#Issue Description I experienced an issue with Tesla’s Full Self-Driving (FSD) system when a trailer hitch was attached to my vehicle. From what I observed, the FSD system appeared to detect the trailer hitch as an object in close proximity to the vehicle. This caused the vehicle to accelerate rapidly to the FSD maximum speed of 85 MPH, regardless of the road conditions or posted speed limits.
... This issue occurred when the lane markings were visible, but the hitch obstructed the rear camera’s view. On the FSD display, this obstruction was represented as a persistent “cloud” following the vehicle. I attempted to adjust the maximum speed limit manually to mitigate the acceleration, but this did not prevent the behavior. Each time I engaged FSD, the car would accelerate rapidly, even in situations where such high speeds were not appropriate for the road conditions. # Steps Taken to Resolve the Issue I covered the rear camera, which allowed FSD to function normally and prevented the rapid acceleration. The “cloud” visualization on the FSD display also disappeared once the camera was covered. # Trailer Mode Tesla offers a “Trailer Mode,” but I did not enable it since I was only using a hitch without towing a trailer. Problem #5 described exactly what had happened to me: 'ONE RECURRING PROBLEM THIS CAR HAS IS THAT THE SYSTEM MALFUNCTIONS SEVERELY. THIS TYPICALLY HAPPENES WHEN THE DRIVER IS TRYING TO PARK THE VEHICLE. REPORTS PAINT THE SAME PICTURES REPEATEDLY: THE DRIVER IS PULLING INTO A PARKING LOT, THEY PUSH THE BREAK PEDAL, AND INSTEAD OF STOPPING, THE CAR ACELERATES RAPIDLY, WHICH RESULTS IN A CRASH." Based on this article, it seems that Lexus should have issued a recall, but no! 5. The strange thing was that NO airbags were exploded/activated, yet the vehicle was damaged to beyond repair, it makes me wonder if there was an issue with the airbags too. If this incident had occurred at an intersection, I dare not imagine the outcome...I could have been killed or critically injured along with other innocent people on the road. Dealer was unable to find the source of the problem but reported that the car was working fine and gave it back to me. The second time was a 10/1/2024. I was able to drive slowly to a parking lot, turn the car off, wait a bit and turn it back on which seemed to reset the warnings. Car is currently at a different dealer and they tell me that unless it happens while they are in the car, they cannot diagnose anything. If this were to happen while driving, it could be very dangerous as the Hummer is a very large heavy vehicle so to lose driving systems in motion is quite a scary thing. Having it happen while stopped is bad since the car doesn't operate normally, steering is erratic and the suspension controls non-existent which impacts safety even with speed limited to 36. Cluster behind the steering wheel was blank, has happen twice within the last 30 days. This is an issue since I had no speedometer and had to use WAZE to guess how fast I was going. Sometime later during the drive a cruise control error message appeared on the dashboard. Subsequently the problem was identified as a faulty cruise control switch (part number 5G0-959-442-AE-ICX). The switch was replaced and the fault has not reoccurred over a few hundred miles of driving. While this malfunction can be overridden using the normal cruise control disconnect procedures, uncommanded acceleration poses a safety risk to driver and other motorists or pedestrians.Read more

