I have a Maverick (24 AWD XLT) and this issue has occurred twice. It happened on my vehicle the first time as I was trying to turn left, up an incline, with oncoming traffic - loss of acceleration. There was simultaneously a service light and power train malfunction message that went to my ford app. The vehicle had 5k miles on it. I took it immediate to the dealer. After testing they said the conn
... ection at the accelerator was loose (pigtail) and replaced. I just had the same failure occur yesterday (~22k miles) on my way home from work, except I lost acceleration on an inclined on-ramp trying to merge onto the highway. I had just slowed on the ramp to let a semi pass, and am thankful I didn’t try to get ahead of it. It wouldn’t have ended well. Still, I could see traffic jamming behind me, and folks were struggling to get on the highway because of my significantly reduced speed. Losing control of acceleration at a point where it is critical is very dangerous. I’ve never experienced a vehicle failure that felt like it risked my life. For this to occur twice in key moments of acceleration, with less than 25k miles is unsettling. Online there are many similar reports, not sure how this isn’t a recall yet.Read more

The acceleration lags or stalls after braking to almost a complete stop. The lag is anywhere from 1 to maybe 3 seconds and I’ve almost gotten into multiple accidents as the vehicle has stopped in the middle of a lane or intersection when I’m trying to turn. The Honda dealership says they have been able to duplicate the issue, but said they don’t have a fix for the problem.

I was driving my 2018 Chevy Volt at about 30 mph on EV mode. The battery was at 50%, the car was performing normally. Then the battery jumped to 100%, the check engine light came on, and the vehicle lost power. I was in rush hour traffic and unable to get the vehicle to go faster than 5-10 mph. I switched to "normal" mode to engage the gasoline engine, hoping this would help but it did not
... . The gas engine RPM fluctuated wildly and did not restore the ability to accelerate the vehicle. I was still unable to go faster than 5-10 mph. I was a few blocks from home and managed to get home safely. The engine code was P1E00. I put the vehicle on the charger and it seemed fine the next day. This exact situation I ran into - reduced engine power - is addressed in Special Coverage N232432680, which includes this vehicles VIN. I took the car to the GM dealer today and they are telling me that the car is fine, they can find no problems. I mentioned that this issue is covered under Special Coverage N232432680, and was told that unless they can reproduce the issue in the shop, that they cannot apply Special Coverage N232432680.Read more

My 2019 Ram 1500 has had a failed ABS control module for several months, which disables critical safety features including ABS, traction control, and cruise control. While entering the interstate on an on-ramp during light rain and under normal driving conditions, the loss of traction control contributed to a sudden loss of stability, causing the rear of the truck to break loose and the vehicle to
... spin. No other vehicles were involved, but the situation created a serious safety hazard and could have resulted in a crash. The vehicle continues to operate without these safety systems, significantly impairing safe handling and braking. I have been informed that the required replacement module is on extended backorder with no estimated availability, leaving the vehicle in an unsafe condition for an extended period of time.Read more

My 2024 Ram 1500 TRX has recurring safety-related failures of the infotainment system affecting both Uconnect 5 (12” screen) and earlier Uconnect 4 units. These issues are widespread in 2021–2024 Ram TRX and Ram 1500 models and began or worsened after forced over-the-air updates. The primary defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses
... all functions controlled through the display, including HVAC/defrost, backup camera, vehicle settings, seat controls, and performance gauges. This creates an immediate safety hazard: I cannot adjust climate/defrost, cannot see the required rear-view camera when shifting to reverse, and cannot access key vehicle information. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including: Cameras not working, screen freezing or blacks out, navigation failing, carplay unstable won’t stay connected or randomly drops and more. Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

