On XXX, my vehicle unexpectedly activated its automatic braking system while driving, resulting in a rear-end collision. My infant daughter was in the vehicle at the time. While we were fortunate to avoid catastrophic injuries, the incident placed both of us in significant danger and could easily have resulted in a much more serious outcome. The other driver may also have sustained injuries as a r
... esult of the collision. The vehicle sustained approximately $20,000 in damage. Since the accident, I have learned of Hyundai’s recall involving this braking system. Given the circumstances, I am deeply concerned that the condition addressed by the recall contributed to the collision. What is particularly troubling is that this was not the first time I raised concerns regarding the vehicle’s braking behavior. On February 23, 2026, at approximately 11:00 a.m., I spoke with my dealership, North Bay Hyundai in [XXX] regarding unexpected braking behavior that I was experiencing with the vehicle. My concerns were not escalated, and I was not advised that a potential safety issue might exist. Several months later, I was involved in a collision involving the same system that is now subject to recall. I am also deeply concerned by the manner in which this recall was communicated. I received only a text message regarding a defect involving a critical vehicle safety system. Given the potential consequences of unexpected braking events, I do not believe the seriousness of this issue was adequately conveyed to affected customers. CHRONOLOGY OF CASCADING VEHICLE SAFETY FAILURES VIN [XXX] Vehicle 2015 Honda CR-V. 1 STRUCTURAL BODY FRAME DEFECT. Vehicle has a chronic upper body structural alignment defect allowing massive water intrusion to bypass factory headliner and moonroof seals. Multiple consecutive factory OEM headliners have been destroyed by water logging, proving the upper tracking architecture is permanently warpe
... d and cannot maintain an environmental seal. 2 ELECTRICAL FAILURE ENGINE STALL HAZARD. Saturated water tracking through the upper frame has migrated into primary engine bay electrical lines. This chronic moisture caused a severe short circuit that fried the vehicle alternator, voltage regulator, and primary battery requiring emergency replacement. This infrastructure breakdown creates an imminent safety risk of sudden loss of vehicle charging power and dangerous engine stalling while operating at highway speeds. 3 ADAS INFRASTRUCTURE FAILURES. Moisture migration has breached the primary dashboard wire harnesses causing active continuous power dropouts and glitching across the center infotainment screen and main driver instrument display interface, blinding the operator to speedometers and critical crash avoidance alerts. 4 AIRBAG HAZARDS ACTIVE BIOHAZARD. Saturated water tracking down internal pillars runs directly over Side Curtain Airbag modules and electronic connectors, risking sensor corrosion. Cabin flooding has saturated seating foam, endangering electronic Occupant Classification Sensors OCS that regulate passenger airbag deployment forces. Unremediated moisture left toxic mold spores festering beneath the seating infrastructure, causing severe respiratory irritation to occupants. REPAIR STATUS. Unresolved safety defect. Repair networks failed to remediate structural hazards. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

While driving a brand new Kia Carnival Hybrid (purchased yesterday), I experienced a severe, life-threatening powertrain acceleration lag from a rolling start. My children were in the car and this was very scary. When attempting to accelerate into oncoming traffic on a wet road surface, the vehicle completely hesitated and dropped power for approximately 1.5 to 2 seconds, leaving the vehicle stran
... ded in the path of oncoming traffic. When the power finally engaged, it delivered a violent surge of torque that caused the front tires to break traction and squeal aggressively. This software-driven hesitation and power-distribution lag represents a severe safety hazard.Read more

My 2025 Cadillac Escalade ESV lost power and shut down while I was driving on the highway. The gas pedal stopped responding, and the vehicle would not restart afterward. This created a serious safety risk because the vehicle suddenly lost propulsion while driving in traffic. The vehicle is less than one year old and is already going to be on its third engine. The original engine was already repla
... ced once before. The previous replacement engine was a brand-new GM engine with 0 miles, but it failed after only about 20,000 miles. Pepe Cadillac confirmed that this is the second replacement engine, which means the third engine counting the original engine. The dealer also confirmed that the engine locked up due to internal bearing material failure and needs to be replaced again. The repair order number is 140134. I am concerned this is a serious safety-related engine defect. A brand-new GM replacement engine should not lock up after only about 22,000 miles, especially while driving on the highway. I am also concerned that simply replacing the engine again may not fully address the safety and reliability issue with this vehicle. Cadillac Customer Care case number: 100539041.Read more

