On September 27th 2023 I brought my vehicle in because it was having shifting and vibration issues. The car was driving fine until early this year when it starts to do the vibration that was the issue previous when my transmission was going out. So I set up appointment at Ford dealership while I was in route to the dealership my car started to sputter and shake and fall out of gear. So I turned around tried to make it home in the car just stopped moving. I wasn't able to move forward or reverse there is no connection within the transmission for my car to move. I got my car towed to the dealership who diagnosed my car and my transmission and they stated that there was metal fragments found in the fluid level and I also said that the torque converter clutch performance never engage when it was testing it. I told him about my previous transmission issue with Ford and I also States them that four has have many federal recalls dealing with their transmissions in their vehicle.Read more

While driving the engine seems to "go out" which is a risk to the driver, passengers, and other motorists near by because it is not expected and can happen anywhere (two lane road, major highway, while turning, etc.). You are able to press the gas and hear the engine but the car does not move - it acts as if it were in neutral. There are no safety or warning lights on the dashboard before or duri
... ng this issue meaning that there is no way for the driver to prepare. There have been times where I was able to pull off the road and restart the car to temporarily remedy the issue but safe and accessible pull offs are not always available (bride, two lane road, etc). I travel with small children and there is a fear that if this continues to happen it can result in fatal consequences. In addition to the safety issue mentioned above there is a perpetual leak coming from the engine. This has progressively gotten worse and after speaking with a representative at Ford they named off a few "possible" causes but were unable to make a definitive decision. I was also informed that the issues that I am experiencing were not unique to just my vehicle but many Ford Escape 2013's in the area that had come to the dealership for guidance and help. While I have stated that the issue occurred in 2013 - it is still happening and continues to get worse.Read more

There is a recall on this motorcycle's front brake switch. Supposedly it was repaired before it was delivered to me in 2021. I have had problems with the cruise control intermittently turning itself off and becoming inoperative, which is a symptom of the problem caused by a defective front brake switch according to the recall. I have had the motorcycle in to the dealership twice, starting in 20