All spark plugs fouled again; all spark plugs replaced. Valve cover gasket was leaking, so that was replaced as well. Yup, I paid for the repairs out-of-pocket. Episode Four: May 02, 2025. Vehicle goes into limp mode again, with CHECK ENGINE LIGHT turning on. Another spark plug fouled up, so it was replaced. Ignition coil replaced as well. Episode Five: June 19, 2025. The primary failure is the engine assembly and turbocharger, specifically caused by the failure of the oil pump drive belt and tensioner system (subject of Safety Recall 23S64 / NHTSA 23V-905). Despite an attempted recall "remedy" on August 25, 2025, the engine suffered internal oil starvation due to belt debris (rubber fibers) remaining in the oil passages. The dealership has confirmed in writing
... that the turbocharger failed and the engine requires a "long block replacement" due to "debris in the oil passages" and a "clogged turbo oil feed filter." The vehicle is currently held at dealership, and the failed components are available for inspection. The vehicle experienced a sudden loss of motive power while in operation, a condition specifically cited in the safety risk section of Recall 23S64. This loss of power occurred during acceleration, creating an immediate risk of a rear-end collision or being stranded in active traffic. Additionally, per the recall notice, this failure path can lead to a loss of the mechanical vacuum pump, which provides power braking assist. A sudden increase in braking effort combined with a stalled engine significantly increases the risk of a crash. The problem has been confirmed by the dealership. On January 5, 2026, the service department diagnosed a failed turbocharger then on April 17th 2026, the service department diagnosed the problem to be a internal engine debris clogging oil feed lines. They verified the engine has oil pressure at the pump but is failing to deliver oil to critical components (turbo/upper block) due to contamination from the defective belt system. The vehicle has been inspected by the authorized Ford manufacturer representative at the dealership. I have also escalated the case to Ford Customer Relationship Center, though they have yet to provide a $0-cost safety remedy as required by federal law. Concern: The 2024 BMW 530i is equipped with "Dynamic Cruise Control" and a "Speed Limit Assistant." The system is designed to automatically adopt posted speed limits and adjust the cruise control set speed. However, because this specific trim lacks a radar-based distance-monitoring sensor (Active Cruise Control), the vehicle initiates aggressive, unintended acceleration toward slower-moving traff
... ic or stopped obstacles whenever it detects a higher speed limit sign. Safety Risk: While the vehicle is using its cameras to "see" and adopt an external speed command, it remains "blind" to the actual traffic directly in its path. This creates a high risk of rear-end collisions, as the car will prioritize reaching the new speed limit even if it means accelerating directly into the bumper of a lead vehicle. Proposed Solution: A "Dynamic" cruise control system that has the authority to control acceleration based on external data MUST also have the authority to maintain a safe following distance. I propose that BMW should be required to include distance-monitoring features in all cruise control systems that utilize automated speed adoption. If the hardware for distance-keeping is not present, the car should not be permitted to automatically increase speed based on roadside signs. Inspection Status: The vehicle is available for inspection. No warning lamps appeared prior to these incidents, as the system is operating exactly as programmed by the manufacturer, which highlights a fundamental logic and design failure.Read more

