I was proceeding to drive home from an appointment with a friend, the car suddenly lost power and I had no control over anything. I was able to keep the steering wheel in the locked position trying to get to the side of the road. Thankfully the speed was only 25mph, as someone had just switched lanes and came in front of me. I was in the process of turning to enter the highway. I've been having a
... problem with this car since I purchased. I told Ford after this incident, that I fear to drive the vehicle with my family, because that's how scared I was when I had no control. I requested multiple times for someone from Ford Corporate to contact me, and still have not heard from anyone. On 11/26 I received a recall for the 12v battery, which is pretty much what keeps the car on and off, and I still have not heard from anyone in Ford corporate.Read more

While driving at highway speed and the adaptive cruise control engaged, the vehicle applied the brakes without driver input. The vehicle slowed gradually over several seconds even though there was no vehicle, pedestrian, or obstacle present in front of the vehicle. Road and environmental conditions were clear and normal, including clear weather, dry roadway, and unobstructed lane of travel. Traffi
... c was flowing normally, and there was no lead vehicle. The driver was attentive and did not press the brake pedal. The braking occurred without any warning, alert, or collision indication and resulted in an unexpected reduction in speed, creating a risk of a rear-end collision from following traffic and on one occasion road rage (which has occurred due to this condition) as the driver thought I "brake checked" him. The vehicle has been inspected by the local Kia dealership (Spitzer Kia of Mansfield) and they could not replicate the condition. Kia has also been notified and a case number assigned (25773402) but with no resolution. This issue has occurred multiple times under similar highway driving conditions, indicating repeatable uncommanded braking behavior. Additionally, KIA issued a “Voluntary Service Campaign” for 2024 Kia Telluride’s on July 31, 2025 for the same vehicle behavior. Unexpected braking at highway speeds creates sudden speed differentials and increases the risk of rear-end collisions, even when deceleration is gradual. The driver has limited ability to anticipate or prevent the event. No warning lights, audible alerts, or collision warnings were displayed. CONCLUSION The vehicle exhibits repeatable false or uncommanded activation of the automatic braking system during highway driving, consistent with a potential defect in the AEB/FCA system. This condition poses a safety risk to vehicle occupants and surrounding traffic and should be reviewed by NHTSARead more

This is in regards to Recall #C4B for the ABS/HCU failure on my 2017 RAM. This recall was issued over a year ago in November of 2024, but Stellantis has yet to DO ANYTHING and can't even tell us when a remedy will be available. I have asked my local dealership and get the same answer, NOTHING!!! The parts must be available, because the dealership told me that I could purchase the part from them
... and pay them to install it. But, I would have to then wait to get reimbursed from Stellantis once the recall is "remedied". Two problems with this; 1) I don't have the $1500+ to spend on this, and 2) I don't trust them to ever pay me back! But, this does tell me that they have the parts. Plus, they have no trouble making and selling new vehicles with parts that don't have this safety failure. Can you make them prioritize this recall and take car of the thousands of RAM trucks with this safety issue. BTW, last month my brother-in-law was driving my truck on I-25 through Denver, CO. He hit an icy spot and lost traction causing him to go into a 360 spin. God was looking over him because he did hit anything or anyone. THIS WOULD NOT HAVE HAPPENED IF THE TRACTION CONTROL SYSTEM WOULD HAVE BEEN WORKING. STELLANTIS CAME VERY CLOSE TO BEING RESPONSIBLE FOR HIS DEATH!!!Read more

I am reporting an unresolved safety defect involving the 12.3-inch instrument panel display on my Toyota RAV4 Prime, which is the subject of NHTSA Recall 25V595 (Manufacturer Recall 25TB08). The defect affects the LED digital display that provides critical driver information, including vehicle speed, warning indicators, and operating status. Initially, the display would intermittently go blank at
... startup and sometimes return during the drive. However, the condition has progressively worsened. The instrument panel display has now been completely blank and unresponsive for over three consecutive days and has not returned at all, including after multiple drive cycles. As a result, the vehicle cannot display critical operating information or malfunction indicators. This issue was first reported to an authorized Toyota dealer in August, prior to the recall being issued. At that time, I was informed that I would be responsible for replacing the entire display/instrument system at my own expense. On September 11, 2025, Toyota issued Recall 25V595 acknowledging that the instrument panel display may be blank at startup and stating that operating the vehicle in this condition reduces the ability to see malfunction indicators and could increase the risk of a crash or injury. The vehicle was most recently serviced on December 19, at which time the dealer confirmed that the issue is related to the recall and that no remedy is currently available. The recall remains incomplete, and Toyota has not provided a repair, interim solution, loaner vehicle, or timeline for resolution. Because the instrument panel display is completely nonfunctional, the vehicle cannot be safely operated. The lack of a remedy for a safety-related defect affecting a core vehicle safety system has exceeded a reasonable timeframe. I am requesting NHTSA review and intervention.Read more