Several times, they claimed that there were no parts available. I just got off the phone for the third time with Yamaha Customer Relations (714-761-7300). The message they left was from a person that said his name was "Jim". When I called back and spoke to him he confirmed that he had called me but said that his name was Jeremy. No extension number was provided. Jeremy said that I had to bring in the motorcycle a third time to have it inspected. I have become a hamster spinning in a wheel. I wanted to share a very scary incident that occurred while I was slowly creeping into a parking space in a parking garage. Additionally, I would like to mention that there was no warning lamps lit up on my dashboard before the incident and none appeared after the crash. My insurance was notified, and they deemed the car undrivable. The insurance representative is scheduled to inspect the car tomorrow. ⸻ Title: Loss of Power While Driving on Freeway – Dangerous Malfunction Similar to Recall 23V-116 Complaint Description: I am reporting a serious and dangerous malfunction with my 2021 Jeep Wrangler Rubicon EcoDiesel. While driving on a high-speed freeway (70–80 MPH), all warning lights on the dashboard illuminated—including the check engine light—and the vehicle suddenly went into li
... mp mode, resulting in a loss of power. This occurred in fast-moving traffic, putting me at immediate risk of a crash. This started happening again every time I got on the freeway for 10 minutes. I’ve owned this Jeep for about a year, have never taken it off-road, and have kept it properly maintained. All electrical components, fuses, and systems have been inspected, and no clear cause has been found. The problem persists and is clearly not due to driver use or negligence. This issue strongly resembles Recall Number 23V-116, which states that affected vehicles may experience a loss of drive power, increasing the risk of a crash. That recall was for 2021 Jeep Wrangler vehicles, the same model and year as mine. However, my VIN was not included, despite the fact that I’m experiencing the exact same symptoms described in the recall notice: “A loss of drive power increases the risk of a crash.” This issue began occurring shortly after that recall was issued, and it has happened multiple times, mostly during highway driving—making the vehicle extremely unsafe. I believe my vehicle should have been included in Recall 23V-116, as this is clearly a safety-related defect involving the powertrain or control module software. This is an urgent safety issue, and I respectfully request that NHTSA investigate why this VIN was excluded from the recall and consider expanding the recall to cover additional affected vehicles before someone gets seriously injured or worse. Component/System That Failed: The 10R80 automatic transmission is malfunctioning. It causes harsh shifting, delayed acceleration, hesitation, and unpredictable surging. These issues have been confirmed as part of Ford’s internal TSB 23-2250, which directly references this transmission defect. The vehicle is available for inspection upon request. Safety Risk: This transmission failure poses a se
... rious safety risk. The hesitation and delay in acceleration occur during merging, turning, and highway driving, which can cause near misses and sudden loss of power. The delayed response time compromises my ability to move safely through traffic. Reproduced/Confirmed: Yes. The issue was confirmed by Liberty Ford in Fayetteville, NC, after I paid for a diagnostic. The service department acknowledged the symptoms but refused to cover the diagnosis or repair under warranty since no recall was showing under my VIN. Inspected by Others: The vehicle has not been inspected by the manufacturer directly or by law enforcement or insurance. However, it was diagnosed and documented by an authorized Ford dealership (Liberty Ford in Fayetteville NC). Warning Lamps or Symptoms: There were no warning lamps or error codes. The symptoms began subtly but became consistent over time. The first noticeable event was approximately in “early 2025”, with increased severity since then. I submitted a written complaint to Ford on June 9, 2025, via USPS Priority Mail (Tracking #[XXX]) and also filed a certified safety complaint with NHTSA (Tracking #[XXX]) but have received no response. On June 5, 2025, I was driving my 2025 Kia Sorento. I proceeded to get onto the highway from an on ramp. At that moment, the engine stalled. The car would not accelerate. I could not go faster than 20-25 mph. This was a huge safety risk to me, to my passengers, and to others on the highway. Eventually I exited the highway and drove home very slowly, since the car would not accelerate beyond 20-25
... mph. I was pressing the gas pedal as far down as it would go. I even tried turning off the car and restarting it. The check engine light was on. The car was towed to a Kia dealer. The Kia mechanics noted that the fault log in the car's computer system noted that the stalling/failure to accelerate event did occur, but that they were unable to reproduce it, even after several attempts driving it around in various conditions. The Kia mechanics told me there was nothing they could do or fix, since they were unable to reproduce the stalling event. Then just yesterday, on July 6, 2025, the Sorento stalled again while it was being driven on the highway. I am having it towed tomorrow to the same Kia dealership to have the same mechanics look at it again. At this point, this is an extreme safety issue for anyone driving the car, in the car while it is being driven, or on the highway when the car is on the highway. Imagine if the car stalls again and another car behind us traveling at a high speed slams into us. We could potentially suffer mortal or grave physical injury.Read more