To Whom It May Concern at NHTSA, I am filing this safety complaint regarding a defective ABS Hydraulic Control Unit (HCU) on my 2017 Ram 1500 Rebel 4x4 5.7L Hemi, VIN: [XXX] , with approximately 88,000 miles. DEFECT DESCRIPTION: The ABS HCU has failed, resulting in complete loss of anti-lock braking, traction control, electronic stability control, and cruise control. This is an identical failure
... mode to the defect addressed under NHTSA Recall 24V-896, which covered 2017-2018 Ram 2500-5500 trucks. My 2017 Ram 1500 uses the same or substantially similar HCU component and is experiencing the identical failure. SAFETY RISK: The loss of ABS, ESC, and traction control significantly increases the risk of a crash, particularly in adverse weather conditions. This is not a minor inconvenience — it is a serious safety hazard that FCA/Stellantis has already acknowledged in heavier-duty Ram platforms. MANUFACTURER KNOWLEDGE: FCA was aware of this defect as early as 2016 based on warranty claims and customer complaints. Despite issuing Recall 24V-896 for heavier Ram trucks with the same HCU failure, FCA has failed to extend recall coverage to the 2017 Ram 1500, leaving thousands of owners without remedy. FINANCIAL IMPACT: Repair estimates range from $1,200 to $2,000. The part (OEM #XXX) remains on backorder or engineering hold, making timely repair impossible through normal channels. REQUESTED ACTION: I respectfully request that NHTSA: 1. Open a formal investigation into the 2017 Ram 1500 ABS HCU failure 2. Compel FCA/Stellantis to expand Recall 24V-896 or issue a new recall to include the 2017 Ram 1500 3. Require FCA to reimburse owners who have already paid for this repair out of pocket I am happy to provide any additional documentation, repair invoices, or diagnostic reports upon request. This failure puts safety at risk by silently disabling AEB and forward collision warning during normal driving — systems a driver may depend on in an emergency braking situation with no advance notice they are unavailable. The contact stated that prior to the failure, the vehicle was repaired an unknown recall was associated with the turbocharger repair. The contact researched and discovered that the failure was a known failure with similar vehicles. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 143,500.Read more

Date of Accident: April 13, 2026 I drove into our church parking lot and then parked next to my husband’s car and was totally stopped and in the parking space. I thought I had my foot on the brake. After a split second, it accelerated and jumped the curb of the parking space at a very fast speed. I had no control of the car. I sheared off a heavy metal sidewalk railing and was headed for the br
... ick church (church is on a berm above parking lot), but I managed to steer away to miss the church and a large tree. I steered off the berm and into empty parking lot – still traveling at high speed. I finally threw it into neutral to get it to stop because it was continuing toward a busy street with lots of traffic. Neutral stopped it. No air bags deployed. The front and side of the car that hit the metal railing sustained $6,500 damage. Towed it to collision repair. When we asked at the Columbia, MO, Honda dealer’s service department, they said because the air bags didn’t deploy, that it was probably operator’s error and nothing they could do. We didn’t feel that it was operator error and reported to the assistant General Manager of Honda in a personal meeting. We asked to have the Honda field engineer exam the car. Instead, they had their district service supervisor exam the car. Their conclusion: they could find nothing wrong with the car. After reading online reports of almost identical accidents with this model, we decided it was too dangerous to drive and sold it back to the Honda dealer.Read more

While driving on interstate at highway speeds (70mph), the transmission experienced sudden downshift, which increased the engine RPMs to redline and rapidly decelerated the vehicle. Occupant safety was put at risk while other drivers were forced to quickly avoid collision due to our much slower speed. Transmission would not select proper gear which made it difficult to proceed to a safe pull-off p
... oint. After restarting the vehicle, the situation repeated itself several times while attempting to drive home. Reoccurring problems include rapid deceleration and high engine revs when transmission suddenly shifts at high speeds, or stalling & lack of acceleration when transmission shifts improperly at low speeds (which makes moving the vehicle safely from the roadway difficult). Researching the problem, it appears to be an issue with the leadframe internal to the 6R80 transmission. Apparently there are safety recalls for other Ford SUVs and trucks with the same leadframe part all related to unexpected transmission shifts.Read more

### Summary: Sudden Unintended Acceleration (SUA) from a stationary position. ### Vehicle Configuration: 2023 Tesla Model Y Long Range AWD. Battery Type: NMC. ### Incident Details: On April 19, 2026, at approximately 10:00 a.m., the vehicle was stationary on a flat surface at 6722 Vista Del Mar Ave, San Diego. I was in the driver's seat with the vehicle "On" but stopped, conversing with two w
... itnesses. My feet were completely clear of both the accelerator and brake pedals. ### The Event: Without driver input, the vehicle suddenly and forcefully accelerated forward. I immediately slammed the service brake with full force. The vehicle traveled approximately 20 feet before the brakes brought the car to a halt. ### Evidence & Gaps: Two eyewitnesses confirmed the vehicle moved autonomously without pedal interaction. A manufacturer data report was requested, but it showed a specific data gap for the exact seconds of the acceleration event, despite providing second-by-second logs for the rest of the day.Read more

