My 2025 Ford Ranger Lariat FX4 has experienced sudden and complete loss of accelerator pedal response while driving at low speed on at least 4 to 6 separate occasions. The engine continues to run normally, but pressing the accelerator produces no power and no vehicle response. The condition has consistently occurred while pulling into my subdivision, which has an inclined entrance. The truck also
... feels noticeably sluggish nearly every time I make this turn, even when full dropout does not occur. No warning lights, check engine light, or messages in the instrument cluster illuminate during these events. No diagnostic trouble codes have been stored that I am aware of. The vehicle is available for inspection upon request. The symptoms closely match two active Ford recalls — 26S10 (EGR valve failure causing unexpected loss of drive power, stated by Ford to be most likely at speeds under 13 mph) and 25V-239 (PCM reset causing temporary loss of acceleration). My vehicle is not currently listed as included in either recall per Ford’s VIN lookup. Numerous owner reports on public Ford Ranger forums describe identical symptoms on 2024 and 2025 Ranger trucks, many of which are also not covered by the existing recalls and for which dealers have been unable to find stored codes. A sudden, unannounced loss of drive power at low speed on an incline or while turning into traffic represents a clear crash risk. I am requesting that NHTSA investigate whether the affected vehicle population should be expanded.Read more

I am writing to report a critical safety failure regarding my vehicle’s display systems. While driving, the left side of the infotainment system (the digital instrument cluster) went completely dark. I lost all visibility of my speedometer, odometer, and critical vehicle warnings. I immediately pulled over and attempted to power-cycle the vehicle, but the display remained unresponsive. Shortly a
... fter, the right side of the infotainment system also failed, resulting in a total loss of control over the HVAC (AC), radio, and integrated garage door opener. This is a recurring issue documented by other owners of this model, and given the loss of the speedometer while in motion, I consider the vehicle unsafe to drive until repaired.Read more

The vehicle has 57000 miles and i am reporting a commonly known issue with the transmission. Despite multiple service appointments with Ford explaining the issue, I was informed there was nothing wrong. Now, 10 months post extended warranty, Ford is telling me it needs a full transmission replacement. Thoroughout, the vehicle shifted eradically up and down, including on the interstate, quickly dro

Sometimes, at low speed or at a stoplight, the vehicle would disengage the drive train and roll in neutral while the RPMs revved when the throttle was pressed. The vehicle was not operating safely, would roughly bounce around gears, when reported to Ford, told that there was nothing wrong and now being told post warranty that the transmission is a total fail that will require replacement. The latest service appointment one month ago, where I was again told that an oil topoff was all that was needed. This is a well reported safety issue by numerous owners and Ford has failed to take any action or initiate a recall.Read more

For approximately 3 years, this vehicle has experienced repeated unintended transmission downshifts at freeway speeds without warning or driver input. The vehicle can suddenly and violently downshift at highway speed, creating a serious risk of loss of control, skidding, rear impact, or crash. This defect appears substantially similar to Ford recall 26S28 / NHTSA campaign 26V237, which covers cer
... tain 2015-2017 Ford F-150 vehicles for unexpected downshifts caused by loss of signal between the transmission range sensor and the powertrain control module. Ford reported that this condition can cause the transmission to unexpectedly downshift and create a loss-of-control hazard. (NHTSA) My vehicle appears to use the same Ford-based engine, transmission, and control architecture family and is exhibiting the same dangerous fault pattern. Diagnostic scans show PCM code P0720 and P0722. Freeze-frame data captured the transmission commanded in 3rd gear with turbine shaft speed present at 1041 rpm, but output shaft speed at 0 rpm. This indicates loss of the output shaft speed signal while driving. The transmission tag reads CK4P-7000-FB. The likely fault area is the output shaft speed sensor circuit, internal transmission lead frame, internal harness, connector, or related transmission signal path. We contacted Mobility Ventures, but they have not meaningfully responded or helped resolve the defect. We also contacted a local Ford dealer, and they stated they do not know how to handle or repair this defect on this vehicle. Based on the same dangerous symptom, same Ford-origin driveline family, and same sensor-signal failure pattern, this vehicle should be evaluated for inclusion in, or equivalent treatment consistent with, Ford recall 26S28 / NHTSA 26V237.Read more