On [XXX] at [XXX] I dropped my wife and 2 children off at the Funland Arcade in Deep Creek, Maryland. The parking lot was full from the holiday weekend and I was planning on taking the Lightning to a nearby charger while the family gamed so I double parked perpendicular to the arcade entrance, put the hazards on, and let them out. After the 3 of them got out, I placed the Lightning in reverse to
... back up 5-10 feet so that I could exit the parking lot through the same way I came in. Once I had enough space from reversing so I could make the turn to exit I placed the Lighting in drive. The moment the Lightning switched to drive it accelerated at a very high speed as if I was flooring the gas pedal, which I was not. The lightning was in one-petal mode which I always use unless I’m on a highway. I immediately began pumping the brakes but the truck would not stop, I probably slammed on the brake petal 4-5 times to no avail. I even tried shifting the truck back into park but it would not shift out of drive. I was able to luckily steer the truck into a hillside approximately 150 feet away. The truck ran over numerous small trees and shrubs before crashing to a stop approximately 16 feet up the hill. Shortly after the shock wore off, I slowly reversed the truck down the hill, exited the truck, and informed dozens of onlookers who came out of the arcade to see what the noise in the parking lot was from, that the truck just took off on its own but that I was fine. I called 911 and the police came. Luckily no people or vehicles were between the arcade entrance and the hillside or the outcome would have been significantly worse. This crash was witnessed first hand by wife and 2 children. I have grave safety concerns about this truck now and will not put my family or self back in it until Ford fixes this problem. The vehicle was a 2023 Toyota RAV4 hybrid. When the driver attempted to park the vehicle in a parking lot, it suddenly began moving forward without use of the accelerator. Toyota was notified of the crash and a Toyota representative made the 911 call. A New York state trooper and two fire trucks arrived. The vehicle was towed and eventually collected by our insurance company. The tow truck driver said the brake pedal went to the floor when he tried it. The car was declared a total loss. 1. The notable failure was in the braking system. The car also seemed to be traveling faster than it should and didn't slow down. The insurance company has the vehicle now. 2. The safety issue was loss of speed and braking control of the vehicle. Steering was normal. Fortunately the parking lot had few cars or people in it. 3. No investigation of the problem aside from assessment of damage that I know of. 4. Liberty Mutual assessed the damage. The NY state trooper made a collision information form and gave us a copy. 5. The vehicle unexpectedly began accelerating from a creep with no accelerator input. Acceleration ceased immediately as soon a the brake pedal was touched. No actual braking was required--simply a touch on the pedal. Acceleration was as if the accelerator pedal had been pressed about 1/4 down. It lasted about 1.5 seconds from initiation to touching the brake pedal. The pavement was not on a grade b
... ut was substantially level. This is the 3rd or 4th such incident in the last few months. In the other incidents I was unsure if it was my imagination or some unrelated factor like grade. In previous incidents I had been about to accelerate in any case, and pressing the accelerator overtook it. In this incident, the car was navigating a parking lot, having just pulled out of the parking space and moved 50-100 ft. The car was moving in "creep" mode, i.e., the EV system simulating the creep of an automatic transmission (ca 0-4 mph). My impression is that each incident occured not long after restarting the car, though I am not fully certain of this. In this occurence there was one passenger; in all previous occurences there was no passengers. In all occurences, the Cruise Control system was on but not engaged. The "driving style" was set for Normal, as opposed to Sport or Eco. No alerts or unexpected lights were observed on the instrument panel. For the latest incident, the driver's floor mat was checked and found to be properly secured and well clear of the accelerator pedal. The car's last (and only) service was the normal Honda "A1" maintenance in May 2025 at ~11000 miles. Service was done at a Honda shop, so any firmware should be current to that date. The shop was not notified of the acceleration issue at that time. The Honda shop has been notified today (July 5).Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The Engine Control Module (ECM) is malfunctioning, triggering persistent P0101 codes related to the Mass Air Flow (MAF) sensor. This specific issue is addressed in Nissan Technical Service Bulletin DE97AA, which recommends an ECM reprogramming. The component is still installed in the vehicle and is a
... vailable for inspection. ⸻ • How was your safety or the safety of others put at risk? When the vehicle is at a complete stop and I attempt to accelerate, it becomes jumpy and unresponsive, refusing to accelerate properly. This creates a serious safety hazard, especially when trying to merge into traffic or cross intersections, as I am unable to match the speed of other vehicles. This increases the risk of being rear-ended or causing an accident due to unexpectedly slow acceleration. ⸻ • Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. An independent mechanic diagnosed the issue and confirmed the code P0101. After reviewing Nissan’s official TSB DE97AA, they advised me to go to a Nissan dealership to have the ECM reprogrammed as outlined in the bulletin. ⸻ • Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle was inspected by an independent mechanic, but not yet by the manufacturer. The dealership is aware of the issue but has refused to perform the repair without payment. ⸻ • Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The Check Engine Light is consistently triggered by code P0101, and the traction control warning light also comes on when the issue is active. The symptoms appeared recently and continue to recur even after the code is reset.Read more

On a sudden the vehicle RPM unintended rev up too high and the vehicle had an unintended accelerating to move forward aggressively causing lose control of the steering . Also she could not brake even though her left foot was on the brake pedal and pressing hard to try to stop the vehicle . . Received a P2138 Throttle/Pedal position sensor/switch "D"/"E" voltage correlation code while driving that caused "Reduced Power" and "stabilitrak" errors. The reduced power mode it went into made the truck unable to accelerate in a safe manner and made the truck "jump" wildly as it attempted to accelerate.