First Incident- volume kept rising on its own. Would turn it down, it would go right back up. Would turn the radio off, it would turn on by itself and would keep rising until full blast. Vehicle was parked, came back from work, turned vehicle on and the volume was fine on it. Second Incident- (this has happened twice) about a month after the volume incident. This time the vehicle would accelerate
... by itself. No cruise control was on, vehicle would keep going faster and faster. First time this happened the vehicle needed to be towed, cops were involved. I finally was able to stop the vehicle at a red light (where it eventually broke down and would not start). Extended warranty covered the tow. There was no check engine light on. Replaced the battery as I was told the errors were pointing back to low voltage. After battery was replaced, maybe three weeks later the vehicle did the same thing on the highway again. Car speed kept increasing on its own, no cruise control or pedal input. Vehicle kept speeding. I took this vehicle to Moon Township, PA Nissan Cochran dealership and they could not replicate the issue. I have video proof the first time this happened where the vehicle kept increasing in speed without pedal input. I have two kids under the age of [XXX] , This is an unsafe vehicle and it has lost my trust completely. The extended warranty company for this vehicle said they can’t do anything since the dealership says no check engine light is on. Third incident- got into vehicle, turned the vehicle on and the parking brake would not release. The parking brake was never applied in the first place so I have no clue how or why it was on when I turned the vehicle on. I just recently called the dealership on this issue now (Moon Township, Nissan Cochran) and will see what they say. Again, no check engine light is present. I am at a loss on this vehicle. 2022 with only 45k miles on it and have never experienced this many issues with a vehicle before! It is unsafe. My car has started breaking randomly without reason. Just a few days ago it slammed on the brakes. I was driving on the interstate and went around a bend, and my car slammed on the breaks. I. Talking from 70- 5mph in seconds. I didn't have time to do anything. Nothing in front of me, nothing on the side. The car behind me had to swerve into the median to avoid hitting me. My car almost caused
... an accident. Im honestly afraid to drive it. Im not sure if the radar system is an issue in other Hondas, but if i was in an accident it would have been my fault. If the person behind me wasn't paying attention my children could have been seriously injured or worse. Please look into this issue. Its more dangerous than the other recalls.Read more

I was driving at 60mph. I heard a sudden pop that felt like it came from behind me / under the car. While I glanced to see if I ran over something (I did not), the car suddenly felt like it was about to stall. It was slowing down quickly and dramatically. My safety was in danger as I quickly dropped speeds in heavy traffic on a freeway without warning. I was able to maintain control, and I turned
... my hazards on. I noticed that I was maintaining speed around 35-40 mph (no matter how heavy I hit the accelerator), and I moved to the right lane and continued while wondering if my car would survive all the way home. Luckily I was only about 1 mile from my house at that point, so I proceeded at a low rate of speed and ultimately made it home. It was scary, sudden, and put me and others on the road in danger. A couple of warnings popped up saying "Electrical System issue" and something else that was related to electronics. The vehicle was towed to the dealership that night, and diagnostics and repairs began the following day. The problem has been confirmed by the dealer to be an ICCU failure that needs to be replaced. There were zero warnings prior to the failure--the car was working perfectly up until that moment. Anything is available for inspection, but as I mentioned it's being replaced as we speak so the broken / malfunctioning part wouldn't be there anymore if someone looked at it.Read more

The gas pedal failed to work on three separate occasions. The car essentially went into “limp” mode where the whole car stop driving out of nowhere. These models of Honda are known for the head gasket prematurely going out which has caused me car to have multiple misfires, warning lights, loss of power. The problem has been confirmed by the dealership and an independent mechanic.
