Vehicle equipped with Tesla Full Self-Driving (Supervised v 14.2) software exhibits unsafe automated following behavior. The system maintains following distances that are too short at steady speeds, including behind stable lead vehicles with no braking or traffic disturbances. Tesla’s own Safety Score system flags this behavior as “following too closely,” indicating elevated collision risk.
... However, the driver has no available control to adjust minimum following distance or impose safer headway while using FSD This represents unsafe autonomous tailgating behavior with loss of driver control authority over safe following distance, creating increased risk of rear-end collision and hazardous automated driving conditions. Tesla offers several controls (Sloth, Chill, Standard, Hurry, and MadMax) modes, none of which solve the issue if the driver in front is driving the speed limit. My car under FSD will crowd (or tailgate) the front car regardless of the mode. In one instance, I was following a car under FSD in sloth mode and it made the car in front of me pull over just to get me off their tail.... this is certainly now how I wish to drive, and don't want my car driving this way. Failure mode: automated headway policy dominance, lack of driver override, unsafe autonomous following distance.Read more

Vehicle equipped with Tesla Full Self-Driving (Supervised v 14.2) software exhibits unsafe automated behavior due to removal of driver speed control. The system infers speed limits and driving speed without allowing the driver to set a safe maximum speed. In residential neighborhoods with children, pedestrians, and shared social spaces, the vehicle drives at model-inferred speeds that are sociall
... y and physically unsafe. Driver is unable to impose a lower safe speed limit without disengaging. Tesla removed previous option for driver to adjust speed. In a state park campground, the system failed to detect a posted 15 mph speed limit and inferred a 55 mph limit. The vehicle accelerated to unsafe speeds on narrow, pedestrian-heavy roads, with no driver ability to cap speed while under FSD. This represents a loss of human override authority and unsafe autonomous system behavior, creating pedestrian hazard and safety risk. Failure mode: automated speed inference dominance, map prior misclassification, lack of driver override, unsafe autonomous acceleration in pedestrian environments.Read more

Subject: Safety Complaint – 2022 Honda HR‑V – Intermittent Unintended Acceleration While Braking I am submitting this complaint regarding a serious safety defect in my 2022 Honda HR‑V. On multiple occasions, the vehicle has unexpectedly accelerated while I was pressing the brake pedal. This behavior is intermittent, unpredictable, and dangerous. Description of the Issue: While slowing dow
... n or coming to a stop, the vehicle will occasionally surge forward despite my foot being firmly on the brake. The engine revs and the vehicle attempts to move forward against braking force. This is not driver error. It occurs without any warning lights, alerts, or diagnostic messages. Operating Conditions When the Problem Occurs: - Low‑speed braking (parking lots, stop signs, traffic - Foot fully on the brake pedal - No cruise control active - No pedals obstructed - Normal road and weather conditions Safety Concerns: This defect creates a clear risk of: - Rear‑end collisions - Pedestrian injury - Property damage Loss of vehicle control The behavior is consistent with an electronic brake/throttle control malfunction, including possible issues with the brake pedal position sensor, accelerator pedal sensor, brake booster pressure sensor, or driver‑assist modules (ABS, ESC, CMBS). Other HR‑V owners have reported similar incidents. Dealer Response: The issue is intermittent, and the dealership has been unable to reproduce it. No repairs have been performed. No diagnostic codes were stored. Requested Action: I am requesting that NHTSA investigate this issue as a potential unintended acceleration defect affecting the 2022 Honda HR‑V. This problem poses a significant safety hazard to me and to the public. Thank you for your attention to this matter. During a normal commute on the highway, the vehicle suddenly lost all engine power and stalled in the middle of high speed traffic. The vehicle could not exceed 8 mph while attempting to pull to the side of the road for safety. There were no warning lights (other than a low tire pressures - normal for cold days) and no error messages. The car was unable to get started for about 10 minutes. Eve

This vehicle has had multiple incidents of surging of the propulsion system both on battery power and when powered by the internal combustion engine, causing loss of propulsion and sudden acceleration almost causing collisions multiple times. - The vehicle has been experiencing shifting issues (its an automatic vehicle) when driving in all instances and at all speeds that has progressively gotten worse over the past 4-8 weeks. - As it seems to be a transmission issue (and a known one pending litigation) the safety issue is if the transmission fails during operation of the car. - It was brought to a dealership to be inspected after it
... was initially felt as more of a vibration than the more harsh shifting/engine sounds and issues that it is currently experiencing. The dealership thought it to be the front hub assembly/bearing that was the issue. I had the front driver side replaced and the issue still remains and has continued to worsen. It has not been inspected by the manufacturer, police, or insurance representative. - There have been no warning lamps or messages at all. - Through research it appears this is a known issue with ongoing litigation (2 class action lawsuits currently in litigation related to UA80F transmissions of which it appears that our VIN is in the affected group).Read more

