I am a long time Corvette owner and have purchased many through the years. I recently purchased a 2008 Corvette Coupe with only 30k miles, one owner vehicle. Unbelievable to me, I have had to spend over $4,000 the past month replacing the ECM and most notably the Fuel Pump, to relieve the constant Fuel Smell, not to mention the rack and pinion steering gear needs to be replaced, with again, only 3
... 0k miles on the vehicle, and that part is discontinued. The failure of the ECM almost cost my wife and I our life when we were coming home to [XXX] from [XXX] . The car completely lost all power on the highway and we almost got railed by traffic. Not a good situation to say the least. I almost had a heart attack. So we had to have it towed and finally made it home, only to find out the next day after a nice ride, my entire garage was overwhelmed with a gas smell that evening which permeated through the whole house. I took notice that GM covered this exact situation with GM Bulletin # xxx back in 2017, so they KNEW there was a problem! My question is, how can GM not cover this NOW. Many 2008 Corvettes are still being bought and sold. I spent over $2800 for this last week! I am asking you and GM to at least cover 50% of this charge. I don't think that is too much to ask. It's much cheaper than having been sued by our family, should me wife and I have not survived that situation on the road, a case I know they would have won. We should definitely be compensated to say the least! Thanks INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

May 7, 2026 National Highway Traffic Safety Administration 1200 New Jersey Avenue SE Washington, DC 20590 RE: Safety Defect Complaint – Brake System / Vehicle Control Concerns 2020 Chevrolet Malibu Dear NHTSA, I am submitting a safety complaint regarding persistent braking and vehicle control issues involving my 2020 Chevrolet Malibu. Prior to December 31, 2025, the vehicle exhibited jerk
... ing during braking, all dashboard gauges go to the left, warning lights, reduced power (20-25mph), and inconsistent braking behavior. This situation happens without warning, at any given time, even in the flow of traffic. An independent inspection indicated the brake system was in proper condition, suggesting a more complex underlying safety issue. The vehicle was taken to Reed-Lallier Chevrolet for diagnosis and remained there for approximately five months. During that time, I was not provided documentation confirming completion of a brake system diagnostic or a clear resolution path. Due to the extended delay without diagnosis, I no longer feel safe operating the vehicle. I am also concerned that prolonged inactivity without proper repair may have worsened the condition and increased risk of failure. Given the nature of the symptoms involving braking and vehicle control, I request review for potential safety-related investigation. Vehicle Information: 2020 Chevrolet Malibu VIN: Mileage:Read more

The headlights and dashboard lose power intermittently while driving at highway speeds, specifically triggered by using the turn signal. This is ongoing since January when I started having to take it for a recalls.More things kept happening. I kept having to have more repairs. Every time I went, I told them about this and it just keeps going on. Today I took a huge list of things they should check
... to see why it’s doing this. This is the following list that I gave them -2017 Kia Forte - Service Request: •Issue: Headlights/dash icons flicker specifically when using turn signals. •Issue: Average MPG keeps resetting (Computer Brownout). •Mandatory Checks: 1Multi-Function Switch (internal short). 2Manual Voltage Drop Test on the G01 Ground Point. 3Alternator Ripple Test (check for "dirty" power). Last night on my way home from my daughter’s house which is a 50 minute drive, it was in the dark pouring rain and no longer than I had got started going home, I used the turn signal and my lights flickered a little which that’s what they’ve done before. Then the icon inside the car for headlights flashed on and off, lights inside got very dim and then my headlights got so dim that I had to follow other cars that had LED headlights down an interstate at 65 mph or less safely just to get to my exit to get home for over 45 minutes. I have mentioned this to them repeatedly, but I gave this list to them this morning. I called a little bit ago just to check on things and I was told “well we’ve been driving it for over an hour and we can’t seem to get it to act up”. I said are you not going to do the things that I stated and actually physically touch and look and not just sticking it on a computer. He rudely asked- what are you talking about? It’s all computer. That’s what we do.I’ve had to deal with this since pretty much the middle of January 2026. We are now here at May 7 and it got at its worst last night and horrified me and my safety. When I have car will upload docsRead more