I purchased a new 2023 Kia Sorento in February 2023 from Ken Garff Kia Avondale. Since shortly after purchase, the vehicle has experienced repeated electrical and mechanical failures that substantially impair safety. In July 2023, the vehicle began displaying flashing lights, dashboard error messages, and system malfunctions while driving, despite no collision or visible damage. The air condition
... ing system malfunctioned and blew hot air even when turned off. The dealership stated a front sensor was “damaged” and not warranty-covered, despite no visible damage, and verbally indicated that many other customers were experiencing similar issues. Repairs took nearly four months, during which Kia Corporate engineers were involved. The issue persisted despite repairs. In December 2024, the vehicle began displaying repeated transmission system error warnings, often within the first 5–10 minutes of driving, causing significant loss of speed and drivability. After multiple service visits, the vehicle required a full transmission replacement in July/August 2025 under warranty. Despite this repair, the same transmission errors returned in December 2025. More critically, the vehicle then experienced multiple sudden and complete electrical shutdowns without warning, including: Shutting off entirely while stopped at a red light, requiring a jump start All screens going black and the vehicle failing to restart Restarting only after being left parked for several minutes These events occurred without any battery or system warning indicators and created a serious safety risk, including risk of being stranded or disabled in traffic. The issues have been documented with photos, videos, repair records, and written communications. Due to the ongoing and dangerous nature of these failures, I am reporting this as a vehicle safety defect.Read more

Upon starting the vehicle, it failed to engage in reverse. Although the backup camera activated, the vehicle remained stationary. After multiple ignition cycles, the vehicle eventually engaged in reverse and then moved forward in Drive. However, shortly after, the vehicle experienced a sudden loss of power and entered "limp mode," restricting speed to approximately 15–20 mph regardless of thrott
... le input. The engine revved excessively high without corresponding acceleration, and a severe burning/heat smell emanated from the transmission/engine compartment. This failure occurred without any prior warning; notably, no dashboard warning lights or "Check Engine" indicators illuminated to alert the driver of a mechanical or electronic fault. The sudden, un-signaled drop in speed on a public roadway created a significant safety hazard for following traffic and risked a rear-end collision. The vehicle was towed to a certified Honda dealership where the Transmission Control Module (TCM) was diagnosed as failed. The TCM is currently being replaced under the Honda Certified Pre-Owned (CPO) warranty. We do not know what else requires replacement and repairs. The dealership will not cover the replacement of the transmission fluid, or engine fluid, despite the fluid being subjected to extreme heat and potential damage during the failure and subsequent limp mode.Read more

While driving at approximately 65 mph on the I-405 freeway in Irvine, CA during rainy conditions, my 2019 Mercedes-Benz C300 suddenly and completely lost acceleration without warning. At the same moment, a Check Engine Light illuminated on the dash. I was forced to coast across all lanes and stop in the emergency shoulder in a very unsafe area with high-speed traffic passing close by in low visib
... ility rainy conditions. Multiple vehicles nearly struck us, including one that had to emergency brake directly behind my car in the shoulder. I contacted AAA and the vehicle had to be towed because it was not safe or driveable. My girlfriend was also in the car and both of us experienced significant fear and distress during and after the incident. My mechanic retrieved fault code P019185 and stated this is a known software problem with the engine control unit that requires a Mercedes software update and reset of mixture adaptations per a manufacturer LI/service document. This means the manufacturer appears aware of the defect and the required software fix, yet I received no recall notice or safety warning. A sudden total loss of engine power at highway speeds creates an extreme crash risk and life-threatening danger to the driver, passengers, and surrounding traffic. There have been other serious drivability issues with this vehicle in the past (misfires, coolant pipe failure, etc.), but this incident was the most dangerous. I am reporting this because this defect appears safety-related and could easily cause a fatality if it happens to someone else at freeway speeds. -- What component or system failed or malfunctioned, and is it available for inspection upon request? This report is regarding a premature transmission failure in our 2017 Toyota Highlander equipped with the UA80F 8-speed automatic transmission. Our highlander is currently stuck at High Tech Automotive in Wenatchee, WA 150+ miles from home after experiencing a transmission failure on the highway.
... -- How was your safety or the safety of others put at risk? During the transmission failure while driving on the highway the Check Engine Light came on as well as ABS light and vehicle went into a limp mode. The transmission failure and subsequent limp mode rapidly decreased our speed while traveling on the highway and forced us to limp the vehicle at low speeds across traffic putting ourselves and others traveling on the highway at risk. This vehicle has been well maintained and serviced in accordance with Toyota’s recommended maintenance schedule. Despite this, the transmission failed unexpectedly during a recent family road trip to Wenatchee, WA, leaving us stranded far from home. -- Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, the vehicle was towed to High Tech Automotive in Wenatchee WA (where it is currently stranded). The diagnostics report there confirmed proper transmission fluid level and a failed transmission with burnt transmission fluid. -- Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Not yet. A request for assistance was sent to Toyota today. -- Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? There were no warning lamps, or messages. We've had the vehicle maintained and there was a whirring sound which we had inspected at Dougs Auto Tune and Repair thinking it could be the timing chain or alternator but there was no indication of transmission issues.Read more

