I was driving my 2016 Ford f150 in Ventura California on August 29th 2025 when my throttle began to accelerate on its own after taking my foot off the gas pedal. My truck sped up out of control, I almost lost control of the vehicle and crashed. Luckily I was able to gain control and get it into neutral and then into park. My trucks rpms raced so high it sounded as if the engine was going to explod
... e. I put it into park and the rpms lowered to a normal level. While this was all happening a wrench icon popped up on my dash. Once the wrench icon came on I noticed my dash wasn’t telling me what gear I was in or if I was in park, neutral or drive. I pulled into the closest parking lot. I put it in park and every time I gave the truck gas the throttle would continue to accelerate on its own and would rev extremely high on its own. I also noticed as I was sitting it the parking lot at idol there is now a thumping sound/ jolt when selecting what gear to go into. I don’t know if this was caused from having to slam the car into neutral and then into park so I wouldn’t crash when the throttle accelerated. I shut my truck off and it wouldn’t turn back on. All of my electronics work but it doesn’t even want to try to start or turn over. I had to tow my truck home and it is now sitting in front of my house unusable. I have videos I can provide.Read more

While driving at highway speed, all of a sudden I lost all power steering and multiple system failure warning lights appeared on the center dashboard including power steering failure, brake system failure and collision mitigation system failure, among others. With great difficulty I was able to steer the car off the highway and was towed to an Acura dealer. The car was 5 months old with just ove
... r 3000 miles at the time and there were no prior issues and no advanced warning when this happened. The dealer (Acura of Berlin in CT) stated they have seen this issues several times. The battery voltage was low (11.98-12.09 volts), although the car was being driven at highway speeds for at least 45 minutes before the incident. They charged and tested the battery, and their diagnostics said the battery was good. They also found a code "DTC U3000-49 EPS CONTROL UNIT INTERNAL CIRCUIT MALFUNCTION". The dealer opened an incident with the Acura Tech line, who indicated that if the problem happens again they should replace the steering rack. No repair other than battery charging was performed. This is an unacceptable safety risk waiting for it to happen again. There are many reports of this exact problem online from 2025 Acura RDX owners. We need an immediate recall.Read more

My 2019 Nissan Leaf SV is under recall R24B2 for battery overheating risk during DC fast charging. Nissan has instructed owners not to use Level 3 fast charging until a remedy is available, but no fix has been provided for almost one year with continued delay in repair promise dates. Yesterday, I was unable to use a fast charger as needed, which forced me to wait several hours at a slower Level 2
... charging station. This delay made it impossible to drive my Leaf for a planned Labor Day weekend trip to Amelia Island. Instead, we had to use my daughter’s car, since the Leaf could not meet our travel needs without fast charging. This ongoing situation shows significant loss of use and hardship. Fast charging is an essential function of an electric vehicle, and without it the car cannot perform as originally marketed. The prolonged lack of remedy (nearly a year since the recall was issued) is unacceptable and has left my vehicle restricted and unreliable. I request NHTSA take action to ensure Nissan provides a timely remedy or interim solution for affected Leaf owners. Attached is letter also sent to Nissan.Read more

The specific component or system which has failed and or is malfunctioning is unknown due to the fact the manufacturer and dealer were unable to properly diagnose and resolve the issue. The vehicle continues to malfunction, as of 8/28/2025. Beginning [XXX], the vehicle's engine began to make a very loud, concerning noise. The vehicle was inspected by the Infiniti of Mobile Dealership, in Mobile
... , Alabama on 12/18/2023. The dealership's service technician diagnosis at the time was the vehicle's radiator fan was stuck wide opened due to a failed idpm and a radiator leak from a crack. The dealership installed a motor and fan assembly and a control unit ipdm. However, the noise persisted after the "repair" and in addition, the heater/ac was not working properly. The vehicle was brought back to the same dealership on 02/22/2024 for further diagnosis. No action was taken by the dealership to repair the issue. The noise and malfunctioning of the A/C/heater persisted and became worse over time and the vehicle was then taken to the Infiniti of Memphis dealership in Memphis TN om 7/2024. The noise was much louder and the A/C stopped working in 98 degree weather as the car overheated. The vehicle's A/C continued to suddenly go out while the vehicle was in motion in high temperatures between 95 to 100 degrees Fahrenheit, causing a health and safety concern. The dealership reviewed the history of the problem and could not properly diagnose the issue. The dealership claimed the vehicle was working exactly as it should be. The vehicle owner insisted it was not working properly and the Infiniti of Memphis Service Manager contacted the Infiniti of Mobile Service Manager and both agreed to replace the fan assembly once again. The replacement failed to repair the problem. Both dealerships stated there was nothing further it could do. The OnStar Telematics Module has malfunctioned and is causing SAFETY FEATURES to malfunction. It is causing the following to occur to the vehicle: 1. The faulty module is causing the car to shut OFF UNEXPECTEDLY MID DRIVE. Losing control of the vehicle instantly. No steering, no gas, no brake, no gear change. THIS can cause a CATASTROPHIC ACCIDENT at higher speeds on the highways. The battery is
... completely drained when technicians inspect it but the alternator is working properly. 2. Causes the vehicle’s safety features to malfunction and give alerts such as “Service Driver Assist”, “Lane Departure Unavailable”, “Forward Collision Assist Unavailable”, “Steering Control Unavailable Soon”. 3. On April 30, 2024, while sitting at a traffic light waiting to turn, my Honda Pilot experienced a catastrophic engine failure. The driver heard a loud knocking sound and the engine failed. The driver was left to navigate to a safe area through traffic. The vehicle was towed to a nearby auto mechanic for evaluation. It was determined that a connecting rod had penetrated the engine block and that en