The vehicle is available for inspection upon request. It is currently at a dealership that is working to reproduce and confirm the problem. There are Reddit threads that discuss other Sorento owners experiencing this same issue. COMPONENT FAILURE: The engine, power train, and speed control systems failed due to the known Nu 2.0L GDI connecting rod bearing defect. SAFETY RISK: My [XXX] daughter was driving in heavy rush-hour traffic when the vehicle suddenly lost motive power and entered a 20 MPH "limp mode." This sudden deceleration created an immediate and severe risk of a rear-end collision, endangering my daughter a
... nd surrounding motorists. DEALER CONFIRMATION: The failure was officially confirmed by Flow Kia of Charlottesville (RO #XXX). On Dec 3, 2025, the dealer confirmed active DTC P1326 (KSDS) and documented failed cylinder compression (as low as 150 PSI; min spec is 178 PSI). The dealer issued a written diagnosis stating: "engine replacement is required." MANUFACTURER CONDUCT: Despite their own dealer's written diagnosis and the active P1326 safety code, Kia Corporate issued a bad-faith warranty denial. The dealer also unlawfully charged me $165 for the mandatory KSDS warranty diagnostics. SUMMARY: This is a textbook safety failure under NHTSA Investigation EA 21-003. Kia's refusal to replace the engine under the court-supervised Engine Litigation II settlement represents a systemic failure by the manufacturer to remedy a known safety hazard. 51-page evidence packet attached. 2016 Ford F-150 experiencing intermittent and dangerous transmission behavior consistent with the current Ford safety recall involving unexpected transmission downshifts on vehicles equipped with the 6R80 transmission. Vehicle previously required a full transmission replacement/rebuild in September 2025 after repeated violent downshifting and drivability problems. Diagnostic codes at that time in
... cluded P0705, P0707, P1702, and P1921 involving transmission range sensor and intermittent transmission signal faults. Following the rebuild, intermittent drivability concerns continued. Over several months, vehicle would periodically not “sound right” during operation, although symptoms were difficult to duplicate consistently. Check engine light would intermittently illuminate after approximately one hour or more of continuous driving and later clear itself after vehicle cooled down overnight. Current symptoms have worsened and include unexpected downshifting, inability to manually shift using dash controls, skipped gears, delayed downshifts/upshifts, remaining in 3rd gear while slowing to a stop, and behavior consistent with being stuck in low gear while traveling above 50 mph with excessively high RPMs. Vehicle intermittently appears unable to properly determine gear position. On May 19, 2026 I experienced uncontrollable downshifting and engine revving that resulted in an inability to safely operate the vehicle. Transmission behavior creates unsafe driving conditions and concern for loss of vehicle control during highway operation. Vehicle is now considered unsafe to operate and will be towed to a Ford dealership for evaluation related to the active safety recall.Read more

Vehicle exhibits repeated brake pulsation and vibration under normal braking conditions, affecting braking performance consistency. The issue began at approximately 10,000 miles and has progressively worsened. The vehicle currently has approximately 30,000 miles. The pulsation is felt through the steering wheel and brake pedal during moderate braking from highway speeds and occurs consistently acr
... oss multiple driving conditions. The condition is significantly more pronounced when using the vehicle’s adaptive cruise control system. During normal operation, the adaptive cruise repeatedly applies the brakes automatically, and this consistently produces increased vibration and uneven braking response. This indicates the issue occurs during normal vehicle operation rather than abnormal use. The repeated pulsation creates inconsistent brake feel and may affect stopping performance, particularly during repeated or sustained braking events. This raises concern regarding braking reliability and stopping consistency, especially in traffic conditions where adaptive cruise control is actively managing following distance. The condition has steadily worsened over time and is present during routine driving. It is not limited to high-speed or heavy braking scenarios. A manufacturer Technical Service Bulletin (TSB CHA126V) exists for this vehicle model addressing front brake disc judder. The symptoms described in the bulletin match the condition experienced in this vehicle. Given the early onset, progressive nature of the issue, and the involvement of system-controlled braking (adaptive cruise), this condition appears to be related to component performance or design rather than normal wear. Kia response .. Dear [XXX] , Thank you for contacting Kia Customer Care. While driving my 2023 Nissan Rogue on Interstate xxx from [XXX] to [XXX] with my xxx daughter in the vehicle, the vehicle suddenly displayed multiple warnings including collision assist/sensor malfunction warnings, start/stop system failure warnings, and “check owner’s manual” messages. Immediately afterward, the vehicle experienced a sudden loss of power and refusal to accelerate. At the
... time of failure, I was traveling approximately 78 mph in the fast lane of the interstate. The vehicle rapidly lost speed and dropped into what appeared to be limp mode, eventually slowing into the upper 50 mph range despite pressing the accelerator. I had to fight across multiple lanes of freeway traffic while unable to maintain highway speed. This created an extremely dangerous situation due to surrounding high-speed traffic and limited safe shoulder access in a remote desert area. The vehicle became unsafe to continue driving and required a tow of over 70 miles to the nearest Nissan dealership. The tow expense alone exceeded $600. Prior to this incident, the vehicle had already experienced a separate unexplained no-power/no-start event. At that time, the battery, starter, and alternator reportedly tested normal. The vehicle remains within the manufacturer’s 5 year/60,000 mile powertrain warranty and currently has active recalls associated with the vehicle. A Nissan case has been opened and the vehicle is pending dealer diagnosis and inspection. After posting publicly about this incident in Nissan owner groups, I identified numerous other owners reporting nearly identical symptoms involving warning light cascades, sudden loss of acceleration, limp mode events, and highway safety concerns. This incident created a serious safety hazard involving sudden highway deceleration and loss of motive power during interstate travel. While driving my 2025 Chevrolet Equinox, the vehicle experienced a sudden power and drivability issue. The vehicle began to decelerate on its own while in motion, even though I was actively driving. A warning notification appeared on the driver dashboard, and shortly after, the vehicle power shut off. This creates a serious safety concern because the vehicle can unexpectedly lose speed and/or pow