On April 17 2026 while driving my 2022 Acura MDX at very low speed in a parking lot, the vehicle suddenly accelerated forward without warning and surged into bushes before I could stop it. I did not mistake the gas for the brake pedal. This felt like a sudden unintended acceleration event. I was preparing to park in a stall and the vehicle lunged forward unexpectedly. I applied the brake, but t
... he braking did not immediately stop the forward surge. Earlier, I experienced another unusual event in which the vehicle abruptly auto-braked, jerked, and made a warning sound while I was slowing for a red light. I noticed the car felt as if it was surging a bit on residential streets and was about to mention it during the next service. This incident has caused me to lose confidence in the safety of the vehicle. I contacted Niello Acura who has been very cooperative. I was told the issue would need to be duplicated or that they could only inspect it if a code had been stored once the car is repaired. The car is currently being repaired with close to 7,000 in front end damage. I am reporting this because a sudden unintended acceleration presents a serious safety risk and could easily have caused a more severe crash or injury. I did research on this issue and have found various reports of unintended accelerations in the Acura MDX forums from various years but was unable to find any recalls or outcomes with these reports My concern is this will be dismissed as "operator error" or a phantom incident that cannot be reproduced.Read more

While taking off in my 2020 Toyota C-HR on the highway, the CVT transmission suddenly failed, resulting in an immediate and significant loss of propulsion power. The vehicle did not fully shut off but lost the ability to maintain normal highway speed, requiring me to rapidly reduce speed and carefully maneuver the vehicle to safety in active traffic conditions. This created a serious safety risk
... due to unexpected loss of acceleration and potential inability to maintain highway speed in traffic. The failure occurred at approximately 117,700 miles and shortly after purchase. The vehicle had a documented history of regular maintenance performed at Toyota dealerships approximately every 3,000 miles. I was later informed by Toyota service personnel that the vehicle had previously been documented on 2/2/26 in the manufacturer’s internal system with a CVT fault and a recommendation for transmission replacement prior to my purchase, which was not disclosed in any public vehicle history reporting system. This raises a major safety concern that the transmission failure may be part of a broader pattern of CVT reliability issues affecting this model and drivetrain family. I am submitting this complaint due to the sudden loss of propulsion at highway speed, which presents a potential safety hazard to drivers and surrounding traffic. And deceptive practices of undisclosed known safety issues by Toyota of USA.Read more

I purchased this car new on December 31, 2024. I have owned it for less than 16 months, and in that time, we have had an issue occur at least 5 times where we get an alert on the screen that says, "Stop Safely Vehicle Will Shut Off Soon". Less than 20 seconds later, the car completely dies. On each of the 5 instances, we were lucky to be going relatively slow and were in a safe place with a shou

We brought this to the attention of Herb Chambers Chrysler Dodge Jeep RAM FIAT of Danvers (not where we bought the car, but the closest Chrysler dealership for maintenance) and they were unable to provide any help. They said they have never seen this, and since they couldn't get any codes, they didn't know what was going on. I own a Hyundai Elantra with approximately 103,000 miles. The vehicle has developed an intermittent issue where the engine will suddenly stall while driving. This most often occurs when I am decelerating and turning, such as when entering or navigating roundabouts. At the time of the stall, multiple warning indicators appear simultaneously, including the check engine light, battery warning light,
... and a “no key detected” message. There are no prior warning signs before the failure occurs—no unusual noises, no loss of power leading up to the event, and no dashboard warnings in advance. The stall is sudden and unexpected. When the vehicle stalls, I must quickly shift into park and restart the engine. The car restarts immediately without hesitation, and all warning lights disappear. The vehicle then operates normally again until the issue randomly reoccurs. This has happened multiple times. This issue creates a safety risk because the vehicle loses power unexpectedly while in motion, particularly during low-speed turns and in roundabouts, which could increase the risk of a collision due to sudden loss of propulsion and reduced control. The problem has not yet been formally diagnosed by a dealer or independent repair shop, but based on research and similar reported cases, it may be related to an intermittent engine or electrical signal failure. The issue has not yet been inspected by the manufacturer, police, or insurance representatives. UNKNOWN as to the exact failed component, but the failure appears to involve a sudden loss of engine operation without warning, followed by immediate recovery upon restart.Read more

