I am 100% certain that I did not depress the accelerator pedal to cause the car to go at that speed in reverse twice! We contacted General Motors which supposedly had their engineers look at the mechanicals of the car but the only information we received from them is that the car did accelerate both times at 99% of capacity. The air bags did not deploy so the crash computer did not activiate. GM either did not examine the soft ware on the car, or, if they did, that information was not shared with us. While driving on the highway at approximately 60MPH, both display screens went completely blank. The A/C stopped working and radio speakers also stopped working. After about 30 seconds, the system had reset and came back on as if it was the start up sequence. I could not tell if it affected steering/braking since I was on a relatively straight road and light traffic but it was highly disconcerting

When turning on to another road and accelerating, engine will suddenly loose all power. releasing accelerator will regain some power until vehicle speed increases. Throttle control is often erratic and transmission shifting is inconsistent resulting in hard clunks during shift. After googling around, this is a common problem on these 2018 ford ecoboost engines and Ford obviously knows there is a problem that could cause accidents or engine fires. In some instances the vehicle has labored up a hill and seemingly doesn't want to downshift and pick go faster. It seems that this issue has triggered a TSB from Kia, but the issue has potential for elevated risks for other drivers on the road. If the vehicle is unable to maintain freeway speeds or climbing speed, then it poses a risk to traffic on either side of the road and the potential need to pull over. It seems that the vehicle loses power or momentum and would make it very dangerous to get off the road or onto the shoulder. ... et a "Check Brake System" flash on my instrument panel cluster, also the light which illuminates for my parking brake turns on, and I lose power brakes, the brake pedal turned extremely stiff. I immediately look for a pull off zone, but because I had lost brakes I did not want to risk keeping the vehicle moving. I get the vehicle stopped on the side of the road, and instantly put it in park along with the parking brake and shut the vehicle off. The vehicle would not start or crank, just a flash across all of the electronics, the lights go dark, then illuminate again as if normal. This is an extremely dangerous situation, the vehicle had not notified me that the motor had stopped turning; not a check engine light or ring at all, and shortly after that the power brakes stop working. This is very dangerous for motorists because of the lack of notification from the vehicle, and the incredibly weak breaking ability. If the vehicle had been moving at highway speeds, any sort of traffic, I could've ended up injured or dead, and even worse, killed another driver. The vehicle was towed to the nearest town where it would be inspected by a dealership; they reported to have found coolant in the oil and that the vehicle needed a new motor. I took my 2021 diesel Jeep in for a Y81 Emissions recall on 02.19.24. Since then, my vehicle has had its check engine light go on SIX TIMES with a P0133 alert which refers to an O2 sensor. I have taken it in to the dealership for a resolution six times. Currently, it is set to be diagnosed and repaired yet one more time on 01.22.25. I tried to go through an arbitration with FCA USA LLC which produces the Jeep brand and it was denied for a repurchase of the vehicle because the arbitrator felt it was not a safety issue since I still drive the vehicle and they are technically not in violation of the warranty which I did not accept as a resolution. Due to my non-resolved safety concern, I am raising the issue in this forum so hopefully something can be done and others that potentially have the same issue can reach a resolution as well. My hypothesis of the cause of the issue is when the Y81 recall happened, it tuned down the emissions output that was originally higher necessary for the vehicle to run, because I didn't have check engine lights before. By this tune-down of emissions, the engine is not able to produce the correct output to run the vehicle and thus the random lack of power causing a safety issue, and is currently adversely affecting components that are causing the P0133 O2 sensor to alert.Read more

However, I was informed that they did not have the parts to fix the issue and had no idea when they would receive them. I asked GM if they would provide me with a rental vehicle, but they declined, stating that if they offered a rental to me, they would have to provide one for others as well, which was not feasible due to the large number of affected vehicles. I explained that my truck is my daily driver for work, and I frequently transport my grandchildren in it. Driving the truck was hazardous given the issues it was already exhibiting. I was told to contact my dealer, and since my truck was under warranty, they should provide me with a rental. When I requested to speak with a manager, I was informed that they would only reiterate the same information. I then contacted my dealer, Sapaugh GM Country, located at 1435 McNutt St., Herculaneum, MO. The contact stated that the brake pedal was depressed but the vehicle failed to respond, and the vehicle failed to stop as needed. There was no warning light illuminated. The contact stated that the air bags deployed as intended; however, a warning light indicating an air bag malfunction illuminated afterwards. The contact stated that the front driver's side and knee air bags had deployed as designed. The contact sustained pain and bruises on the left side of the chest, on the upper part of the left breast, on the left leg, and on the right wrist and right arm. The vehicle automatically called 9-1-1 for Emergency Assistance, but the contact declined to receive medical assistance. The contact was able to walk away from the scene. No Police report was filed. The contact was unsure whether there was any property damage. The vehicle was towed from the scene and totaled by the Insurance Company. The contact stated that the failure had occurred once before while driving at approximately 60-65 MPH, and the vehicle unintendedly accelerated with an abnormal sound coming from the vehicle, but the contact was able to depress the brake pedal and regained control of the vehicle. The vehicle was being driven on a public road, going roughly 45mph. I felt a very harsh studder/clunk and completely lost responsiveness from the accelerator. There was no throttle response even pumping the pedal all the way to the floor. I was able to coast to a parking area and make a safe stop. ... was the most severe instance of the problem, but both primary drivers of the car have experienced the accelerator going unresponsive at different times. Since this was a severe safety related concern, we took the car in for service at the dealer, but were told they were unable to reproduce the problem and did not make any repairs. We provided the dealer with a video we captured toward the end of the latest incident, but they haven't been able to find any cause. There were multiple warning lights on the dash, but no codes were stored. We did have another warning light for a brake system error, but there was no code stored and the dealer was unable to located the source of a voltage anomaly in the parking brake system. We've taken it in to another dealer service center for additional diagnostics but the original shop is refusing to provide the complete details of any diagnostics and corrections they have made, so we don't have the original data that was pulled from the car.Read more

A police report was taken at the scene and the vehicle was towed to the home. The cause of the failure was not yet determined. The vehicle was a total loss. Driving my Rivian R1S on a road trip with family from Seattle, WA to Ketchum, ID (about 600miles.) Stopped to recharge outside of Boise, ID for about 20mins. Started driving again and in about 5 minutes the car started warning (visual message and audible alarm). The following events then occurred: -- The warning message stated something along the line of "Pull over immediately the car will sh
... ut down imminently." -- I was driving in traffic and was able to make it through an intersection and saw a gas station in the distance. -- The power steering and braking began to go unresponsive and the car began slowing down as I drove through the intersection. I tried to guide the car to the gas station. -- While pulling in to the gas station the car could barely be controlled. Once dropping below 5mph or so it immediately shut itself down. -- The inability to effectively control the vehicle was extremely unsafe. If the intersection I had to pass through had a red light the car would likely have shut itself down and stranded our family at a very busy intersection. -- The car began flashing a ton of different warning messages. "Suspension needs to be serviced", "Front trunk unable to close please service", "Battery system overheating" are a few as an example. -- After calling customer service I reset the vehicle (using prescribed sequence of button pushes.) The car then went dark and nothing worked from there out. -- Called a tow truck and found hotel for family. Rivian was towed to the Boise service center. -- Rivian service center has been unable to diagnose or recreate the issue to date - they have been in possession of the vehicle for 17 days and counting.Read more