I notified the dealership and they gave me a loaner vehicle and they took it to complete auto care in Denver Colorado to have the vehicle fixed. I was told that the
... vehicle had a cracked turbo manifold, and the turbo manifold and the entirety of the setup of the turbo needed to be replaced. I find that odd that the check engine light came on with 50 miles of purchasing the new vehicle on the second day. It appears to me that the check engine light was cleared from the vehicle, and the vehicle was sold only for the light to return more quickly than they anticipated.I received the vehicle back no check engine light no exhaust fumes. It appeared had been repaired. I went through a 500 mile break-in of the new turbo. Within that 500 miles of the new turbo, I began to smell a gasoline, also coming from the vents and the engine bay once again entering the cabin. I documented all of this information and I contacted the dealership. Hooshies Auto Sales in Denver Colorado. And I also contacted the shop that completed the repair repairs. The dealership has not responded to multiple emails, indicating there’s an immediate risk to myself and my daughters in the vehicle from carbon monoxide from the exhaust fumes entering the cabin, not only that fuel fumes are still entering the cabin. The check engine light is back on P0299 turbo underboost. I have parked the vehicle as it is no longer drivable. I am in need of a vehicle for work and for personal needs. The dealership has escalated this to the owners, which I have tried to contact multiple times via phone and email and they will not respond nor will they call me back.Read more

I am reporting a repeated safety defect that happens only in rainy conditions. My 2022 Tesla Model Y repeatedly shows critical safety warnings including “Automatic Emergency Braking unavailable”, traction and stability control disabled, and “Vehicle Hold unavailable”. This has occurred during normal driving in rain every rainy season for 3 years, despite 4 service visits and parts replacem
... ent including the inverter. It started when I was pregnant and is now ongoing with my newborn in the car. The system failures happen while driving in normal wet/slippery conditions and greatly increase the risk of a crash. This appears to be a systemic safety defect, not driver error, and Tesla has not fixed it after multiple service attempts. Events & Dates: •First occurrence: Feb/2024 •Service visits: 02/01/2024 1. This report is being made within 1 1/2 hours after the incident. It has not been inspected by a dealer, service center, nor any other agency. I was stopped at a local fast food restaurant waiting to pay for my purchase. The 2021 Ford F-150 was in drive with my foot on the brake. No warning lights or other indication were present. Without warning, the engine revved to a high rpm and the truck shot
... forward about 15-20 feet before I could react by putting my full weight on the brake pedal. The engine quickly slowed. I then put the vehicle in reverse and backed up to the window and placed the transmission in park. The truck is equipped with adaptive cruise control but it was not active at the time of the incident. Had there been another vehicle in front of me, I would have rear-ended them. I work from home and use the truck a couple of times per week and it is available for inspection most days.Read more

Mileage: Approximately 45,000 Current Status: Inoperable; held at Joe Machens Nissan pending evaluation by Nissan Consumer Affairs. Description of the Problem The vehicle has become completely inoperable, making it unsafe to operate. In the months prior to the failure, traction control warning indicators intermittently appeared on the dashboard. I did not recognize their significance at the time a
... nd did not document them, but they may have been early warning signs of the current operational failure. Approximately 22 days prior to the vehicle becoming completely inoperable, it underwent a recall update, oil change, and tire rotation at Joe Machens Nissan. Staff reported no concerns at the time. Shortly afterward, the vehicle began exhibiting reduced driving function and erratic power, which eventually led to complete inoperability. On [XXX], while commuting approximately 40 miles at highway speeds (up to 70 mph), the vehicle exhibited unpredictable engine response. The RPMs fluctuated erratically, and the vehicle would not accelerate beyond 70 mph. There were no dashboard warning indicators at the time. The vehicle is currently in the care of Joe Machens Nissan pending evaluation by Nissan Consumer Affairs. I am submitting this complaint to document potential safety concerns and to contribute to investigations regarding this vehicle model, including similar complaints reported by other 2021 Nissan Versa owners. Safety Impact Statement The operational failure poses a risk of reduced vehicle control and potential accident while driving at highway speeds, even in the absence of dashboard warnings. I am submitting a safety complaint regarding a recurring infotainment system failure involving the 2025/2026 BMW X3. While driving, the center display screen suddenly blacks out, which creates a serious safety hazard. This issue first occurred with my personal BMW X3. More concerning, the exact same failure occurred again while I was driving a BMW X3 loaner vehicle provided by the dealership. Thi
... s indicates the problem is not isolated to a single vehicle but may be systemic. I was able to capture video evidence of the incident while driving, which clearly shows the screen going completely black: [XXX] When the screen blacks out, access to navigation, vehicle settings, driver assistance displays, and other critical information is lost while the vehicle is in motion. This creates a dangerous distraction and increases the risk of an accident. Despite previous service visits, the issue has not been resolved, and the occurrence in a second vehicle raises serious concerns about the reliability and safety of BMW’s infotainment system in the X3. And most importantly, this is the brand-new redesign BMW X3 2025 and 2026 model. Not any previous model, which raises more concerns. I believe this defect warrants investigation, as it directly impacts driver safety and may affect multiple vehicles. I am uncomfortable continuing to drive the vehicle under these conditions. Thank you for your attention to this matter. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