The loss of acceleration and sudden deceleration could increase the risk of a crash, especially at highway speeds or in heavy traffic. This incident occurred while the vehicle was being driven, not while parked. The warning message appeared on the dash before or during the power loss. I am reporting this because the vehicle’s sudden deceleration and power shutdown while driving could be a safety defect. Other than the lurch that occurred while I was pressing hard on the brake, I didn’t move the car until after I had put it in neutral and it settled down. Then I put it in drive and moved forward. I don't recall any warning lamps or messages. No report yet from the Honda dealer [XXX] ), but I wanted to report this since this should never happen -- regardless of the root cause. I was driving the vehicle on the highway the cruise control was on. All of a sudden I got a warning on vehicle info panel that stated vehicle power is reduced. The vehicle decelerated on its own and I could not accelerate past 55. The check engine light came on. I pulled off the road and turned the vehicle off. After about 10 minutes, I started the car back up and proceeded down the highway at nor

They could find no error codes and could not duplicate the problem. I was sent on my way. This has happened multiple times while driving on roads in my area. Mostly while on the highway.Read more

My car is experiencing a harsh down shift from 3rd to 4th gear with lack of power on acceleration on my vehicle since 10/25/2024. I have taken it in to three service departments including Karl Flammer Ford in Tarpon Springs, FL. Each service department has said the only remedy is a New Transmission estimated to be $7,000-$8,000 (receipts of diagnostics can be provided). We have already spent $52
... 4.95 for diagnostic testing. Because we have been unable to afford to have the problem fixed we are currently driving around in an extremely dangerous vehicle. I have received the notice of Ford's "Important Safety Recall" Notice 26S28/ NHTSA Recall 26V237 and have researched that other F-150's from previous years have experienced similar issues with the transmission and have been recalled. I would like to be on record, incase something were to happen because of this transmission, that the NHTSA as well as Ford Motor is completely aware of how dangerous this situation is. Quoting from the recall notice: an unexpected downshift to a lower gear can occur causing an abrupt wheel speed reduction. This may result in a loss of vehicle control, increasing the risk of a crash.Read more

UPDATE to previous complaint – Reference Number: [11738920] VIN: [XXX] Case: Tesla [XXX] UPDATE to my previous NHTSA complaint regarding sudden unintended acceleration in my Tesla Model Y (VIN: [XXX] ) I previously reported a sudden acceleration incident on [XXX], at [XXX] 4 parking garage ramp. Tesla has now provided the Vehicle Data Report and EDR data (Case # [XXX]). The data shows accelera
... tor pedal position increasing to 100%, with brake input occurring late. However, I am 100% certain that I did not press the accelerator pedal at any time. I have a clean driving record of over 27 years. This discrepancy between the logged data and actual driver input strongly suggests a potential safety defect in the accelerator pedal sensor system (both redundant sensors), drive-by-wire system, or brake override / Automatic Emergency Braking functionality. Attachments included with this update: Tesla Vehicle Data Report (VDR) Tesla EDR Report High-resolution photos of vehicle damage and airbag deployment I request that NHTSA review this additional technical information as part of their evaluation for a possible safety defect investigation. BTW, it is really difficult to contact Tesla to report such incidents. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I own a 2019 BMW X5 xDrive40i with the B58 3.0L turbocharged inline-six engine. While driving in Minnesota, the vehicle suddenly displayed an urgent warning stating “oil pressure too low” and instructed me to stop the engine. This occurred while the vehicle was being operated on the road, requiring me to stop driving immediately. The vehicle was later inspected by an authorized BMW dealer. Th
... e dealer diagnosed the failure as a defective oil pump and stated that the oil pump must be replaced. The estimated repair cost was approximately $7,500 or more. This is a serious safety concern because a sudden loss of engine oil pressure can lead to engine seizure, loss of motive power, and loss of vehicle control risk depending on traffic conditions. The oil pump is a critical lubrication component and should not fail as a normal wear item. I have found numerous similar owner reports involving BMW B58 oil pump failures, including reports suggesting that cold-weather operation may contribute to or expose the failure. My vehicle is operated in Minnesota, a cold-weather state. The vehicle was inspected by Park Place BMW in Rochester, MN. I am requesting that NHTSA investigate whether this is a safety-related defect affecting 2019 BMW X5 xDrive40i vehicles and other BMW models equipped with the B58 engine.Read more

While driving the car immediately downshifted and I could not accelerate. The Hill assist came on and service advance trac came on. This happened 3 times. Luckly I was able to stop and start it quickly to drive it to a near dealership. We picked up the truck today, and it stalled again on the way home. We drove it back to the dealership today, May 23rd, hopefully they can figure it out. We are again without a car for a long period of time. We have paid $1,964.10 in repairs that have not fixed the problem. The way the truck stops so suddenly is very dangerous as it happened once getting on the highway and once in front of a train track.Read more