The vehicle was towed again to another auto mechanic, where the engine was removed and replaced with a used engine of the same make and model with other required support equipment (timing belt, water pump, miscellaneous belts and hardware) at a cost of $8656.00 Given the passage of approximately 15 months since the engine was replaced, the catastrophically failed engine is no longer available to be inspected. Thankfully the vehicle was stationary when the failure occurred or the driver may have been at greater risk. He did need to walk through a busy intersection to get to safety. The engine failure and replacement was performed and documented by an independent service center. The failed engine was not inspected by the manufacturer, police, or insurance carrier’s representatives. Only observed by the two independent service centers. Summary: Unintended full brake engagement at highway speeds caused by electronic parking brake/Auto Hold button defect. Extreme risk of collision. Details: On [XXX], my wife was driving our 2023 Kia Forte GT Line when the Auto Hold and electronic parking brake system malfunctioned. The system fully engaged the brakes without warning while traveling at highway speed, scraping the brake rotors and
... creating an immediate risk of being rear-ended. This occurred multiple times. The following day ([XXX], the brake pedal would not depress at all, rendering the vehicle undrivable. The dealership (McFarland Kia, North Hampton, NH) diagnosed “a sticky substance” inside the electronic parking brake button, located next to the cupholder. There have never been any liquid spills in this vehicle. The more likely cause is a combination of factory lubrication and normal dust/debris accumulation over time. If such a small amount of dust or residue can trigger unintended full braking while driving, it is a dangerous design flaw that poses a life-threatening hazard to drivers and passengers. The vehicle was repaired after I paid $284.99 out-of-pocket, but I am pursuing reimbursement through my Zurich Vehicle Service Contract. I have also requested an independent third-party brake and rotor inspection, as this defect may have caused further damage. Why this is a safety issue: Sudden, uncommanded braking at highway speeds can cause high-speed rear-end collisions. The defect can occur without visible warning and without any driver action. The defect can be triggered by minimal non-liquid debris, which is unavoidable over the life of a vehicle. I am requesting NHTSA investigate whether this is an isolated case or part of a larger design defect in the Kia Forte’s electronic parking brake/Auto Hold system. This condition should not be possible in a vehicle that meets federal safety standards. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I purchased my vehicle in 2020. In 2022 I experienced an issue where my car stopped in the middle of the road without warning. Despite several attempts to try and move the vehicle off of the road the police were notified and my vehicle was towed. The vehicle gave an error code that conditions not met for driving. One office tried to put the vehicle in neutral but the wheels locked and the shifter
... was locked as well. The battery has been replaced 3 different times since I have owned the vehicle. A mechanic friend of mine (who is a trainer for GMC) hooked his computer up to it and found no issues. He did find a loose wire which he the reconnected. I was able to drive my car for 6 months before the same thing happened, same situation the vehicle just stopped in the middle of the road only this time I couldn't turn the vehicle off, we had to manually disconnect the battery and again the police came out due to traffic hold up. My car was towed twice afterwards as I could get the car running for a few minutes before another error. To date my car has been sitting in my driveway since March of this year, this time it won't hold a charge and will not even start. Im not sure if this is an engine or an electrical issue. I did call 2 Buick dealerships that told me that it would take about 5 hours to diagnose the issue where I would be charged per hour at the mechanic hourly rate. At this point I think my vehicle needs to be recalled, even if I could get it repaired I dont feel safe driving it. Thankfully it stopped on a minor freeway and no major incidents occurred.Read more