The vehicle began experiencing repeated Autopilot/FSD failures while driving. Symptoms included sudden Autopilot disengagements, cruise control becoming unavailable, and loss of road visualization/navigation display functionality. The issue progressively worsened over time. The vehicle was first brought to Tesla service in 2025 for the same overheating-related concern. At that time, Tesla identi
... fied a coolant-related issue, topped off the coolant system, and returned the vehicle to service. However, the problem continued and gradually became more frequent and severe afterward. During the most recent service visit, Tesla diagnosed the HW3 Autopilot computer as overheating due to a heat transfer failure between the Autopilot board and cooling cold plate. Tesla stated that heat generated by the board could no longer be efficiently transferred to the coolant system, causing the Autopilot computer to overheat. Tesla further advised that continued operation without repair could eventually impact additional vehicle functions, including the center display screen shutting off due to overheating-related failures. Given the loss of driver-assistance functionality and potential impact to core vehicle systems, I am submitting this report so the issue is documented as a potential thermal management or hardware reliability concern involving the HW3 Autopilot computer system.Read more

The vehicle experienced a failure involving the automatic transmission gear shift cable (recall AOC 2013–2016). The gear selector indicated the vehicle was in “Park,” but the transmission was not fully engaged in Park due to the defective shift cable/bushing. This created a significant safety risk, as the vehicle was capable of rolling unexpectedly after being placed in Park. This condition
... puts the driver, passengers, pedestrians, and surrounding property at risk of injury or damage, especially when parked on an incline or in populated areas. There were no clear or consistent warning indicators prior to the issue. The vehicle appeared to be safely secured, which increases the possibility of an accident or death because the driver may exit the vehicle believing it will remain stationary. This issue is consistent with a known recall involving the shift cable assembly. The defect appears to be intermittent and not always easily reproducible, but it directly impacts the vehicle’s ability to remain securely in Park. At this time, the vehicle has not received a complete or reliable remedy, and the condition raises ongoing safety concerns. The component can be made available for inspection upon request. Because this defect affects a critical safety function and can lead to unintended vehicle movement, it presents a serious hazard. I am submitting this complaint to document the issue and request further investigation and resolution.Read more

SUMMARY OF SAFETY HAZARD: I am reporting a catastrophic and unpredictable loss of motive power in a 2023 Volkswagen Taos (4Motion with DQ381 DSG Transmission). The vehicle presents a critical electronic sensor failure within the transmission mechatronics unit that causes the vehicle to enter an immediate "Limp Mode" without warning. This has resulted in several near-miss incidents where the veh
... icle became immobilized in active traffic and on high-speed roadways. TECHNICAL DATA OF FAILURE: Factory-level ODIS diagnostic logs (recorded Feb 3 and March 17, 2026) reveal that the transmission’s "Actual position clutch 1" reports a constant value of -0.69 bar. In an automotive hydraulic system engineered for positive pressure, a reading of -0.69 bar is a physical impossibility (a vacuum state). This indicates a total failure of the pressure sensor/TCU logic, leading to the unintentional disengagement of the primary drive clutch. DECEPTIVE REPORTING RISK: Authorized service centers have been documented falsifying the safety status of this vehicle. Despite possessing internal diagnostic logs showing the -0.69 bar vacuum failure, a dealership issued a final Service Order certifying "No issues found" and stating "Actual values match." This reporting practice is forcing a vehicle with a known, documented powertrain failure back onto public roads, creating a severe life-safety risk for occupants and other motorists. The vehicle is currently grounded to prevent a collision.Read more

