While driving with Tesla's Full Self-Driving (FSD) system engaged, my vehicle struck two standard traffic cones in a clearly marked construction zone. The system failed to detect the cones or make any evasive action. I have video evidence of the incident. The situation was extremely dangerous. The cones could easily have been construction workers, pedestrians, or something more severe. I hesitate
... d to intervene because it happened in a split second, and I was trusting the system to respond — which it completely failed to do. In that moment, I was also faced with a difficult choice: swerve and potentially cause a more serious accident, or trust that FSD would navigate correctly. It didn’t. The impact caused damage to my vehicle. When I brought it to Tesla, the technician dismissed my concern, laughed, and sarcastically told me, “There’s no way we’re paying for it.” He said it was my responsibility to avoid the cones — even while FSD was fully engaged. That kind of response is unacceptable, especially when Tesla markets this system as being capable of handling real-world driving environments like construction zones. This incident shows a significant failure in Tesla’s FSD recognition and decision-making systems. There were no warnings or alerts prior to the impact. The event raises major safety concerns, not only for the driver but for anyone on or near the road. I’m reporting this because I believe Tesla's current version of FSD has critical deficiencies that could lead to injury or worse. The system should never ignore basic road hazards like cones, and the company's dismissive response after the fact is alarming.Read more

Dear NHTSA, I am writing to report a serious and potentially life-threatening safety issue with my 2013 Ford F-150. I recently experienced a sudden and dangerous failure while driving on the freeway, which nearly caused a major accident. After doing some research, I discovered that this issue was previously addressed under a Ford Customer Satisfaction Program, but the coverage period has since ex
... pired. This is a common and well-documented issue among 2013 F-150 owners, and it is unacceptable that Ford has not issued a full recall. Just because the program has expired does not mean the risk has. In my case, the issue escalated to a near-death situation, and I’m extremely concerned that this will happen to someone else—if it hasn't already. I urge NHTSA to investigate this further and hold Ford accountable. There is no reason such a serious safety defect should go unresolved simply because the time window has closed. A dangerous defect is still dangerous, regardless of the date. Please let me know what steps I can take to further assist with this report or if you need additional documentation. I hope this can help prevent future tragedies. Sincerely, [XXX] [XXX] VIN: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I bought a 2019 Chevy Blazer in February 2023 . The vehicle has issues when I fill it with gas. As soon as I gas up to full it starts jerking immediately afterwards. It makes loud popping sounds. The engine dies completely. It starts to tremble and will not go. When I try to turn in back on , the starter works very hard before it will turn back on. It does turn back on eventually but I have

This is a big safety concern for me since these episodes happen every week when I fill up my car with gas. I have almost been hit from behind during these episodes and it is very scary and such a hassle to have to deal with this every week. I have already had several Chevy dealers and mechanics work on this problem and it continues to act up like this. I own a 2023 Honda CR-V Sport Hybrid and have experienced multiple instances of sudden unintended acceleration. The issue occurs intermittently, typically at low speeds (25–30 mph) during stop-and-go traffic, or when the vehicle transitions from EV to gas mode. During these incidents, the vehicle unexpectedly surges forward for 1–2 seconds, even while the brake pedal is applied. The suspected
... issue involves the electronic throttle control system or a malfunction during the hybrid system's EV-to-gas transition. There are no warning lights or error codes, and the issue occurs sporadically. The vehicle is available for inspection upon request. This behavior puts both my safety and that of others at risk, especially in heavy traffic or near pedestrians. The unexpected acceleration while braking can lead to rear-end collisions, property damage, or injury, as the vehicle moves unpredictably. Yes. A Honda service technician experienced the issue firsthand while driving my vehicle. He then tested a 2025 CR-V Hybrid and reported similar behavior, suggesting it might be “normal.” However, I strongly disagree, as the acceleration feels unsafe and abnormal. The vehicle has been inspected by the Honda dealership. A formal diagnosis was performed, but they did not find anything unusual. The service center stated they will not proceed further unless I can replicate the issue, which I cannot due to its intermittent nature. The vehicle has not been inspected by police, insurance, or independent mechanics yet. No. There were no warning lamps, alerts, or error messages before, during, or after the sudden acceleration events.Read more