My 2014 Jeep Cherokee began exhibiting repeated transmission and electrical malfunctions, particularly after startup or while idling. The most concerning issue is the appearance of a “Switch to Park” warning message, which occurs while the vehicle is already in Drive. When this happens, the transmission fails to engage properly, and I’m unable to shift into gear without cycling through Park
... multiple times. **seems to start happening after filling gas tank to full** This poses a major safety risk, especially in intersections or during stop-and-go traffic. The car has also experienced flashing odometer issues, rough idle, and erratic behavior when shifting — all of which point to a communication failure between core modules (BCM, TCM, PCM). A Chrysler-certified dealership performed a proxy alignment after acknowledging that the modules were “talking at the same time.” While no fault codes appeared on my OBD-II scanner, the dealer determined that the issue required a software-level alignment. However, I was told this is not part of any current recall or TSB, despite my symptoms being identical to other complaints already logged (e.g., NHTSA ID 11425007). This problem has occurred multiple times, both before and after dealership service. I was also informed there are no current recalls, but VIN-based systems like VinSeeker.com have suggested there may be open recall alerts related to transmission or software issues. This type of malfunction places the driver and passengers at risk of unexpected stalling, transmission disengagement, or inability to accelerate — especially dangerous in high-traffic areas. The issue appears to be known, systemic, and potentially tied to faulty transmission control software or defective module synchronization.Read more

The vehicle experienced multiple critical system failures within one week of purchase: Transmission failure (slipping, delayed shifting, and stalling during acceleration) Braking system malfunction (brakes jump or pulse when pressed, inconsistent stopping) Fluid leakage inside the cabin near the gas and brake pedals (believed to be either brake fluid or transmission fluid) The vehicle is curre
... ntly undrivable and can be made available for inspection upon request. The vehicle became a severe safety hazard while I was operating it with my infant child in the car. The brakes were unresponsive and unpredictable, creating near-miss situations when trying to stop. The transmission failure caused stalling at intersections and hesitation when accelerating, putting me at risk of being rear-ended. The fluid leak near the pedals presents a possible fire hazard or the risk of further brake failure. I feared for my life and my child’s safety. The problem has not been confirmed by a mechanic yet because the dealership, Platinum Auto Sales in Arlington, VA, refused to take the vehicle back or inspect it after I reported the issues. I was dismissed when I returned to report the failures. I am currently seeking an independent inspection, but I want this formally documented as an urgent safety matter. I am in the process of filing reports with the Federal Trade Commission, state consumer protection, and local police regarding fraud and sale of a dangerous vehicle. I can make the vehicle available for inspection by NHTSA or a certified safety inspector. There were no warning lights or error messages at the time of purchase. The problems appeared within 24–48 hours of driving the vehicle: Difficulty shifting and hesitation when accelerating began almost immediately Brake issues and pedal fluid leakage appeared by Day 3 By Day 5, the vehicle became unsafe to drive altogetherRead more

I am filing this complaint regarding a known and documented issue affecting my 2012 Toyota Camry (VIN: [XXX] ), which has developed a transmission shudder and vibration between 20–50 mph. This problem is directly aligned with Toyota Technical Service Bulletin T-SB-0079-15 and Customer Support Program ZF4, both of which describe issues with the torque converter and ECM software. My vehicle has 1
... 16,670 miles, which falls within the mileage eligibility of the support program (ZF4: 150,000 miles), though it is just outside the time limitation (8 years). I reached out to Toyota (Case #: [XXX]) to request goodwill assistance due to the safety-related nature of the defect and the fact that it is a well-documented product flaw, but they declined support solely based on time — not on mileage or vehicle condition. The vibration affects the transmission and drivability, particularly during city and highway acceleration, creating potential hazards when merging or navigating traffic. I believe this qualifies as a design flaw with potential safety implications. I respectfully request that NHTSA review Toyota’s handling of this issue and determine whether further consumer protection or a recall is warranted, especially for owners who were not adequately notified or who fall just outside the support window despite clear symptoms.