The vehicle frequently fails to engage gas engine when depressing gas pedal to any degree. A full push of the gas pedal does not result in gas engine engagement. This has happened over 15 ties since the recall computer update last summer (2025). This creates extreme danger because the operator expects the full power of the engine to engage when pulling out of intersections or crossing oncoming tra
... ffic, but the engine does not engage causing a slow, creeping acceleration from the electric motor only. I took it into the Kia dealer and they found nothing wrong. No warning lights occur when the malfunction happens. And no check engine lights occur either. The only correction is to fully turn the vehicle off and back on. Doing that resets everything back to normal. This malfunction usually occurs within the first 25-30 minutes of vehicle operation.Read more

I bought a Subaru Forrester 2025 in March of 2025. I drove this car for almost 9 months to the day. On 12-19 -25 I was driving down the aisle in the Walmart parking lot at less than 10 mph. I turned right into a handicap parking lot and applied my brake as I was settling into the parking space. I was driving less than 5 mph as one normally would when parking and coming to a stop. My car accelera
... ted quickly and with great speed and since my front wheels were still turned toward the right, my car sped toward the cement encased parking sign at a high rate of speed. I was in disbelief. I backed up to be in the proper space and got out and surveyed the damages. Thankfully I was alone in the car and due to the handicapped parking signs ahead and to the right of the parking spot I did not cross the marked lot lines and no other vehicles were involved. The car was not driveable so I had the it towed to the dealership and found they did not have a body shop. Then engaged a body shop to repair. Estimate of damages is $6,820. My car is still there today. It is now Jan 25 and we are waiting on a few replacement parts. At first I thought I might have stepped on the gas instead of the brake as I was parking that night. But all common sense tells me that my foot was properly on the brake as I was finishing up my parking maneuver at a very slow rate of speed. I looked around the internet and see that these same circumstances were replicated by other drivers esp. on the 2012-2020 models of Forresters. I had no prior knowledge of what I consider to be a significant fault in this vehicle. I did receive widespread bruising to my chest from the seatbelt. Once my car is repaired I will take it back to the dealer to have several systems checked for errors. I am leery of driving it again. I am really not in a position to buy a different vehicle but it may come to that. This malfunction is ongoing and the dealership cannot fix the problem. The first time it happened in September 2025. I was driving down the road and the gauges started to go crazy. The screen on the dash said reduced acceleration. Then it said Service ESC. Then it said Service Traction Control. It would not let me go over 40 mph. I took it to the dealership and they told me I needed a valve body r
... eplacement. They installed the valve body and that did not fix the problem. It happened again and said reduced acceleration took over forcing me to pull over on the side of the road. The GMC dealership said to bring it in again but they could not reproduce the malfunction after driving for a week. It happens randomly. The third time this happened I was traveling up interstate 24 at about 70 mph. There were semi trucks around me when the malfunction happened and my acceleration and speed was reduced throwing the RPMs as high as they would go, and I was almost hit by a semi trying to move out of the way and onto the shoulder of the interstate I tried to accelerate to get out of the way of the semi truck but could not. I had to swerve over almost losing control of my truck. I informed the dealership that this had happened again and they asked me to bring it in. I did. They told me they were going to clean corrosion off of ground wires, and that should fix the problem. It did not. This keeps happening and I feel that the vehicle is not safe to drive. I do not know when it will happen again. I travel for a living and I feel the vehicle is unsafe to operate.Read more