I brought my vehicle to the dealership due to serious safety concerns, including the engine randomly shutting off while driving, sudden unintended acceleration, unresponsiveness when pressing the gas, difficulty turning the key to start the engine, loud noises while running, and the engine, battery, and oil lights coming on. This is the second time I’ve brought the vehicle to the Kia dealership
... for the same issue. The first time, I was told the transmission had been replaced and the problem was resolved. However, the same issue resumed shortly after I got the car back. I called the dealership that day but they were already closed, so I dropped the vehicle off the next morning when they opened. At drop-off, I was told no repairs could begin until the recall was “officially” announced, but that they would look into the issue and keep me updated. Unfortunately, I never received any updates and had to reach out myself multiple times. This morning, I was told the sensor lights showing were “old codes” and that everything seemed fine. They said they’d take the car for a test drive and that I’d have it back by the end of the day. No one called me back. I followed up around 4:30–5:00 PM and was told the service department had closed, but the notes said that after the test drive, more codes appeared and the vehicle would require additional work. When I first dropped the car off, I was told I’d have it back the next morning, and if not, I’d be provided with a rental. Neither happened. When I expressed that I didn’t feel safe driving the car—even if the test drive went well—especially after a mechanic told me he wasn’t sure how to fix the current recall, I was dismissed. As I was saying, “Okay, just call me after the test drive so I can come by and check it out,” Greg hung up on me mid-sentence. The Anti-lock Braking System (ABS) module on my 2015 Jeep Wrangler failed at approximately 100,000 miles, causing a complete loss of ABS functionality. The component remains installed in the vehicle and is available for inspection upon request. This failure significantly impacted my safety and the safety of others, as it compromised braking performance, making the vehicle more difficult to control
... during sudden stops or slippery road conditions. Without ABS, there was an increased risk of skidding, longer stopping distances, and potential collisions, particularly in emergency braking situations or adverse weather conditions. Additionally, my cruise control is no longer operable due to this issue, further impacting the vehicle's drivability. The problem was diagnosed and confirmed by Suncoast Chrysler Jeep Dodge Ram, which identified the ABS module failure as the cause and provided a repair estimate for replacement. While the vehicle has not been inspected by the manufacturer, police, or insurance representatives, the dealership's diagnosis serves as confirmation of the defect. At approximately 100,000 miles, the ABS warning light, traction control light, and brake light all illuminated on the dashboard simultaneously, signaling a system-wide failure. Since then, these warning lights have remained illuminated, indicating an ongoing issue with the vehicle’s braking and stability systems. The failure immediately coincided with a noticeable decline in braking performance and the loss of cruise control functionality. No other warning messages or alerts appeared before the failure. After researching this issue, I have found that this is a very common problem affecting numerous Jeep owners with models from 2007 to the present (JK and JKU models). Many owners have reported similar ABS module failures, leading to the loss of ABS, traction control, and cruise control.Read more

Since the incident happened today, my vehicle has not yet been inspected. No warning lamps of any kind had been activated. The car was nearly brand new. Upon further investigation, there are numerous reports of similar events happening to Toyota hybrids over the years. A simple google search for this turns up many reports. These reports center around sudden loss of braking from the regenerative braking system and a "lag" before conventional braking is applied that causes the forward lurch due to the momentum of the car. Many others on online forums seem to also suggest running into this issue specifically when the wheels hit a bump. This sounds exactly like what happened to my car, causing it to lose control despite my having the brake applied. It is insane that such a problem has existed for years in the braking system without being fixed by Toyota.Read more

Component/system that failed: The transmission control module in my 2020 Chevrolet Silverado 1500 has malfunctioned. The vehicle has issues with slipping between gears, rough shifting, and hesitation when accelerating. This component has already been inspected and diagnosed by a certified Chevrolet dealership, and the part is currently on backorder. The vehicle is available for further inspection
... upon request. Safety risk: This issue poses a serious safety risk to myself and others. The gear slipping and delayed response when driving—especially when merging onto highways or changing lanes—creates unpredictable behavior that can lead to an accident. The truck sometimes fails to engage gears properly, which makes it dangerous in traffic situations. Diagnosis confirmation: Yes, the problem has been confirmed by a Chevy dealership service advisor (Jose Diaz), who provided a formal quote and diagnosis for the replacement of the transmission control module. Inspection by others: The vehicle has not been inspected by police or insurance representatives, but Chevrolet Customer Assistance is currently reviewing the case, and I was advised to submit this documentation. Warning lamps or symptoms: There were no warning lamps initially, Symptoms include slipping gears, hesitation when pressing the gas pedal, jerky shifting, and difficulty accelerating smoothly. These symptoms have progressively worsened over time. Additional context: I have learned that many 2020 Chevrolet Silverado trucks have been recalled for the exact same issue, but my VIN is currently not included. I believe this is a manufacturing defect and that my vehicle should be considered for inclusion in the recall or further investigation.Read more