I am writing to open a formal complaint regarding a serious transmission defect in my 2019 Honda Odyssey (VIN: [XXX] ). American Honda Motor Co.), which allege that the 9-speed ZF 9HP transmission in my model year suffers from a design defect. These transmission failures create a significant safety hazard. Recently my van report code as transmission system problem the second time. We got mechanic checked and confirmed it is transmission issue. The first time when it happened, we drove the van on highway and suddenly it lose power and slow down by itself. there was no response even we applied gas. We went down highway and drove to a close-by repair shop. they replaced some part for us but it didn't resolve the issue. This time since the error comes, it won't speed up in lower speed except the speed goes beyond 40-50 miles plus. when we back the car, it responses slow or none. it comes with big noise also now. it definitely gets worst. The very common issue when you stop and start to move, it never responses from the beginning even we applied gas, even the passenger can feel the reluctance. it also happens when the van turns. we also had the experience when driving on the highway, it speeded automatically without driver applied the gas. While traveling at normal highway speeds (55 MPH), the vehicle's battery capacity rapidly dropped from 80-90% down to 10% in less than 30 seconds, resulting in activation of the vehicle's "turtle mode" and power throttling, which made it impossible to maintain posted highway speed of 55 MPH. A warning appeared on the dashboard, which stated, "Warning Power Reduced". The vehicle traveled at approxi
... mately 40 MPH creating a serious hazard to others on the roadway who were traveling at the posted speed limit of 55 MPH. Upon pulling over to the side of the freeway, the battery charge rebounded back up to 75%, but maintained power throttle, making it unsafe to reenter the roadway, as the vehicle was incapable of accelerating to the posted speed limit of 55 MPH. It was extremely slow to accelerate at all, making it from 0 - 40 in about a minute, which again, was extremely hazardous to others on the roadway. Nissan has since acknowledged that this was due to a factory defect, and covered under their factory warranty. The issue is regarding the “Traffic Sign Assist” (Owner’s Manual page 221) feature of the Adaptive Cruise Control System. Vehicle will be made available if requested. The function of the "Traffic Sign Assist" is to monitor roadside "Speed Limit" signs and the cruise control will increase or decrease speed to stay within the permissible legal speed limit. The system worked except occasion
... ally it would change the indicated permissible speed limit on the instrument display when there is no corresponding roadside speed limit sign and the permissible speed limit for that stretch of roadway is unchanged. When this happens, suddenly and abruptly, the vehicle display would indicate a "phantom" Higher or Lower Speed Limit sign and rapidly and abruptly accelerate or decelerate. Again there were no roadside signs posted indicating a higher or lower speed limit! The risk is potential for rear end collisions from or to other traffic due to the abrupt acceleration or deceleration. when we returned home from a 4556 mile road trip, we took the vehicle to the Mercedes dealer for routine service and mentioned this to the Service Advisor. He indicated that when this defect is reported their solution is to deactivate the system. I don't feel this is adequate since another driver could reactivate the system through the settings. It has not been inspected by anyone except the dealer. There were no warning lamps, messages or other symptoms before the display changed and the vehicle abruptly accelerated or decelerated.Read more

My spouse related this has happened several times to her at various speed settings. Today, I was on the highway with it set at 60 MPH, the vehicle began accelerating and I noticed the set sp
... eed had changed to 75 MPH. I disengaged the cruise control and turned it off. We plan to notify Ford and take it to the dealership for evaluation.Read more

The car acceleration issues, taking a long time to go from 0-50 with the tachometer going up to 5 to do so. My MPG has gone from 33 to 28 mpg. There are times when I press on the gas but my car doesn’t move. It also makes loud “rumbling” or “whining” sounds at times.

I took car to dealer to get latest software update version 4.2.13 since my car hasnt been received updates OTA. After the update, the cars accelerating from a stand still had a lag of almost 1 to 2 seconds which is dangerous. I went online to find out several polestar 2 owners complaining about the same issue. My car has the performance upgrade which has a 0-60 time in 4 seconds. Now after thi
... s issue, car is hitting 0-60 in under 6.5 seconds. I was in a situation yesterday where i backed out of my driveway and saw a car behind me coming in very fast, I floored the acceleration pedal and the car did not move until 1.3 seconds. This is extremely unsafe and needs immediate attention. Before this software update I had no issues in acceleration and have owned the car for over 2 years. Please help.Read more

I was pulling into a parking spot and my vehicle suddenly lurch forward, and I had to slam on the brakes quickly, scraping the front of the curb. I checked there was no warning lights. I checked the floor mat, and this was not impeding the accelerator pedal I am certainly I did not accidentally hit the accelerator instead of the brake.

