because this could easily cause a rear-end collision if someone were following too close. I've notified my local dealer about this issue more than once, but they cannot find anything wrong.Read more

The engine began making an metallic clicking noise that increased with engine RPM. I made an appointment with the local Honda dealership service center as soon as possible. The options I was provided with were to either replace the engine or replace the connecting rod bearings and connecting rod bolts with no warranty on the work as they could not guarantee the crankshaft had not been damaged. I provided all receipts for engine oil changes, engine oil filter changes, and engine air filter changes to Honda USA. Despite having an active recall for this exact problem (connecting rod bearing failure) on other 2017 Honda Ridgeline's with the same 3.5L V6 Engine, and performing all maintenance as needed with proof provided to Honda USA they denied my claim for vehicle age and mileage. My VIN was not included on the recall. I purchased a new 2019 VW Jetta GLI. Since the first day I bought the car four years ago, whenever I call someone or receive a call, and my iPhone is plugged in to the USB socket to connect to hands-free calling, the person on the other end hears a very big echo and voice lag. 100% of the time this happens. I end up having to hang up and call just using the speaker on my phone because no one can s
... tand the giant echo reverb they hear on their end. I have brought the car into a few different VW dealerships to have this fixed and they never fix it. I paid $500 to a local mechanic to have the entire hardware unit replaced with a new VW --manufactured unit and the problem still exists. I googled to see if other owners of a 2019 VW Jetta are having this issue and it appears to be a very, very widespread and persistent problem across the entire brand. VW has known about this problem for five years! They do not fix it ever. They need to be forced into fixing it by a government agency like you please! This is a safety and legal issue, since I am not able to use the hands-free calling feature in my car. It is illegal in my state to use a cell phone while driving unless it is a hands-free call. If you google this problem, you will find thousands of owners with this problem. Please issue a recall as it is a safety issue and a legal liability. Thank you! I am filing this complaint regarding serious safety issues with my 2018 Mitsubishi Outlander Sport, VIN: [XXX] , purchased from Carvana in 2019. Recurring transmission problems have created a significant safety risk for me and others. About a year after purchase, the vehicle’s transmission began failing, and I replaced it in 2021. Despite this, the transmission warning light activates intermitt
... ently in stop-and-go traffic, and now the vehicle has started stalling on highways, creating a critical hazard by suddenly losing power. In 2021, I took the vehicle to a now-closed Mitsubishi dealership in Brooklyn, where I was informed that this transmission issue was not covered, despite it being a drivetrain problem that should have been under the 100,000-mile powertrain warranty. My research suggests this model may have a Transmission Control Unit (CVT-ECU) defect, potentially causing stalling. Carvana has stated this issue is Mitsubishi’s responsibility, as it is likely manufacturing-related rather than due to prior use. Given Carvana’s stance and the severe safety implications of stalling, I urge NHTSA to investigate if this vehicle, or others with similar transmissions, pose a broader safety risk. Summary of Safety Concerns: 1. Recurring Transmission Problems: Transmission was replaced in 2021, yet the warning light persists, indicating unresolved issues. 2. Stalling on Highways: Vehicle loses power without warning, posing a major hazard in high-speed traffic. 3. Potential Manufacturing Defect: Research suggests a CVT-ECU defect, likely explaining the stalling. These issues leave me unable to rely on my vehicle safely. For my safety and others, I request an investigation into these transmission issues in the 2018 Mitsubishi Outlander Sport to determine if there is a larger safety concern. Thank you for your attention. I am available to provide further information as needed. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

We purchased this vehicle in mid 2018. In October of 2018 Lexus issued a customer support process ZLZ which covered our vehicle for problems with the dashboard. At that time, we had no deterioration of our dashboard so we did not respond. The Lexus program expired in 2021. In the last two years, our dashboard has deteriorated quite substantially becoming sticky and more importantly, becoming very
... reflective. This condition is a serious safety issue for day and night driving as light reflects up on the windshield and obscures vision out of the windshield. We took the vehicle to Lexus San Diego in August of 2024 and they declined to address the repair as a defect and offered to replace the dashboard for approximately $3000. At that time I called the Lexus Corporate Dept and they said that it could fall under the support program again if they re-opened the program. The representative did not offer any timeline for that happening. I sent correspondence to Lexus Corporate in Oct of 2024 and finally heard back on 11/08/2024 that they would only offer a customer paid fix of approximately $2200.00. The dashboard is definitely defective (soon to be inoperable) and creates a very significant safety problem. I do not understand or agree with Lexus not addressing it properly.Read more

I had just completed filling my car with gas and was still parked at the gas pump. The gas cap had been replaced and I got back into the car and started it. I thought that the engine had fully started, but when I shifted into drive, the engine sputtered a little. I put it back in park, and attempted to start the car again. There was a huge bang under the hood and a puff of smoke. Upon opening
... the hood, the cover over the intake manifold had exploded into pieces. There was enough force that one of the shards of plastic dented the hood from the underside. Luckily nothing caught fire, but I fear that it could have. I had the car towed to a repair shop, and they confirmed that at minimum, the intake manifold needs replacing, but really couldn't guess as to the cause of the failure at this point. They are the only people who have had an official look at the vehicle. It would be available for inspection by someone else. My engine had been running fine with no warning signs in sound or performance. I am someone who keeps up with regular car maintenance. I did have a check engine light on back in late spring that indicated (I'm looking around to see if I still have the print out) a minor issue with the gas cap seal, but the light cleared itself before my appointment at the dealership to have them investigate it and the technician told me that in the absence of other issues (noise or performance) it should be fine to drive. I would like to report a serious safety issue with the 2021 Acura TLX Type s. Recently in April 2024, while driving, I heard a loud pop from the vehicle, and it turned out that the transfer case had burst into the transmission. This failure has rendered the rear-wheel drive non-functional, resulting in severe oversteer. This oversteer is especially dangerous because the vehicle is now operating a
... s front-wheel drive, which creates an unpredictable and hazardous driving experience. This issue could lead to a catastrophic transmission failure, particularly at highway speeds, posing a severe safety risk to me and other drivers. I have contacted the dealer about the problem, but they have refused to take any corrective action. I’ve also found that multiple other owners have reported similar issues with this model, highlighting a potential widespread problem with the transfer case and transmission interface. Given the safety risks associated with this failure, I urge NHTSA to investigate this defect and take appropriate action to ensure public safety.Read more