... BMW certified master technicians, with 50+ years of working on MBW cars said, they never seen this defect nor this module failed before. They shared that this looks like a design flaw, with no recall and no history. I sent a certified letter dated Dec. 22, 2025 to BMW North America CEO Sebastian Mackensen and BMW Global CEO Oliver Zipse. Unfortunately, both did not read the letter. I was told they are too busy with other things, which is appalling. The letter was passed to their customer relations. They shared with me and confirmed that although this defect is rare, they will not reimburse me the cost of the repair since my car is out of warranty. I am aware that the car is out of warranty however they should stand and held accountable to the quality of the module since there are certified technicians shared with me that could have been a design flaw. This is not a wear and tear tear. Even BMW Service acknowledged that fact, especially with the vehicle that has been driven 34,000 miles. I believe this is a Safety issue. Suspension drops, uneven load shifts, and potential next corner could roll. Again, BMW admits it’s rare—meaning they know it happens—yet they won’t cover it. That’s not quality commitment. How are you? The problem with our Hyundai involves the engine and engine control system. My vehicle is equipped with the same 2.0-liter Nu GDI engine that Hyundai recalled in other 2021 and 2022 vehicles for engine stalling, internal engine damage, and fire risk. Although my VIN is not currently listed, the engine itself is identical to the recalled engines and is available for inspection. Since
... early ownership, the vehicle has repeatedly hesitated, surged, and jumped when accelerating from a stop, especially at stoplights and intersections. This unpredictable behavior creates a clear safety risk because the vehicle does not respond consistently in traffic and increases the risk of a collision. These issues are ongoing and reproducible under normal driving conditions. The symptoms have occurred many times and were not isolated events. The vehicle has been presented for service and the issues described. No misuse or maintenance-related cause has been identified. Prior to and during these events, the vehicle has shown reduced power, inconsistent throttle response, and hesitation. At times, warning lights and engine behavior consistent with internal engine distress have appeared. These symptoms match those described by Hyundai, Consumer Reports, and NHTSA in recalls involving this same engine. Hyundai has already acknowledged that this engine defect can lead to stalling, seizure, oil leaks, and fire, and that warning systems may not fully detect the problem until damage has already begun. Because my vehicle uses the same recalled engine and exhibits the same failure characteristics, I believe it should be reviewed as part of a recall expansion or defect investigation.Read more

While driving in the Los Angelas area on March 24, 2025, on the 210 Freeway approaching the 605 Freeway the vehicle abruptly decelerated from 65mph to 45mph. We contacted Jeep Connected who directed us to Cerritos CDJR. As we were heading so
... uth on the 605 Freeway the vehicle again abruptly decelerated to 28mph. This placed us in a dangerous safety situation as we were at the height of the afternoon traffic. The Service Advisor was able to determine that the Valve Body(VB) and Transmission Control Module(TCM) had failed. Also, both batteries were fully drained of their charge. The mileage was 4095. The comment was that the batteries contributed to the VB/TCM failure. We have continued to experience battery degradation with five (5) batteries having been replaced within the first year of ownership. We continue to identify battery degradation to the dealership but have received no correction to this defect. Over the past four (4) months we have documented "resting" battery voltage at a level where they are at or below 50% of their expected lifetime. While it is suggested that parasitic drain is the cause, we continually ensure that lights, radio, heating and air-conditioning and Jeep Connected are turned off when parked. I have documented to the Utah Division of Consumer Protection and NCDS these conditions. The ruling from NCDS is: DENIED. "I have reached this conclusion for the following reason: the customer did prove by shop work orders a Manufacturer defect does exist with the batteries discharging, which has affected the use and safety of the vehicle. However, there has only been three repair attempts with replacing batteries which is not considered unreasonable in my opinion." With the recognition by an Arbitrator that a manufacture defect exists which affects safety, it cannot be limited to my vehicle.Read more

I wanted to register a complaint on an issue we have experienced in our 2024 Honda Prologue EX Elite with the "CV axles." This vehicle was sold to us with like-new mileage and was also certified, pre-owned by Honda. Symptoms of the problem were knocking sounds heard from the lower, front section of the car when accelerating from a stop and turning at the same time. It was first noticed around 4
... 000 miles and diagnosed by the local Honda dealer at around 7300 miles. Based on many blog postings about consumer experiences with this issue, I believe it would have been difficult for Honda to unknowingly sell this car with this as a design defect, and it does not have a fix other than replacing with the same part which is prone to failure. Again, this claim can be backed up by many other consumer postings which reference defective CV axles being replaced multiple times. We would have made a wiser decision on purchase selection and are now concerned about resale value of the car. We are asking for your help in a recall mandate after evaluating the problem for Honda Prologues and GM vehicles using the same platform.Read more

The transmission of this model car is very poorly programmed a low speeds. it is constantly jerking in and out of gears making it difficult to moderate speed. There is also a substantial delay between hitting the pedal and accelerating the vehicle. The issues with the transmission paired with the very delayed accelleration leads to very ineffective control of the vehicles speed, leading to what I
... believe is dangerous driving scenarios. Turning through inersections, braking at a light, accellerating out of the way of traffic, are all made much more difficult/unpredictible due to how unpredictible and inconsitent the transmission behaves. It feels impossible to 'learn' how it behaves. Here is a forumn of other users confirming similar issues: https://www.mdxers.org/threads/jerky-transmission.181658/ I believe this should be addressed via software update or recall as it i'm sure will lead to accidents/injuries.Read more

