After evaluation by 4 mechanics, all reported the cylinder had lost compression and that one of the intake valves on the cylinder were likely the problem. Ultimately, a Ford dealership was the one who performed the engine replacement and also agreed it was likely an intake valve. I am reporting this after seeing numerous intake valve recalls for Ford engines of similar years since my event occurred. 2022 Tucson (NX4, Alabama built) 2.5L Engine experienced a catastrophic failure leading to Hyundai dealer diagnosis that "Cylinder #3 is not contributing properly," which requires a full engine replacement. The vehicle is currently located at home and is available for inspection upon request. The engine scuffing has resulted in a severe loss of power contribution from Cylinder #3, leading to pote
... ntial for sudden engine stall or complete loss of motive power while driving. If the engine seizes or stalls at highway speeds or during a critical maneuver (e.g., merging onto a freeway or turning across traffic), it creates an unreasonable risk of a rear-end collision or a catastrophic accident, endangering both the driver and others on the road. Furthermore, the internal mechanical failure creates a significant FIRE RISK due to potential oil leakage onto hot exhaust components. The problem has been confirmed by Hyundai Service Center in El Paso, Texas and three independent service centers in Arizona and Texas. All four assessments concluded that the vehicle requires a full engine replacement due to severe internal damage. Inspected by Hyundai Service Center in El Paso, TX. Despite confirming failure, Hyundai refuses to provide critical diagnostic data (Power Balance Test, Endoscopic Inspection, DTCs) and the determined root cause. This refusal hinders warranty transparency. The Check Engine Light (CEL) did not illuminate. Symptoms appeared late September 2025 at approximately 88,000 miles (audible engine noise/scuffing, rough idle, significant power loss). This failure at non-typical mileage demonstrates a severe internal defect and a safety system malfunction (failure of the ECU/CEL to adequately warn the driver). Note: Dashcam video documentation of the engine failure symptoms (audible noise and power loss) is available and can be presented to investigators upon request.Read more

I then contacted an Acura dealership by phone and was advised that I may be required to replace the entire rear hatch, rather than just the glas
... s. I have since learned that Technical Service Bulletin (TSB) 22-014 acknowledges an issue affecting rear-glass failure, but it is limited to certain 2019–2020 models and VIN ranges. No recall has been issued, despite numerous reports of this same defect. Additionally, there is an ongoing class-action lawsuit related to this issue (Tyzcko & Zavareei LLP). I am submitting this complaint because Acura is refusing to repair or replace the shattered rear glass despite its being a known design defect. This failure represents a serious safety hazard, as occupants seated in the rear of the vehicle could be severely injured if the glass were to shatter while in motion. I believe Acura must be held responsible and required to issue a recall, as well as provide repair coverage, especially for vehicles now out of warranty. I respectfully request that NHTSA investigate this issue due to the clear safety concerns and growing number of similar incidents.Read more

In 2022, my 2019 Acura TLX A-Spec received a warranty engine replacement due to a spun bearing. At that time, recall 23V-751 (connecting rod bearing defect) did not yet exist. Recently, the replacement engine developed a pronounced knocking/ticking noise at idle and during light acceleration. Multiple independent mechanics and an Acura service department confirmed the noise is abnormal and consist
... ent with connecting rod bearing wear as described in the recall. Acura advised that my VIN is not included in the recall because the original engine was replaced. However, the replacement engine was supplied by Acura prior to the recall and may contain the same defective components. The dealership and Acura corporate did not inspect the internal components or verify whether the replacement engine used the revised bearing parts — they only referenced the VIN and engine number to deny recall eligibility. I currently have an open case with Acura, and the vehicle is scheduled for diagnostic inspection. I was informed that if the issue is determined to be related to recall 23V-751, diagnostic costs may be reimbursed, but there is no guarantee. This situation raises concern because my symptoms, timeline, and component failure are consistent with the known defect, yet my vehicle remains excluded from coverage. Given the safety implications of possible engine seizure or loss of power while driving, I am requesting that NHTSA review whether vehicles with Acura-supplied replacement engines should also fall under recall 23V-751. All service records and warranty history with Acura are available for review upon request.Read more

Shortly after purchasing my 2022 Tesla Model 3 RWD in February 2025, I began noticing a persistent musty, mold-like odor coming from the HVAC vents whenever the air conditioning or defrost was on. The odor worsens when driving with the windows down. I reported this issue to Tesla multiple times between May and October 2025. Tesla replaced the cabin filters but refused to perform a full evaporator
... or HVAC disinfection, stating the odor was “normal.” Despite the filter changes, the smell always returns within a few weeks. Between these visits, I also tried to resolve the issue myself by replacing filters, using odor-eliminating products, and placing moisture absorbers inside the vehicle, but nothing has resolved the problem. This ongoing issue has led to repeated respiratory and eye irritation, including a documented pneumonia case in April 2025. The symptoms clearly worsen after time spent in the car. Tesla has not offered any permanent fix, warranty repair, or replacement. The problem appears to stem from mold or microbial buildup within the HVAC evaporator and duct system, which could affect air quality and occupant health. This raises safety concerns regarding possible HVAC design flaws or moisture retention issues that may affect other Tesla vehicles. The issue is ongoing as of November 2025 and has been reported to the Florida Department of Agriculture and Consumer Services (FDACS Case #2511-00831) for further review.Read more