While driving with a trailer after leaving a jobsite in [XXX] , the vehicle experienced a sudden electrical failure while in motion. The instrument cluster/speedometer screen went completely black and the vehicle lights stopped functioning properly while driving at night. During the incident, we lost the speedometer display and visibility from the vehicle lighting system, creating a serious safet
... y hazard while towing a trailer on public roads. Because of the electrical failure, we were forced to pull off on the side of the road to avoid a potentially dangerous situation. After shutting the truck down and waiting approximately one hour, the vehicle was restarted and the systems eventually began functioning again. This vehicle is used regularly for commercial towing and jobsite operations. A sudden loss of instrument display, lighting systems, and electrical functionality while towing creates a major safety concern for both the occupants of the vehicle and surrounding traffic. The failure appeared intermittent because the vehicle resumed operation after sitting for a period of time. However, the incident involved complete loss of critical vehicle display and lighting systems while driving. The vehicle is available for inspection upon request. On 4/27/26, my family and I were traveling south on Colorado C470 at 8 AM. Our 2021 Chevy Silverado 1500 had catastrophic engine failure at highway speed as we were passing an 18 wheeler. The failure caused the vehicle to lose power and engine, and the display said to put the truck in Neutral and restart the engine. After coasting to the shoulder, the engine would not restart. We were stranded on
... the highway during rush hour for an hour and 1/2 waiting for a tow truck. This scenario is identical to the safety risks outlined in NHTSA Recall 25V274 (and investigation PE 24-007). This failure resulted in the intake manifold being physically displaced and damage to the timing assembly. The dealership is attempting to deny coverage by claiming the "upper gallery" damage (timing/intake) is unrelated to the "lower end" issues covered under the special coverage/recall [ this is a Dealer Quote]. This logic is technically flawed: Consequential Damage: A catastrophic lower-end failure (e.g., a broken connecting rod or seized crankshaft bearing) at 80 mph creates the massive internal pressure required to shatter or "blow off" a plastic intake manifold. Timing Linkage: The timing chain is physically connected to the crankshaft. It is mechanically impossible to have a catastrophic crankshaft failure at high RPMs without causing secondary damage to the timing gears and "upper gallery". Recall Precedence: My vehicle exhibits the exact failure mode—loss of motive power due to bearing failure—currently being investigated by the NHTSA for 2019-2024 L87 engines.Read more

Multiple service visits to an authorized MINI dealer have not resolved the issue. The dealership service team has stated verbally that "all the early 2025 Countrymen seem to have these issues." Dealer is now indicating that full head unit replacement may be required, with extended out-of-service time. The defect creates a documented driver-distraction risk and aligns with multiple existing NHTSA complaints and BMW Technical Service Bulletins for the 2025 Countryman platform. My 2017 Hyundai Tucson is experiencing dangerous, sudden engine stalling, which are symptoms directly matching Service Campaign T1B. These critical safety issues surfaced following a total engine replacement in June 2025. Despite the vehicle being within the 12-month/12,000 mile workmanship warranty, the dealership is refusing to honor this coverage or offer goodwill assistance. My claim is base
... d on a documented pattern of physical and technical negligence: - Failure to Update Software: The dealership failed to perform the mandatory ECM/TCM software integration (Campaign T1B) during engine swap, which is required to prevent the exact stalling I am now experiencing. - Dismissal of Warning Lights (Code P0456): One day after the engine replacement was “completed,” I returned to the dealership because the Check Engine Light illuminated with Code P0456. This was dismissed by the service provider without a proper diagnostic resolution, despite being a direct indicator of improper vacuum line reconnection during the engine installation. - Mechanical Negligence (Coolant Leak): Post-engine swap, the vehicle developed a coolant leak, which was a direct result of improper reassembly. Additional proof of the lack of due diligence during the repair. The lack of follow-through, combined with the dismissal of a life-safety defect and poor physical workmanship, suggests a total failure in the case management process. Safety Defect Complaint – Tesla Model Y FSD v14.2.2.5 I am reporting unsafe speed behavior in Tesla Model Y Full Self-Driving (FSD) software v14.2.2.5. FSD uses selectable driving profiles (Sloth, Chill, Standard, Hurry, Mad Max) to control speed, but provides no definition of how these profiles affect speed or limits. Tesla also removed the ability to manually adjust speed incrementally, redu
... cing driver control. While in Standard mode, the vehicle exceeded the speed limit by 12 mph, resulting in a photo radar citation. This was not driver-initiated. I have experienced at least six incidents where the vehicle exceeded the speed limit by 10+ mph without warning. Two design issues create significant safety risk: The right scroll wheel now controls both microphone and FSD profile selection. Its multi-directional design allows unintended inputs that can change speed profiles (e.g., to Hurry or Mad Max) without driver awareness. There is no audible or visual alert when this occurs. The vehicle can accelerate beyond safe/legal speeds before the driver realizes and disengages FSD. The driver-set speed warning (set to 8 mph over limit) failed to activate during multiple overspeed events, including the citation incident, despite being enabled. This removes a critical safety safeguard. Speed increases have occurred both gradually and rapidly, reducing reaction time. Safety Risk: unintended acceleration, loss of predictable speed control, lack of driver alerts, and failure of safety warnings. These conditions could lead to violations or collisions. I request NHTSA investigate whether this constitutes a safety defect.Read more