I own a 2019 Ram ProMaster. The vehicle previously received the manufacturer recall repair for the transmission park issue (software update). After the recall repair, the vehicle experienced a safety issue where it would roll freely while in “Park,” as if it were in neutral, even though the dashboard indicated “P.” This created a rollaway risk. This happened August 2025 intermittently. M
... iles on the vehicle 68k. I recently (4/17/2025) serviced the transmission (approximately 73,000 miles) and found significant internal damage evidence. The transmission fluid was heavily contaminated, and I found approximately 11 square metal fragments around 1/4 inch in size inside the transmission pan and filter. This amount and size of metal debris indicates internal transmission failure. The prior rollaway condition combined with internal debris suggests the recall repair did not fully resolve the defect. Although the vehicle is currently engaging Park, the condition was intermittent and may reoccur without warning. This presents an ongoing safety risk of unintended vehicle movement. I am concerned that the recall remedy (software update) is insufficient to address mechanical failure inside the transmission, and that other vehicles may have the same unresolved defect.Read more

This is not a failed component, the issue is a design interaction issue. This truck when it gets stuck as it unfortunately very easily does even on wet grass goes into a traction control mode where the throttle is VERY damped but the issue is not only is the opening very dampened the closing is also VERY damped. This is made significantly worse by the fact that traction control cannot actually
... be disabled. There is a switch and it indicates it is disabled but the functionality remains the same. This combination means whenever you're working to get the truck unstuck, like yesterday when it was on wet grass you have to expect that the vehicle will suddenly accelerate as soon as it finds traction. normally you can counteract this by expecting to apply significant brakes as soon as it finds traction but if the front wheels are still on a slippery surface the ABS system will lose it's mind and essentially disable brakes as the vehicle accelerates violently backwards with the new found traction. This combination of software decisions makes it so you can very easily replicate an unintended acceleration scenario where the vehicle jumps back a solid 20' when this happens, not respecting any brake input given by the driver and certainly not respecting that the throttle was released long ago. The throttle closing should not be damped when traction control is running and the ABS system should not be behaving the way it does. I understand getting the vehicle unstuck from wet grass is not normal operation but that's what the traction control disable button should be for and Ford makes that button useless also. When taken together, all these engineering decisions make for a very unsafe situation that has zero reason to exist. I've attached pictures that show the overview of the situation as well as the characteristic ABS marks showing that it was disabling brakes.Read more

The vehicle exhibits unintended acceleration behavior when the brake pedal is applied. Instead of slowing down, the engine revs and the vehicle does not decelerate as expected. On March 16, 2026, while merging onto a highway and applying the brakes, the engine suddenly revved to approximately 3,500 RPMs while the vehicle failed to slow down. There have been previous issues with the vehicle not sl
... owing when breaks are being applied, nut this is the first instance when it was noticed that the RPMs increased while breaking. This issue has also been reported to the dealership, with service documentation noting that when applying the brakes, the engine revs and the vehicle does not slow or accelerate properly. This creates a dangerous condition where braking input does not result in expected deceleration and may instead cause acceleration-related behavior. The issue has not been resolved. This issue significantly increases the risk of collision and loss of vehicle control.Read more