Following a manufacturer over-the-air (OTA) software update released in Fall 2025 and subsequent completion of Safety Recall 68C, my vehicle exhibits new and dangerous driving behavior that did not exist prior to these updates. While operating in electric mode, the vehicle can unexpectedly lose power during acceleration, including at roadway speeds and while climbing hills, creating a risk of sudd
... en deceleration in traffic. Under electric operation, the vehicle accelerates normally up to approximately 45 mph. Beyond that point, electric power output progressively decreases even as accelerator input is maintained or increased. Vehicle speed can decline while the accelerator is further depressed, and the electric power indicator visibly drops. This often results in abrupt internal combustion engine (ICE) engagement, aggressive downshifting, and sudden acceleration. This loss-and-surge behavior is unpredictable and hazardous, particularly in traffic or on inclines. Prior to the OTA update and recall action, the vehicle provided consistent electric power well beyond 45 mph, and ICE engagement was smooth and nearly imperceptible. I consistently achieved approximately 22–23 miles of electric range on the same daily route. Post-update and post-recall, usable electric range has been reduced to approximately 14–15 miles under similar conditions, regardless of weather. The issue also occurs at low speeds, including complete loss of electric momentum while climbing hills, followed by harsh ICE engagement that can cause wheel spin. This behavior raises safety concerns related to power loss and unexpected acceleration. Based on the timing and nature of these changes, I am concerned that software updates may be intentionally limiting battery power output and usable capacity to reduce stress on potentially defective high-voltage batteries rather than correcting the underlying hardware defect. This materially alters vehicle performance and safety and requires investigation.Read more

I am filing a safety defect complaint regarding my 2017 ford explorer. The B pillar has broke in half a couple of years ago and nearly struck a vehicle behind me and if the remaining trim piece detaches, it could become a serious risk of collision, injury or property damage. it states NHTSA works to prevent unsafe traveling which is why I'm writing this. I have contacted Ford customer service and
... dealerships multiple times with no luck of anything. I have had to continuously tape the area to door which is not ordeal nor a forever fix. The b pillar is held on by wires that are slowly breaking. The wires lead to a hole that is inside my door. the opening exposes from the outside, meaning if it rains or snows, it is getting inside the hole, into my car door. If the recall piece ever came in, my door has already experienced neglect and issues. I'm concerned this will lead to corrosion, electrical complications and additional mechanical failures. Also, if it is not taped up correctly, air blows directly into the car door, into the cabin while driving onto my feet causing extreme cold exposure and physical harm and making it hard to operate the vehicle. It's very painful and also not ideal, especially when I also have a full time job and 3 kids to worry about. I currently owe 12k still on the vehicle and want to replace it with a safer vehicle. I should not have to be miserable every single day and put me and my kids in danger and also let my vehicle deteriorate while i wait for this recall piece on top of having to pay the remaining balance of this vehicle. I respectfully request that nhtsa review this complaint and investigate before anymore physical, emotional, financially harm is done.Read more

While driving the anti-skid anti-lock failure light flashes with an audible warning. This has routinely been happening since early 2025, with the only local dealer advise is to rapidly depress the gas pedal with the key on (engine off). There’s been zero advancement to provide remedy to date. When I ask the dealer, he says “we haven’t been notified yet of parts being available. You’ll be n

Lost all power to vehicle after driving for approximately 40 minutes, around 60-70 MPH. I was driving on the highway and then lost all power. I was able to pull off of the interstate and got a few blinking messages. The only one I remember was "Shift to P to Start". I could not accelerate at all and probably had less than a minute to get to safety before the car coasted to a stop and I was forced
... to put the car in park and turn it back on. It seemed like a forced restart on a computer but you could not choose the time or place to do it and there was very little warning. This is a huge safety issue!Read more

Unknown. Intermittently when merging into highway traffic from the onramp, the truck will stall and I will lose acceleration. There will be a loss in power and the truck speed will not change. instead it will just coast with no ability to change speed. I have owned the truck for 1 year and 3months. I advised the dealer and they can not duplicate the issue.

Piston oil ring recall update complete in July 20 25. A month later key fob battery dies. Replaced key fob battery every other month since recall. September 20 25 had brakes and rotors changed at dealership since then anti-slip alert has triggered twice on dry roads. Light comes on then immediately goes off. Remote car starter stopped working in December. Dealership had no resolution for key fob i
... ssues and has not been able to replicate anti-slip issue. Dealership currently has vehicle to attempt to replicate issue and to look into the remote car starter issue. This is unsafe because the car automatically shutters and feels uncontrolled slip comes on also causes issues when key fob consistently dies every other month and get alert, letting me know that the key fob has died. And while the remote car starter is not a safety issue, it is all tied in with the electrical issues that are happening. The issues started after the recall was added to my vehicle.Read more