I own a 2016 Model S P100D equipped with the Large Drive Unit (LDU). The component that fails is the rotor coolant seal in the rear drive unit motor. Coolant leaks internally into the motor and inverter, causing bearing wear, corrosion, isolation faults, and eventual complete drive unit failure. The affected drive units are still installed and available for inspection upon request. This defect put
... s my safety and the safety of others at risk because it can cause sudden loss of propulsion (especially at highway speeds), power reduction, or total drivetrain shutdown, potentially leading to a crash. Coolant in high-voltage areas also raises electrical fault/fire risks. The problem is widely reproduced and confirmed: Tesla service centers are familiar with it and replace failed units with updated “-U” revisions that include a coolant bypass. Independent EV shops confirm the exact root cause (rotor seal failure) via encoder inspections showing coolant intrusion and through disassembly. This is a documented pattern failure in all 2012–2020 LDU vehicles. No police or insurance inspections have occurred yet, as no crash or total failure has happened. Tesla is aware of the recurring issue through service records on similar vehicles. Early symptoms before full failure typically include rear motor whining/grinding (especially under load), reduced power warnings, “Rear Drive Unit Fault” messages, and visible coolant on the speed sensor. These can appear gradually. My vehicles have not yet experienced total failure, but the defect is known to be inevitable. I am proactively monitoring encoder sensors. Because this is a systemic design defect affecting thousands of vehicles with clear safety implications, I request NHTSA open an investigation and require Tesla to issue a recall for free repairs/replacements on all affected 2012–2020 LDU vehicles.Read more

I own a 2016 Tesla Model X P100D equipped with the Large Drive Unit (LDU). The component that fails is the rotor coolant seal in the rear drive unit motor. Coolant leaks internally into the motor and inverter, causing bearing wear, corrosion, isolation faults, and eventual complete drive unit failure. The affected drive units are still installed and available for inspection upon request. This defe
... ct puts my safety and the safety of others at risk because it can cause sudden loss of propulsion (especially at highway speeds), power reduction, or total drivetrain shutdown, potentially leading to a crash. Vehicle slammed on brakes on highway with no obstruction in front. Emergency system reacts to shadows
Incident date: Apr 19, 2026
Driving using cruise control on interstate. Car in front of me exited. My car began to slow even though there was nothing in front of me but a slight incline. I pressed on the accelerator and the car continued to slow. I turned off the A/C and cruise, but the car continued to slow and came to a complete stop on the interstate. There was a semi-truck behind me. Cars were passing me. I put the car i
... n park and turned off the car. I started the car back up and drove away. I was completely stopped on an interstate with construction barriers on each side of the lanes, so there was no shoulder to pull off on. I was fearful of getting in an accident or causing one. I was very scared for my safety. There were no warning lamps or messages to assist in the moment. I took my car into the dealership and explained what happened. Dealership checked the cruise control and said "test drove vehicle and cruise control operation is normal." They reset cruise control follow distance from closest to farthest and could not duplicate the concern. I am no longer using the cruise control because I feel it is a safety issue that was not addressed.Read more