My car started getting hot, began to overheat. I was on my way home on freeway. Second lane from slow lane. My car began to loose power, I could no longer excelerate. There was a big rig coming up behind me he was not slowing down as he got closer began honking I was trying to flag him that I could not move, as I was trying it started to go like 5 miles per hour where i began slowly to get over,
... cars in the other lane slowed down. Big rig went around me and avoided any collision. Thank God. I slowly made my way home on the streets with heater blasting. I towed my car to the dealer. I was told, my head gasket needed to be replaced, more than likely I would need a knew engine. Dealership said it was because when taken my car in for service I did not bring to the dealership. Cashier told me that there have been several incidents as mine.To my surprise I googled 2019 Honda Accord engine, and there were many with the same issue same car same year. Yet no recalls! I was lucky not to have been killed or have caused a multiple car collision. Are we waiting for someone to be killed to recall these engines. I missed several days of work, borrowed cars and rented a car to get back and forth to work not to mention the tramatic experience of a big rig coming at you honking on a FREEWAY. A week prior to this incident, my lights on the dashboard came on. I did call the dealership to set up an appointment. At that point they were no signs of overheating. I have taken my care in faithfully for oil changes where all fluids are checked, i was not told not once that any fluids were ever low, last visit was approx 5 months ago. I have called to have them send me all receipts. However in hindsight I do not need theses records as it has been proven by other people they have had the same experience. We are not talking about a faulty air conditioner we are speaking of an engine, in my case power failure. In the even records of oil changes and dealership diagnosis is required. I can email.Read more

While driving on a two-lane highway at approximately 80–85 mph and attempting to pass another vehicle, the engine suddenly shut off without warning. The vehicle lost all propulsion, and steering assist was significantly reduced, creating a dangerous situation and nearly resulting in a head-on collision with oncoming traffic. Warning lights including check engine and low power steering illuminat
... ed immediately after the stall. The vehicle would not restart for several minutes and was temporarily disabled in the roadway. The vehicle was later inspected by a dealership, which confirmed through diagnostic data that multiple engine stall events occurred at highway speeds. The inspection also identified system-wide electrical voltage drops and multiple communication faults between control modules. The dealership attributed the issue to a loose battery terminal; however, the diagnostic report indicates prior voltage irregularities and multiple affected systems, suggesting a broader failure condition. This incident is consistent with an active safety recall affecting this vehicle, which states that the engine may stall during driving. The failure has not been reproduced since the initial incident, but the vehicle has documented evidence of multiple stall events and remains a safety concern. The vehicle is available for inspection upon request. Due to the sudden loss of power at highway speed and the inability to safely control or accelerate the vehicle, this issue poses a serious risk to the driver and others on the road.Read more

Adaptive cruise control fails nearly every time the car is driven more than 40 minutes with ACC activated. Failure indicated by warning lights and statement on DIC. DIC instructs operator to clean the front "H" emblem on grille, however, the emblem is always completely clean. The failure has been reported 4+ times to the dealership. Dealership reports the failure is a known problem and there i
... s no fix available from Honda. Upon system shut down the vehicle disengages cruise control and has rapid deceleration causing unsafe driving conditions while in traffic. System usually resets itself without cleaning or other remedies within 10-20 minutes after fail, and will usually fail again +/- 40 minutes later. ACC is marketed as part of the Driver safety system and is unreliable. first occurrence was at 670 miles since new. Car now has < 10k miles and has failed 12+ times.Read more

My 2020 Audi SQ5 (3.0L turbo V6, EA839 engine) experienced premature water pump failure at 64,361 miles. Coolant leaked into the vacuum system, damaging the vacuum unit and related components. Failed parts are available for inspection. Safety Risk: Loss of coolant circulation causes rapid overheating and risk of sudden engine failure at highway speeds, creating a collision hazard. Coolant intrusio
... n into the vacuum system can also impair brake booster function, compromising stopping ability. Confirmed by Dealer: Diagnosed and confirmed by Audi Reno Tahoe (Reno, NV) on April 9, 2026. Written estimate #XXX documents replacement of the water pump, pulley, valve, vacuum unit, hose, and tank — totaling $6,591.37 out of pocket. No recall or manufacturer assistance has been offered even though this is a known issue dating back to 2013. Warnings: On 4/9/2026, dealer reports that after a non related repair test drive the technician observed "Drive system malfunction ON during hard acceleration" as well as visible signs of coolant leakage. Prior to having the non-related service completed, I did not see any warning lights or receive a temperature warning. Known Defect — Recall Warranted: This failure is consistent with a documented defect across 2013–2022 Audi/VW 3.0T engines. A class action — XXX et al. v. Volkswagen Group of America, Case No. XXX— specifically names the 2020 SQ5. Despite known complaints, internal testing data, and dealer reports, no recall has been issued. I urge NHTSA to investigate and mandate a recall of all affected vehicles.Read more