Truck continued to downshift, so I went into Gus dealership and they explained the recall fix. I was told there are no recall on transmission and eventually Transmission would streamline. AT 156,000 miles- Since reprograming truck has been double downshifting/bucking in first gear, which I have been stating to Ford dealership since purchase IS A SAFETY CONCERN. To date NO LIGHTS ON DASH DISPLAYS OF Transmission, but BUCKING/DOWNSHIFTING CONTINUES (A SAFTY CONCERN). 05/17/24, I called Ford and stated my concerns. I received an engine failure warning message when the vehicle stalled on me. The dealership looked at the engine and stated that there is lose debris in the engine. The dealership has stated that they are unable to give me a diagnoses letter or a repair order at this time. Once they repair is made, they can give me something in writing. This issue appears to be similar Safety Recall 24TA07. However, Toyota is stating that my vehicle does not fall under 24TA07. The cruise control had again engaged to 15mph and was green signifying it was active and accelerating to catch up to the 15mph. This occurs immediately when shifting from reverse to drive. The only common factor I can isolate to is when backing, if I simply shift to drive after slowing to a near stop, without braking, it engages. While using Traffic Aware Cruise Control the vehicle would rapidly decelerate with no warnings of potential collision and with no actual emergency situation existing on the road. In a single trip that spanned both day and night there were three rapid declaration events occurring both when light or dark outside. The dealer kept the car overnight stating that they were too busy on 10/14/24 and promised me that they will provide alternate vehicle on 10/15/24 morning as per my phone call. I went to the dealership on the 15th at 8:50 AM to pick up the alternate vehicle, the service advisor said I can take my vehicle since he had changed the battery and it was safe to drive and did not provide me the invoice for the replaced battery. I asked the service advisor for the diagnostic report and he informed me that everything was okay in the report. I insisted on taking the print out of the diagnostic report and had to wait to collect it. Seeing the time on the report I am able to understand that they ran the diagnostic report while I was waiting. After driving my vehicle from the dealership and not having new battery invoice in my hand I called Honda motor company customer service and narrated my dealership visit details along with being uncomfortable on the safety of the vehicle. I didn't get any help from them as well. They created a case number for me. They said that someone will reach out to me and no one has till date. I have experienced the engine surging while driving 2 times in the last 3 months. 10/18/2024. The RPMs slowly increase regardless of taking my foot off the gas pedal, slowing, or coming to a stop. A bump or vibration in the road can cause it to engage or disengage. Ram dealer service dept indicates Hydraulic Control Unit needs to be replaced. ... parking lines on its own to a small hill continuously. My wife tried to press the brake multiple times with no success and the motor continued accelerating on its own. My wife turned the steering wheel to the right to avoid running into the hill or flipping the vehicle over. Once turned, the vehicle continued accelerating crossing over a neighborhood street and finally crashing into a neighbor's wall. Once it crashed the wall, all air bags were deployed and the vehicle rebounded back into the street where my wife was eventually able to shift it into parking. Once parked, the vehicle wouldn't start or move at all. Thankfully, there were no fatalities but my wife crashed into a wall and had to be taken to the hospital to be treated for the bruises in her legs and body from the air bag impact. Additionally, our community is gated and thankfully there is little traffic but this problem in a main road or highway could easily injure or kill people. We reported the accident to the local police and filed a report with them. We also notified insurance of the crash and have started a repair claim but the vehicle has not been inspected for any damages or problems. We have video recording of the incident which we will use for insurance and can provide as part of this complaint.Read more

Vehicle speeds up and breaks uncontrollably. When stopped it displays several codes on the dashboard 1) power steering fail, increased effort, driving permitted, 2) GRA/ACC fault driving permitted 3) faulty windshield wipers No fault codes are found with a code reader.

Everytime I start my car in the cold I get all these warning messages on my dash, in this case it just let me choose 2 options but I also get Auto idle stop problem, mitagation problem etc. I took it to the dealer they dont know what could be the proble. Sporadically, when in snow mode, the car refuses to accelerate at all. Moderate pressing of the gas pedal or completely flooring it, the car will not respond and accelerate. I was backing out of an outdoor shopping center slot located at the end by the curb. I had to attempt this three times due to traffic and another car that was also trying to back out. I tried to brake but was flying over 2 curb hit the road and flew over 2 more curbs with plants in the middle. The braking did not stop the vehicle. Coming up over the last curb the vehicle hit a parked car sending backward and it hit 2 more vehicles. No one was injured, since the vehicles were empty. The police did come and wrote an incident report, but not a police report because in California since it was property only they do not generate a report. However, this could have been fatal since people were waking about the parking lot. There were not any warning lights or messages. It has not as yet been inspected by by anyone . The car is available for inspection at a collision center. I am not sure which component failed and why it accelerated, or did not respond to the brake, or deploy the air bag upon impact.Read more