2021 Mazda CX5 AWD- Non Turbo Engine. Sunny Day, Dry. My Car was clean. No Precipitation. Some shadows of the interstate light poles across the road 200-300 feet in front of me, but they were thin shadows. Sun Was to my Left. I was driving South on May 01 2026 about 10am on the Interstate going Highway Speeds, with adaptive cruise control "on", in the 2nd lane of a 4 lane interstate. Thankf
... ully no other traffic ahead or behind me. Car slammed on the brakes suddenly, pretty hard. I counteracted and added more gas pedal to keep in going. I turned off the Cruise Control. After a while I re-engaged the Cruise Control and had no issues. This could have been a huge accident in busier traffic. Will get the car checked out at Mazda Dealership Service this week.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? Power Steering While driving yesterday, doing shopping and online order deliveries, the steering wheel started having slight resistance during turns. It became worse shortly after. As I attempted to leave a grocery store, less than 5 minutes away from home, the steering wheel was extremely difficu
... lt to turn. I didn't see any leaks under the hood nor on the ground underneath my car. Nor was there any smells nor odors. On 5/1/2026, I did hear a slight noise when I turned the steering wheel to back into the driveway at home. How was your safety or the safety of others put at risk? Steering wheel became difficult to turn right or left. I barely made it home to park it, as the longer I drove to get home, the more difficult it became to make turns. I parked it immediately as I didn't want to make too wide of a turn on a road, risking any accidents. Has the problem been reproduced or confirmed by a dealer or independent service center? Not yet. I will need the car towed to a local Ford/Lincoln dealership for assessment. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Not yet, in the process of contacting my auto insurance about it. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The message on the dashboard was "Service Power Steering" and it appeared on 5/2/2026 for the very first time. Also, the power steering icon showed in a stable amber color. This message did not appear before.Read more

NHTSA Complaint SummaryVehicle: 2017 Ford Escape SEEngine: 1.5L EcoBoostVIN: [XXX] Defect: Engine Block Design Flaw / Coolant IntrusionTechnical Description of the Issue:My vehicle is experiencing a well-documented manufacturing defect specific to the 1.5L EcoBoost engine. The engine block was designed with a "v-groove" between the cylinders that is prone to structural failure. This allows engine
... coolant to leak into the combustion chambers (coolant intrusion) and allows engine oil to contaminate the cooling system. My vehicle currently exhibits "milky," tan-brown sludge in the coolant reservoir, confirming that oil and coolant are cross-contaminating due to internal engine failure.Extreme Safety Hazards and Dangers:This defect creates several critical safety risks for the driver, passengers, and other motorists:Sudden Loss of Power at High Speeds: The mixing of oil and coolant destroys the engine’s ability to lubricate internal bearings. This can lead to a catastrophic engine seizure while the vehicle is in motion, causing an immediate loss of power and power steering, which creates a high risk of collisions on highways.Engine Fire Risk: When coolant enters the combustion chamber, it can cause severe engine misfires and overheating. Overheated oil or pressurized coolant spraying onto a hot exhaust manifold during a mechanical failure presents a significant fire hazard.Hydro-locking: If coolant floods the cylinder while the vehicle is in operation, the engine can hydro-lock, causing the engine to stall instantly and potentially throwing metal debris onto the roadway, endangering other drivers.Visibility Hazards: Excessive white smoke from the exhaust (caused by burning coolant) can suddenly obscure the driver’s rear-view visibility and the visibility of trailing motorists. Conclusion:Ford Motor Company was aware of this defect, as evidenced by Customer Satisfaction Program 21N08 and TSB 19-2346. However, because these programs have arbitrary expira INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (Read more
